BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is a genetically inherited disorder characterized by progressive renal cyst formation, leading to end-stage renal disease (ESRD). The disease causes a continuous decline in renal function, resulting in significantly enlarged kidneys that adversely affect the patient's quality of life (QOL). Despite the identification of causative gene mutations, no approved therapeutic intervention halts cystogenesis or prevents renal failure.
OBJECTIVES: This review aims to evaluate the potential of emerging pharmacological interventions and in

BACKGROUND: The prevalence of intracranial aneurysms (ICAs) is higher in patients with autosomal dominant polycystic kidney disease (ADPKD) than in the general population. This extrarenal manifestation carries significant mortality and morbidity risks. Our study aims to estimate the prevalence of, and to characterize patients with ICA(s) among the ADPKD cohort of South Australia's statewide transplantation centre.
METHODS: A retrospective review of prospectively collected data was conducted for all patients who underwent nephrectomy/ies between 1 January 1995 and 31 December 2021. Incidences

Autosomal dominant polycystic kidney disease (ADPKD) is the leading genetic form of KD. Although rare causal variants in the PKD1 and PKD2 genes have been identified, their penetrance and the disease progression and outcome are known to vary, and treatment efficacy in these carriers lags compared to patients with other forms of chronic KD (CKD). To develop a population screening strategy with high sensitivity to individuals likely to develop disease, we characterize the presentation and progression of ADPKD in variant carriers, identified in a multi-center all-comers cohort, as well as the UK

UNLABELLED: <p>Introduction: Autosomal dominant polycystic kidney disease (ADPKD) is a genetic disorder that is characterized by the development of fluid-filled kidney cysts, which often lead to kidney failure. The vasopressin receptor 2 antagonist, tolvaptan, is the only approved treatment that slows the progression of ADPKD. There is an unmet need for treatment options for patients with ADPKD because tolvaptan has limited efficacy and non-negligible side effects. In vitro and in vivo data suggest that inhibition of the cystic fibrosis transmembrane conductance regulator (CFTR) may be a suita

Over the last few decades, there has been growing interest in the use of ketogenic diets, mainly as a weight loss strategy. Obesity and diabetes are major risk factors for kidney disease, and obese patients with chronic kidney disease (CKD) are potential candidates for weight reducing diets, among them ketogenic diets. Of further relevance to nephrology, a limited number of recent studies suggests a favorable effect of ketogenic diets in patients affected by autosomal dominant polycystic kidney disease (ADPKD). However, concerns remain about safety in patients with CKD, particularly in the lon

Autosomal dominant polycystic kidney disease (ADPKD), the most prevalent genetic kidney disorder, is characterized by diffuse kidney cysts, hypertension, and progressive kidney function decline, often leading to kidney failure by the age of 60 years. Compared with the general population, patients with ADPKD have an increased risk for development of saccular intracranial aneurysms (IAs), which can lead to intracranial bleeding and result in significant disability and mortality. Of both modifiable and nonmodifiable risk factors, the most significant is a family history of IAs or aneurysm rupture

The measurement of glomerular filtration rate (GFR) is essential in diagnosing and managing chronic kidney disease (CKD) and autosomal dominant polycystic kidney disease (ADPKD), both requiring precise renal function assessment. Traditionally, GFR has been estimated using endogenous markers like creatinine and cystatin C, though these can be inaccurate due to factors unrelated to kidney function, such as muscle mass and diet. The iohexol clearance method provides a more accurate and less invasive alternative to traditional markers like inulin or radioactive markers. Iohexol, a non-ionic, water

INTRODUCTION: Positive effects on pain reduction following renal denervation (RDN) have been described in patients with kidney-related pain syndromes, yet data on the safety and efficacy of redo RDN procedures are limited.
METHODS: Consecutive patients with a history of chronic loin pain, hypertension and preserved renal function (eGFR ≥40 ml/min) scheduled for RDN were included. Changes in perceived pain, opioid and antihypertensive medication use, and blood pressure were prospectively assessed 3 months after each procedure.
RESULTS: Two patients presented with a history of Autosomal Domina

Mutations in polycystin-1 (PC1) or polycystin-2 (PC2) cause autosomal-dominant polycystic kidney disease (ADPKD). Structural data suggest that one PC1 and three PC2 form heterotetrameric ion channels with an ion permeation pathway blocked by PC1 (R4100, R4107, and H4111) and PC2 (L677, N681) residues. Here, we demonstrate that replacing these residues with alanines results in a gain-of-function (GOF) PC2/PC1 construct with distinct selectivity properties compared to PC2 homomers. We also show preferential formation of PC2/PC1 heteromeric complexes over PC2 homomers. Re-interpretation of publis

Autosomal Dominant Polycystic Kidney Disease (ADPKD) is the most common inheritable disease of cystic degeneration in the kidney. ADPKD is a significant cause of end-stage renal disease (ESRD). Autosomal Dominant Polycystic Liver Disease (ADPLD) results in substantial PLD with minimal PKD. Currently, there are eight genes which have been associated with ADPKD (PKD1 and PKD2), ADPLD (PRKCSH, SEC63, LRP5, ALG8, and SEC61B), or both (GANAB). The severity of ADPKD can show an extremely broad range, but the evolution to ESRD is doubtless unavoidable. In some patients, carcinogenesis develops with i

