CART for ascites accumulation and dialysis difficulty after hepatic cyst fenestration and bilateral nephrectomy in ADPKD: a case report.

Hepatomegaly with hepatic cysts constitutes a common extrarenal manifestation of autosomal dominant polycystic kidney disease (ADPKD), often resulting in mass effects and occasionally necessitating cyst fenestration to alleviate abdominal distension. Nonetheless, persistent cyst fluid leakage as a postoperative complication frequently leads to refractory ascites, for which treatment options are limited. In this report, we present a 62-year-old female patient with ADPKD undergoing maintenance hemodialysis. She received bilateral nephrectomy and hepatic cyst fenestration to mitigate abdominal di

Perioperative complications in native nephrectomy as a therapeutic approach for symptomatic polycystic kidney disease: a comprehensive study.

PURPOSE: To assess perioperative complications after native nephrectomy in autosomal dominant polycystic kidney disease (ADPKD), identify risk factors and evaluate surgical timing and approach. METHODS: We retrospectively analyzed 124 nephrectomy episodes in patients with ADPKD treated at a single tertiary center between October 2012 and October 2022. Complications were graded according to the Clavien-Dindo classification. Multivariable logistic regression was used to assess predictors of overall and major complications. RESULTS: Perioperative complications occurred in 73/124 episodes (58.9%

Successful arteriovenous fistula creation in a chronic kidney disease G5 patient with bilateral ulnar artery absence: A case report.

Hemodialysis access induced distal ischemia (HAIDI) represents a serious complication following arteriovenous fistula (AVF) creation, with increased risk in elderly patients with arterial abnormalities. Bilateral congenital absence of ulnar arteries is an exceptionally rare anatomical variant with optimal AVF creation strategies remain undefined due to the theoretical high risk of HAIDI. We report a female patient in her 80s with chronic kidney disease G5. She had bilateral congenital absence of ulnar arteries and required vascular access creation. Preoperative assessment revealed radial arter

Biallelic NPHP3 Variants Causing Late Onset Chronic Kidney Insufficiency in Nephronophthisis: A Case Report.

Emerging evidence suggests that the overall prevalence of nephronophthisis (NPHP) in adult-onset end-stage kidney disease (ESKD) is very likely underestimated. Thus, a comprehensive understanding of the genetic basis and clinicopathologic features of adult-onset NPHP is essential. In this case, we report a 38-year-old woman who exhibited no growth disorders or urinary abnormalities in prior physical examinations. However, at 38 years of age, she presented with renal dysfunction (eGFR 29.5 mL/min/1.73 m2). Urine tests revealed a low specific gravity of urine, but no proteinuria or microscopic h

Tolvaptan improves the diurnal blood pressure profile in patients with hypertension and autosomal-dominant polycystic kidney disease.

BACKGROUND: Autosomal-dominant polycystic kidney disease (ADPKD) is a genetic disorder frequently associated with non-dipper hypertension, with tolvaptan being the only currently approved treatment. However, the effects of tolvaptan on blood pressure (BP) in patients with ADPKD and hypertension remain unclear. METHODS: This retrospective study examined the effect of tolvaptan on 24-h ambulatory BP monitoring parameters. Changes in ambulatory BP-related parameters, including annual rate of change in total kidney volume (TKV), were assessed in 23 patients with ADPKD and hypertension before and

World's first surgical repair of bilateral common iliac artery agenesis.

We present a rare case of a 23-year-old woman with congenitally absent bilateral common iliac arteries, identified incidentally during a computed tomography scan of the abdomen. Imaging revealed occlusion of the aorta below the renal arteries with flow to the lower limbs provided by hypertrophied collaterals. Despite initial conservative management, the patient experienced worsening claudication and functional limitations, prompting surgical revascularisation. An aorto-bifemoral bypass using an axillary-bifemoral graft was performed with successful recovery - the first documented case of surgi

Aberrant activation of renal functional reserve and reduced tubule flow, but not obstruction, is associated with cystogenesis in Pkd2 mutant kidneys.

Disruption of PKD2 function causes polycystic kidney disease (PKD) and leads to the formation of cysts in the kidney in both mice and humans. PKD is progressive, initiating with the formation of tubule dilations that expand into large cysts, causing the destruction of renal parenchyma and loss of kidney function. The mechanisms initiating tubule dilation and expansion are uncertain. Tubule obstruction has been proposed as a factor contributing to cystogenesis. Here, we utilize a proximal tubule fluorescent dextran uptake assay to evaluate changes in tubule flow that occur during cyst initiatio

Attribute-based cross-classification reveals sex- and age-specific prognostic impact of anemia in ADPKD.

BACKGROUND: Anemia is less prevalent in autosomal dominant polycystic kidney disease (ADPKD) owing to preserved erythropoietin production. However, its impact on kidney prognosis remains unclear. Given sex-related differences in hemoglobin (Hb) levels, we hypothesized that the prognostic relevance of anemia may vary by sex and age. Therefore, we aimed to identify subgroup-specific risk patterns using an attribute-based cross-classification approach to support individualized anemia management in ADPKD. METHODS: We analyzed 552 Japanese patients with ADPKD from a single-center cohort. The prima

Carbon dioxide contrast angiography-and computed tomography-guided percutaneous embolization of the renal allograft artery in a patient with graft intolerance syndrome and iodinated contrast allergy: a case report.

