Triglyceride-glucose index as a cardiovascular risk marker: updated evidence, limitations, and future perspectives.

Cardiovascular diseases (CVDs) remain the foremost cause of morbidity and mortality worldwide, accounting for a substantial burden on healthcare systems, economies, and society. Traditional risk factors do not fully explain residual cardiovascular risk, highlighting the role of insulin resistance (IR) as a key pathophysiological link between metabolic disorders and CVD. The triglyceride-glucose (TyG) index, a simple surrogate marker derived from fasting triglyceride and glucose levels, has emerged as a practical tool for assessing IR and cardiovascular risk. This review summarizes current evid

A postpartum woman with recurrent headaches and transient neurological deficits: a case report.

BACKGROUND: Reversible cerebral vasoconstriction syndrome (RCVS) is an under-recognized clinical entity characterized by transient segmental vasoconstriction of cerebral arteries. It commonly presents with thunderclap headaches and can mimic other acute neurological conditions, often delaying diagnosis. Known risk factors include the female sex, postpartum state and exposure to vasoactive substances. CASE PRESENTATION: We report the case of a 26-year-old, white woman presenting ten days postpartum with severe, recurrent headaches. Her medical history included opioid maintenance therapy and pa

Physical Performance Moderates the Association Between Age and Executive Function in Older Adults at Elevated Risk of Dementia: Cross-Sectional Analysis From the CITA GO-ON Trial.

BACKGROUND: Preservation of executive function is vital for independence and dementia prevention, yet the physical performance levels at which age-related executive vulnerability becomes detectable remain uncertain. This study examined whether gait speed, handgrip strength and leg muscle power influence the relationship between age and executive function in older adults at increased dementia risk and assessed how sex and white matter hyperintensity (WMH) burden affect this relationship. METHODS: Cross-sectional analysis of 920 community-dwelling older adults (aged 60-85) at high dementia risk

The impact of P-Element-induced hybrid dysgenesis on the male germline in Drosophila simulans.

The P-element, a DNA transposon, has independently invaded two Drosophila species, accompanied by rapid evolution of suppression. In the germline, suppression is mediated primarily by maternally expressed piRNAs, a class of regulatory small RNAs associated with PIWI proteins. The offspring of females that lack P-element-specific piRNAs and males that contain P-elements suffer a syndrome of deleterious phenotypes, including sterility, genome rearrangements, gonadal atrophy, and mutations, while the offspring of the reciprocal cross are normal. These effects, collectively termed hybrid dysgenesi

Diagnostic and Clinical Outcomes of Sex Chromosome Abnormalities Detected on Noninvasive Prenatal Screening.

OBJECTIVE: Noninvasive prenatal screening (NIPS) screens for aneuploidy, but its positive predictive value for sex chromosome aneuploidies (SCA) is variable. NIPS reports include "atypical sex chromosome findings," which may indicate fetal/maternal SCAs or mosaicism, although sensitivity is unknown. Previous studies are limited by small cohorts and insufficient maternal testing and ultrasound data. This study evaluates confirmation rates and outcomes for screen-positive SCAs, including "atypical sex chromosome" on NIPS. STUDY DESIGN: This retrospective study included singleton pregnancies tha

Clinical Validation of the Behavioral Evaluation Scale of Frontotemporal Dementia: A Pilot Study.

OBJECTIVE: Besides cognitive deficits, dementias are characterized by behavioral symptoms, hampering efforts to distinguish between different types of dementia, such as Alzheimer's disease (AD) and behavioral variant frontotemporal dementia (bvFTD). Moreover, instruments adapted to the local sociocultural context are lacking in non-English-speaking countries. The authors developed a scoring instrument (the Behavioral Evaluation Scale of Frontotemporal Dementia [BES-FTD]) for use in the Latin American context to assess behavioral changes among patients with bvFTD and to test its accuracy to dif

Sexually-dimorphic offspring outcomes following antenatal corticosteroids: a review of the evidence for congenital adrenal hyperplasia and preterm birth risk.

BACKGROUND: Pregnant individuals may receive antenatal corticosteroids for various conditions, including congenital adrenal hyperplasia (to prevent virilisation in female fetuses) and preterm birth risk (to hasten fetal maturation and improve perinatal survival). This narrative review aimed to evaluate the evidence of sexually-dimorphic offspring outcomes following antenatal corticosteroids for such conditions and explore the potential underlying mechanisms. METHODS: A comprehensive MEDLINE search identified twenty-eight studies for inclusion: seven focused on congenital adrenal hyperplasia a

Parent-child alienation, peer alienation, and sleep disturbance in Chinese college students: a longitudinal study of bidirectional relationships and gender differences.

BACKGROUND: Sleep disturbance is highly prevalent among college students and poses significant risks to mental health. However, the longitudinal relationships among parent-child alienation, peer alienation, and sleep disturbance remain poorly understood, particularly regarding gender differences. METHODS: A three-wave longitudinal study involving 4,852 Chinese college students was conducted. The Random Intercept Cross-Lagged Panel Model (RI-CLPM) was used to examine bidirectional associations and mediating effects, and multigroup analyses were conducted to assess gender differences. RESULTS:

Sex chromosome dosage effects on human brain structure and function: a systematic review of neuroimaging findings in sex chromosome aneuploidies.

Sex chromosomal aneuploidies (SCAs) are a group of genetic disorders characterized by an abnormal number of X or Y chromosomes compared to the typical karyotype of 46,XX in females and 46,XY in males. In the last three decades, there has been an increased interest in the neurobiological effects in SCAs, including effects on brain structure and function. The current work aimed to provide a comprehensive overview of results of neuroimaging studies in SCAs to summarize sex chromosome dosage effects on human brain structure and function. We included 48 studies, and results revealed a complex patte

Prenatal exposure to hypertensive disorders and adolescent brain white matter development.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) are associated with altered in utero neurodevelopment, yet long-term impact on adolescent white matter architecture remains unclear, particularly by HDP subtype and sex. While preeclampsia/eclampsia (P/E) is clinically more severe, pregnancy-related hypertension (PR-HTN) may involve longer in-utero exposure to hemodynamic stress. This study examined how distinct HDP exposures influence longitudinal white matter measures in adolescents. METHODS: Using a longitudinal cohort with two MRI timepoints (baseline ages 9-11 years; 2-year follow-up

Two-Year Cumulative Risk of Mental Health Disorders After Pediatric Orthopaedic Surgery: A Retrospective Cohort Study Comparing Procedure Types.

BACKGROUND: Surgery can be psychologically challenging for pediatric patients and their families, yet the impact of pediatric orthopaedic surgery on the future development of mental health disorders (MHDs) is incompletely understood. The purpose of this study was to determine whether the rate of the 2-year cumulative risk of common MHDs differed by the type of pediatric orthopaedic procedure. METHODS: All patients aged ≤18 years undergoing pediatric orthopaedic surgery at a single academic medical center over a 10-year period were retrospectively identified by Current Procedural Terminology c

Familial 46,XY disorder of sex development due to a novel CYB5A variant: a rare cause of isolated 17,20-lyase deficiency.

Primary amenorrhea may be the first clinical presentation of an underlying 46,XY disorder of sex development (DSD). We report 2 siblings assigned female at birth; the index case presented with primary amenorrhea and the younger sibling was identified through family screening. Both had female external genitalia with minimal virilization, absent Mullerian structures, and testes located in the inguinal/labioscrotal region on imaging. Hormonal evaluation showed impaired androgen synthesis with very low testosterone and markedly elevated 17-hydroxyprogesterone:androstenedione ratios, suggestive of
