Plasma proteomic signatures in bipolar disorder and their implications for data-driven patient stratification.

Bipolar disorder (BD) is a heterogeneous mood disorder that lacks robust biomarkers for biological stratification. We used high-throughput plasma proteomics to characterize case-control differences, identify proteomic subgroups within BD, and examine their associations with blood-based markers and clinical features. Plasma samples from the French I-GIVE (Immuno-Genetics, Inflammation, retroVirus, Environment) cohort, including 248 participants with BD and 76 healthy controls, were assessed using the Olink Reveal panel (1,034 proteins). Age-, sex-, and BMI-adjusted case-control analyses identif

Clinical characteristics and genotype-phenotype correlation analysis in 279 patients with 5α-reductase type 2 deficiency.

OBJECTIVE: 5α-Reductase type 2 deficiency (5α-RD2) is an autosomal recessive differences/disorders of sex development caused by SRD5A2 gene mutations, characterized by impaired testosterone-to-dihydrotestosterone conversion and highly heterogeneous clinical phenotypes. The genotype-phenotype correlation of 5α-RD2 remains incompletely understood, and large-scale multicenter evidence is lacking for clinical management. METHODS: We systematically searched PubMed, Embase, CNKI, Wanfang, and Chinese Medical Journals databases from inception to December 2025. A total of 279 genetically confirmed pa

Gender differences in the association between anxiety and behavioral problems in 31,400 Chinese children: a cross-sectional regression and network analysis.

BACKGROUND: Child anxiety and behavioral problems are significant global public health concerns. Traditional methods often fail to capture intricate symptom interactions. This study used network analysis to explore the relationship between anxiety and behavioral problems, focusing on gender differences in a large sample of Chinese children. METHODS: Data from 31,400 students (aged 9-16) were analyzed. Anxiety and behavioral problems were assessed using SCARED and PSQ. To control for developmental variance, symptom domains were residualized against chronological age prior to network estimation

Non-pharmacological executive function interventions for children and adolescents with neurodevelopmental disorders: a systematic review and network meta-analysis.

BACKGROUND: Children and adolescents with neurodevelopmental disorders (NDDs) frequently experience executive dysfunction. Although non-pharmacological interventions are commonly recommended to address these challenges, their relative effectiveness across intervention types and executive-function domains remain unclear. We aimed to systematically compare the effectiveness of non-pharmacological interventions for improving overall and domain-specific task-based executive function outcomes in children and adolescents with NDDs. METHODS: We conducted a systematic review, pairwise meta-analysis,

Testosterone and DHEA-S Secreting Adrenal Cortical Adenoma in a Postmenopausal Woman: A Rare Functional Incidentaloma.

Postmenopausal hyperandrogenism is uncommon and should prompt evaluation for an underlying androgen-secreting source. Although ovarian causes are more frequent, adrenal tumors remain an important consideration and may occasionally be malignant. We report a 53-year-old postmenopausal woman who presented with progressive hirsutism, scalp hair loss, and mild clitoromegaly. Laboratory evaluation showed markedly elevated total testosterone and dehydroepiandrosterone sulfate levels with suppressed gonadotropins. Pelvic ultrasonography showed no ovarian mass. CT demonstrated a 3.5 × 2.4 cm left adren

Early-life adversity alters the adult blood proteome primarily in female mice.

BACKGROUND: Early-life adversity (ELA) is one of the strongest predictors of poor mental and physical health later in life, increasing an individual's risk for disorders ranging from depression to metabolic syndrome. However, whether and how ELA shapes immune memory through adulthood, subsequently affecting susceptibility to immune-associated diseases such as autoimmunity, remains elusive. METHODS: To address this question, we employed a translationally and ethologically relevant mouse model of resource deprivation, the limited bedding and nesting model, to study the long-term effects of ELA

Predicting PANSS symptoms in schizophrenia spectrum disorders using speech only: an international, multi-centre, computational secondary analysis across multiple languages.

BACKGROUND: Speech carries cues to variation in mental state in schizophrenia, typically indexed with clinician-rated scales such as Positive and Negative Syndrome Scale (PANSS). In this study, we aggregate multi-centre recordings to assemble a large corpus and assess symptom-prediction models at scale, to enable rater-independent, efficient, and reproducible assessments for early detection of relapse-related signals from speech. METHODS: In this computational secondary analysis, we compiled data from 453 patients with schizophrenia spectrum disorders, recruited between 2005 and 2026 from ten

Vitamin D receptor gene variants and their association with clinical phenotypes of multiple sclerosis.

Multiple sclerosis (MS) is a complex neuroinflammatory disorder shaped by environmental and genetic factors. Vitamin D, a well-known environmental factor associated with the disease, exerts its immunoregulatory effects mainly through vitamin D receptor (VDR). Single nucleotide variants (SNVs) within the VDR gene may modify receptor function. Although previous studies have examined VDR SNVs in patients with MS, results have been inconsistent. In addition, their involvement in disease phenotypic variability remains poorly understood. The study aimed to examine potential correlation between FokI

Sex differences in responses to neurosteroid therapy after preterm birth.

