Fanconi Anemia in a Young Adult Male Presenting with Pancytopenia and Bilateral Ectopic Kidneys.

Fanconi anemia (FA) is a rare inherited disorder of bone marrow failure that presents with progressive pancytopenia, multiple congenital abnormalities, and an increased predisposition to various malignancies. We describe a 19-year-old male who reported generalized weakness, shortness of breath on exertion, and a dry cough persisting for 1 month. He had undergone three blood transfusions within the preceding 3 months. Clinical examination revealed marked pallor without evidence of jaundice, cyanosis, clubbing, or lymphadenopathy. Laboratory investigations showed pancytopenia, while bone marrow

Characteristics of pediatric-onset anti-contactin1 antibody-associated autoimmune nodopathies with concomitant membranous nephropathy.

BACKGROUND: Anti-contactin-1 antibody-associated autoimmune nodopathy (CNTN1-AN) is a rare disorder predominantly affecting older individuals, characterized by sensorimotor peripheral neuropathy, with over 50% of cases presenting with proteinuria and membranous nephropathy (MN). Pediatric-onset CNTN1-AN is exceptionally rare, and its clinical profile remains poorly characterized. CASE PRESENTATION AND LITERATURE REVIEW: We report a pediatric case of CNTN1-AN with MN and conduct a literature review to elucidate the pathogenesis and clinical features of CNTN1-AN combined with MN in children. The

Nephrocalcinosis Following Roux-en-Y Gastric Bypass Secondary to Suspected Enteric Hyperoxaluria: A Case Report.

Nephrocalcinosis refers to the deposition of calcium oxalate or calcium phosphate in the kidney, most commonly within the renal medulla, and is frequently linked with underlying metabolic abnormalities. Roux-en-Y gastric bypass (RYGB) is a well-established risk factor for enteric hyperoxaluria secondary to fat malabsorption, thereby increasing the risk of nephrolithiasis, oxalate nephropathy, and nephrocalcinosis. Despite this association, nephrocalcinosis following RYGB remains an uncommon and underrecognized clinical entity. A 36-year-old male with a history of complicated peptic ulcer disea

Hajdu-Cheney Syndrome as a Rare Cause of Acroosteolysis: A Case Report.

Hajdu-Cheney syndrome (HCS) is a rare autosomal dominant multisystem disorder caused by mutations in the NOTCH2 gene, leading to dysregulation of the Notch signaling pathway. It is characterized by a broad and variable clinical spectrum, including acroosteolysis, osteoporosis, short stature, craniofacial dysmorphism, and potential cardiovascular, renal, and neurological involvement. We report the case of a 16-year-old boy, born to consanguineous parents, who was referred for evaluation of facial dysmorphism. Clinical examination revealed typical features of HCS, including brachycephaly with a

Guillouet-Gordon Syndrome With Severe Microtia in an Infant With a Homozygous Variant in MED16.

Bi-allelic MED16 variants underlie the recently described MEDopathy termed Guillouet-Gordon syndrome (GGNS, MIM #621220), characterized by neurodevelopmental and structural anomalies. To date, 27 individuals with GGNS have been reported, collectively harboring 28 distinct compound heterozygous or homozygous MED16 variants. Here we present a male infant, born to unrelated parents, presenting with multiple congenital anomalies overlapping with the established GGNS phenotypic spectrum. Specifically, the clinical presentation included structural brain defects and neurodevelopmental deficits alongs

Hypoplastic vestibular aqueduct in congenital temporal bone anomalies - implications for subtype diagnosis of Meniere's disease.

BACKGROUND AND PURPOSE: As the endolymphatic (ES) undergoes normal postnatal maturation, the surrounding vestibular aqueduct (VA) undergoes a corresponding change in morphology, quantified by its angular trajectory (ATVA). In adult temporal bones with Meniere's disease (MD), a fetal orientation ATVA (≥140°) indicates underlying ES hypoplasia and defines the so called hypoplastic disease endotype (MD-hp). However, ES hypoplasia has also been described histologically in other inner ear syndromes and congenital conditions, independent of MD. We aimed to investigate whether ATVA ≥140° occurs in ma

Severe arterial hypertension, a silent complication of Williams-Beuren syndrome: A case report with literature review.

