Father and son with a pathogenic variant c.614dup p.(Gln206Thrfs*20) in the NR5A1 gene: a case report.

BACKGROUND: Steroidogenic factor 1 (SF-1), encoded by the NR5A1 gene, is a critical transcriptional regulator of adrenal and gonadal development. Pathogenic NR5A1 variants lead to a broad phenotypic spectrum characterized by insufficient virilization, including gonadal and testicular dysgenesis, ambiguous genitalia, hypospadias, micropenis, cryptorchidism, anorchia, and male infertility. This case report characterizes the clinical and genetic findings in a boy and his father with differences of sex development (DSD) caused by a heterozygous NR5A1 frameshift variant, thereby expanding our under

Molecular analysis of individuals with suspected 46,XY differences of sex development in a homogenous and understudied population.

Differences of sex development (DSD) are a group of rare congenital conditions defined by atypical chromosomal, gonadal, and/or hormonal sex. Despite advances in massively parallel sequencing (MPS), more than half of DSD cases have an unknown genetic aetiology. We recruited and analysed 21 individuals with 46,XY DSD from the Greater Middle East population using chromosomal microarray and whole exome sequencing. Participants had DSD ranging from micropenis to anorchia (absence of testes) with extra-genital features reported in four individuals (19%). Using a combination of microarray and WES, a

The Impact of Congenital Anomalies of the Male and Female Reproductive Organs on Infertility and Recurrent Pregnancy Loss: A Review.

Congenital anomalies of the reproductive system represent a heterogeneous group of structural and functional abnormalities affecting both male and female genital organs. These anomalies typically arise during embryogenesis and may remain asymptomatic until they are incidentally identified during evaluation for infertility, recurrent pregnancy loss, or disorders of sexual development. In females, abnormalities include Müllerian duct anomalies and congenital malformations of the uterus, cervix, vagina, and ovaries, such as Mayer-Rokitansky-Küster-Hauser syndrome, septate, unicornuate, bicornuate

The Aesthetic Form of Perezhivanie: An Ethnographic Study of Becoming an Environmental Scholar-Activist.

End-stage renal disease is closely associated with changes in bone and mineral metabolism. In recent times, osteoporosis has become important among hemodialysis (HD) patients. In this study, the investigators sought to evaluate the relationship between bone mineral density (BMD) and biochemical markers of bone turnover among HD patients. A total of 70 uremic patients on a maintenance HD program for at least 1 y were enrolled in the study. All patients were treated with conventional bicarbonated HD for 5 h through the use of low-flux hollow-fiber dialyzers. Bone densitometry was measured by dua

Prior internal carotid artery embolization to optimize oncological and surgical outcomes in salvage endoscopic nasopharyngectomy for recurrent nasopharyngeal carcinoma.

UNLABELLED: BACKGROUND Salvage endoscopic nasopharyngectomy (SEN) for recurrent nasopharyngeal carcinoma (rNPC) with internal carotid artery (ICA) involvement carries prohibitive risks of catastrophic hemorrhage and incomplete resection. This study aims to evaluate the efficacy and safety of preoperative ICA embolization. METHODS: Between January 2006 and December 2022, 535 patients who underwent SEN for rNPC were retrospectively screened. Among 140 patients with radiological ICA involvement, 114 eligible patients were identified after predefined exclusions. Propensity score matching (1:1) yi

And Then There Was One: Elective Induction and Contralateral Septopexy for Management of Prenatal Testicular Torsion.

Prenatal testicular torsion (PTT) is very rare and poses a unique challenge as the contralateral testis cannot be easily monitored in utero. Delays to induction or delivery could increase the risk of testicular loss and anorchia due to contralateral torsion. We present a case of unilateral PTT in a near-term fetus managed with elective induction who underwent orchiectomy of the ipsilateral testis and septopexy of the contralateral testis, with a specific focus on multispecialty discussion weighing the maternal-fetal risk of early delivery with the risk of contralateral torsion.

Inverted V-I MeatoGlanuloplasty (IV-IMG): Twenty years' experience with a new technique for distal hypospadias repair.

PURPOSE: No Urethroplasty Techniques (NUTs) are technically simple, low-morbidity alternatives to urethroplasty for the correction of distal hypospadias. Along with functional outcomes, cosmetic results have become increasingly relevant in recent years. Despite the high number of techniques, some drawbacks remain in achieving these goals. The aim of this paper is to describe the Inverted V-I MeatoGlanuloplasty technique, developed at our center for the treatment of distal hypospadias and to evaluate its outcomes. MATERIALS AD METHODS: A retrospective, single-center study was conducted on pati

AOP report: Adverse outcome pathway network for developmental androgen signaling inhibition leading to hypospadias.

