Modulation of cyst growth in autosomal dominant polycystic kidney disease: mechanistic insights and therapeutic opportunities.

Autosomal dominant polycystic kidney disease (ADPKD) is one of the most common inherited kidney disorders and is characterized by the progressive formation and expansion of fluid-filled cysts, ultimately leading to kidney failure. Although caused by reduced dosage of the polycystin proteins, the disease phenotype arises from a broad disruption of epithelial physiology rather than a single linear pathway. Loss of polycystin function destabilizes epithelial homeostasis and sensitizes cyst-lining cells to proliferative and secretory cues. A central consequence is the emergence of a self-reinforci

β-Hydroxybutyrate: a renoprotective hormone in polycystic kidney disease.

Autosomal dominant polycystic kidney disease (ADPKD) is a common genetic form of chronic kidney disease characterized by highly variable rates of progression. This variability suggests that nutritional, lifestyle, and environmental factors may significantly influence disease course. Despite its prevalence, pharmacological treatment options for ADPKD remain very limited, underscoring the urgent need for safe and effective therapeutic strategies. Recent research has identified ADPKD as a metabolic disorder at the cellular level, marked by impaired mitochondrial function and a pathological relian

Are Renal Cysts a Barrier to Donation and Kidney Transplant?

OBJECTIVES: We evaluated the long-term outcomes of kidney transplants performed using grafts that contain simple renal cysts, to determine whether these cysts constitute a barrier to kidney donation or transplant. MATERIALS AND METHODS: All kidney transplants performed by our team from February 2012 to May 2025 were retrospectively reviewed. Cases in which the graft contained at least 1 simple renal cyst were included. Demographic characteristics, cyst size, perioperative complications, postoperative renal function, and cyst size during follow-up ultrasonography were recorded. Postoperative s

Fracture risk assessment in early-stage autosomal dominant polycystic kidney disease: a cross-sectional study using FRAX®, QFracture®, and Garvan models.

BACKGROUND: Compared with individuals with other chronic kidney disease (CKD), individuals with autosomal dominant polycystic kidney disease (ADPKD) may face increased fracture risk. This study provides the first clinical evaluation of fracture risk prediction in early-stage ADPKD using the FRAX®, QFracture®, and Garvan models. METHODS: In this cross-sectional study, adult ADPKD patients with preserved kidney function were screened for fracture risk. Due to age eligibility criteria and data availability, fracture risk estimates could be calculated in 67 individuals using QFracture®, in 46 usi

Deep learning-supported image quantification of epithelial cell shapes and its application to polycystic kidney disease.

Cell shape is a fundamental determinant of tissue architecture and organ function. In epithelial tissues, cytoskeletal organization and apical junctions regulate cell geometry, shaping functional tissue units. Disruption of these mechanisms is associated with diseases such as autosomal dominant polycystic kidney disease (ADPKD), in which epithelial organization is altered leading to cyst formation. Quantitative analysis of epithelial morphology can provide mechanistic insight, but existing approaches are often manual, low-throughput, and difficult to standardize. Here, we present a fully autom

Papillary Hyperplasia in Autosomal Dominant Polycystic Kidney Disease: A Multi-Institutional Study With Immunohistochemical and Molecular Characterization.

Renal cysts in autosomal dominant polycystic kidney disease (ADPKD) frequently harbor small intracystic epithelial proliferations that arise in continuity with the cyst lining and are distinct from other recognized proliferative lesions or well-defined renal tumors and lack a formal designation. We aim to provide the histomorphologic, immunohistochemical (IHC), and molecular characteristics of these lesions, determine their incidence, and employ the term papillary hyperplasia (PH), consistent with the recent International Society of Urological Pathology (ISUP) consensus meeting report on precu

Single-Molecule Real-Time Sequencing for MUC1 Variable Number of Tandem Repeat Variation to Improve Autosomal Dominant Tubulointerstitial Kidney Disease Diagnosis.

KEY POINTS: Single-molecule real-time sequencing with the PacMUC1 script resolved exact MUC1 variable tandem repeat structure and full allelic variation. In 300 individuals, the protocol identified 215 distinct MUC1 tandem repeat alleles with 80 repeat units and nine frameshift mutation types. Probe extension assay identified 90% of families with frameshift mutations, detection of frameshifted mucin-1 aided genetically unresolved cases. BACKGROUND: ADTKD- MUC1 is caused by frameshift mutations in the MUC1 gene, producing a frameshifted neoprotein (MUC1fs) toxic to kidney cells. The gene's var

Adult Diagnosis of Solitary Kidney and Renal Dysplasia in a Male Born Prematurely as a Twin: A Case Report.

BACKGROUND: Congenital anomalies of the kidney and urinary tract (CAKUT) are a common cause of chronic renal disease and usually manifest either antenatally or in the early years of life. Solitary Kidney with Renal Dysplasia presents early in life due to regular antenatal sonographic scans. The rarity of diagnosis being deferred until adulthood has important implications when there is prematurity and other associated congenital anomalies, such as Congenital Heart Disease. CASE PRESENTATION: We present a 23-year-old male experiencing sequential generalized edematous changes, decreased urine ou

Meckel-Gruber syndrome: a rare and fatal congenital disorder (case report).

