OBJECTIVE: To determine the frequency of fetal urinary tract anomalies and to characterize the types of such abnormalities detected on ultrasonography and the outcome of affected patients during a 15-year period at our institution.
DESIGN: We retrospectively reviewed the findings on maternal prenatal ultrasound examinations and the postnatal medical records of 56 children with urinary tract abnormalities detected by prenatal ultrasound examination at Mayo Clinic Rochester from November 1979 to June 1994.
RESULTS: Of the 56 children, 18 (32%) had severe urinary tract anomalies in conjunction

Once viewed as hopelessly incurable disorders and the dustbin for careers in academic medicine, the polycystic kidney diseases have emerged as prime targets of pathophysiologic study and palliative and definitive treatment in the era of molecular medicine. Polycystic kidney disease (PKD) may be hereditary or acquired. The major inherited types are autosomal dominant (AD) and autosomal recessive (AR). ADPKD is caused by at least two (and possibly three) genes located on separate chromosomes, while ADPKD-1 is due to a 14 kb transcript in a duplicated region on the short arm of chromosome 16 very

Bilateral renal agenesis (Potter's sequence) is usually a sporadic occurrence. Recurrence in the same family is rare. A family study of bilateral and unilateral renal agenesis was undertaken in three cases of renal agenesis in the same family. The grand-father and the father have a unilateral renal agenesis and his two stillborn infants had a Potter's syndrome. The author considered the familial nature of congenital absence of the kidneys and the relationship between hereditary bilateral renal agenesis and unilateral renal agenesis.

Out of 13,252 cases in which fetal bilateral echogenic kidneys were detected by transvaginal sonography between 12 and 18 weeks' gestation, there were nine fetuses where oval hypoechogenic masses were detected in the renal bed. In five fetuses where hypoechogenic masses in the renal bed were sonographically visualized, postabortal examination was compatible with renal agenesis and the hypoechogenic masses proved to be enlarged adrenals. In three additional cases, unilateral renal agenesis was accompanied by unilateral enlarged adrenals, radiologically confirmed postnatally. In one case, a fals

We report on a male twin born with no functional renal tissue and without the extrarenal manifestations of Potter facies, skin changes, club feet and pulmonary hypoplasia. The monoamniotic co-twin had normal renal function, thereby maintaining sufficient amniotic fluid to avoid the classic presentation of Potter's syndrome. Prenatal diagnosis of this condition allowed prompt confirmation of these findings and support for the parents without unnecessary intervention. This experiment of nature demonstrates the necessity of normal renal function in the maintenance of amniotic fluid and its relati

Renal tubular dysgenesis (RTD), with hypoplasia especially of renal proximal convoluted tubules and clinical neonatal anuria or oliguria, has been reported as a congenital familial (autosomal recessive) disease, variably with features of oligohydramnios, Potter syndrome, or pulmonary hypoplasia. A similar tubular lesion due to antenatal tubular atrophy has been reported for conjoined twins with twin-twin transfusion syndrome or acardia and in infants of mothers given antihypertensive agents, including angiotensin-converting enzyme (ACE) inhibitors, during pregnancy, and it has been seen as a u

In our study, complications of oligohydramnios were examined. In medical records of all 5,210 deliveries of our department between 1987 and 1990 181 cases of oligohydramnios were identified. The diagnosis was confirmed by ultrasound and based on the method described by Mannings and Platt: amniotic fluid depots of less than 1 cm were defined as oligohydramnios. All 181 cases met the above definition, which results in an incidence of 3.5% pregnancies with reduced amniotic fluid. We examined the records of mother and foetus, the delivery and perinatal complications and follow-up of the newborn. I

To study the effects of cocaine use in pregnancy in Amsterdam, clinical data on cocaine-using pregnant women (n = 21) and their offspring (n = 23) were obtained retrospectively (1987-1994) at the Academic Medical Center, Amsterdam. Infants exposed to cocaine had a median gestational age of 39 weeks and a median birth weight of 3090 g. There were six preterm infants, two small-for-gestational-age infants and five infants with a small head circumference. Three infants had a congenital malformation. One infant (Potter's syndrome) died shortly before birth. One infant had congenital syphilis, four

