Bilateral Obstructing Ureteroceles Presenting As Hypertension in a Young Adult Without Urinary Symptoms: A Case Report.

We present a 35-year-old man with long-standing untreated hypertension who had no urinary symptoms, flank pain, or history of urinary tract infections and was later found to have bilateral ureteroceles with bilateral hydronephrosis and renal cortical thinning. A ureterocele is a congenital cystic dilation of the distal ureter as it inserts into the bladder that can impede urinary drainage. The initial laboratory workup for secondary hypertension was unremarkable, and he had a minimal response to first-line blood pressure management. Imaging was then sought to evaluate for structural causes of

Urinary incontinence revealing Mayer-Rokitansky-Küster-Hauser syndrome in an early adolescent.

Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome is a rare congenital condition marked by absence of the uterus and upper two-thirds of the vagina in females with a 46, XX karyotype. Type II MRKH includes renal, skeletal and occasionally cardiac or auditory anomalies. An early adolescent girl presented with lifelong dribbling of urine and recurrent urinary tract infection. She had normal external genitalia, was at Tanner Stage II and showed mild thoracic scoliosis. Abdominal ultrasonography demonstrated a horseshoe kidney with bilateral ectopic ureters, uterine agenesis and absence of the upper

KILT syndrome revealed by right iliac fossa pain in an 11-year-old boy: A case report.

Kidney and Inferior Vena Cava abnormalities with Leg Thrombosis syndrome, an acronym for kidney and inferior vena cava abnormalities with leg thrombosis, is a rare congenital condition that predisposes young patients to deep venous thrombosis. We report on the case of an 11-year-old boy with a recent diagnosis of pulmonary tuberculosis who presented with right iliac fossa pain and inability to walk because of a painful flexed right lower limb. Initial ultrasound was inconclusive for appendicitis because the appendix was not visualized due to bowel gas; it showed no peritoneal fluid or intraper

Severe Sepsis in an Adolescent With a De Novo Variant in the EP300 Gene Associated With Rubinstein-Taybi Syndrome Type 2: A Case Report.

Rubinstein-Taybi syndrome type 2 (RSTS2) results from pathogenic variants in the EP300 gene, manifesting with intellectual disability, facial dysmorphism, and multisystem anomalies. Variants of uncertain significance (VUS) require family segregation studies for reclassification. A 17-year-old male adolescent with intellectual developmental disorder and autism spectrum disorder (ASD) presented at the age of 12 with pneumonia complicated by severe infection, bilateral pleural effusion, and rupture of the left renal excretory system. A multigene panel identified a VUS in the EP300 gene (c.4331_43

Disseminated histiocytic sarcoma in 2 FeLV-positive cats in Brazil.

Histiocytic sarcomas (HSs) are uncommon malignant neoplasms in cats, typically affecting older animals and not previously associated with feline leukemia virus (FeLV). Here, we describe 2 cases of disseminated HS in young FeLV-positive domestic shorthair cats that clinically and grossly mimicked lymphoma. Case 1, a 1-y-old female cat, was presented with hemiplegia and dysuria and was euthanized because of the clinical suspicion of spinal lymphoma. Case 2, a 4-y-old male cat, had hyporexia, ocular abnormalities, and bilateral renal enlargement and was euthanized because of the clinical suspicio

A 50-Year-Old Woman With Cough and Cystic Lesions Accompanied by Diffuse Interstitial Abnormalities.

A 50-year-old woman was admitted to our hospital with a 2-year history of productive cough and intermittent low-grade fever, which had shown only temporary improvement with short courses of antibiotics. Her past medical history was notable for hysteromyoma, treated with hysterectomy 6 years earlier. She did not smoke, did not report any occupational chemical exposures, and reported a family history of cor pulmonale in her father. Approximately 2 years before admission, a thoracic CT scan at a local hospital revealed diffuse cystic lesions with scattered bilateral nodules and calcifications. Br

Mayer-Rokitansky-Küster-Hauser syndrome type II complicated by strangulated left inguinal ovarian-fallopian tube hernia in a pediatric patient: a case report.

