[Chinese expert consensus on the diagnosis and treatment of pneumoconiosis complicated with tuberculosis].

Pneumoconiosis complicated with pulmonary tuberculosis is characterized by high prevalence and disability rates, as well as difficulty in early diagnosis, constituting a serious public health problem. The Chinese Society of Tuberculosis (Chinese Medical Association) and the Society of Labor Hygiene and Occupational Diseases (Chinese Preventive Medicine Association) organized multidisciplinary experts in respiratory diseases, occupational diseases, tuberculosis and other related fields to formulate the Chinese expert consensus on the diagnosis and treatment of pneumoconiosis complicated with tu

Do separate bronchial brushing smears provide additional diagnostic information in bronchoscopic evaluation of mycobacterial infection? A retrospective paired study.

BACKGROUND: Tuberculosis and nontuberculous mycobacterial infections remain important public health concerns. Although acid-fast bacillus (AFB) smear microscopy is widely used because of its rapid turnaround time, the incremental diagnostic value of separate bronchial brushing smears during bronchoscopy remains unclear. We evaluated whether separate brushing smears provide additional information beyond routinely processed bronchoscopic specimens. METHODS: This single-center retrospective study included 43 patients aged ≥20 years who underwent bronchoscopy at Shimane University Hospital betwee

Development and validation of a blood-based diagnostic model for pulmonary tuberculosis combining GBP5 expression and routine laboratory indicators.

WHO reports that only 54% of tuberculosis (TB) patients received rapid diagnostic tests at their initial presentation. Conventional laboratory methods in TB detection have high specificity (>90%) but low sensitivity (<50%). This means a large number of tuberculosis patients are missed. A better diagnostic approach is essential to decrease the TB burden. Studies on multi-indicator blood-based models for rapid TB diagnosis remain limited. We developed a rapid, non-sputum-based model to improve the efficiency of TB diagnosis. This was a retrospective case-control study including 301 patients with

Cross-Reactivity of GeneXpert MTB/RIF Probe C with Nocardia Species as a Suggestive Ancillary Indicator.

BACKGROUND: The slow-growing, fastidious nature of Nocardia species makes rapid molecular tools essential for diagnosis. Although the GeneXpert MTB/RIF assay targets Mycobacterium tuberculosis (MTB), the cross-reactivity of its Probe C with Nocardia has not previously been characterized. METHODS: We retrospectively analyzed 14,141 non-duplicate clinical samples from February 2020 to June 2023. Probe C reactivity was first assessed against Nocardia culture results. To investigate the molecular basis, we subsequently compared the rpoB target region of Probe C with the genomes of 1082 Nocardia s

Ultrasound-Guided Needle Aspiration Biopsy for Superficial Cervical Lymphadenopathy: Diagnostic Yield Beyond Tuberculosis.

Background: Superficial cervical lymphadenopathy frequently requires tissue sampling because infectious, inflammatory, and malignant diseases often share overlapping clinical and radiological features. Although ultrasound-guided sampling of superficial cervical lymph nodes is well established, the available literature predominantly focuses on fine-needle aspiration cytology (FNAC) or core needle biopsy (CNB). Comparatively less data specifically describe aspiration-based tissue acquisition providing material for combined histopathological, microbiological, and molecular analyses, particularly

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Intraoperative hypotension and postoperative pneumonia in patients after selective intracranial tumor resection: a retrospective cohort study.

BACKGROUND: Postoperative pneumonia is common and associated with increased postoperative mortality. Intraoperative hypotension is suggested to be associated with an increased risk of postoperative surgical infection. We aim to explore whether intraoperative hypotension could increase the risk of postoperative pneumonia in adult patients undergoing intracranial tumor resection. METHODS: A total of 341 patients who received selective intracranial tumor resection under general anesthesia between January 2018, and December 2022 in a single university hospital were reviewed. Univariate and multiv

Active tuberculosis in household contacts of bacteriologically confirmed pulmonary tuberculosis patients: A multicenter study finding the 'Missed One' in Central Ethiopia.

BACKGROUND: There was a 'missing millions' gap between the incidence of tuberculosis (TB) cases and the notified cases. In many TB high-burden countries, only about 25% of household contacts (HHCs) completing household TB evaluation and 20-89% of eligible contacts did not adhere to TB screening. The study was conducted to assess the yield of door-to-door TB household contact investigation among household contact of bacteriologically confirmed pulmonary TB cases in central Ethiopia. METHODS: This cross-sectional study was carried out in selected health facilities of central Ethiopia from Janua

Challenges in Diagnosis and Management of Atlantoaxial Tuberculosis: A Case Report.

