Extending the Graft Harvesting Area in Severe Penoscrotal Hypospadias: The Rainbow Technique.

INTRODUCTION: Proximal hypospadias presents a significant challenge for urethral reconstruction due to the extent of the defect requiring repair. Staged repair with grafting is employed to address this challenge (1). Buccal mucosa and post-auricular skin grafts have been described, but prepuce tends to have lower contraction and failure rates (2, 3). However, the conventional rectangular preputial graft is often of insufficient length for larger defects. In this video, we introduce a novel rainbow technique for preputial skin harvesting in cases of severe proximal hypospadias where an extended

Enhancing the Structural Integrity of 3D Bioprinted Urethral Grafts for Clinical Applications using GelMA-Fibrinogen Bioinks Infused with Human Primary Cells.

The demand for organ transplantation continues to grow, since the shortage of organ donors and complications associated with existing treatments remain. A promising alternative to transplantation is the implantation of artificial organs, which can be produced using 3D bioprinting technology. This approach offers the advantage of patient-specific solutions tailored to individual needs. Patients with urethral stricture or hypospadias who are unable to urinate require urethral reconstruction, which carries risks of complications and stricture recurrence. These patients may benefit from a tissue-e

Evaluation of the Clinical Usefulness of a Bioartificial Urethral Substitute Generated by Tissue Engineering in Laboratory Rabbits: Protocol for an In Vivo Study.

BACKGROUND: Surgical management of diseases compromising the integrity of the human urethra remains a major clinical challenge, and congenital conditions, such as hypospadias and epispadias, and acquired diseases, such as trauma, tumors, and strictures, are difficult to treat, highlighting the need for novel regenerative approaches capable of restoring urethral architecture and function. Recent advances in tissue engineering have allowed the generation of bioartificial tissues, including several models of artificial urethra generated with different types of cells and biomaterials. Although pot

Meier-GORLIN Syndrome Caused by Compound Heterozygous CDC6 Variants.

Meier-Gorlin syndrome (MGORS) is a rare primordial dwarfism syndrome characterized by the triad of pre- and postnatal growth restriction, microtia, and patellar aplasia or hypoplasia. Pathogenic variants in genes involved in DNA replication licensing and initiation have been implicated in MGORS, including the uncommon disease-associated gene CDC6, for which only two affected families have been reported to date. We report a male infant born at 37 weeks' gestation to a 40-year-old primigravida via cesarean delivery due to intrauterine growth restriction and fetal intolerance of labor. He present

Pregnancy outcome after first-trimester exposure to JAK inhibitors - an evaluation of the German Embryotox cohort.

BACKGROUND: Janus kinase inhibitors (JAKi) are increasingly being used in the management of autoimmune diseases across rheumatology, gastroenterology, and dermatology, including in women of childbearing age. Consequently, pregnancies occurring during JAKi treatment are becoming more common. However, uncertainties regarding their safety during pregnancy remain, and current human data are insufficient to allow a comprehensive assessment of fetal risk. METHODS: Observational data of pregnancies with first trimester JAKi exposure, recorded in the German Embryotox database, was analyzed. Pregnanci

Hypospadias surgery in Sub-Saharan Africa: A systematic review of anatomical assessment and outcomes.

BACKGROUND: Hypospadias is a common congenital anomaly whose surgical management aims to restore both functional and anatomical outcomes. Although numerous studies have been conducted in Sub-Saharan Africa, surgical practices, anatomical assessment, and outcomes remain poorly synthesized. This study aimed to systematically review the literature on the anatomical assessment, management and outcomes of hypospadias in Sub-Saharan Africa. METHODS: A systematic review of studies published between 2000 and 2025 was conducted following PRISMA guidelines. Studies reporting on hypospadias management i

Genotype-phenotype correlation in partial combined 17α-hydroxylase/17,20-lyase deficiency: functional characterization of CYP17A1 variants in ten 46,XY patients.