This report summarizes an adverse outcome pathway network (AOPN) describing how reduced androgen signaling during fetal life may lead to hypospadias in male offspring. Hypospadias is a penile malformation caused by disrupted masculinization of the genital tubercle. Under normal physiological conditions, androgen action promotes differentiation of the genital tubercle to a penis in male fetuses. Suboptimal androgen receptor signaling can impair genital tubercle development, causing hypospadias in humans and other mammals. In rodent toxicity studies, including test guideline studies from the Org

Fertility-Preserving Laparoscopic Spermatic Cord Elongation in Bilateral Intraabdominal Testes: A Case of Fused Spermatic Cords.

INTRODUCTION: The optimal surgical approach for bilateral intraabdominal testes remains controversial. The Shehata technique has shown promising results as a fertility-preserving method by elongating the gonadal vessels without vessel division. MATERIALS AND METHODS: We present a case of bilateral intraabdominal testes treated with simultaneous bilateral Shehata technique. SURGICAL TECHNIQUE: At 9 months of age, bilateral testes were identified laparoscopically. After dissection of the spermatic cords, a one-stage bilateral orchidopexy was attempted. Due to significant tension in the testicu

Clinical characterization and analysis of the SRD5A2 gene in 16 Chinese patients with 5α-reductase type 2 deficiency: a single-center retrospective study.

BACKGROUND: 5α-reductase type 2 deficiency(5α-RD2) is a rare autosomal recessive disorder primarily caused by variants in the SRD5A2 gene. This condition is characterized by a spectrum of undervirilization in 46, XY individuals, ranging from ambiguous genitalia to predominantly male external genitalia with minimal undervirilization. Early diagnosis and effective management of 5α-reductase type 2 deficiency patients are major challenges for healthcare providers. The mutational analysis of SRD5A2 gene is regarded as the gold standard for the diagnosis of the disease. This study aimed to summariz

Could miRNAs be used as markers for distinguishing undescended testes from retractile testes? A prospective controlled pilot/exploratory study.

BACKGROUND: Undescended testes (UDTs) are common in male infants. Untreated UDT poses risks such as infertility (IF), testicular cancer (TC), and testicular torsion (TT). Retractile testes (RTs) sporadically ascend from the scrotum. UDT requires early surgical correction, whereas RT requires only periodic follow-up. Differentiating these conditions is challenging, making clinical biomarkers potentially useful. The aim of our study was to examine the use of miRNAs as biomarkers in the differential diagnosis of UDT and RT which are difficult to differentiate. METHODS: This prospective pilot/exp

Ectopic Adrenocortical Cancer Originating From the Pancreas: A Case Report With Literature Review.

Malignancies arising from adrenal rests are extremely rare. They may occur in locations such as the retroperitoneum, testes/scrotum, ovaries, kidneys, anterior abdominal wall, spinal cord, and liver, in association with embryological development. In this article, we present a patient with ectopic adrenocortical cancer (ACC) located in the pancreas, along with a review of the relevant literature. A 57-year-old female patient was diagnosed with mixed-type breast cancer (lobular and ductal carcinoma). During further diagnostic evaluations, in addition to a malignant-appearing mass in the left bre

Congenital anterior urethrocutaneous fistula.

Congenital anterior urethrocutaneous fistula (CAUF) is an uncommon penile malformation characterized by the urethra's connection to the skin on the ventral aspect of the penis. It can occur alone or with genitourinary or anorectal anomalies. Due to the scarcity of reported cases, treatment strategies vary, prompting a systematic review to evaluate their clinical presentation, surgical outcomes, and recurrence rates. A systematic review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, searching PubMed, ScienceDirect, EBSCO, Cochrane, and

Laparoscopic transabdominal preperitoneal repair for atypical inguinal hernia associated with adult anorchia: a case report.

Adult anorchia complicated by inguinal hernia is rare, and its influence on intraoperative anatomy has been insufficiently described. We report a 54-year-old man with left inguinal hernia and congenital absence of testes who underwent laparoscopic transabdominal preperitoneal (TAPP) repair. Preoperative imaging demonstrated a lateral hernia orifice without identifiable spermatic cord structures. Laparoscopic findings revealed absence of normal vas deferens and testicular vessels, resulting in an indistinct internal inguinal ring and atypical inguinal anatomy. Standard anatomical landmarks used

Laterality and Breed Distribution of Cryptorchidism in 251 Dogs: A Retrospective Clinical Study.

Cryptorchidism is one of the most frequently diagnosed developmental disorders of the male canine reproductive system, defined as the failure of one or both testes to descend into the scrotum. Physiologically, testicular descent is typically completed by six to eight weeks of age, although some authors extend this period to sixteen weeks. Failure of testicular descent beyond this timeframe is considered pathological. The condition has multiple causes and affects between 1% and 10% of the canine population. Genetics is the most significant factor, indicating the hereditary basis of cryptorchidi