OBJECTIVE: To study fetuses who had the sonographic diagnosis of hemivertebrae made in utero (excluding open neural tube defects) and to determine the incidence of associated anomalies, karyotypes, and neonatal outcome.
METHODS: Twenty-seven fetuses with sonographically detected vertebral abnormalities (excluding neural tube defects) and postnatal follow-up were included in the study. Associated structural defects and karyotypic anomalies were tabulated. The outcome for fetuses with isolated vertebral abnormalities on prenatal sonogram was compared with that of fetuses with associated anomali

Potter's syndrome is a rare disorder but on which should not be overlooked. Ultrasound signs such as oligoaminos, retarded intra-uterine growth. (R.I.U.G.) and failure to detect the bladder and kidneys constitute fairly clear signs. Early and specific diagnosis leads to therapeutic pregnancy termination. This allows the parents to avoid the painful and late birth of a malformed child.

Histopathological findings in seven temporal bones from four infants diagnosed as Potter's syndrome are described. The infants were labelled as Potter's syndrome after autopsy confirmed bilateral renal agenesis. Extrarenal manifestations included pulmonary hypoplasia and facial anomalies. The middle ear volume in infants with Potter's syndrome remained unchanged with age resulting in a significantly higher percentage of residual middle ear mesenchyme in these infants as compared to normal infants. Dehiscence of the facial nerve was seen in all the temporal bones studied. One ear showed the pre

Defects of neuronal migration were detected in the brains of five unrelated infants with Potter syndrome (oligohydramnios sequence). These consisted of abnormal lamination of cerebral cortex, white matter heterotopias, and meningeal and molecular zone neuronal-glial ectopias. Besides, various other brain anomalies were sometimes found. They comprised one or more of the followings: abnormal gyration patterns (gyral fusion, cerebellar microgyria), cerebellar granule and Purkinje cell heterotopias, brain stem heterotopias, adysplasia of basal ganglia, gliosis, mineralization, and hydrocephalus. We describe an autopsy case of congenital cystic adenomatoid malformation of the lung (CCAM) associated with bilateral renal agenesis. Prenatal ultrasound examination showed additional left heart hypoplastic syndrome. A therapeutic abortion was induced at 23 weeks of gestation. The association CCAM-bilateral renal agenesis is a rare condition (5 cases previously described) which has to be known because of the mitigation effect of the CCAM on the oligohydramnios determined by bilateral renal agenesis. However, this instance is usually associated with oligohydramnios. The pathogenesis of polyhyd

Potter's sequence, commonly known as Potter's syndrome, may be due to very different pathogenetic and etiological conditions. The phenotype and the pathogenesis are described. The heredity differs, depending on the etiology. The result of an ultrasound investigation of a family shows the probably most common mode of heredity: autosomally dominant with lowered penetration and variable expressivity. When genetic counselling is required, meticulous examination of the foetus and the family is recommended in order to obtain a basis for counselling of the family. Future pregnancies in the parents an

The authors present a case of a 73 year old female patient. She had a left side pleural mesothelioma and had been operated thrice. At the third operation an abdominal propagation joined to the thoracic mesothelioma. Starting from this third surgery severe hypoglycemic episodes illustrated the clinical pictures. The electronmicroscopy of the resected tumor mass documented the presence of neurosecretory granules. This strongly suggests that tumor mass produced a substance which could have been responsible for the hypoglycemic episodes. Due to the fact the insulin levels measured during severe hy

The occurrence of hypoglycemia with an intrathoracic tumor was reported in 1930 independently by Doege and Potter. Since then, the presence of a nonpancreatic tumor associated with low blood sugars has often been referred to as the Doege-Potter syndrome. We report a patient with a recurrent solitary fibrous tumor of the pleura who experienced symptomatic hypoglycemia attributable to his tumor.

The 58 cases of bilateral renal agenesis (Potter syndrome) registered in the Genetic Counselling unit of our institute in the last 12 years are reviewed. The only familial recurrent case which has been prenatally diagnosed is described in detail. A urinary bladder anomaly like that of the subsequent third child has not been previously reported. The authors analyze the possible inheritance patterns. They suggest the malformation is a genetically heterogeneous entity. They emphasize that nowadays the birth of a newborn with bilateral renal agenesis can be prevented in all cases.
