BACKGROUND: Executive dysfunction is the most prominent feature of cognitive impairment in patients with end-stage renal disease (ESRD). The dorsolateral prefrontal cortex (DLPFC) is a central region for the regulation of executive functions. The aim of our study was to examine alterations in neural activity and functional connectivity (FC) of the DLPFC in relation to cognitive assessments and clinical indicators in patients with ESRD using the resting-state functional magnetic resonance imaging (rs-fMRI) technique, and to further predict cognitive-related brain damage in this population.

Presented here is an extremely rare case of bilateral ureteral obstruction due to Candida albicans fungus balls, which led to acute kidney dysfunction and candidemia. An 83-year-old man was brought to our hospital after falling due to poor physical condition. He had been receiving abiraterone acetate for 1 month for metastatic castration-resistant prostate cancer, while past medical history included type II diabetes, cardiovascular disease, and dementia. Blood test results revealed severe liver dysfunction, though whole-body computed tomography (CT) findings showed no abnormalities. Based on t

BACKGROUND AND AIMS: Congenital anomalies of the kidney and urinary tract (CAKUT) are a group of disorders responsible for the majority of pediatric end-stage renal disease cases. The CAKUT group is phenotypically variable and can affect the kidney alone and/or the lower urinary tract. The aim of the study was to assess the risk factors for the onset of chronic kidney disease (CKD) and the progression to kidney failure (KF, i.e., the need for dialysis or kidney transplantation) in a cohort of patients with CAKUT.
METHOD: We conducted a longitudinal study at the Nephrology, Dialysis, and Trans

PURPOSE: To describe a case of CBAVD due to a pathogenic variant in ADGRG2, detail the reproductive outcomes of the couple, and provide an up-to-date review of ADGRG2 variant pathophysiology as well as speculate on the mechanism by which defects in ADGRG2 function result in vasal agenesis.
METHODS: Review of the first reported case in the USA and expound on ADGRG2 dysfunction vis-à-vis male reproductive ductal anatomy.
RESULTS: A pathogenic variant in ADGRG2 was the genetic etiology of CBAVD. The couple underwent sperm harvesting coupled with ICSI and delivered two daughters.
CONCLUSION: Pa

Takayasu arteritis (TA) is a rare large-vessel vasculitis, typically affecting young females. We report a young Indian male with resistant hypertension and bilateral renal artery stenosis, who developed extensive collateral and accessory renal arteries as adaptive vascular remodelling in the setting of chronic, inactive large-vessel vasculitis, with preserved renal function due to extensive collateralisation. This constellation of findings is rare, particularly in males. The case underscores the role of CT angiography in delineating complex vascular remodelling in chronic TA and the importance

GM3 synthase deficiency (GM3SD), also known as salt and pepper developmental regression syndrome, is a rare autosomal recessive congenital disorder of glycosylation caused by biallelic pathogenic variants in ST3GAL5. It is characterized by early onset epileptic encephalopathy, severe neurodevelopmental impairment, and sensory deficits. Routine metabolic screening tests are frequently non-specific, which may contribute to delayed diagnosis. We report a Turkish male infant who presented in early infancy with developmental delay and hypotonia, followed by early onset, refractory epilepsy and prog

Bilateral renal agenesis (BRA) is typically considered a lethal condition due to severe pulmonary hypoplasia caused by early-onset anhydramnios. We report the case of a male infant who survived without prenatal intervention, underwent complex postnatal nephrological and surgical management, and ultimately received a successful renal transplant. This case highlights the exceptional possibility of survival of a patient with BRA and underscores the importance of multidisciplinary care.

BACKGROUND: Mutations in the HNF1B gene can cause developmental and functional abnormalities in multiple organs such as the pancreas, kidneys, and liver. The patient is a 19-year-old male with normal growth and development, presenting with a 9-year history of diabetes that remains poorly controlled despite intensive subcutaneous insulin therapy. His clinical features include surgically managed bilateral cataracts, hyperuricemia, and bilateral renal cysts. Family history is significant for diabetes in paternal and maternal uncles.
METHODS: This combination of early onset, multiorgan involvemen

