BACKGROUND: Progress in nephrology is still constrained by the limited ability of conventional experimental models to faithfully recapitulate the complex structure and function of the human kidney. Human pluripotent stem cell-derived kidney organoids provide a three-dimensional in vitro experimental system that allows investigation of kidney development and disease-related mechanisms in a human cellular context, although still limited in their ability to reproduce the full structural and physiological complexity of the native kidney.
SUMMARY: Kidney organoids have emerged as a powerful system

Autosomal dominant polycystic kidney disease (ADPKD) is marked by the progressive development of bilateral kidney cysts, leading to compression of intact surrounding tissue and a subsequent decline in kidney function. Accumulation of ATP within the cyst fluid significantly drives cyst expansion by activating purinergic receptors, thereby promoting calcium-dependent chloride secretion. Moreover, the induction of hypoxia-inducible factor 1α (HIF-1α) in cyst-lining epithelial cells further enhances chloride conductance. However, the mechanisms underlying ATP release into the cyst fluid and the ro

INTRODUCTION: We have previously reported that overweight and obesity are independently associated with more rapid disease progression in adults with autosomal dominant polycystic kidney disease (ADPKD). In this study, we perform a cross-sectional analysis to evaluate whether overweight and obesity are also associated with renal blood flow (RBF) in this patient population.
METHODS: A total of 134 non-diabetic adults with ADPKD and estimated glomerular filtration rate (eGFR) ≥60 mL/min/1.73 m2 who participated in a randomized controlled trial of pravastatin therapy were categorized as normal w

The endocannabinoid system is a complex regulatory network whose functioning is associated with maintaining homeostasis and regulating immune response. The aim of this study was to evaluate the relationship between endocannabinoid system activity, inflammation, and the progression of chronic kidney disease (CKD) in patients with autosomal dominant polycystic kidney disease (ADPKD). The study included 105 participants: 60 individuals with ADPKD and 45 healthy volunteers. From a single venous blood draw, concentrations of anandamide (AEA), 2-arachidonoylglycerol (2-AG), tumor necrosis factor α (

OBJECTIVE: Inherited kidney diseases (IKDs) significantly contribute to chronic kidney disease (CKD), particularly in regions with high consanguinity rates. Despite the increasing availability of next-generation sequencing, evidence regarding its diagnostic and clinical effectiveness in adult populations remains limited. This study evaluated the diagnostic yield and clinical implications of genetic testing in adults with suspected IKD.
MATERIALS AND METHODS: A retrospective analysis of 63 adults who underwent genetic evaluation for unexplained or familial CKD was performed. Genetic testing us

BACKGROUND AND AIMS: Congenital anomalies of the kidney and urinary tract (CAKUT) are a group of disorders responsible for the majority of pediatric end-stage renal disease cases. The CAKUT group is phenotypically variable and can affect the kidney alone and/or the lower urinary tract. The aim of the study was to assess the risk factors for the onset of chronic kidney disease (CKD) and the progression to kidney failure (KF, i.e., the need for dialysis or kidney transplantation) in a cohort of patients with CAKUT.
METHOD: We conducted a longitudinal study at the Nephrology, Dialysis, and Trans

Autosomal dominant polycystic kidney disease (ADPKD) is one of the most common monogenic genetic disorders, caused by mutations in receptor PKD1 or ion channel PKD2, and is characterized by progressive renal cyst development with additional hepatic and extrarenal manifestations. As effective treatments for ADPKD remain limited, further investigation into the function and regulation of PKD proteins is needed. Using biotin-based proximity labeling combined with mass spectrometry in human embryonic kidney (HEK) cells, here we identify Ras association domain family member 4 (RASSF4) as a potential

Autosomal recessive polycystic kidney disease (ARPKD) is a pediatric genetic nephropathy caused by mutations in PKHD1, which encodes fibrocystin. The cellular basis and epithelial dynamics of cyst formation remain incompletely understood. We used lineage-tracing systems in rat and mouse to define epithelial behavior during cystogenesis. Clonal labeling revealed that renal and biliary epithelial cells undergo marked expansion as cysts form. Genetic mosaic analysis showed that individual Pkhd1-deficient cholangiocytes can generate millimeter-scale cysts within one year. Mathematical modeling dem

