[Value of determining the activity of adenosine deaminase (ADA) in the differential diagnosis of pleural effusions].

BACKGROUND: Pleural effusion (PE) is a common problem in medical practice. Transudative effusions usually do not present difficulty in diagnosis, in most populations. Exudative PE however, require careful differential diagnosis that include necessarily tuberculosis and metastatic cancer. METHODS: We have conducted a cross-sectional study of 221 patients with persistent PE, in order to evaluate accuracy of determining adenosine deaminase activity (ADA) for diagnosis of tuberculosis pleuritis. This group of patients, taken mostly from the State Hospital for pulmonary diseases, consecutively, wa

[Tuberculosis in Greenland 1994. A minor outbreak of new cases of tuberculosis in the village of Kullorsuaq].

An outbreak of pulmonary tuberculosis was detected in 1994 in a small village, Kullorsuaq, located 300 kilometres north of Upernavik in the north-western part of Greenland. The inhabitants of Kullorsuaq mainly live off subsistence, hunting, seal-hunting and fishing. Too many people were living in bad housing and with a general lack of sanitation facilities. There had been a few serious cases of pulmonary tuberculosis in 1993, and when three new cases of tuberculosis in school children were diagnosed in April 1994, the authorities arranged for all 292 inhabitants of Kullorsuaq to be examined by

[The incidence of tuberculosis in Romania in 1994].

The 1994 TB notification rate structure in Romania was based on the data reported in "New-case/relapse TB notification form", registered between 1.01.-31.12.1994 and stored in "National TB-Register" data base. The 21,422 TB patients notified in 1994 correspond to an annual incidence of 94.2%000, which means an increase of 5.41% as compared with 1993, of 34.6% as compared to 1990 and of 68.8% as compared to 1985. As compared with 1985 tb notifications, this increase supplied a commulative overload of 32,546 patients, out of which, 18,233 only in the last 5 years. Tb notification rate show large

Non-tuberculous pulmonary infections in Scotland: a cluster in Lothian?

BACKGROUND: A retrospective study was carried out to confirm the clinical impression that, in Lothian, non-tuberculous mycobacterial infections are as common as pulmonary tuberculosis. METHODS: All pulmonary isolates of Mycobacterium tuberculosis/bovis and non-tuberculous mycobacteria in Scotland from April 1990 to March 1993, and the notes of all patients with M malmoense isolates in Lothian, were reviewed. Information on mycobacterial culture procedures in Scottish laboratories was obtained as part of an audit project. RESULTS: Of all pulmonary isolates of mycobacteria in Lothian 53% (108/

Mycobacterium xenopi infection masquerading as pulmonary tuberculosis in two patients infected with the human immunodeficiency virus.

Mycobacterium xenopi infections have rarely been reported among patients infected with the human immunodeficiency virus (HIV). We recently treated two HIV-infected men, neither of whom had a history of pulmonary disease or AIDS-defining conditions, and who had M. xenopi lung infections. Both patients presented with night sweats, cough, and pleuritic chest pain. Chest radiographs showed an upper-lobe nodule in the first patient and a perihilar cavitary infiltrate in the second patient. Both patients were initially believed to have pulmonary tuberculosis and were treated accordingly; however, on

Identification of Mycobacterium intracellulare by a polymerase chain reaction using species-specific primers.

SETTING: The polymerase chain reaction (PCR) is a rapid and specific method used to amplify a certain DNA fragment. It is applicable to rapid diagnosis of mycobacterial infections. By use of species-specific primers, it is possible to identify mycobacteria by PCR. In this study, a newly constructed primer was tested for specificity for Mycobacterium intracellulare in the PCR. OBJECTIVE: M. intracellulare is one of the most frequently found bacteria in opportunistic infection in AIDS, and rapid identification of this species is important. The purpose of this study was to construct a primer spe

[Primary gastric tuberculosis].

Intestinal tuberculosis (TB) comprises 5% of all cases of TB and may be a major problem in immigrant communities, although the incidence of pulmonary TB is declining. Gastric TB is rare, constituting 0.1-2% of all cases of TB. Gastric TB usually develops secondary to other tuberculous lesions, most commonly pulmonary. On endoscopy antral infiltrative lesions are found. Primary gastric TB is very rare, only 8 cases having been reported in the English literature. We report a case of primary gastric TB in a 55-year-old woman who presented with abdominal pain and gastric outlet obstruction. The di

Tracheobronchial involvement in 84 cases of pulmonary tuberculosis.

In suspected pulmonary tuberculosis (TB), tracheobronchial examination is required for patients with clinical and radiographic features consistent with tuberculosis disease but with negative sputum for Mycobacterium tuberculosis. We report the endoscopic findings of 84 patients who underwent fibreoptic bronchoscopy in recent years, and whose cultures of biological specimens grew M tuberculosis. Cough (86%) and fever (69%) were the most common symptoms, followed by sputum (67%), dyspnoea (30%) and haemoptysis (27%). Chest radiographic abnormalities were mostly localized (60%) with prevalence in

Improved outcomes for patients with multidrug-resistant tuberculosis.

