Prenatal body fluid analysis in the evaluation of CAKUT.

Congenital anomalies of the kidney and urinary tract (CAKUT) are the leading cause of chronic kidney disease in children. Although prenatal ultrasound enables early detection, its ability to predict postnatal renal outcome, particularly in bilateral cases, remains limited due to substantial phenotypic variability and the inability of imaging to capture ongoing disease processes. Genetic testing improves etiological classification but shows weak genotype-phenotype correlations and limited prognostic value, reflecting the multifactorial nature of CAKUT and prompting investigation of molecular bi

Detection of female genital tract abnormalities in antenatally detected fetal unilateral renal agenesis: Original retrospective case series with review of literature.

OBJECTIVE: In females, unilateral renal agenesis is associated with genital tract abnormalities, which can cause complications in childhood and beyond. However, its prevalence and characteristics are not consistently integrated into antenatal counselling. METHODS: An original retrospective case series study (2005-2024) at Guy's and St Thomas' Hospital identified fetuses with isolated antenatally diagnosed unilateral renal agenesis from an Astraia database, with standardized 6-week postnatal imaging. RESULTS: 110 cases of renal agenesis were identified. Of these, fifteen cases (n = 15) were e

Neurological manifestations and vasculopathy in PLCG2-associated antibody deficiency and immune dysregulation (APLAID): a case series.

BACKGROUND: Autosomal dominant pathogenic variants in PLCG2 are associated with autoinflammation and PLCG2-associated antibody deficiency and immune dysregulation (APLAID), characterized by immunological abnormalities and multi-system inflammation. Descriptions of associated neurological manifestations and vascular anomalies are limited. We describe three patients with APLAID, vasculopathies, neuroinflammation, and cerebral vascular tortuosity. These cases highlight underrecognized and clinically important manifestations of APLAID. CASE PRESENTATIONS: All three patients had de novo variants i

Atypical Renal Hypoplasia in HNF1B-Related Disease: The Clinical Impact of Early Genetic Diagnosis.

Background: Genetic testing is an increasingly important component in the diagnostic evaluation of kidney diseases. Pathogenic variants in the HNF1B gene cause a disorder classically known as renal cysts and diabetes (RCAD) syndrome, typically characterised by bilateral renal cysts, diabetes mellitus, and a broad spectrum of other extrarenal abnormalities. However, non-cystic presentations often lead to diagnostic delays. Case Report: We report the case of a 28-year-old woman evaluated for chronic kidney disease of unknown aetiology who presented with atypical radiological findings. Imaging re

[X-linked hypophosphatemia caused by a novel variant in the PHEX gene].

X-linked hypophosphatemia (XLH) (OMIM #307800) is a rare, chronic, and progressive genetic musculoskeletal disorder caused by loss-of-function variants in the PHEX gene located at locus Xp22.11. Clinical manifestations of XLH include short stature, dental abnormalities, and lower limb deformities, although the phenotype is variable. Diagnosis is clinical, biochemical, and radiological, but genetic testing is also recommended. This case report describes a 29-year-old female patient with no family history related to her disease, presenting with short stature, dental abnormalities, lower limb def

Severe growth retardation in Dent disease: a case report of a novel OCRL variant.

INTRODUCTION: Dent disease type 2 (DD-2) is a rare X-linked recessive renal tubulopathy caused by mutations in the OCRL gene. Clinically it is characterised by low molecular weight proteinuria (LMWP), hypercalciuria, and nephrocalcinosis. The phenotypic spectrum of DD-2 is expanding with the identification of new pathogenic variants. CASE REPORT: A 12-year-old male presenting with marked short stature and poor weight gain was found to have LMWP, haematuria, hypercalciuria, and bilateral nephrocalcinosis on initial clinical evaluation. Renal function was normal. There was no relevant family hi

Case Report: Computed tomographic diagnosis of asymmetric bilateral uterine horn torsion secondary to benign uterine leiomyomas in a non-gravid dog.

