Intraoperative vascular ligation uncovers the role of tumor vasculature in Doege-Potter syndrome: a case report.

Solitary fibrous tumor (SFT) is a rare mesenchymal neoplasm that may present with non-islet cell tumor hypoglycemia (NICTH), also referred to as Doege-Potter syndrome, primarily due to aberrant secretion of big-IGF2. However, not all big-IGF2-producing SFTs result in hypoglycemia, implying the involvement of additional pathogenic factors. This article reports a case of a 60-year-old male who presented with hypoglycemic syncope secondary to Doege-Potter syndrome. Preoperative imaging identified a large, highly vascularized SFT, approximately 20 cm in diameter, located in the right thoracic cavi

Correlation of pre-operative sonographic and nuclear scan parameters with intraoperative renal histopathology in children with congenital pelviureteric junction obstruction.

BACKGROUND: Pelviureteric junction obstruction (PUJO) is the most common cause of pediatric hydronephrosis; however, distinguishing obstructive and non-obstructive dilatation remains challenging. Existing diagnostic modalities are limited by the lack of validation against objective measures of renal injury. Preoperative imaging is critical for guiding surgical decision-making; however, its correlation with histopathologic renal damage has not been fully elucidated. OBJECTIVES: To evaluate the association between preoperative sonographic and nuclear scintigraphy parameters and intraoperative r

Townes-Brocks Syndrome as Familial Isolated Bilateral Cup Ear Deformity: A Case Report.

Townes-Brocks syndrome (TBS) is a rare autosomal dominant disorder caused by pathogenic variants in SALL1. Although classically characterized by external ear anomalies, anorectal malformations, and thumb/radial defects, its expressivity is highly variable and mild presentations may be overlooked. We report a 7-year-old boy presenting for cosmetic correction of bilateral cup ear deformities. Initial hearing screening and renal ultrasonography showed no abnormalities, and no other anomalies were identified on clinical examination. Detailed history-taking and construction of a three-generation pe

Hypoglycaemia in a patient with undiagnosed Doege-Potter syndrome: A case report and narrative literature review.

Doege-Potter syndrome (DPS) is a rare paraneoplastic disorder characterised by non-islet cell tumour hypoglycaemia (NICTH), resulting from ectopic secretion of insulin-like growth factor 2 (IGF-II) by solitary fibrous tumours (SFTs). We describe the case of an 85-year-old woman, presenting in October 2024, with a chronic subdural haematoma and profound intraoperative hypoglycaemia. Further investigation revealed a large pleural mass; the pathophysiological findings were consistent with DPS, and histopathology confirmed a malignant SFT. Hypoglycaemia resolved after complete tumour resection. Th

Acute asymptomatic pulmonary thromboembolism detected using dynamic digital radiography following total hip arthroplasty: a case report.

BACKGROUND: Acute pulmonary thromboembolism is a serious complication of total hip arthroplasty that may be asymptomatic, underscoring the importance of early detection. Contrast-enhanced computed tomography is the standard diagnostic modality; however, its use is limited by its invasiveness, radiation exposure, contrast allergy, and renal dysfunction. Dynamic digital radiography is a novel, minimally invasive imaging technique that enables visualization of pulmonary perfusion without contrast agents. CASE SUMMARY: We report a case of a 58-year-old woman who underwent primary total hip arthro

Seronegative Doege-Potter syndrome in a patient with a pleural-based solitary fibrous tumour.

Doege-Potter syndrome (DPS) is a paraneoplastic syndrome characterised by episodic hypoglycaemia associated with solitary fibrous tumours (SFTs). It is thought to occur secondary to the secretion of a prohormone of insulin-like growth factor II by the tumour. Here we describe the diagnosis and management of a rare case of seronegative DPS in a patient with a pleural-based SFT.

Multistage Reconstruction of a Severe Cloacal Exstrophy Variant With Associated Myelomeningocele and Multisystem Involvement: A Case Report.

