Two 46,XY agonadal siblings with variable degrees of sexual ambiguity are described. The eldest child is a phenotypic male with micropenis. The younger patient, a phenotypic female with slight fusion of the genital folds and absent müllerian ducts, conforms to the criteria usually accepted for the diagnosis of true agonadism. Coexistence of anorchia and true agonadism in the same sibship supports the hypothesis, suggested by others, that both disorders are related and are due to the regression of the embryonic testes.

The pharmacodynamics of plasma testosterone (T) and androstenedione (A) levels were studied in ten hypogonadal boys after oral administration of testosterone undecanoate (TU). Plasma T and A levels were measured by specific radioimmunoassays. An abdominal extraperitoneal approach is described for use in patients who have a severe degree of incomplete descent of the testicle. In 86 such operations, the testicle was placed in the scrotum in 85. A patient with Beckwitt-Wiedemann's syndrome is described in which associated malformations were hemihypertrophy of the body, medullary sponge kidneys and absence of testes and internal ducts. We examined the capacity of reaction in the endocrine tissue of testes in prepubertal boys. 18 boys were endocrinologically healthy, six suffered from cryptorchidism on one or both sides. After a single dose of HCG (5000 IU/m) we found an increase of pregnantriol excretion in the 24 h urine in all subjects studied except in the case of surgically proved anorchia. In all except one boy we found a sharp increase in testosterone after HCG administration.