Uterine torsion is uncommon in dogs and most commonly involves a single uterine horn. An 11-year-old intact female Pomeranian was presented with a 2- to 3-day history of abdominal distension, dyspnea, anorexia, vomiting, and oliguria. Clinicopathologic findings indicated renal compromise, electrolyte abnormalities, tissue injury, and marked systemic inflammation. Radiography and ultrasonography revealed large bilateral abdominal masses with peritoneal effusion, although the organ of origin could not be determined. Contrast-enhanced computed tomography (CT) demonstrated masses arising from both

Case Report: Co-occurrence of Wilson disease and tuberous sclerosis complex in a Chinese patient.

BACKGROUND: Wilson disease (WD) and tuberous sclerosis complex (TSC) are rare genetic disorders with distinct pathophysiologies. Their co-occurrence has not been previously reported. When atypical clinical features emerge in a patient with an established genetic diagnosis, comprehensive reassessment, including thorough physical examination and expanded genetic testing, becomes critical. CASE PRESENTATION: We describe a 28-year-old Chinese woman with a confirmed diagnosis of WD who developed focal seizures after substantial clinical improvement during copper chelation therapy. Neonatal extracorporeal membrane oxygenation in congenital urinary tract obstruction with pulmonary hypoplasia: a case complicated by alveolar capillary dysplasia emphasizing importance of early rapid genetic testing.

A late preterm male infant born at 36 weeks and 2 days of gestation with birth weight of 2.5 kg and prenatally diagnosed bilateral hydronephrosis, megaureter, urinomas, and failed fetal shunt placement in the setting of anhydramnios developed severe hypoxemic respiratory failure shortly after birth. Despite surfactant, inhaled nitric oxide, and vasoactive support, refractory hypoxemia with severe pulmonary hypertension (oxygenation index of 56) necessitated venoarterial extracorporeal membrane oxygenation (ECMO) at 9 h of life. His course was complicated by hemodynamic instability, transfusion

Microcirculatory, micro-rheological, metabolic and histomorphological investigation of experimental mesoblastic nephroma in rats treated with Vincristine.

The development of malignant tumors and their abnormal vasculature can lead to severe changes in tissue perfusion. In this pilot study, we aimed to investigate changes in renal microcirculation and parenchymal abnormalities without treatment and after Vincristine treatment in a rat model of mesenchymal mesoblastic nephroma (Ne/De) tumor cell implantation of rat origin, which closely resembles Wilms' tumor. On the 1st, 4th, and 9th post-operative days, blood samples were collected to test for hematological parameters, erythrocyte deformability, and aggregation. In the Vincristine-treated group

Severe bilateral ocular coloboma and microphthalmia in a neonate with WDR37-associated neurooculocardiogenitourinary syndrome.

We describe a neonate with severe bilateral ocular abnormalities associated with WDR37-related neurooculocardiogenitourinary syndrome. Ocular findings identified at birth included bilateral microphthalmia, microcornea, corneal edema, marked anterior segment dysgenesis, and bilateral colobomatous posterior pole abnormalities. The patient also had multiple systemic anomalies, including complex congenital heart disease, renal dysplasia, skeletal abnormalities, and cerebellar malformation. Clinical exome sequencing identified a de novo pathogenic variant in the WDR37 gene, confirming the diagnosis

Uremic Optic Neuropathy as the Initial Manifestation of Advanced Chronic Kidney Disease.

Uremic optic neuropathy (UON) is a rare but potentially reversible complication of advanced kidney failure characterized by optic disc edema and visual decline that improves with dialysis. We report the case of a 45-year-old man with no prior diagnosis of chronic kidney disease who presented with 3 months of progressive bilateral blurry vision, eye pain, and exertional fatigue. Ophthalmologic examination revealed bilateral optic disc edema with reduced visual acuity. Laboratory evaluation showed profound azotemia (blood urea nitrogen 182.9 mg/dL; creatinine 29.5 mg/dL), hyperkalemia, anemia, m

Necrotizing Myopathy Presenting With Guillain-Barré-Like Areflexic Paraparesis, Severe Rhabdomyolysis, and Dialysis-Requiring Acute Kidney Injury.

