Retroperitoneal solitary fibrous tumour with Doege-Potter-Syndrome - successful interventional therapy.

BACKGROUND: Solitary fibrous tumours (SFTs) are rare mesenchymal neoplasms that may cause insulin-independent hypoglycaemia (Doege-Potter-syndrome). Although surgical resection is the standard treatment, non-surgical options may be required for elderly or multimorbid patients. CASE PRESENTATION: An 82-year-old woman presented with severe nocturnal hypoglycaemia (<40mg/dl) and progressive weakness. Imaging revealed a large hypervascular retroperitoneal mass. Laboratory findings showed low insulin and C-peptide levels with an IGF-II/IGF-I ratio of 9.3, indicating paraneoplastic hypoglycaemia. B

Surgical resection of a solitary fibrous tumour causing refractory hypoglycaemia: Doege-Potter syndrome.

Solitary fibrous tumours (SFTs) are rare soft tissue masses that are often clinically silent until they cause mass effect. A paraneoplastic syndrome manifesting as persistent hypoglycaemia, termed Doege-Potter syndrome (DPS), can be associated with these lesions. Surgical treatment is recommended for the management of these tumours.A case of a man in his 60s who was admitted for altered mental status and hypoglycaemia is herein described. Physical examination revealed a distended, asymmetric abdomen with a firm palpable mass in the left upper quadrant. Imaging demonstrated a 24 cm abdominal ma

Recent progress in xenotransplantation and its application to pediatric kidney disease.

Patients with kidney failure require dialysis or kidney transplantation. Kidney transplantation offers great benefits, including reduced mortality; however, many patients who wish to undergo kidney transplantation are unable to do so due to a shortage of donor organs. This shortage is a global issue, and xenotransplantation has emerged as a potential solution. The history of xenotransplantation is characterized by overcoming the immunological challenge of hyperacute rejection. Recently, breakthroughs such as gene-edited pigs and novel immunosuppressants have successfully lowered rejection rate

Refractory Hypoglycaemia Caused by a Pleuropulmonary Solitary Fibrous Tumour: A Case of Doege-Potter Syndrome.

Hypoglycaemia has a very broad differential diagnosis. It is important to perform a thorough examination of the possible causes. Doege-Potter syndrome is a rare paraneoplastic syndrome that is associated with refractory hypoglycaemia and is caused by solitary fibrous tumours (SFTs). Diagnosis can be suspected based on imaging and clinical presentation, but the definitive diagnosis requires histologic confirmation. Most SFTs are located in the thorax. Therefore, imaging plays a major role in the diagnosis. CT scan should be performed when diaphragm elevation is seen on X-ray to exclude underlyi

[Rare cause of recurrent hypoglycemia].

Non-islet cell tumor hypoglycemia (NICTH) is a rare cause of hypoglycemia, resulting from excessive secretion of insulin-like growth factor 2 (IGF-2) or pro-IGF‑2, which leads to subsequent activation of the insulin receptor. Doege-Potter syndrome is often used synonymously with NICTH, especially in cases with fibrous primary tumors in the thorax. In the current patient, tumor progression was observed over a short period without the possibility of debulking surgery. Therefore, oral therapy with prednisolone was initiated. Treatment with diazoxide or the somatostatin analogue octreotide is not

Seizures caused by a solitary fibrous tumour of the pleura: an unusual presentation of Doege-Potter syndrome.

Doege-Potter syndrome (DPS) is a rare condition that causes hypoinsulinaemic hypoglycaemia due to solitary fibrous tumours (SFTs). This case report details the treatment of a man in middle adulthood (ages 40-65) diagnosed with DPS after experiencing syncope and vasovagal episodes, which led to the discovery of a large mass in the pleura. Following various diagnostic tests confirming the SFT, the patient required anticonvulsant therapy and glucose infusions due to recurrent seizures and severe hypoglycaemia prior to surgery. After the surgical removal of the tumour, both hypoglycaemia and the n

Distinct Ultrasound Features of Giant Solitary Fibrous Tumour of the Pleura: A Case Report.

Solitary fibrous tumour of the pleura (SFTP) accounts for < 5% of pleural tumours, and the ultrasound (US) features of giant SFTP (> 20 cm) remain unknown. We present the case of a woman diagnosed with giant SFTP associated with Doege-Potter syndrome, who experienced dyspnea and recurrent hypoglycemia. Chest x-ray revealed a large left hemithorax opacity, initially misdiagnosed as massive pleural effusion, leading to an inappropriate thoracentesis. Subsequent US demonstrated a large heterogeneous mass with intralesional cystic changes and calcifications, which lacks respiratory movement. Compu

Case Report: Doege-Potter syndrome: a giant solitary fibrous pleural tumor causing severe hypoglycemia.

