Unraveling the Mechanistic Spectrum of Myhre Syndrome: SMAD4 Signaling Disruption, Skeletal Phenotypes, and Translational Innovation.

Myhre syndrome (MYHRS) is an ultra-rare, progressive multisystem disorder caused by recurrent heterozygous missense variants in the SMAD4 gene, a central mediator of TGF-β and BMP signaling. Skeletal abnormalities-including postnatal short stature, brachydactyly, thickened calvarium, and craniofacial dysmorphism-are cardinal features, often accompanied by joint contractures and progressive soft tissue fibrosis. Extensive cellular and genetic evidence supports a gain-of-function (GoF) mechanism wherein mutant SMAD4 displays increased protein stability and prolonged nuclear localization, enhanci

Therapy for Myhre Syndrome: Goals, Misconceptions, and Current Agents.

Myhre Syndrome (MYHRS, MIM #139210) is a rare, multisystem connective tissue disorder caused by recurrent heterozygous gain-of-function pathogenic variants in the SMAD4 gene, a key player in TGF-β signaling and a regulator of extracellular matrix homeostasis. MYHRS is characterized by a progressive fibrotic phenotype affecting multiple organ systems, including the skeletal, cardiovascular, respiratory, and integumentary systems. MYHRS individuals often present with short stature, joint contractures, cardiac valve defects, subglottic stenosis, and skin thickening. Neurodevelopmental disorders,

Microdeletion and microduplication syndromes, including recurrent rearrangements at 16p11.2 and 22q11.21, are enriched in unexplained male infertility.

STUDY QUESTION: What is the impact of undiagnosed microdeletion and microduplication syndromes (MMS) for men with idiopathic low sperm count? SUMMARY ANSWER: Among idiopathic male infertility, ∼2% of cases harbour known disease-causing microdeletions and duplications linked to clinically well-established syndromes, representing ∼2.5-fold higher prevalence than in the general population. WHAT IS KNOWN ALREADY: While infertility affects up to 10% of men, a substantial proportion of cases remain with no identifiable underlying cause. Recurrent submicroscopic losses or gains cause MMS, some of w

Risk of testicular cancer and exposure to welding fumes.

The incidence of germ-cell testicular cancer (TC) has increased in recent decades. Current evidence does not indicate a unanimous association between occupational exposure to welding fumes and TC risk. However, most publications do not provide information on exposure levels to welding fumes. We investigated the association between occupational exposure to welding fumes and the risk of TC in a German case-control study (268 cases and 797 control subjects). A measurement-based welding-exposure matrix was used to estimate year-specific exposure values which were linked to job-task descriptions. O

Disorders of sex development associated with MPI and RSPH1 variants expand the phenotypic spectrum of CDG and PCD in Morocco.

BACKGROUND: Human sex development is a highly regulated process guiding undifferentiated gonads toward a testicular or ovarian fate. Disruptions in this pathway result in disorders of sexual development (DSD), characterized by atypical chromosomal, gonadal, or anatomical sex. These conditions usually appear as ambiguous genitalia at birth or as atypical pubertal development during adolescence. Different etiologic, phenotypic, and genotypic factors can cause DSD. Advances in next-generation sequencing (NGS) have significantly accelerated the identification of genetic variants through targeted p

Elevated anti-müllerian hormone concentrations in blood serum of dogs with oligozoospermia or azoospermia.

Reduced semen quality such as oligozoospermia or azoospermia is a frequent reason for subfertility in dogs. Anti-Müllerian hormone (AMH), produced by Sertoli cells, is known to play a role in spermatogenesis. Elevated serum AMH concentrations have been reported in dogs with Deslorelin-induced testicular atrophy resulting in azoospermia. This led us to hypothesize that congenital or acquired oligozoospermia or azoospermia often due to testicular degeneration or atrophy may also result in increased AMH serum levels, and that AMH could potentially serve as a diagnostic marker. The study included

A Hemizygous MED12 Variant in Three Brothers with Hypomasculinized Genitalia and Additional Clinical Features: A Case Report.

INTRODUCTION: MED12 is a causative gene for congenital malformation syndromes. The association between MED12 variants and 46,XY disorders/differences of sex development (DSD) remains unclear, although several variant-positive patients exhibited genital abnormalities. CASE PRESENTATION: Three siblings manifested hypomasculinized genitalia, including hypospadias and cryptorchidism, with normal or mildly increased gonadotropin levels. Two patients showed normal spontaneous puberty. Whole exome sequencing identified a maternally derived hemizygous MED12 variant (c.3064A>G, p.Met1022Val). One pati

Decoding mechanisms and therapeutic opportunities in testicular development diseases through macrophage immunology.

Testicular development is a complex biological process that relies on three critical elements: proper anatomical development of the reproductive system, precise hormonal regulation, and coordinated interactions among various cell populations sustaining a functional micro-environment within the testis. The testicular immune micro-environment is essential for normal testicular development. As principal regulatory components of this specialized niche, testicular macrophages demonstrate remarkable functional and phenotypic diversity, serving as critical mediators of immune homeostasis within the t

Analysis of the Relationship Between Early Estrogen Response and Cryptorchidism.

