[A CASE REPORT OF A METACHRONOUS BILATERAL TESTICULAR TUMOR AFTER LEFT UNDESCENDED TESTIS SURGERY].

A 53-year-old man underwent testicular fixation for a left undescended testicle at the age of 10 and a right high orchiectomy for a testicular tumor at the age of 35. He presented to our hospital with awareness of swelling in the left scrotal contents for the past month. Scrotal ultrasonography revealed a left testicular tumor, and contrast-enhanced abdominal CT showed a heterogeneous mass in the left scrotum with infiltration of the scrotal skin, as well as pathologically enlarged left inguinal and para-aortic lymph nodes. The pathology confirmed seminoma. The patient was treated with four co

Could miRNAs be used as markers for distinguishing undescended testes from retractile testes? A prospective controlled pilot/exploratory study.

BACKGROUND: Undescended testes (UDTs) are common in male infants. Untreated UDT poses risks such as infertility (IF), testicular cancer (TC), and testicular torsion (TT). Retractile testes (RTs) sporadically ascend from the scrotum. UDT requires early surgical correction, whereas RT requires only periodic follow-up. Differentiating these conditions is challenging, making clinical biomarkers potentially useful. The aim of our study was to examine the use of miRNAs as biomarkers in the differential diagnosis of UDT and RT which are difficult to differentiate. METHODS: This prospective pilot/exp

Optimizing Care for Undescended Testicles in Children and Adolescents-Diagnosis, Management, and Outcomes: A Narrative Review of Current Evidence.

Cryptorchidism is the most prevalent congenital anomaly of the male genitourinary tract, with an incidence of approximately 1 to 9 percent in full-term male infants, decreasing with age due to spontaneous descent. It encompasses testes that fail to descend into the scrotum, which may be intra-abdominal, inguinal, or ectopic, and can be associated with syndromic, genetic, or environmental factors. The descent process occurs in two phases: intra-abdominal, driven by gubernacular development and androgen-independent mechanisms, and inguinoscrotal, regulated by hormonal and mechanical factors incl

Accuracy of Referral for Undescended Testes in Paediatric Patients Referred to Specialist Paediatric Surgical Centres: A Multicentre Prospective Cohort Study.

AIM: Undescended testes (UDT) are a common congenital disorder in boys, yet delays in diagnosis and referral still occur, especially in low-and middle-income countries where access to specialist care may vary. Our study aimed to examine the accuracy and timing of referrals of UDT to paediatric surgical services across different healthcare settings and to evaluate knowledge regarding UDT among paediatric and family medicine trainees. METHOD: A prospective cohort study was conducted from January to December 2022 involving patients referred for UDT at three paediatric surgical centres representi

The potential role of dynein heavy chain genes in the pathophysiology of undescended testis.

BACKGROUND: Failures of testicular descent may result in the condition known as an undescended testis (UDT). While mechanical and hormonal causes have been researched quite extensively, not much is known about the underlying genetics. Developmental processes dependent on ciliary function may involve outer dynein arm heavy chain genes. Its role in testicular descent has never been explored. This study attempts to investigate the potential role of Outer Dynein Arm Heavy Chain (ODNAH) genes in UDT. METHODS: 24 male juvenile UDT patients and 24 age-matched controls undergoing circumcision were in

Transverse testicular ectopia with fused vas deferens: A systematic review.

BACKGROUND: Transverse testicular ectopia (TTE) with fused vas deferens is an extremely rare anomaly, often diagnosed intraoperatively. Current TTE classifications do not address internal ductal variations, limiting surgical guidance. OBJECTIVE: To systematically review cases of TTE with fused vas deferens, summarize presentation, operative strategies, outcomes and identify patterns that highlight the need for classification refinement. METHODS: A PRISMA 2020-compliant systematic review (PROSPERO; CRD420251247785) was performed across PubMed, ScienceDirect and citation of included articles t

Comparison of Microvascular Flow Imaging With Other Doppler Methods in Ultrasonography for Assessment of Parenchymal Perfusion in Undescended Testis in Young Children.

BACKGROUND: Cryptorchidism is a common congenital anomaly in male children, where accurate assessment of testicular vascularity is important for imaging evaluation and characterization of intratesticular blood flow. Conventional Doppler techniques have limitations in detecting low-velocity microvascular flow, particularly in small pediatric testes. These limitations may affect the ability to adequately visualize subtle vascular signals using standard ultrasound techniques. METHODS: This cross-sectional study was conducted at a tertiary care center over six months and included 40 children aged

Follicle-Stimulating Hormone to Inhibin B Ratio Among Primary Infertile Men With Low Testosterone-A Biochemical Marker of Testicular Reserve.