Kidney diseases, whether acute or chronic, remain among the most challenging conditions to diagnose due to their complex and often asymptomatic progression. Emerging biomarkers such as neutrophil gelatinase-associated lipocalin (NGAL) and cystatin C (CysC) correlate with early pathogenesis and offer complementary insights into disease etiology. However, quantification of their levels, which is essential for patient staging, is available only in centralized laboratories. We present the lateral-flow microfluidic bead-based assay (LMBA), the first rapid, self-contained system for NGAL and CysC an

Takayasu arteritis (TA) is a rare large-vessel vasculitis, typically affecting young females. We report a young Indian male with resistant hypertension and bilateral renal artery stenosis, who developed extensive collateral and accessory renal arteries as adaptive vascular remodelling in the setting of chronic, inactive large-vessel vasculitis, with preserved renal function due to extensive collateralisation. This constellation of findings is rare, particularly in males. The case underscores the role of CT angiography in delineating complex vascular remodelling in chronic TA and the importance

INTRODUCTION: Tolvaptan, a vasopressin V2 receptor (V2R) inverse agonist, is the only treatment specifically approved for autosomal dominant polycystic kidney disease (ADPKD). In addition to inhibiting cAMP synthesis, it has other effects on V2R signaling. Our goal was to mechanistically characterize the effects of MQ232, an optimized peptide of the green mamba snake and a new V2R inverse agonist, in Pkd1RC/RC mice.
METHODS: Pkd1RC/RC mice were treated with MQ232 (0.5, 2, 4, or 8 nmols/kg/hour via minipump), tolvaptan (0.2% in chow), or vehicle from four to 16 weeks of age. MRI measurements o

BACKGROUND AND PURPOSE: Autosomal dominant polycystic kidney disease (ADPKD) is a leading cause of end-stage renal disease with limited treatment options. Drug repurposing offers a promising strategy to find effective treatments.
EXPERIMENTAL APPROACH: We identified birinapant, bardoxolone methyl and salicylic acid as repurposing candidates for ADPKD and applied thermal proteome profiling to identify disease-relevant targets.
KEY RESULTS: Our results uncovered shared effects between the three drugs, including a thermal shift in ADP/ATP translocases (SLC25A4/5/6) caused by reduced ATP product

BACKGROUND: Accurate epidemiological data on autosomal dominant polycystic kidney disease (ADPKD) are essential for healthcare planning and therapeutic strategies. However, nationwide prevalence estimates in Japan have not been updated for decades.
METHODS: This was a nationwide survey conducted in Japan. Departments at hospitals were selected using stratified random sampling based on specialty and hospital size. The number of patients with ADPKD not receiving kidney replacement therapy (KRT), defined in this study as those who did not receive dialysis and did not newly undergo kidney transpl

GM3 synthase deficiency (GM3SD), also known as salt and pepper developmental regression syndrome, is a rare autosomal recessive congenital disorder of glycosylation caused by biallelic pathogenic variants in ST3GAL5. It is characterized by early onset epileptic encephalopathy, severe neurodevelopmental impairment, and sensory deficits. Routine metabolic screening tests are frequently non-specific, which may contribute to delayed diagnosis. We report a Turkish male infant who presented in early infancy with developmental delay and hypotonia, followed by early onset, refractory epilepsy and prog

Metabolic and mitochondrial alterations are central in the pathogenesis of autosomal dominant polycystic kidney disease (ADPKD) since therapies targeting these alterations slow kidney disease progression in orthologous animal models. To investigate metabolic and mitochondrial defects in an animal model orthologous to ADPKD, we used male mice homozygous for a point variant in the GPS cleavage site of the polycystin-1 (Pkd1V/V) and evaluated oxygen consumption, Ca2+ uptake, and redox state in the mitochondrial fraction of Pkd1V/V kidneys and wild-type controls (WT). Our findings revealed mitocho