Acute pancreatitis most commonly results from gallstone disease, alcohol use, metabolic abnormalities, or medication effects. Malignant infiltration of the pancreas is a rare cause of pancreatic inflammation and may present with symptoms indistinguishable from more typical forms of pancreatitis. Pancreatic involvement by lymphoma is particularly uncommon and may mimic pancreatic neoplasms or recurrent inflammatory pancreatic disease. We present the case of a 29-year-old woman with a recently diagnosed CD-20-positive B-cell lymphoma who developed recurrent severe abdominal pain and markedly ele

INTRODUCTION: Multicystic dysplastic kidney (MCDK) is a congenital condition where noncommunicating cysts replace normal renal parenchyma, affecting about 1 in 4,300 live births. Unilateral MCDK is often asymptomatic, it may cause flank pain, hematuria, or hypertension. CASE DESCRIPTION: A 40-year-old male with history of hypertension since age 20 was referred to our clinic for evaluation of cystic kidney. He had no family history of kidney disease. His blood pressure was 145/85 mmHg. Serum creatinine was 0.8 mg/dL. Urinalysis did not show hematuria or proteinuria. Renal ultrasound showed an e

Doege-Potter Syndrome (DPS) is a rare paraneoplastic cause of hypoglycaemia, arising from solitary fibrous tumours (SFTs) that ectopically secrete insulin-like growth factor II (IGF-II). We report a case of DPS in a 76-year-old man presenting with recurrent severe hypoglycaemia. Biochemistry investigations revealed hypoglycaemia with suppressed insulin, C-peptide and beta-hydroxybutyrate levels. Magnetic resonance imaging (MRI) confirmed a giant (205 × 190 × 169 mm) right pleurally-based SFT. Hypoglycaemia was managed with intravenous dextrose infusion and glucagon until surgical resection via

Loss of Kif26b function causes renal agenesis or hypoplasia in mice, and a KIF26B mutation resulting in deletion of the C-terminal region has been identified in a human patient with a congenital kidney disorder. However, the role of this region during kidney development is unclear; thus, to investigate this, we generated mice lacking the C-terminal region of KIF26B (Kif26b-C) using CRISPR/Cas9. Homozygous Kif26b-C mice showed bilateral renal agenesis, resembling Kif26b knockout mice. Histological analysis revealed loss of the nephrogenic zone and impaired nephron progenitor condensation at emb

Fraser syndrome is a rare autosomal recessive congenital malformation syndrome characterized by a wide clinical spectrum, including ocular, limb, genital, and visceral anomalies. Diagnosis is suggested by the association of multiple characteristic malformations. We report the case of a newborn admitted to the Neonatology and Neonatal Intensive Care Unit of Mohammed VI University Hospital in Tangier, born to consanguineous parents, who presented with a polymalformative syndrome associating bilateral anophthalmia with ankyloblepharon, syndactyly, genital anomaly, anal imperforation, and low-set

OBJECTIVE: To describe the management of a rare chest wall solitary fibrous tumor (SFT) complicated by Doege-Potter syndrome (DPS), emphasizing the critical roles of multidisciplinary consultation and preoperative vascular embolization in optimizing outcomes.
METHODS: A 56-year-old male presented with exertional dyspnea, dizziness, recurrent hypoglycemia, and hypokalemia. Contrast-enhanced computed tomography (CT) identified a large chest wall mass (19.2 × 13.3 × 21.6 cm), with biopsy confirming SFT. Multidisciplinary evaluation established a diagnosis of DPS. The patient underwent three cycl

Solitary fibrous tumours can cause non-islet cell tumour-induced hypoglycaemia, a paraneoplastic syndrome resulting from extrapancreatic tumours secreting insulin-like growth factor -II. This is also known as Doege-Potter syndrome. A male patient in his 70s presented to our hospital with loss of consciousness because of a second relapse of the solitary fibrous tumour with Doege-Potter syndrome. A third surgery was performed after transcatheter arterial embolization. After surgery, blood glucose levels stabilized. Repeated relapses can occur in solitary fibrous tumours even after the complete r

IgG4-related disease (IgG4-RD) is a systemic immune-mediated fibroinflammatory condition that can involve virtually any organ. Renal involvement, referred to as IgG4-related kidney disease (IgG4-RKD), represents an uncommon manifestation and may pose significant diagnostic challenges, particularly when renal dysfunction is the only presenting feature. We report the case of an 80-year-old man who presented with severe renal insufficiency, manifested by markedly elevated serum creatinine levels, in the absence of prior renal disease or significant comorbidities. Imaging evaluation revealed no ab