Williams-Beuren syndrome is a rare chromosomal microdeletion (7q11.23) associated with psychomotor delay, a characteristic dysmorphic profile, neurocognitive disorders, and, especially, an increased risk of cardiovascular and renal complications leading to hypertension. We report a case of severe hypertensive emergency in a child with this syndrome. Our patient is a 7-year-old boy who presented with headaches, morning projectile vomiting, and polyuria over the past month. On admission, the blood pressure was 180/120 mmHg, and the cardiovascular examination revealed no cardiac or vascular murmu

Neonatal Survival After Serial Amnioinfusions for Anhydramnios Due to Fetal Kidney Failure: The RAFT Clinical Trial.

IMPORTANCE: Anhydramnios from fetal kidney failure causes lethal pulmonary hypoplasia. Serial amnioinfusions have shown promise in mitigating pulmonary disease from bilateral renal agenesis, but efficacy and safety for other causes of fetal kidney failure are unproven. OBJECTIVE: To assess the efficacy of serial amnioinfusions initiated before 26 weeks' gestation to mitigate lethal pulmonary hypoplasia in cases of anhydramnios due to fetal kidney failure. DESIGN, SETTING, AND PARTICIPANTS: Prospective, nonrandomized clinical trial conducted at 13 US fetal therapy centers from December 2018 t

Red flags in asymmetric axonal neuropathy revealing MPO-ANCA-associated microscopic polyangiitis: a case report.

BACKGROUND: Mononeuritis multiplex is a recognized, though sometimes overlooked, clinical feature of myeloperoxidase antineutrophil cytoplasmic antibody (MPO-ANCA)-associated vasculitis. When renal or pulmonary involvement is initially mild, the condition may mimic common neurological disorders and diagnosis can be delayed. CASE PRESENTATION: A 51-year-old woman presented with a five-month history of progressive bilateral lower-limb pain and edema, followed by three weeks of rapidly progressive asymmetric weakness affecting all four limbs. Nerve conduction studies showed severe multifocal axo

Bilateral Simultaneous Patellar Tendon Rupture in an Adolescent Athlete: A Case Report and Literature Review Highlighting the Role of Vitamin D Deficiency.

Simultaneous bilateral patellar tendon rupture is an exceptionally rare injury in the paediatric population. While bilateral patellar tendon rupture in adults is frequently associated with systemic disease or corticosteroid exposure, the few reported paediatric cases have been linked to localised tendinopathy, connective tissue disorders, or repetitive mechanical stress. This report presents a case of simultaneous bilateral patellar tendon rupture in a 15-year-old male following a rugby-related injury. Biochemical investigation revealed severe vitamin D deficiency with secondary hyperparathyro

A Rare Presentation of Chronic Nonsteroidal Anti-inflammatory Drug-induced Acute-on-chronic Kidney Disease with Secondary Hyperaldosteronism and Hyperparathyroidism in a Young Male with Refractory Hypertension.

Refractory hypertension in young adults warrants evaluation for secondary causes. Chronic nonsteroidal anti-inflammatory drug (NSAID) use, though common, is an underrecognized cause of chronic kidney disease that may lead to biochemical abnormalities, including electrolyte disturbances and hormonal dysregulation. A 37-year-old male presented with headache, vomiting, and abdominal pain. Examination revealed severe hypertension (210/190 mmHg) with epigastric tenderness. Electrocardiography (ECG) showed a Wellens type A pattern with Q waves. Initial laboratories revealed hyponatremia, hypokalemia

Bilateral, non-syndromic second branchial cleft fistula in an 8-year-old child: a case report.

BACKGROUND: Second branchial cleft anomalies are the most common congenital cervical malformations, accounting for approximately 90% of branchial cleft lesions. They usually present unilaterally, while bilateral involvement is exceedingly rare and most often associated with genetic syndromes such as branchio-oto-renal (BOR) syndrome. CASE PRESENTATION: We report the case of an 8-year-old Moroccan boy with no relevant past medical history who presented with bilateral, low-lying paramedian cervical fistulas that had been recurrently infected since early childhood. Clinical examination revealed

Severe Hypothyroidism Presenting as Rhabdomyolysis and Acute Kidney Injury: A Report of a Rare Case.

