Fetal hydronephrosis is the most frequently detected prenatal urinary tract abnormality and encompasses a broad clinical spectrum, ranging from transient physiological dilation to clinically significant obstructive uropathy with potential long-term renal consequences. The broader term urinary tract dilation integrates renal pelvic and calyceal dilation with abnormalities of the renal parenchyma, ureters, bladder, and amniotic fluid volume, thereby supporting more comprehensive risk stratification. This narrative review synthesizes evidence on fetal urinary tract development, prenatal diagnosis

Papillorenal syndrome (PAPRS), or renal coloboma syndrome, is a rare autosomal dominant disorder caused by PAX2 mutations. It classically manifests with renal hypodysplasia and optic nerve anomalies. However, recent literature suggests an expanding phenotypic spectrum. We report a 7-year-8-month-old boy born to consanguineous parents, presenting with stage 4 chronic kidney disease (CKD), nephrotic-range proteinuria, visual impairment, and ADHD. Renal biopsy revealed focal segmental glomerulosclerosis (FSGS), and ocular examination showed bilateral peripheral scalloped chorioretinal atrophy wit

A cat with bilateral ureteral obstruction and long-term urolithiasis that had been medically managed for a year was presented to the emergency department of our hospital. Severe azotemia and metabolic acidosis were identified. Ultrasonography and computed tomography supported a diagnosis of ureteral calculi, with no evidence of other underlying abnormalities. During exploratory laparotomy, irreversible renal failure secondary to bilateral ureteral obstruction was suspected, and euthanasia was elected. Histopathological and immunohistochemical examination of the ureters revealed tubulopapillary

BACKGROUND: Distal renal tubular acidosis (dRTA) is a rare inherited disorder characterized by impaired urinary acidification, leading to metabolic acidosis, hypokalemia, nephrocalcinosis, and growth impairment. Pathogenic variants in ATP6V0A4 are among the most common genetic causes of autosomal recessive dRTA.
METHODS AND RESULTS: We report a Lebanese infant presenting with failure to thrive, recurrent vomiting, severe hyperchloremic metabolic acidosis, hypokalemia, and bilateral nephrocalcinosis, in whom whole-exome sequencing (WES) was performed to establish the molecular diagnosis and pe

OBJECTIVES: Clinical and radiological criteria for predicting surgical approach or spontaneous resolution have been evaluated mainly in primary non-refluxing megaureters detected prenatally. We aimed to analyze these criteria in a retrospectively collected cohort of children with either prenatal or postnatal diagnosis of non-obstructive non-refluxing (NONR) megaureter.
METHODS: Hospital files of the children with NONR megaureter were evaluated retrospectively for age at diagnosis, presenting complaint, follow-up period, accompanying urologic abnormalities, complications (renal scar and urinar

Urinary bladder agenesis is an extraordinarily rare congenital anomaly, often associated with complex genitourinary malformations and poses significant diagnostic challenges in infancy. We report an infant girl with progressive abdominal distension, continuous urinary dribbling and bilateral hydronephrosis, ultimately found to have bladder agenesis with bilateral pelviureteric junction obstruction, ectopic ureteric insertion into the vagina and malrotated kidneys. Multimodality imaging, including ultrasonography, radionuclide diuretic renography with single-photon emission CT/CT and contrast-e

This case report describes the concurrent occurrence of rare renal vascular and pericalyceal variations and highlights their surgical significance. The renal vasculature and pelvis exhibit a wide spectrum of anatomical variations, many of which are clinically and surgically significant. Awareness of these deviations is of immense importance during retroperitoneal and renal surgeries and interventions. This report presents concurrent variations in both renal arteries and veins, as well as the renal pelvis, observed in a cadaver during routine dissection at the All India Institute of Medical Sci

A 42-year-old woman with a 25 pack-year smoking history, who quit 5 years prior to presentation, was referred to our outpatient clinic for reevaluation of multiple bilateral pulmonary nodules first identified 4 years earlier on chest computed tomography, performed following a self-limited flu-like illness with transient rash and nonproductive cough. Her symptoms resolved without intervention, and she remained clinically stable in the years that followed, with only occasional episodes of dry cough; however, the radiologic findings and her medical history prompted progressive diagnostic workup.