Background and Objectives: Atlantoaxial tuberculosis (TB) is rare, and its diagnosis is difficult. Herein, we present a rare case with a challenging diagnostic journey of atlantoaxial TB spanning over two years. Materials and Methods: A 70-year-old immunocompetent female patient presented with a four-week history of nuchal pain, stiffness, and headache. She did not have any TB-associated constitutional symptoms. The result of the initial biopsy indicated only a nonfermenting Gram-negative bacillus and the histopathological report revealed concurrent acute and chronic inflammation. Posterior fu

Cell-free DNA next-generation sequencing for Mycobacterium tuberculosis obtained from plasma of children with active tuberculosis.

BACKGROUND: Difficulties in microbiologically confirming childhood tuberculosis (TB) can result in delayed treatment and increased disease severity. METHODS: In this study, we for the first time used whole genome next-generation sequencing (NGS) to detect cell-free DNA (cfDNA) from Mycobacterium tuberculosis (MTB) in plasma from children. RESULTS: We enrolled 94 children with active TB and 32 children with other respiratory infections. Combining NGS with probe capture enrichment (targeted cfNGS) showed higher coverage and detecting capability than did NGS alone. The targeted cfNGS showed sli

A novel method for detecting Lipoarabinomannan in urine with the promise of meeting the WHO target product profile for the diagnosis of tuberculosis.

The diagnosis of tuberculosis largely relies on the detection of Mycobacterium tuberculosis (M. tuberculosis) via pathogen-specific DNA or bacterial culture from sputum samples. As the only point-of-care test so far, urinary lipoarabinomannan (LAM) has been endorsed by the World Health Organization for the diagnosis of tuberculosis in people living with HIV. In this study, the electrochemiluminescence LAM research assay (EclLAM) was used to measure LAM in the urine of HIV-sero-negative individuals with pulmonary tuberculosis and to monitor anti-tuberculosis treatment. Urine samples from 18 pat

Comparative Analysis of Culture and Sputum Smear Conversion Timelines and Their Associated Factors in Smokers Versus Non-smokers With Drug-Resistant Tuberculosis.

Background Drug-resistant tuberculosis (DR-TB) is a global health challenge, with smoking potentially affecting treatment outcomes. Smoking compromises immune function and may interfere with the pharmacokinetics of anti-TB drugs. Delayed sputum smear and culture conversion are key indicators of prolonged treatment and infectiousness. This study explores the impact of smoking on these conversion timelines in DR-TB patients. Objective To identify and assess the overall treatment outcomes and the factors associated with delayed culture and sputum smear conversion in smokers compared to non-smoker

Evaluation of techniques for post-mortem diagnosis of Mycobacterium tuberculosis complex infection in goats.

Goats are highly susceptible to Mycobacterium tuberculosis complex (MTBC) infection and can play an important role in the epidemiology of animal tuberculosis (TB) in certain epidemiological scenarios. Historically, culture has been considered the only gold standard technique for post-mortem confirmation of MTBC infection. However, it is constrained by its low sensitivity, the slow growth of MTBC, and stringent biosecurity requirements. Thus, alternative post-mortem techniques are essential for effective TB control and eradication. This study aimed to compare the use of different diagnosis tech

Usefulness of the GeneXpert MTB/RIF Ct for predicting tuberculosis infectiousness.

INTRODUCTION: The GeneXpert® MTB/RIF test is faster and more sensitive than smear microscopy (SM) for the diagnosis of pulmonary tuberculosis (pTB). We evaluated the contribution of the Ct ("Cycle threshold") of the GeneXpert MTB/RIF test to estimate infectiousness. METHODS: We performed a multicenter retrospective analysis of patients with pTB confirmed by positivity of culture and GeneXpert. The median Ct was calculated on Day 0 (D0). We studied the following correlations: between the Ct and SM on D0, Ct and Time-To-Detection (TTD) of the culture in liquid medium on D0 and then the correlat

Clustering and risk factor analysis of pulmonary tuberculosis in a district in Ethiopia: a population-based cohort study.

OBJECTIVE: A 'cluster' is an area with a higher occurrence of tuberculosis (TB) than would be expected in an average random distribution of that area. Tuberculosis clustering is commonly reported in Ethiopia, but most studies rely on registered data, which may miss patients who do not visit health facilities or those who attend but are not identified as having TB. This makes the detection of actual clusters challenging. This study analysed the clustering of pulmonary TB and associated risk factors using symptom-based population screening in Dale, Ethiopia. DESIGN: A prospective population-bas

Impact of vitamin C deficiency on imaging patterns and ventilatory function in pulmonary tuberculosis.

BACKGROUND: Studies have shown that vitamin C is essential for the immune response to tuberculosis (TB), and that its deficiency may elevate the risk of TB and related complications. This prospective study investigated the association between disease severity, imaging findings and vitamin C levels. METHODS: This study enrolled 109 patients with confirmed pulmonary tuberculosis based on Mycobacterium tuberculosis culture. Patients were divided into two groups based on serum vitamin C levels: 59 patients (54.13%) with normal levels and 50 (45.87%) with low levels. RESULTS: At baseline, patient