Meckel-Gruber syndrome is a rare congenital disorder characterized by multiple malformations. It transmits via a recessive autosomal mode. It is characterized by an occipital encephalocele, polydactyly, and polycystic renal dysplasia. The diagnosis could be established via ultrasound. In fact, it is actually the key method for the early screening of this lethal malformation with at least two of its main characteristics. However, the diagnosis is confirmed by karyotype analysis. Herein, we present a case of Meckel-Gruber syndrome diagnosed in a fetus from a consanguineous marriage in a 40-year-

Vaptans: A Narrative Review of Pharmacokinetics, Pharmacogenetics, and Clinical Applications.

Vaptans are non-peptide arginine vasopressin (AVP) receptor antagonists essential for modulating water homeostasis. This review evaluates their chemical evolution from peptide analogs to orally bioavailable molecules and summarizes the pharmacokinetic profiles of key agents, including tolvaptan, conivaptan, and mozavaptan. Significant emphasis is placed on the clinical role of tolvaptan as a disease-modifying therapy in autosomal dominant polycystic kidney disease (ADPKD) and its efficacy in syndrome of inappropriate antidiuretic hormone secretion (SIADH). Furthermore, the impact of pharmacoge

Premature Rupture of Membranes after Serial Amnioinfusions in Fetuses with Early Anhydramnios due to Severe Renal Disease/Lower Urinary Obstruction.

INTRODUCTION: This study estimates the frequency of preterm premature rupture of membranes (PPROM) in fetuses with early anhydramnios due to severe renal or urinary disease treated with serial amnioinfusions (AIs). METHODS: Fetuses diagnosed with early anhydramnios (<25 weeks gestation) secondary to severe renal disease or urinary tract obstruction underwent serial AIs at two academic centers, to reduce uterine compression and prevent pulmonary hypoplasia. Fetuses with structural or chromosomal anomalies were excluded. AIs were performed to maintain an amniotic fluid index (AFI) between 15 an

Transient neonatal diabetes mellitus as an early diagnostic clue to HNF1B-related disease - two case reports and a literature review.

BACKGROUND: Pathogenic variants in the HNF1B gene cause a multi system disorder encompassing organ abnormalities-primarily affecting the kidneys and pancreas-as well as metabolic disturbances, collectively referred to as HNF1B-related disease. While maturity-onset diabetes of the young type 5 is a well-recognized manifestation, neonatal diabetes mellitus (NDM) associated with HNF1B is exceedingly rare, and has only been reported in patients harboring single nucleotide variants. CASE PRESENTATION: We describe two unrelated female children presenting with transient NDM caused by a complete HNF1

Double Threat in a Newborn: High Anorectal Malformation with Pouch Colon and Bilateral Multicystic Dysplastic Kidneys - A Rare Case Report.

The coexistence of bilateral multicystic dysplastic kidneys (MCDKs) and high anorectal malformation with pouch colon is exceptionally rare. The combination results in a nonsurvivable condition due to complete renal failure, posing complex diagnostic and management challenges. A full-term male neonate presented with absent anal opening, limb and auricular deformities, and progressive anuria since birth. Antenatal history was unavailable. Clinical examination revealed bilateral clubfoot and normal male genitalia. Laboratory evaluation showed steadily worsening renal function. Ultrasound demonstr

An unusual segmental retinal venous abnormality detected during retinopathy of prematurity screening in an infant with a PAX2 variant.

PURPOSE: Congenital abnormalities of retinal venous development are rare and may be difficult to distinguish from prematurity-related vascular changes when detected during retinopathy of prematurity screening. In premature infants, atypical posterior pole vascular findings should not be automatically attributed to retinopathy of prematurity alone. The purpose of this report is to describe an unusual segmental retinal venous abnormality detected during retinopathy of prematurity screening in a premature infant with a PAX2 variant and renal dysplasia. METHODS: We report a premature male infant

Detection of the NPHP4 c.2999_3005del (p.Asn1000SerfsTer4) variant in an Iranian family with nephronophthisis-4.

Nephronophthisis type 4 (NPHP4) is a rare genetic kidney disorder progressing to end-stage renal disease (ESRD). In this study, we aimed to identify the genetic cause of NPHP4 in a pedigree with three affected individuals. Whole-exome sequencing was performed on the proband, and variants were filtered, analyzed, and evaluated using in silico tools. Co-segregation analysis was conducted using Sanger sequencing. Protein modeling for the wild-type and mutant forms was performed using AlphFold3. We identified a homozygous deletion (c.2999_3005delTGTGTGT/ p.Asn1000SerfsTer4) in exon 21 of NPHP4, wh

Can MQ232 guide to novel options for treatment of ADPKD?

Wang et al. showcase a green mamba-derived compound, MQ232, which interferes with cyst growth in a mouse model of autosomal dominant polycystic kidney disease, similar to the only approved drug for autosomal dominant polycystic kidney disease treatment, tolvaptan. However, the authors reveal cyclic adenosine monophosphate-dependent and -independent components in MQ232's mode of action and gender differences in efficacy. Therefore, MQ232 may pave the way to novel treatment options for autosomal dominant polycystic kidney disease.