INTRODUCTION: Pulmonary hypoplasia and severe pulmonary hypertension associated with severe oligo/anhydramnios may be mitigated by amnioinfusion in fetal bilateral renal agenesis (BRA). We report fetal cardiovascular testing in live-born BRA subjects of the Renal Anhydramnios Fetal Therapy (RAFT) trial.
METHODS: Fetuses in the BRA RAFT trial cohort underwent third trimester echocardiography with maternal hyperoxia (MH). Echocardiographic parameters, compared between baseline and MH, are presented for expectant management and amnioinfusion participants using descriptive and inferential statist

BACKGROUND: Foetal megacystis (FM) may be associated with a wide range of conditions beyond posterior urethral valves (PUV). This study aimed to describe the outcomes of FM cases, excluding PUV, in order to better understand the long-term consequences of these conditions and improve prenatal counselling.
METHODS: This retrospective multicentre study, conducted between 2009 and 2023, included pregnant women referred for suspected FM across three foetal medicine units. Patients with confirmed PUV, diagnosed either postnatally or through pathological examination following medical termination of

INTRODUCTION AND IMPORTANCE: Unilateral agenesis of the ovary and fallopian tube is a rare reproductive anomaly that may occur with or without associated uterine malformations. Both variations have been reported in the literature. As patients are often asymptomatic, diagnosis typically occurs incidentally during surgery. This case presents a rare combination of ipsilateral ovarian and fallopian tube agenesis with a unicornuate uterus and normal bilateral kidneys. It underscores the need for a multidisciplinary approach involving obstetricians, neonatologists, and nursing care to improve matern

Solitary fibrous tumors are mesenchymal neoplasms that make up 2% of soft tissue tumors. Doege-Potter syndrome (DPS) occurs between 5-10% of patients with solitary fibrous tumor of the pleura (SFTP). Little is mentioned about DPS in the relapse setting, where malignant transformation should always be ruled out (until 12% of cases) according to tumor size, histopathologic aspects (including mitotic index) and immunohistochemical markers. This is the first case of Doege-Potter syndrome in the setting of a fibrous tumor relapse of the pleura without malignant transformation described in Colombia.

Pierson syndrome, characterized by congenital nephrotic syndrome, ocular abnormalities, and neurological defects, is caused by biallelic pathogenic variants in LAMB2. LAMB2 encodes laminin β2, a key component of basement membranes that is predominantly expressed in the glomeruli, eyes, and neuromuscular junctions. The renal histopathology of Pierson syndrome typically shows diffuse mesangial sclerosis (DMS), with occasional tubulointerstitial atrophy and fibrosis. We report a case of Pierson syndrome characterized by DMS and prominent tubular dilatation. A fetal ultrasound at 23 weeks of gesta

OBJECTIVE: Primary aldosteronism (PA) adversely affects cardiovascular and renal health and has been linked to increased urinary calcium excretion, elevated parathyroid hormone (PTH) levels, and reduced bone mineral density. Although adrenalectomy and mineralocorticoid receptor antagonists (MRAs) are established treatments, their differential effects on calcium dynamics across PA subtypes remain unclear.
DESIGN: We conducted a retrospective observational study, analyzing a large, well-characterized cohort on calcium metabolism in PA. A total of 352 patients with PA and 57 controls were evalua

BACKGROUND: Percutaneous nephrolithotomy (PCNL) may cause severe thoracic and abdominal complications. Because their onset can be insidious and their intraoperative manifestations atypical, recognition may be delayed. We report a case of sudden bilateral hemorrhagic pleural effusion with hemorrhagic ascites during PCNL and discuss strategies for intraoperative identification and management.
CASE PRESENTATION: A 63-year-old woman underwent PCNL for multiple right renal calculi. During the procedure, renal pelvic bleeding occurred, followed by a sudden increase in airway pressure to 40 cmH2O, a

BACKGROUND: HNF1B-associated disease is a genetic disorder caused by heterozygous pathogenic variants in the HNF1B gene, leading to a variety of clinical phenotypes primarily affecting the kidneys, pancreas, liver, and genitourinary tract. First identified in 1997 as the cause of MODY5 (maturity-onset diabetes of the young, type 5), the disease spectrum has since expanded to include kidney disease, pancreatic hypoplasia, genital malformations, gout, hypomagnesemia, liver abnormalities, and primary hyperparathyroidism. Variable expressivity and limited penetrance complicate diagnosis, with some

Fibromuscular dysplasia (FMD) is a vascular disease characterized by abnormal cellular growth in medium-sized arteries progressing to stenosis, aneurysm, or arterial dissection. Renal and carotid artery involvement is common. FMD is not characterized by inflammation or atherosclerosis and predominantly impacts young women. Refractory hypertension is a common primary clinical manifestation of renal artery FMD. Severe or untreated FMD can lead to stroke, aneurysm rupture, or death. Despite the need for early diagnosis and aggressive management, FMD is frequently overlooked because of nonspecific

Sirenomelia (mermaid syndrome) is an extremely rare and lethal congenital malformation characterized by fusion of the lower limbs, with a global incidence of approximately one in 60,000-100,000 live births. No proven hereditary pattern or genetic basis has been established. Sympus dipus (Type I, Stocker-Heifetz classification) is the variant in which both feet are preserved despite lower-limb fusion. We report the autopsy findings of a 19-week male fetus received at the Department of Anatomical Pathology of Hospital Metropolitano "Dr. Bernardo Sepúlveda" in Monterrey, México. External examinat
