PKHD1 biallelic defects caused Severe Hepatic Fibrosis and polycystic kidney: Identification of a novel pathogenic variant in a Chinese family.

BACKGROUND: Autosomal recessive polycystic kidney disease (ARPKD) is a rare hereditary ciliopathy, clinically characterized by fibrocystic changes in the kidneys and liver. The disease is manifested by hepatic fibrosis and significantly enlarged, echogenic kidneys with fusiform dilation of the collecting ducts. ARPKD is predominantly associated with homozygous or compound heterozygous mutations in the PKHD1 gene, which encodes the protein fibrocystin. CLINICAL CASE: In this study, we report a case involving a young Chinese female who presented with recurrent upper gastrointestinal bleeding, a

Identification of Pathogenic PKHD1 Variants in Infants with Autosomal Recessive Polycystic Kidney Disease from the Dhofar Region, Oman.

BACKGROUND: Autosomal recessive polycystic kidney disease (ARPKD) is a rare, inherited disorder primarily affecting the kidneys and liver. Disease-causing variants in PKHD1 lead to a disruption of the encoded protein fibrocystin/polyductin. This study aims to identify disease causing variants in PKHD1 in families from the Dhofar region of Oman. METHODS: We conducted a case series of six families with antenatal diagnoses of ARPKD and postnatal deaths. Genetic testing was performed on neonates using Sanger sequencing and next-generation sequencing (NGS) to detect variants in PKHD1. In silico an

Investigation of Urinary Extracellular Vesicles as Novel and Safe Therapeutics for Autosomal Recessive Polycystic Kidney Disease (ARPKD).

Autosomal recessive polycystic kidney disease (ARPKD) is a severe inherited disorder caused primarily by mutations in PKHD1 and in a minority of cases, CYS1. These genes encode fibrocystin and cystin, respectively. ARPKD typically manifests in infancy with enlarged kidneys, progressive cyst formation, and an estimated peri-natal high mortality rate of 20%. Given the lack of efficient therapies and the genetic complexity of many rare diseases such as ARPKD, strategies that restore functional proteins defective in the disease may offer a disease-modifying approach. Urinary extracellular vesicles

Disruption of the human cystin-1 myristoyl-electrostatic switch causes polycystic kidney disease that phenocopies autosomal recessive polycystic kidney disease.

INTRODUCTION: Autosomal recessive polycystic kidney disease (ARPKD) is caused primarily by pathogenic variants in PKHD1, encoding fibrocystin/polyductin. In Cys1cpk/cpk (cpk) mice, the kidney and liver lesions closely phenocopy ARPKD. Cys1 encodes cystin, a myristoylated protein that traffics to the primary cilium and nucleus. We recently reported the first patient with ARPKD due to a homozygous CYS1 splicing variant. METHODS: To study this, we utilized high-resolution ultrasound, array Comparative Genomic Hybridization and exon sequencing to identify an ARPKD-like kidney lesion and CYS1 defe

Deciphering the Impact of RAC1-SPTAN1 in ARPKD Cystogenesis Using Multifaceted Models.

Autosomal recessive polycystic kidney disease (ARPKD) leads to severe renal cysts and progressive kidney dysfunction, with no approved treatments. The absence of such cystic phenotypes in Pkhd1-/- mice underscores the need for novel models that better recapitulate the human disease. We developed kidney organoid-on-chip models that mimic patients' distal-nephron cysts, identifying RAC1/c-FOS as potential therapeutic targets. However, critical questions remain regarding RAC1 activation during cyst formation, cyst origins, and underlying molecular mechanisms. Using a multifaceted approach, organo

Next-Generation Sequencing Defines a Molecularly Confirmed ARPKD Core Within the Broader PKHD1-Associated Disease Spectrum.