To our knowledge, this case represents one of the rarest and most severe forms of cloacal exstrophy (CE), distinguished by extensive multisystem involvement, including a large omphalocele, imperforate anus, bladder exstrophy, bilateral clubfeet, and the unusual concomitant presence of a lumbosacral myelomeningocele. This constellation of findings is exceptionally uncommon and poses significant surgical and multidisciplinary challenges from birth. Prenatal MRI demonstrated a large omphalocele containing liver, bowel, and gallbladder; a lumbar myelomeningocele (L5-S5); nonvisualization of the bl

Terminal 4q duplication and extended 10q deletion in a preterm infant with linear growth restriction: transcriptomic evidence of disrupted developmental and metabolic pathways.

BACKGROUND: Distal duplication 4q syndrome and distal 10q26 deletion syndrome are rare chromosomal abnormalities associated with complex and overlapping phenotypes. Most cases result from unbalanced inheritance of a parental translocation, yet the specific contributions of each chromosomal segment to the clinical phenotype remain poorly defined. CASE PRESENTATION: We report on a male preterm infant born at 28 + 6 weeks' gestation who carried a derivative chromosome 10 due to a paternally inherited unbalanced translocation t(4;10)(q31.22;q26.13), leading to a 42.46 Mb duplication of 4q31.22-q3

Acromegalic features and hypoglycemia: Doege-Potter syndrome.

BACKGROUND: Doege-Potter syndrome (DPS) is a condition characterized by severe, symptomatic, and sustained hypoglycemia, refractory to medical treatment, due to non-insular tumor cells. CASE PRESENTATION: We describe the case of a 73-year-old male with acromegalic features and two months of hypoglycemic episodes, in whom a giant mass was identified in the right hemithorax via radiography. Surgical resection of the tumor resolved hypoglycemia and improved the acromegalic traits.Solitary fibrous tumors (SFTs) are rare neoplasms that predominantly affect individuals aged 50 to 60 years. Impaired Metabolic Remodeling Characterizes Injured Niches during Post-ischemic Kidney Repair.

KEY POINTS: Spatial metabolomics revealed persistent injured tissue niches after ischemic injury despite the presence of apparently healthy epithelial cells. Proximal tubule cells residing in injured niches exhibited succinate accumulation and linoleic acid depletion. Integration with spatial transcriptomics showed that injured niches were characterized by dysregulated oxidative and lipid metabolism. BACKGROUND: The role of the tissue microenvironment in the transition from AKI to CKD remains poorly understood. While the persistence of failed-repair proximal tubule (FR-PT) cells is postulated

Long-term survival with eribulin in an elderly patient with inoperable retroperitoneal well-differentiated liposarcoma: a case report and literature review.

BACKGROUND: The management of retroperitoneal well-differentiated liposarcoma (WDLPS) in elderly, surgically ineligible patients remains a therapeutic challenge. Eribulin, a microtubule inhibitor, has demonstrated activity in advanced non-WDLPS liposarcoma subtypes; however, its efficacy in pure well-differentiated liposarcoma (WDLPS) remains exploratory, and long-term outcomes in elderly patients are not well documented. CASE PRESENTATION: An 82-year-old female presented with a large retroperitoneal mass. Biopsy confirmed WDLPS with MDM2 amplification. Due to age, comorbidities, and unresect

Case Report: A rare cause of neonatal respiratory distress: extralobar pulmonary sequestration.

Pulmonary sequestration (PS) is a rare congenital malformation characterized by a mass of non-functioning, dysplastic lung tissue that lacks normal tracheobronchial communication and receives its blood supply from an aberrant systemic artery. Although advances in prenatal ultrasonography have significantly improved early detection rates, a subset of cases remains undiagnosed until postnatal clinical symptoms emerge. Extralobar sequestration (ELS) in particular typically presents during the neonatal period, frequently manifesting as acute respiratory distress. We present the case of a term male

Delayed Diagnosis of Tuberous Sclerosis Complex Presenting as Abnormal Uterine Bleeding: A Case Report and Literature Review.