INTRODUCTION: Cryptorchidism, a common congenital malformation in male newborns due to abnormal testicular descent, is closely linked to male infertility and germ cell tumors. The study suggested that estrogen response contributes to its occurrence. METHODS: Data were collected from Gene Expression Omnibus (GEO). Enrichment scores were calculated by single-sample gene set enrichment analysis (GSEA), while pathway analysis was performed using GSEA. Key modules were identified through weighted gene co-expression network analysis (WGCNA), followed by the screening of differentially expressed gen

Gonadotropins in Mini-Puberty: Pathophysiological and Therapeutic Implications for Male Congenital Hypogonadism.

Background: Mini-puberty is a transient but critical postnatal activation of the hypothalamic-pituitary-gonadal axis, essential for male gonadal maturation, penile and testicular growth, and future reproductive potential: this physiological hormonal surge is absent or blunted in congenital hypogonadotropic hypogonadism (CHH), often manifesting as micropenis, cryptorchidism, and impaired Sertoli cell proliferation. Objective: The aim of this review is to summarize current evidence on the impact of early gonadotropin therapy in male infants with CHH. Methods: We conducted a comprehensive literat

Genetic Association of RXFP2 T222P Variant With Cryptorchidism: A Meta-Analytic Study.

BACKGROUND: The RXFP2 receptor plays a key role in testicular descent, mediating the action of relaxin on gubernaculum development. The T222P polymorphism in the RXFP2 gene has previously been investigated in relation to cryptorchidism, but reported associations have been inconsistent across populations. OBJECTIVES: The aim of this meta-analysis was to assess the association of the T222P variant in the RXFP2 gene with the risk of cryptorchidism and to analyze effect-modifying factors, including population structure and allele frequencies in control groups. MATERIALS AND METHODS: A systematic

Management of andrological disorders from childhood and adolescence to transition age: guidelines from the Italian Society of Andrology and Sexual Medicine (SIAMS) in collaboration with the Italian Society for Pediatric Endocrinology and Diabetology (SIEDP) - part-2.

PURPOSE: Andrological pathologies in adulthood often originate from conditions arising during childhood, adolescence, or even as early as the gestational and neonatal periods. Despite their clinical relevance, pediatric andrological disorders remain underrepresented in the literature and no shared position statements or comprehensive guidelines are currently available. The present paper complements a previous publication by the same societies, entitled “Management of andrological disorders from infancy to adolescence and transitional age: Part-1 – The SIAMS–SIEDP position statement”, by furthe

Safety of acetaminophen use in pregnancy: review of existing evidence.

Acetaminophen is the most widely used antipyretic and analgesic medication in pregnancy, but recent concerns have challenged its reproductive safety. Review articles and regulatory advisories have highlighted associations between prenatal acetaminophen exposure and adverse neurodevelopmental outcomes, including autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD); reproductive issues; and endocrine abnormalities. This review examines the biological plausibility, epidemiological evidence, and methodological limitations underlying observed associations. Systematic r

ZMYND11 p.Arg600Trp variant associated with a distinctive neurodevelopmental phenotype.

Zinc finger MYND-type containing 11 (ZMYND11)-related neurodevelopmental disorder is an autosomal dominant condition caused by pathogenic variants in ZMYND11. Most previously reported patients harbor loss-of-function (LoF) variants, whereas missense variants are rare and their clinical and mechanistic characteristics remain insufficiently defined. Here we report a patient with a heterozygous ZMYND11 c.1798C>T, p.(Arg600Trp) variant identified through the Initiative on Rare and Undiagnosed Diseases program. Detailed clinical evaluation, developmental assessment and whole-exome sequencing were p

Comprehensively exploring the roles of MAGEA1 in cryptorchidism and its related transcription factor regulatory network.

BACKGROUND: Cryptorchidism is caused by a combination of environmental and genetic variables. This study aimed to identify cryptorchidism-associated susceptibility genes through bioinformatics approaches and validate their dynamic expression patterns in rat models, with the exploration of upstream transcriptional factor (TF) regulatory networks. METHODS: Three cryptorchidism-related datasets were obtained from the online GEO database. According to the connectivity degrees in the PPI network, the hub genes were identified. Moreover, a surgical cryptorchidism rat model was successfully establis

[A case report of Diets-Jongmans syndrome caused by a KDM3B gene variant].

A 5-year-3-month-old boy presented with intellectual disability, autism spectrum disorder, short stature, long ears, large auricles, a broad nasal tip, a pointed chin, and cryptorchidism. Whole-exome sequencing revealed a de novo likely pathogenic heterozygous missense variant in KDM3B (c.5147T>C, p.Leu1716Pro), supporting a diagnosis of Diets‑Jongmans syndrome. This case further enriches the mutation spectrum of Diets‑Jongmans syndrome and suggests that early genetic testing helps clarify the etiology in children with developmental delay or intellectual disability, autism spectrum disorder, a