Necrotizing myopathy typically presents with subacute symmetric proximal weakness and marked creatine kinase elevation. Acute areflexic paraparesis with sensory abnormalities may instead suggest Guillain-Barré syndrome and create uncertainty regarding neuromuscular localization. A man in his 50s presented after collapse with abrupt bilateral lower-extremity paralysis. Examination demonstrated 0/5 strength across proximal and distal lower-extremity muscle groups, marked rigidity and hypercontracture, areflexia, and sensory impairment, with preserved upper-extremity function. Multisystem developmental abnormalities and insulin resistance in ADTKD-HNF1B with the p.E105K variant: A clinical conundrum.

AIMS: HNF1B variants cause autosomal dominant tubulointerstitial kidney disease (ADTKD-HNF1B), but the phenotypic spectrum of variants of uncertain significance (VUS) remains incompletely defined. We aimed to characterise the multisystem phenotype associated with the HNF1B p.E105K variant and to examine the mechanism underlying insulin resistance in this setting. METHODS: A 15-year-old male proband with chronic kidney disease and multisystem developmental abnormalities underwent clinical, biochemical and genetic evaluation, including trio-based whole-exome sequencing. The identified variant w

[SINGLE-PORT LAPAROSCOPIC NEPHRECTOMY FOR CONGENITAL HYDRONEPHROSIS WITH A DISC KIDNEY AFTER THE EPISODE OF TRAUMATIC URINE LEAKAGE : A CASE REPORT].

A 7-year-old boy who had been under observation after being diagnosed with a disc kidney, right hydronephrosis and poor right renal function was urgently transported to our hospital for treatment of a right perirenal urinary leakage caused by abdominal trauma. We performed interventional radiology-computed tomography (IVR-CT) before nephrectomy of the nonfunctioning right hydronephrotic kidney. Preoperative IVR-CT revealed five and four arterial branches supplying the right and left kidney, respectively, derived from the abdominal aorta, bilateral common iliac arteries, and the sacral artery.

CAKUT subtypes determine the rate of progression to kidney failure-an adult patient cohort study.

BACKGROUND: Congenital anomalies of the kidney and urinary tract (CAKUT) are the leading cause of paediatric kidney failure (KF) and a significant contributor to KF in adults. Progression to KF varies widely. Early renal risk stratification is challenging due to a lack of data on long-term kidney outcomes during adulthood. This multicentre study aims to correlate progression to KF with CAKUT phenotypes, including the extent of extrarenal involvement and genetic findings. METHODS: We conducted an observational, retrospective cohort study. A total of 229 adult CAKUT patients with KF either befo

Successful arteriovenous fistula creation in a chronic kidney disease G5 patient with bilateral ulnar artery absence: A case report.

Hemodialysis access induced distal ischemia (HAIDI) represents a serious complication following arteriovenous fistula (AVF) creation, with increased risk in elderly patients with arterial abnormalities. Bilateral congenital absence of ulnar arteries is an exceptionally rare anatomical variant with optimal AVF creation strategies remain undefined due to the theoretical high risk of HAIDI. We report a female patient in her 80s with chronic kidney disease G5. She had bilateral congenital absence of ulnar arteries and required vascular access creation. Preoperative assessment revealed radial arter

World's first surgical repair of bilateral common iliac artery agenesis.

We present a rare case of a 23-year-old woman with congenitally absent bilateral common iliac arteries, identified incidentally during a computed tomography scan of the abdomen. Imaging revealed occlusion of the aorta below the renal arteries with flow to the lower limbs provided by hypertrophied collaterals. Despite initial conservative management, the patient experienced worsening claudication and functional limitations, prompting surgical revascularisation. An aorto-bifemoral bypass using an axillary-bifemoral graft was performed with successful recovery - the first documented case of surgi