BACKGROUND: Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome is a congenital disorder characterized by Müllerian duct anomalies. Patients have a significantly higher incidence of inguinal hernia (6.4%-15.6%) compared to the general female population. These hernias frequently contain ovarian or tubal tissue and are associated with a high risk of strangulation, posing a threat to future fertility. This case report details a rare pediatric presentation of MRKH syndrome type II with a strangulated inguinal ovarian-tubal hernia. CASE PRESENTATION: A 7-year-5-month-old girl presented with a 28-hour h

Apparent mineralocorticoid excess with a novel HSD11B2 variant: longitudinal clinical and biochemical follow-Up.

Apparent mineralocorticoid excess (AME) is a rare autosomal recessive disorder caused by loss-of-function mutations in HSD11B2, leading to impaired conversion of cortisol to cortisone and inappropriate mineralocorticoid receptor activation. Early recognition is critical, as untreated disease results in progressive cardiovascular and renal complications. We describe a 2-year-old boy with severe early-onset hypertension, polyuria, polydipsia, abdominal distension, and growth retardation. Biochemistry revealed hypokalemic metabolic alkalosis with suppressed renin and low aldosterone. Renal ultras

Isolated Bilateral Nephromegaly as the Initial Presentation of Acute Lymphoblastic Leukaemia: A Rare Case and Review of the Literature.

INTRODUCTION: Acute lymphoblastic leukaemia (ALL) is an aggressive hematologic malignancy, with T-cell ALL (T-ALL) accounting for approximately 25% of adult cases. Renal infiltration is recognized but rarely presents as isolated bilateral nephromegaly with acute kidney injury (AKI) in adults. CASE DESCRIPTION: A 32-year-old previously healthy man presented with progressive bilateral flank pain, abdominal heaviness, fatigue, intermittent fever, night sweats, and unintentional weight loss. Physical examination revealed large bilateral flank masses without peripheral lymphadenopathy. Laboratory

An autopsy case of extensive aortic thrombosis with acute kidney injury as the sole clinical manifestation.

Aortic thrombosis is a rare but life-threatening condition. It typically presents with abdominal or lower-limb ischemia. Although acute kidney injury (AKI) can accompany aortic thrombosis, AKI as the sole clinical manifestation has not been previously reported. A 69-year-old man with extensive atherosclerotic disease and chronic kidney disease presented with severe oliguria. Initial evaluation, including non-contrast computed tomography (CT), did not reveal any aortic abnormalities. Renal Doppler ultrasonography findings and markedly elevated D-dimer levels prompted contrast-enhanced CT, which

Persistent Unilateral Nephrolithiasis: Do 24-h Urine Parameters Account for It?

Background: Bilateral nephrolithiasis is typically attributed to systemic metabolic abnormalities reflected in 24 h urine studies. Yet, some individuals form stones exclusively in one kidney over long-term follow-up, while the contralateral kidney remains unaffected. This study aimed to determine whether differences in urinary metabolites can explain persistent unilateral stone formation. Methods: We retrospectively reviewed our 2011-2024 nephrolithiasis database and analyzed 24 h urine compositions obtained under regular diet and daily activity conditions. Patients were classified into three

Kidney failure in a child with Pierpont syndrome: a case report highlight challenges in kidney replacement therapy.

BACKGROUND: Pierpont syndrome is a rare autosomal dominant disorder caused by pathogenic variants in TBL1XR1 gene, characterized by distinctive craniofacial features, plantar lipomatosis, and neurodevelopmental disorders. Urological malformations were reported, but signifcant kidney function impairment has never been described in previous cases. CASE PRESENTATION: We report an 8-year-old girl with bilateral dysplastic kidneys identified in neonatal period and progression to kidney failure, requiring peritoneal dialysis from age four. Subsequent genetic evaluation confirmed a diagnosis of Pier

Case Report: Pembrolizumab unmasks a capillary leak syndrome with chylothorax in early TNBC.