Background/Objectives: Autosomal recessive polycystic kidney disease (ARPKD) is a severe ciliopathy caused by biallelic pathogenic variants in PKHD1, characterized by variable renal and hepatobiliary involvement. The widespread use of next-generation sequencing (NGS) has revealed a large number of rare PKHD1 variants, creating major challenges in distinguishing molecularly confirmed ARPKD from a broader spectrum of PKHD1-associated disease. Methods: We performed an integrated clinical and molecular analysis of 68 individuals referred for suspected ARPKD. Using phase-aware and family-informed A

Genetic and Clinical Evaluation of Pediatric Cystic Kidney Diseases: Autosomal Dominant and Autosomal Recessive Polycystic Kidney Disease.

Cystic kidney diseases are an important cause of pediatric morbidity and mortality. Most genetic cystic kidney diseases are "ciliopathies", a group of disorders that share a common feature of abnormal structure or function of the primary cilia. Ciliopathies represent a broad range of clinically and genetically heterogenous disorders, which affect multiple organ systems. Autosomal dominant polycystic kidney disease (ADPKD) and autosomal recessive polycystic kidney disease (ARPKD) are the 2 most common etiologies, affecting 1:1000-2500 and 1:20,000-40,000 individuals, respectively. Although hist

Fibrocystin/polyductin (FPC): new functional insights into ARPKD pathogenesis revealed by informatics, comparative genomics, and model systems.

Autosomal recessive polycystic kidney disease (ARPKD) is the prototype of the hepato-renal fibrocystic diseases, a subset of the broader ciliopathy disorders. As a severe form of PKD, ARPKD typically manifests in utero with 21% perinatal mortality and progressive loss of kidney function in most post-natal survivors. Congenital hepatic fibrosis is an invariant feature of ARPKD. PKHD1-encoded fibrocystin/polyductin (FPC) is a large 4074 amino acid glycoprotein that likely functions as a receptor molecule and appears to play a key role in maintaining differentiated renal tubular epithelium. Pierson syndrome with numerous dilated tubules masquerading as autosomal recessive polycystic kidney disease: a case report.

Pierson syndrome, characterized by congenital nephrotic syndrome, ocular abnormalities, and neurological defects, is caused by biallelic pathogenic variants in LAMB2. LAMB2 encodes laminin β2, a key component of basement membranes that is predominantly expressed in the glomeruli, eyes, and neuromuscular junctions. The renal histopathology of Pierson syndrome typically shows diffuse mesangial sclerosis (DMS), with occasional tubulointerstitial atrophy and fibrosis. We report a case of Pierson syndrome characterized by DMS and prominent tubular dilatation. A fetal ultrasound at 23 weeks of gesta

Congenital biliary abnormalities in adults: a practical CT and MRI guide.

Congenital abnormalities of the biliary tree are increasingly identified in adulthood as incidental findings on cross-sectional imaging. They range from clinically silent anatomical variants to complex malformations with relevant clinical or surgical implications. This review provides a practical framework for radiologists who encounter these entities in adult patients, organizing them into four categories: congenital cystic biliary dilation, fibropolycystic liver disorders arising from ductal plate malformation, anatomical variants of the biliary tree, and rare developmental anomalies such as

The fibrocystin C-terminal domain inhibits Src/STAT3 signal-induced cystogenesis of kidney epithelial cells.

Autosomal recessive polycystic kidney disease (ARPKD) is caused by impaired function of fibrocystin/polyductin (FPC) in collecting duct epithelia, resulting in cyst formation. We hypothesized that the membrane-bound C-terminal FPC domain (FPCct) is necessary to suppress cystogenesis and facilitate epithelial homeostasis. In ARPKD, cystic kidney epithelia are characterized by a secretory phenotype associated with high intracellular cAMP levels and enhanced STAT3-dependent transcription. Moreover, impaired FPC function may lead to enhanced activation of Src tyrosine kinase, thereby activating ST

Hepatopulmonary syndrome from liver disease associated with autosomal recessive polycystic kidney disease.