Vasopressin plays a major role in the pathogenesis of autosomal dominant polycystic kidney disease (PKD), the fourth leading cause of end-stage kidney disease. The vasopressin V2 receptor (V2R) antagonist tolvaptan is the only approved treatment. The role of vasopressin V1a and V1b receptors (V1aR and V1bR) has not been studied. Pkd1RC/RC mice were allocated to control and 5 experimental groups treated with tolvaptan, OPC21268 (V1aR antagonist), SSR149415 (V1bR antagonist), tolvaptan plus OPC21268, or tolvaptan plus SSR149415, from 4 to 16 wk of age, to compare their separate effects on PKD an

INTRODUCTION: Autosomal Dominant Polycystic kidney disease (ADPKD) is defined as one of the most common genetic disorders and the cause of kidney failure or end-stage kidney disease (ESKD). Several studies have shown that renin-angiotensin system has an important role in pathogenesis of ADPKD. The aim of this study was to examine the association between the angiotensin converting enzyme (ACE) gene Deletion/Deletion (D/D) polymorphism and risk of ADPKD among Iranian patients from west Azerbaijan province of Iran.
METHODS: This case-control study was conducted on 40 patients and 72 controls. Ge

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited kidney disease, affecting approximately 1 in 1000 individuals. This genetically heterogeneous condition is primarily caused by monoallelic pathogenic or likely pathogenic variants in the PKD1 and PKD2 genes, accounting for 78% and 15% of typical cases, respectively. Recently, the application of NGS methods has led to the identification of additional genes associated with ADPKD, which have been incorporated into routine diagnostic testing for detecting phenocopies of the disease.
METHODS: In this stud

BACKGROUND: Autosomal Dominant Polycystic Kidney Disease (ADPKD) is the most common genetic cause of renal failure. Uncontrolled proliferation drives ADPKD, which manifests with cystic kidney enlargement. Yet, the mechanisms by which renal epithelial cells lose cell cycle control are largely unknown. To investigate this, we examined the expression and function of the Ankyrin Repeat and single KH Domain 1 (ANKHD1), which positively regulates proliferation in cancer, yet its role in ADPKD is unexplored. RESULTS: We report elevated proliferation (Ki67 and Cyclin D1) in three independent mouse mod

GLP-1 receptor agonists revolutionized diabetes therapy and offer cardiorenal protection in diabetic kidney disease. Their potential in Autosomal Dominant Polycystic Kidney Disease (ADPKD) is now being explored. Preclinical findings and metabolic improvements (obesity, hypertension, inflammation) indicate promise. Further clinical studies are required to ensure efficacy and safety before clinical integration can be considered. Clinical trial number: Not applicable.

Autosomal dominant polycystic kidney disease (ADPKD) causes progressive cyst formation and renal failure. Tank-binding kinase 1 (TBK1), a key regulator of inflammation, represents a promising target for ADPKD treatment. In this study, we designed and synthesized a series of TBK1 degraders, including both PROTACs and molecular glues. Among the compounds evaluated, degrader 30 demonstrated superior efficacy, inducing TBK1 degradation in a dose- and time-dependent manner. Mechanistic studies revealed that 30 mediates TBK1 degradation through the ubiquitin-proteasome system via E3 ligase RNF126. C

Kidney organoids are an increasingly established model of polycystic kidney disease (PKD). Derived from human pluripotent stem cells (hPSCs), organoids may be generated from induced pluripotent stem cells (iPSCs) of patients that bear naturally occurring mutations or from CRISPR mutant hPSCs by virtue of their genetic tractability. PKD is the leading inheritable cause of kidney failure (KF), accounting for ∼5%-10% of the kidney transplant and dialysis needs worldwide. PKD is a disorder of considerable genetic heterogeneity, composed of typical adult-onset autosomal dominant (ADPKD) and fetal-o

INTRODUCTION: The diagnosis of cyst infection in autosomal dominant polycystic kidney disease (ADPKD) is difficult. [18F]FDG PET/CT imaging is helpful, but early diagnosis remains challenging. Procalcitonin (PCT), a serum biomarker for bacterial infections, has not been evaluated in ADPKD-related cyst infections.
METHODS: A retrospective review (between 2009 and 2023) identified all ADPKD patients who were (i) hospitalized (ii) with serum PCT measurements. Cyst infection was conventionally defined. Univariate and multivariate logistic regressions assessed the association between PCT and cyst

PKD2 is a member of the polycystin subfamily of transient receptor potential (TRP) ion channel subunits which traffic and function in primary cilia organelle membranes. Millions of individuals carry pathogenic genetic variants in PKD2 that cause a life-threatening condition called autosomal dominant polycystic kidney disease (ADPKD). Although ADPKD is a common monogenetic disorder, there is no drug cure or available therapeutics which address the underlying channel dysregulation. Furthermore, the structural and mechanistic impacts of most disease-causing variants are uncharacterized. Using dir