Progesterone production by the placenta leads to elevated levels of neurosteroids in both the maternal and fetal circulations during late gestation. Allopregnanolone levels are crucial because they provide both neuroprotective and trophic support in the fetal brain. These include protecting against excessive excitation and supporting oligodendrocyte development and myelination. Pregnancy compromises and particularly premature birth reduce or totally deprive the newborn brain of this neurosteroid mediated protection and support. The premature fall in allopregnanolone levels may increase suscept

Sex and age effects on binge eating spectrum disorders in young adults: a path analysis.

BACKGROUND AND AIMS: Binge spectrum eating disorders (BSED) are heterogeneous conditions characterized by recurrent binge eating and associated features. While demographic factors are known to influence eating disorder expression, the specific contributions of sex and age remain unclear. This study examined the impact of sex and age on the clinical presentation of BSED in young adults and explored the underlying pathways using path analysis. METHOD: The sample comprised 187 young adults (119 women, 68 men; 20-45 years) diagnosed with bulimia nervosa (BN, N = 41), binge eating disorder (BED, N

Association of Night-Time Stabilization Splint Use With Condylar and Posterior Eminence Remodeling in Adolescents With Unilateral Temporomandibular Joint Osteoarthritis: A CBCT-Based Retrospective Study.

BACKGROUND: Temporomandibular joint osteoarthritis (TMJ OA) in adolescents is clinically important because condylar degeneration during growth may adversely affect mandibular development. However, three-dimensional evidence regarding the effect of stabilization splints on joint remodelling remains limited. METHODS: This retrospective observational study included adolescents with unilateral TMJ OA and baseline condylar erosion who underwent baseline and follow-up cone-beam computed tomography over approximately two years. Patients were classified into splint and no-splint groups. Changes in co

Comparison of Clinical, Metabolic, and Hormonal Profiles in Obese Versus Lean Women With Polycystic Ovary Syndrome: A Systematic Review and Meta-Analysis.

INTRODUCTION: Obesity substantially influences clinical and metabolic manifestations of polycystic ovary syndrome (PCOS). However, differences in clinical, metabolic, and hormonal profiles between obese and lean women with PCOS have not been explored in large-scale meta-analyses. METHODS: Databases were searched from inception through 31 December 2025 for studies reporting clinical, metabolic, and hormonal profiles in obese and lean PCOS phenotypes. Random-effects meta-analyses in R yielded mean differences (MDs) or risk ratios (RRs) with 95% confidence intervals. RESULTS: Forty-two studies

EARLY-ALS: a multicentre study on prodromal amyotrophic lateral sclerosis.

BACKGROUND: Neurodegenerative diseases often feature a prolonged phase of subtle changes preceding diagnosis. In amyotrophic lateral sclerosis (ALS), defining this prodromal period is critical for identifying early disease features and intervention windows. METHODS: In this case-control study, 475 patients with ALS and 285 controls from 20 centres in Germany and Switzerland completed a digital questionnaire on prodromal symptoms and health-related factors. Logistic regression included subgroup analyses by sex and clinical strata. FINDINGS: Patients with ALS reported a higher burden of prodro

Efficacy of letrozole combined with recombinant human growth hormone in boys aged ≥10 years with advanced bone age and impaired growth potential: a retrospective cohort study.

INTRODUCTION: In boys aged ≥10 years with advanced bone age and impaired growth potential (baseline height SDS < -1.5, BA-CA ≥1.5 years, and PAH < -2 SDS), growth hormone monotherapy, while promoting linear growth, fails to delay skeletal maturation, thus limiting therapeutic efficacy. Although letrozole combined with growth hormone may theoretically "decouple" height gain from epiphyseal closure, high-quality evidence specific to the advanced bone age population remains insufficient, and concerns regarding baseline bias and lack of long-term safety data persist. This study, based on real-worl

Real-world river water organic pollutant mixture induces ovarian hyperandrogenism via aryl hydrocarbon receptor activation.

Humans are exposed to complex mixtures of endocrine-disrupting chemicals (EDCs) in the real world. Most prior studies, however, focus on individual EDCs. Here, we investigated the ovarian impact of organic pollutant mixtures extracted from the Raritan River, a heavily industrialized watershed in central New Jersey, using a three-dimensional (3D) in vitro ovarian follicle culture system. Ultra-high-performance liquid chromatography-high-resolution mass spectrometry (UHPLC-HRMS) identified organic pollutant mixtures dominated by alkylphenols, pharmaceuticals, per- and polyfluoroalkyl substances

A Qualitative Study of Parental Decision-Making Following a Prenatal Diagnosis of Turner Syndrome: "How Serious Is This?".

A prenatal diagnosis of Turner syndrome (TS) presents expectant parents with complex and emotionally demanding decisions under conditions of uncertainty. However, there is limited insight into how parents interpret and integrate medical information, personal experiences, and values during this process. This qualitative study explores how parents who chose to continue a pregnancy following a prenatal TS diagnosis experienced and navigated decision-making. The study was conducted within the Danish healthcare system between September 2024 and October 2025. During this period, we conducted 12 semi