Hypothyroidism is a rare cause of rhabdomyolysis and an even less common cause of acute kidney injury (AKI). Because the symptoms may be mild or non-specific, the diagnosis can easily be overlooked. We report the case of a 44-year-old man with no significant past medical history who was admitted after renal impairment was discovered on routine blood testing. On clinical examination, he had sinus bradycardia, puffy facies with bilateral periorbital edema, Hertoghe sign, muffled heart sounds, psychomotor slowing, and constipation. Laboratory investigations showed stage II AKI, severe rhabdomyoly

Combined impact of smoking, alcohol and social inequalities on premature mortality in urban India.

BACKGROUND: Vitamin A and D are essential for the proper growth and development of a child. Due to the complex political circumstances in the state of Palestine, research on micronutrient deficiency is scarce. METHODS: The Palestinian Ministry of Health (MOH) and UNICEF conducted a national cross-sectional survey in 2013 after the implementation of various micronutrient supplementation and fortification programs. Risk factors for levels of vitamin A (n = 1054) and vitamin D (n = 150) were assessed among children aged 6 to 59 months using chi-square tests and logistic regression with each of t

Concurrent thalamic and cerebellar involvement in pediatric neuropsychiatric lupus: Expanding the radiologic spectrum.

BackgroundNeuropsychiatric systemic lupus erythematosus (NP-SLE) in children is uncommon but often severe, with a wide spectrum of neurologic presentations and sometimes atypical or initially normal neuroimaging. Among reported manifestations, acute cerebellitis is rare, and simultaneous thalamic involvement has scarcely been described. Differentiating inflammatory NP-SLE from mimics such as posterior reversible encephalopathy syndrome (PRES), infection, or ischemia is essential, since management strategies and prognosis differ substantially.CaseWe describe a 13-year-old female with newly diag

Acute Q Fever Presenting with Severe Cholestatic Jaundice and Acute Kidney Injury in the Absence of Classic Exposure: A Rare Case Report.

Q fever, caused by Coxiella burnetii, is a zoonotic infection with highly variable clinical manifestations, often leading to diagnostic delays. Although typically associated with livestock exposure and presenting with nonspecific febrile illness or hepatitis, its clinical spectrum is broad. We report an unusual case of a 53-year-old previously healthy male who presented with high-grade fever, epigastric pain radiating to both upper quadrants, marked jaundice, severe acute kidney injury, and dark urine. Initial imaging revealed bilateral perinephric and periureteral stranding, gallbladder wall

Monospecific antibodies reveal reduction of granulins in frontotemporal dementia caused by GRN mutations.

PURPOSE: Primary open-angle glaucoma (POAG) is a common ocular disease, associated with abnormalities in aqueous humor circulation and an increase in intraocular pressure (IOP), leading to progressive optical neuropathy and loss of vision. POAG pathogenesis includes alterations of the structural properties of the sclera, especially in the optic nerve head area, contributing to the degeneration of the retinal ganglion cells. Abnormal sclera biomechanics hinder adequate compensation of IOP fluctuations, thus aggravating POAG progression. The proteomic basis of biomechanical disorders in glaucoma

Porcelain bladder with ascending ureteral calcifications and seminal vesicle calcifications: CT imaging features of advanced urogenital schistosomiasis.

Urogenital schistosomiasis remains endemic in many regions of Africa and is a well-recognized cause of chronic lower urinary tract pathology. Longstanding infection may lead to bladder wall fibrosis, calcification, ureteral strictures, and hydronephrosis. We report the case of a middle-aged man presenting with chronic lower urinary tract symptoms and intermittent hematuria. Non-contrast computed tomography demonstrated circumferential bladder wall calcification consistent with a porcelain bladder, associated with bilateral ureteral mural calcifications and right-sided ureteropyelocaliceal dila

Perioperative Continuous Glucose Monitoring During Resection of a Giant Intrathoracic Solitary Fibrous Tumour Associated With Doege-Potter Syndrome.

A man in his late 60s with hypertension and a history of heavy smoking presented with a large left thoracic mass. A biopsy confirmed a solitary fibrous tumour. Before his scheduled operation, he collapsed; his blood glucose level was 40 mg/dL, consistent with insulin-like growth factor-II-mediated hypoglycaemia (Doege-Potter syndrome). Continuous glucose monitoring was initiated to characterize perioperative glucose trends and support glucose management. As his consciousness and oral intake improved with supportive care and adjunctive corticosteroids, his glucose level stabilized, allowing gra