Doege-Potter syndrome or non-islet cell tumor hypoglycemia (NICTH) is a rare entity usually due to a solitary fibrous tumor of the pleura (pSFT). The diagnosis of NICTH is challenging because of its non-specific clinical presentation and the rarity of pSFT. We present a case report of a 44-year-old woman diagnosed with NICTH to demonstrate the difficulty of establishing this diagnosis and the importance of preoperative preparation based on the characteristics of the lesion. We performed an embolization followed by an en bloc resection with diaphragmatic patch and lung wedge resection, and we s

Solitary Fibrous Tumor With Doege-Potter Syndrome Successfully Treated With Preoperative Transcatheter Arterial Embolization and Complete Excision; A Case Report.

INTRODUCTION: Doege-Potter syndrome (DPS) is a rare paraneoplastic phenomenon of severe hypoglycemia associated with solitary fibrous tumors (SFT). We report a case of a giant pelvic SFT with DPS, which was managed with preoperative arterial embolization and complete excision. CASE PRESENTATION: This report describes the case of a 77-year-old patient with persistent hypoglycemia and a giant pelvic mass. He required continuous total parenteral nutrition (TPN) for severe hypoglycemia. CT showed a giant hypervascular mass (20 × 18 × 15 cm) in the pelvic space. Tumor biopsy showed SFT. To avoid i

The Volume of Fluid Corresponding to Each Centimeter of Amniotic Fluid Index Changes throughout Pregnancy: A Prospective Cohort Observational Study.

INTRODUCTION: The normal values of the amniotic fluid index (AFI) throughout pregnancy have been established as 5-24 cm. However, a similar AFI may correspond to a different AF volume at different gestational ages. The aim of this study was to calculate the amount of fluid corresponding to each cm of AFI when a known volume of fluid is infused into the uterine cavity. METHODS: This was a secondary analysis of a prospective cohort observational study. Serial amnioinfusions (AIs) were performed between 10 and 34 weeks of gestation in 19 fetuses with anhydramnios due to severe renal damage. The

Paraneoplastic Hypoglycaemia in a Patient With Solitary Fibrous Tumor of Pleura.

Solitary fibrous tumours (SFTs) are rare spindle cell neoplasms, predominantly benign, comprising less than 2% of all soft tissue masses. In rare cases, SFTs can lead to Doege-Potter syndrome, a paraneoplastic condition characterised by hypoglycaemia due to elevated levels of insulin-like growth factor II (IGF-II). Patients typically present with symptomatic hypoglycaemia related to this hormonal dysregulation. We report the case of a 70-year-old man who presented with weight loss, cough, fatigue, tingling, and episodes of confusion associated with recurrent hypoglycaemia. Chest radiography re

Doege-Potter syndrome in a patient with solitary fibrous tumor of the retroperitoneum: a case report and literature review.

Doege-Potter syndrome in solitary fibrous tumor of the retroperitoneum is rare. Here, we present a 45 years old Chinese male patient who was admitted to our center after discovering an abdominal mass for 5 years and intermittent episodes of hypoglycemia for 3 months. Imaging examinations confirmed a 19.2*12.3*22.9 cm mass in the abdominal cavity. Through multidisciplinary meeting and rapid preparations, the tumor was completely removed and patient's hypoglycemia symptoms disappeared. Total operation time was 130 min, and total blood loss during surgery was 10 mL. Post operative pathology resul

Doege-Potter Syndrome: A Solitary Fibrous Tumor Causing Non-Islet Cell Tumor Hypoglycemia.

Doege-Potter syndrome occurs when incompletely processed insulin-like growth factor 2 (IGF-2), also known as big IGF-2, is produced by a solitary fibrous tumor (SFT) and results in non-islet cell tumor hypoglycemia (NICTH). We discuss here the case of a 66-year-old male who presented with a 2-week history of increasing confusion and a serum glucose of 34 mg/dL. The patient's symptoms immediately improved with dextrose. The patient did not use insulin, serum sulfonylurea screen was negative, and testing for adrenal insufficiency was unremarkable. Outpatient laboratory evaluation revealed a seru

Recurrent Falls Due to Hypoglycemia: Case of an IGF-2-producing Fibrous Tumor of the Pleura.

This case report delineates the clinical presentation of a 77-year-old male who experienced falls and sustained a humerus fracture attributed to hypoglycemia. Despite the absence of insulin use and normal laboratory results for cortisol, TSH, blood count, and liver and kidney function, a fasting test revealed diminished C-peptide and insulin levels, ruling out insulinoma, exogenous insulin use, or β-cell hyperplasia. Subsequent laboratory investigations demonstrated lowered IGF-1 and elevated IGF-2 levels, indicative of an IGF-2-producing tumor as the etiology of the hypoglycemia. A positron e

[Rapidly growing giant solitary fibrous tumor of the pleura].

Solitary fibrous tumor of the pleura is a relatively rare tumor originating from the mesencymal cells. Most patients, even with large tumor sizes, are completely asymptomatic for a long time. They are usually benign, but local recurrence is common, so it is especially important to remove them with an intact edge. A 77-year-old male patient had a chest X-ray for shortness of breath. A chest CT scan described a right-sided, sharp-edged, dorsally located mass above the aperture. A transthoracic biopsy was performed which confirmed a solitary fibrous tumor of the pleura. During surgery, a 17 × 16