We conducted a retrospective study of patients with culture-confirmed multidrug-resistant tuberculosis (MDR-TB) at Bronx-Lebanon Hospital Center (South Bronx, NY) to determine what factors affected clinical and microbiological responses and survival. For the 38 patients with MDR-TB, reporting of first-line drug susceptibilities was relatively rapid (median time, 30 days). Thirty-four patients (89%) were infected with human immunodeficiency virus (HIV), and initial and overall response rates were 59% and 50%, respectively; the median survival was 315 days; and 50% of these patients died of tube

[Reappearance of tuberculosis, risks and consequences!].

WHO estimates that worldwide cases of TB will amount to 8.8 million in 1995 and 11.9 million in 2005. Compared with the estimated amount of TB cases of 7.5 million in 1990 there is a rise by 16.3 percent for 1995 and by 57.6 per cent for 2005. Treatment costs for TB are relatively low. If properly administered the treatment is effective in more than 95 percent of all cases. The mortality for TB estimated worldwide in 1992 and related to a population of 100,000 confirms that South East Asia and Subsaharian Africa show the highest mortality. The existing statistical data for Germany give an indi

Value of the lateral chest radiograph in tuberculosis in children.

The value of the lateral chest radiograph, often considered a useful adjunct in the detection of hilar adenopathy, was evaluated in a prospective study of 449 children assessed for tuberculosis. Of these children 298 presented to the hospital with signs and symptoms suggestive of tuberculosis, while 151 were investigated in a regional clinic solely because they were in close contact with an adult household member on treatment for tuberculosis. Tuberculosis was confirmed by culture in 176 of the 449 children (39%). In 40 of these (23%) hilar adenopathy was visible on frontal and lateral view, i

Tuberculosis case-finding in Western Australia.

The records of a total of 497 patients notified to have active tuberculosis (TB) in Western Australia from 1986 to 1990 inclusive have been reviewed in relation to case-finding. Of these, 276 (55.5%) were Asian migrants, 92 (18.5%) Europeans and 110 (22.1%) Australians. The disease was confirmed bacteriologically in 247 (49.7%). The most common mode of diagnosis was symptomatic presentation (39.8%), followed by migrant health surveillance (36.8%), incidental finding (7.5%), regular follow-up (3.8%) and pre-employment chest radiography (1.8%). Prevalence of human immunodeficiency virus (HIV) antibodies in tuberculosis patients in Lagos, Nigeria.

To establish the prevalence of HIV antibodies in patients with pulmonary tuberculosis, 536 new cases presenting with symptoms of bronchopulmonary disorders were randomly selected from the six referral chest clinics in Lagos and screened for tuberculosis and HIV infections. Sputum and serum samples were obtained from all the patients. The sputum samples were examined for acid-fast bacilli (AFB) by both microscopy and culture. The sera were screened for HIV-1 and HIV-2 antibodies by ELISA and confirmed by Western blot (WB). Of the 536 cases studied, 188 (35%) were positive for AFB while 13 (2.4%

Takayasu's arteritis in Mexico: a clinical review of 44 consecutive cases.

OBJECTIVES: To review our experience in clinical diagnosis of the non-specific arteritis called Takayasu's arteritis (TA), and to assess its possible relationship with previous mycobacterial infections as judged by delayed hypersensitivity skin test. METHODS: We examined 44 consecutive patients in Mexico City. All of them fulfilled the ACR criteria for the classification of TA and had a characteristic panaortogram in the absence of any other arterial or systemic disease. RESULTS: Forty-three of our patients were Mexican mestizos; only one had a Caucasian appearance. 38 were women, and the ag

[Should the presence of Mycobacterium tuberculosis be demonstrated before the treatment of patients with suspected pulmonary tuberculosis?].

In Denmark, treatment of tuberculosis is generally only recommended if the diagnosis is confirmed bacteriologically, which may cause a delay in treatment. We investigated the duration of the treatment delay, and if the delay would cause any serious health problems for the individual or risk of contact infections by a retrospective examination of 324 cases of pulmonary tuberculosis. The mean treatment delay was longer in older age groups. Regarding death due to delay, we found no risk for the patients who are not weakened by old age or another disease. Only 11 (3.6%) above the age of ten years

[Have the incidence and resistance of tuberculosis increased? Epidemiologic follow-up based on diseases confirmed by culture].

The unexpected increase in the incidence of tuberculosis in many industrialized countries, which was recorded in Germany for the first time in 1992, provided the incentive for this retrospective analysis over the past 13 years of the epidemiology and resistance situation for Tbc in the Rhine/Main area. Basis for the evaluation were the data from our laboratory which is the only institution in the region performing type and resistance determinations on mycobacteria. During the period of the investigation, in which 3,738 tuberculoses were confirmed in cultures, the number of cases of the disease

[Incompatibility of tuberculosis therapy in a patient with AIDS].

A man with advanced HIV infection (CD4 lymphocytes 90/microliter, CD4/CD8 ratio 0.2) was admitted to hospital with fever, cough and weight loss. The radiological and bronchoscopic findings, together with the presence of acid-fast bacilli in the sputum, pointed to open pulmonary tuberculosis caused by Mycobacterium tuberculosis, a diagnosis confirmed by histological examination and culture. Quadruple antibiotic therapy with isoniazid (INH), rifampicin (RMP), ethambutol (EMB) and amikacin was immediately begun and was at first clinically successful.