INTRODUCTION: In adult males, Inhibin B (InhB) production depends on follicle-stimulating hormone (FSH)-which stimulates InhB as part of a negative feedback loop on the pituitary gland-and on spermatogenic activity, which reflects the functional state of the seminiferous epithelium. We aimed to (i) investigate the role of the FSH/InhB ratio in a homogeneous cohort of primary infertile men with low testosterone levels and (ii) evaluate its reliability as a marker of impairment severity. METHODS: Data from 1568 consecutive primary infertile men, defined according to World Health Organization (W

Seminiferous Tubules-Targeted Resveratrol Carbon Dots With Sertoli Cell Biomimetic Vesicles Fusion for Improving Spermatogenesis of Cryptorchidism.

Impaired blood-testis barrier (BTB) integrity, induced by oxidative stress, is one of the primary factors leading to spermatogenic disorders in cryptorchidism. Therefore, the targeted elimination of testicular oxidative stress and repair of the BTB are promising strategies for promoting spermatogenesis in patients with cryptorchidism. This study aimed to demonstrate the potential of Sertoli cell membrane-camouflaged biomimetic vesicles loaded with resveratrol carbon dots (RCD@PCy@EVs) as a therapeutic nanoagent for cryptorchidism treatment in vivo. The prepared RCD@PCy@EVs are actively targete

Posterior bladder-associated pelvic cystic lesion with histologic features consistent with mesenteric cyst in an infant with a functionally solitary kidney: a diagnostic uncertainty case report.

BACKGROUND: Posterior bladder-associated pelvic cystic lesions in male infants are diagnostically challenging, particularly when congenital anomalies of the kidney and urinary tract coexist. Deep pelvic cystic lesions may mimic mesenteric cyst, bladder duplication, bladder diverticulum, ectopic ureter-related pathology, seminal vesicle cyst, or Zinner-spectrum malformation. CASE PRESENTATION: We report a male infant born at 39 weeks' gestation with severe right kidney atrophy, a functionally solitary left kidney with hydroureteronephrosis, a posterior bladder-associated pelvic cystic lesion,

Influence of Epididymal Anomalies and Development of Processus Vaginalis on Testicular Migration And Cryptorchidism. A Narrative Review.

The objective of this narrative review is to show the most relevant aspects of the structure of epidydimal anomalies (EAs) and processus vaginalis (PV) during testicular migration and in patients with undescended testis. Smooth and striated muscles develop around the processus vaginalis (PV) within the gubernaculum during fetal life. The testis migrates through the PV by the means of peristaltic activity generated by those muscles. After migration is completed, the smooth muscle undergoes programmed cell death, resulting in obliteration of PV and leaving only the dartos muscle. Initiation of p

Genetic landscape of a neonatal hypogonadotropic hypogonadism series: Novel variants and phenotypic spectrum.

To describe clinical presentation and genetic findings in a cohort of infants with congenital hypogonadotropic hypogonadism (CHH) diagnosed before 2 years of age. From a large cohort of patients who underwent next-generation sequencing (NGS) for CHH between 2019 and 2025, we identified all patients tested at ≤2 years of age. Genetic analysis consisted of a targeted CHH gene panel, followed by a CPHD-NGS panel. In one remaining patient with neurodevelopmental defects, whole-exome sequencing was performed. Phenotypic data were collected through the GENHYPOPIT registry. Twelve male infants (1.5%

How I do it: "3-In-1" single incision bilateral scrotal orchiopexy and penile webbing repair with circumcision.

BACKGROUND: Cryptorchidism and penoscrotal webbing are common pediatric urologic conditions that may coexist and require surgical correction. For palpable low-lying undescended testes, the scrotal approach to orchiopexy is well established and associated with excellent cosmetic outcomes and reduced operative time. In children undergoing circumcision, concurrent correction of penoscrotal webbing is important to avoid ventral penile skin deficiency and sinking of the penis in the suprapubic fat pad. We present a novel single-incision technique combining bilateral scrotal orchiopexy and penoscrot

Congenital aphallia: From embryology to management - a focused minireview.

Aphallia, or penile agenesis, is an extremely rare congenital anomaly with fewer than 100 cases reported worldwide. It results from failed development of the genital tubercle during early embryogenesis, leading to complete absence of penile structures and abnormal urethral opening. This defect is often part of a broader caudal developmental disturbance and is frequently associated with genitourinary anomalies such as urethrorectal fistula, renal agenesis, renal dysplasia, hydronephrosis, and cryptorchidism, all of which significantly influence prognosis. Diagnosis requires careful differentiat

Anogenital distance as a cross‑species biomarker of fetal androgen action: Origins, translation, and implications for reproductive toxicology.

Anogenital distance (AGD) is a dose-responsive biomarker of disrupted fetal androgen signalling. Because AGD remains relatively stable after birth, its measurement provides a retrospective indicator of fetal androgen action and has proven valuable in toxicity research, particularly for identifying chemical exposures with anti-androgenic properties. Although initially described as an anatomical trait for sexing neonatal animals, rodent toxicity studies in the 1990s established AGD as an androgen-sensitive endpoint following in utero exposure. Importantly, reduced AGD was consistently associated