Pembrolizumab has become standard of care in early-stage triple-negative breast cancer (TNBC), but its expanding use may induce rare adverse events involving vascular and lymphatic endothelium. Capillary leak syndrome (CLS) remains an exceptionally rare toxicity of PD-1 blockade, scarcely reported in breast cancer, especially in the adjuvant setting. We report the case of a 40-year-old female with early-stage TNBC who presented with progressive dyspnea, diffuse subcutaneous edema and bilateral pleural and peritoneal effusions after two cycles of adjuvant pembrolizumab. Imaging confirmed polyse

Doege-Potter syndrome caused by a solitary fibrous tumor arising from the greater omentum: a case report.

Doege-Potter syndrome is a rare paraneoplastic syndrome characterized by non-islet cell tumor hypoglycemia (NICTH) caused by the excessive production of insulin-like growth factor-II (IGF-II), which occurs in less than 5% of solitary fibrous tumors (SFT). SFTs most commonly arise from the pleura, whereas primary omental origins are exceedingly rare. We report a case of Doege-Potter syndrome in an 83-year-old woman who presented with recurrent hypoglycemic episodes caused by a large primary omental SFT. Low serum insulin levels during hypoglycemia and normal adrenocortical hormone levels exclud

Doege-Potter syndrome in a patient with solitary fibrous tumor of the pleura: a rare cause of refractory hypoglycemia-a case report.

BACKGROUND: Solitary fibrous tumors of the pleura (SFTPs) are rare mesenchymal neoplasms that usually follow a benign clinical course. Infrequently, they present with Doege-Potter syndrome (DPS), a paraneoplastic condition characterized by recurrent non-insulin-mediated hypoglycemia caused by excessive secretion of incompletely processed insulin-like growth factor II. This entity is reported in fewer than 5% of cases and can lead to life-threatening metabolic disturbances. CASE DESCRIPTION: A 65-year-old man presented with progressive dyspnea and recurrent refractory hypoglycemia. Chest radio

Pelvic Retroperitoneal Solitary Fibrous Tumor: Two Cases Including Doege-Potter Syndrome.

Solitary fibrous tumor (SFT) is a rare mesenchymal lesion that typically exhibits no aggressive behavior and has low metastatic potential. While it is commonly described in the pleura and thorax, its occurrence in the retroperitoneal pelvic cavity is uncommon. We report two cases of pelvic retroperitoneal SFT treated at the Instituto Nacional de Cancerología in Colombia. The first case involved a 76-year-old man with a pelvic mass that developed over one year. Computed tomography (CT) showed a retroperitoneal tumor near the prostate and a suspicious pulmonary nodule. Biopsy confirmed high-risk

Liposomal amphotericin B as an antifungal prophylaxis in bilateral lung transplantation: a safety analysis.

BACKGROUND: Lung transplantation (LT) is the definitive treatment for end-stage chronic respiratory failure. Despite improvements in surgical and perioperative care, fungal infections remain a major complication in LT recipients, and antifungal prophylaxis with liposomal amphotericin B (L-AmB) is commonly used. However, evidence on the safety of L-AmB administration in LT recipients remains limited. This retrospective observational study aimed to assess the profile of renal, hepatic and hematologic safety, and the incidence of hypersensitivity adverse events following prophylactic L-AmB use in

Formin-1 maintains cochlear microtubule architecture required for hearing in humans and mice.

Formin proteins contribute to the cytoskeletal organization of multiple mammalian organ systems. Genetic defects in formins lead to neurologic, renal, reproductive, and cardiac disorders, but no human phenotype has been described for mutation of FMN1, encoding formin-1, the first-identified formin. In an extended Palestinian kindred, autosomal recessive congenital hearing loss proved due to homozygosity for FMN1 c.2162-2A>G, which leads to aberrant splicing, nonsense mediated decay, and absence of detectable formin-1 protein. The hearing loss is bilateral, moderate, and stable, and accompanied