Tuberous sclerosis complex (TSC) is a rare multisystem genetic disorder characterized by the development of hamartomatous lesions in multiple organs. Although neurologic and dermatologic manifestations commonly lead to diagnosis during childhood, some patients remain undiagnosed until adulthood because of atypical presentations. We report the case of a 28-year-old Ethiopian woman with a longstanding seizure disorder who presented with a two-year history of abnormal uterine bleeding. Physical examination revealed facial angiofibromas and hypomelanotic macules. Further evaluation demonstrated bi

Hypoglycemia unmasking a giant pulmonary solitary fibrous tumor: a case report.

BACKGROUND: Hypoglycemia may result from multiple clinical conditions which are categorized based on clinical status. The first principal step in clinical diagnostic evaluation is measurement of plasma insulin, c-peptide, pro-insulin, and beta-hydroxybutyrate levels. One of the rare causes of hypoglycemia is a group of disorders known as non-islet cell tumor hypoglycemia (NICT), which results from the paraneoplastic secretion of insulin-like growth factor 2 (IGF-2). In rare cases, Solitary fibrous tumors SFTs are associated with recurrent hypoglycemia and represent the most frequent subtype of

Solitary Fibrous Tumor Generated From Uterine Corpus With Doege-Potter Syndrome in Primary Disease: A Rare Case of Recurrent Disease Without Hypoglycemia.

INTRODUCTION: Solitary fibrous tumors (SFTs) generated from uterine corpus are extremely rare and hard to distinguish from myomas, but their prognoses are wildly different. Doege-Potter syndrome (DPS) is a form of nonislet cell tumor hypoglycemia (NITCH) caused by a SFT. Vigilance is required when a pelvic mass is found concurrent with NITCH. CASE PRESENTATION: A 65-year-old female with huge pelvic masses generated from uterine corpus presented with mild symptoms of transient slow response, accompanied by memory loss once, and was found to have hypoglycemia. The pituitary glucocorticoid axis,

A Giant Thoracic Mass and Hypoglycemia: Suspected Solitary Fibrous Tumor-related Doege-Potter Syndrome.

A 96-year-old woman was brought to the emergency department with impaired mobility and generalized edema. She appeared drowsy, and blood glucose testing demonstrated severe hypoglycemia (38 mg/dL). Chest radiography showed an opacity in the right lower lung field, and computed tomography revealed a large intrathoracic mass. Laboratory testing revealed low serum insulin and C-peptide levels, suggesting non-islet cell tumor hypoglycemia. She had previously been diagnosed with a solitary fibrous tumor via a percutaneous biopsy at another hospital. Corticosteroids and glucose supplementation resol

Prenatal diagnosis of lowe syndrome: identification of a novel pathogenic variation in the OCRL1gene.

BACKGROUND: Lowe syndrome is an extremely rare X-linked genetic condition caused by disruptions in phosphatidylinositol metabolism, primarily impacting the eyes, brain, and kidneys. The condition is driven by over 200 identified mutations in the OCRL1 gene, which codes for an inositol polyphosphate 5-phosphatas. METHODS: This report details an Egyptian family seeking prenatal diagnosis after having a child with Lowe syndrome (LS). The affected child exhibited bilateral congenital cataracts, profound developmental delay, heart abnormalities, hypotonia, and proximal renal tubular dysfunction, an

[Posterior Reversible Encephalopathy Syndrome in a Patient with Chronic Kidney Disease on Hemodialysis and Refractory Hypertension: Case Report].

Posterior reversible encephalopathy syndrome (PRES) is a clinical-radiological entity characterized by an acute or subacute neurological onset, typically reversible within a few days following the removal or control of the underlying triggering factor. Diagnosis is based on clinical presentation and imaging findings, particularly brain magnetic resonance imaging (MRI), which in most cases reveals bilateral and symmetrical areas of vasogenic edema, predominantly involving the parieto-occipital regions. PRES is frequently associated with predisposing conditions such as severe arterial hypertensi