BACKGROUND: Partial combined 17α-hydroxylase/17,20-lyase deficiency (PC-17OHD) is a rare 17OHD subtype with residual 17α-hydroxylase and 17,20-lyase activities. In 46,XY individuals, genital virilization is highly variable, and genotype-phenotype correlations remain poorly defined. OBJECTIVE: To characterize the clinical and endocrine spectrum of 46,XY PC-17OHD, functionally validate uncharacterized CYP17A1 variants, and establish genotype-phenotype correlations. METHODS: We enrolled ten 46,XY PC-17OHD patients from 9 families. Clinical phenotypes were characterized, and long-read CYP17A1 se

Changes in birth certificate-reported prevalence of congenital anomalies at live birth during the COVID-19 pandemic: a nationwide study of 26 million United States live births.

BACKGROUND: Although the majority of studies have indicated that maternal severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and coronavirus disease 2019 (COVID-19) vaccination are not associated with neonatal birth defects, it is relevant to examine whether the disease burden changed during the COVID-19 pandemic, as other influencing factors may have also changed. We aimed to compare the birth certificate-reported prevalence of congenital anomalies at live birth before and during the pandemic. METHODS: Using public birth data, all live births from 2016 to 2022 in the Unit

Early diagnosis and resection of Müllerian duct remnants: a strategy to improve outcomes in hypospadias repair.

This retrospective study evaluated whether early diagnosis and resection of Müllerian duct remnants (MDRs) improve outcomes in hypospadias repair. We analyzed patients with hypospadias and coexisting MDRs treated at The First Affiliated Hospital of Sun Yat-sen University (Guangzhou, China) from August 2003 and June 2024. Patients were divided into a study group (n = 22, MDR resection before urethroplasty) and a control group (n = 40, no MDR resection before urethroplasty). MDRs were diagnosed using voiding cystourethrography and cystoscopy. Compared with the control group, the study group had

Clinical characteristics and genotype-phenotype correlation analysis in 279 patients with 5α-reductase type 2 deficiency.

OBJECTIVE: 5α-Reductase type 2 deficiency (5α-RD2) is an autosomal recessive differences/disorders of sex development caused by SRD5A2 gene mutations, characterized by impaired testosterone-to-dihydrotestosterone conversion and highly heterogeneous clinical phenotypes. The genotype-phenotype correlation of 5α-RD2 remains incompletely understood, and large-scale multicenter evidence is lacking for clinical management. METHODS: We systematically searched PubMed, Embase, CNKI, Wanfang, and Chinese Medical Journals databases from inception to December 2025. A total of 279 genetically confirmed pa

Father and son with a pathogenic variant c.614dup p.(Gln206Thrfs*20) in the NR5A1 gene: a case report.

BACKGROUND: Steroidogenic factor 1 (SF-1), encoded by the NR5A1 gene, is a critical transcriptional regulator of adrenal and gonadal development. Pathogenic NR5A1 variants lead to a broad phenotypic spectrum characterized by insufficient virilization, including gonadal and testicular dysgenesis, ambiguous genitalia, hypospadias, micropenis, cryptorchidism, anorchia, and male infertility. This case report characterizes the clinical and genetic findings in a boy and his father with differences of sex development (DSD) caused by a heterozygous NR5A1 frameshift variant, thereby expanding our under

Effect of Massage with Topical Corticosteroids on Recurrent Curvature in Staged Repair of Proximal Hypospadias: A Retrospective Study.

OBJECTIVES: Ventral penile curvature often accompanies severe hypospadias and poses a risk of recurrence after repair. We postulated that altered myofibroblastic activity at the repair site contributes to recurrent curvature. To address this mechanism, we adopted a specific clinical strategy in 2017. We instructed patients to apply upward massage to the ventral side of the penis using a topical corticosteroid ointment between the two stages of Bracka repair. The aim of this study was to assess the impact of this intervention on the recurrence of curvature after surgery. METHODS: A retrospecti

Morphological and immunohistochemical analysis of unilateral testicular hypoplasia in an XY calf: correlation with the timing of bovine hypospadias.

Hypospadias is a common disorder of sex development (DSD), resulting from the impaired formation of the penile urethra. In mammals, penile urethral development depends on androgens secreted during a limited time window immediately after the onset of testis development. In this study, we performed morphological and immunohistochemical analyses of a 15-day-old genetically XY Holstein calf with perineal hypospadias to characterize a unique phenotype providing critical insights into its pathogenic mechanisms. Although the penis and prostate were grossly developed, the penile urethra was not formed