Graft intolerance syndrome (GIS) is a well-recognized complication after kidney allograft failure, characterized by fever, graft pain, and systemic inflammation. Management options include continued immunosuppression, graft nephrectomy, and renal artery embolization, each associated with specific risks. We report a case of GIS successfully treated with percutaneous renal allograft artery embolization using carbon dioxide angiography in a patient with iodinated contrast allergy. A 66-year-old woman with autosomal dominant polycystic kidney disease underwent living-donor kidney transplantation a

Miniaturization of CRISPRa plasmids for efficient delivery into renal epithelial cells and Pkd1 transactivation.

BACKGROUND: Autosomal Dominant Polycystic Kidney Disease is caused by loss-of-function mutations in PKD1 or PKD2 genes, leading to reduced polycystin protein levels. Increasing PKD1 expression via CRISPR activation (CRISPRa) represents a promising therapeutic strategy; however, delivery of large CRISPRa plasmids into renal epithelial cells, and particularly primary cells, remains inefficient due to size-related barriers. We aimed to enable Pkd1 transactivation by miniaturizing CRISPRa plasmids into ~ 6 kb vectors using a one-pot method to enhance cellular uptake in mouse kidney epithelial cell

Monogenic Etiologies of Kidney Cysts in the Pediatric Population: An Observational Cohort Study.

KEY POINTS: In a cohort of children, 79% of families with kidney cysts had a monogenic diagnosis. Resolved cases were more likely to have a family history of kidney cysts and unresolved cases more often had unilateral cysts. Broad genetic testing revealed genetic diversity and informed prognosis and clinical management in childhood kidney cysts. BACKGROUND: Pediatric kidney cysts may indicate an underlying genetic disorder, yet the full spectrum of causes remains incompletely defined. With advances in genetic testing enabling broader evaluation, this study assessed the diagnostic utility of c

Altered Neural Activity and Functional Connectivity of Dorsolateral Prefrontal Cortex Associated With Cognitive Impairment in Patients With End-Stage Renal Disease.

BACKGROUND: Executive dysfunction is the most prominent feature of cognitive impairment in patients with end-stage renal disease (ESRD). The dorsolateral prefrontal cortex (DLPFC) is a central region for the regulation of executive functions. The aim of our study was to examine alterations in neural activity and functional connectivity (FC) of the DLPFC in relation to cognitive assessments and clinical indicators in patients with ESRD using the resting-state functional magnetic resonance imaging (rs-fMRI) technique, and to further predict cognitive-related brain damage in this population. The Kinetics and Mechanism of Niche Filling in Healthy and Cystic Mice.

KEY POINTS: The rate and mechanism of kidney-resident macrophage niche filling differs between healthy and cystic mice. An incompletely filled kidney-resident macrophage niche is associated with worsened polycystic kidney disease outcomes. BACKGROUND: Previous data indicate that bone marrow-derived Ly6c hi monocytes can become tissue-resident macrophages when the niche is available. However, the balance between monocyte recruitment and tissue-resident macrophage self-proliferation following partial depletion is unclear. METHODS: We investigated the kinetics and mechanism of kidney-resident m

A deep intronic IFT172 variant causing pseudoexon inclusion identified by whole-genome sequencing in nephronophthisis.

Nephronophthisis is an autosomal recessive ciliopathy and a major genetic cause of end-stage kidney disease in children and young adults. Although next-generation sequencing panels have improved diagnostic yield, some patients remain genetically unresolved, partly due to deep intronic variants that disrupt pre-mRNA splicing and are not captured by exon-focused approaches. We report a 13-year-old boy who presented with advanced kidney dysfunction, small renal cysts, and kidney histopathology consistent with nephronophthisis. Targeted gene panel sequencing failed to identify causative pathogenic

Designing genome editing experiments with EditABLE.

While many computational tools exist for designing CRISPR-Cas experiments, there is a need for a centralized resource that combines individual tools to predict the most efficient genome editing strategy for a given application. To fill this gap, we develop EditABLE (EditABLE-app.stanford.edu), an online resource that provides optimal CRISPR editors and guide RNAs based on user provided sequence data with functionalities for base editing, prime editing, and integrase-mediated editing. We demonstrate the utility of EditABLE by applying it to one of the most common monogenic disorders, autosomal

The Role of Mitochondria in Polycystic Kidney Disease.

Polycystic kidney disease (PKD) is a genetic disorder characterized by renal cyst formation and progressive renal dysfunction, where inflammation, immune responses, and metabolic dysregulation critically drive disease progression, while emerging evidence increasingly links its pathogenesis to mitochondrial dysfunction. Mitochondria, central to cellular energy production, metabolism, and redox homeostasis, exhibit profound abnormalities in PKD, contributing to disease pathogenesis. Current evidence on mitochondrial mechanisms driving PKD progression includes metabolic reprogramming, oxidative s

Modelling kidney cystogenesis using human kidney tubuloid cultures.

BACKGROUND: Renal cyst formation, as observed in autosomal dominant polycystic kidney disease (ADPKD), is a life-threatening condition with no effective cure yet. The molecular mechanisms underlying primary cilia dysfunction, which causes cyst formation and disease development, are not well understood. Human kidney tubuloids offer a promising model system to investigate the disease mechanisms of PKD in physiologically relevant 3D structures. However, their inherent cystic morphology poses a challenge in effectively modelling kidney cystogenesis. Therefore, our study aims to refine the culture