Hepatopulmonary syndrome (HPS) is a life-threatening complication of chronic liver disease (CLD) that currently can be managed only by liver transplant. Though uncommon, some children with kidney disease have coexistent CLD and hence are at risk of developing HPS. Paediatric cases of HPS are rarely described in the nephrology literature. We present a patient with autosomal recessive polycystic kidney disease (ARPKD) and persistent hypoxaemia diagnosed as HPS and successfully managed with a liver transplant. We suggest that nephrologists caring for children with kidney and concomitant CLD consi

Elucidating the Molecular Landscape of Cystic Kidney Disease: Old Friends, New Friends and Some Surprises.

Cystic kidney diseases (CyKD) are a diverse group of disorders affecting more than 1 in 1000 individuals. Over 120 genes are implicated, primarily encoding components of the primary cilium, transcription factors, and morphogens. Prognosis varies greatly by molecular diagnosis. Causal variants are not identified in 10%-60% of individuals due to our limited understanding of CyKD. To elucidate the molecular landscape of CyKD, we queried the CAG Biobank using the ICD10 codes N28.1, Q61.1, Q61.11, Q61.19, Q61.2, Q61.3, and Q61.8 to identify individuals with CyKD. One hundred eight individuals met c

A risk score to predict kidney survival in patients with autosomal recessive polycystic kidney disease at the age of two months.

Autosomal recessive polycystic kidney disease (ARPKD) is a severe hepatorenal fibrocystic disorder. Its rareness and the variability of disease courses have been major obstacles for the establishment of clinical trials on treatment of kidney disease in ARPKD. In this observational study we characterized kidney disease progression in a very large cohort of up to 658 patients with the clinical diagnosis of ARPKD and identified risk factors associated with rapid kidney disease progression. The estimated probability of kidney failure by the age of 20 years was 50.1% (95% confidence interval 42.2%‒

Electrolyte and metabolite composition of cystic fluid from a rat model of ARPKD.

Fluid-filled cysts are the key feature of polycystic kidney disease, which eventually leads to renal failure. We analyzed the composition of cyst fluid from a rat model of autosomal recessive polycystic kidney disease, the PCK rat, and identified sexual differences. Our results demonstrate that the ion composition of cyst fluid differs from that of urine or plasma. Untargeted metabolomics combined with transcriptomic data identified tryptophan metabolism, enzyme metabolism, steroid hormone biosynthesis, and fatty acid metabolism as pathways differing between male and female PCK rats. We quanti

Polycystic Kidney Disease in Children: The Current Status and the Next Horizon.

Autosomal dominant polycystic kidney disease (ADPKD) and autosomal recessive polycystic kidney disease (ARPKD) are inherited disorders that share many features such as kidney cysts, hypertension, urinary concentrating defects, and progressive chronic kidney disease. The underlying pathogenic mechanisms for both include cilia dysfunction and dysregulated intracellular signaling. ADPKD has been traditionally regarded as an adult-onset disease whereas ARPKD has been classically described as an infantile or childhood condition. However, clinicians must recognize that both disorders can present acr

Progress, challenges, and pragmatic concessions in predicting relative risk of kidney survival in ARPKD.

Autosomal recessive polycystic kidney disease is rare, with heterogeneous disease progression toward kidney failure. Risk stratification tools are needed to identify patients at higher risk of progression. Burgmaier et al. developed a relative risk score model in the international ARPKD registry for children older than 2 months of age without kidney failure. Their regression-based model included 5 predictors and yielded a simple prognostic score that classified "lower-risk" and "higher-risk" groups. Discrimination separating these 2 groups was good, but there are potential future opportunities

Pilot study using a discrete mathematical approach for topological analysis and ssGSEA of gene expression in autosomal recessive polycystic kidney disease.

Autosomal recessive polycystic kidney disease (ARPKD) is a severe genetic disorder characterized by renal cystogenesis and hepatic fibrosis, primarily associated with PKHD1 mutations. While differential expression analysis (DEG) has identified key genes involved in ARPKD, their network-level interactions remain unclear. Recent studies have implicated WNT signaling in ARPKD pathogenesis, but a topological framework may provide additional insights into gene community structures. This study applied a network-based approach integrating single-sample gene set enrichment analysis (ssGSEA) and topolo
