Robotic-Assisted Simultaneous Bilateral Native Nephrectomy and Living Donor Kidney Transplantation.

INTRODUCTION: Simultaneous bilateral nephrectomy and living donor kidney transplantation (LDKT) is often indicated for autosomal dominant polycystic kidney disease (ADPKD) with end-stage kidney disease (ESKD). Robotic-assisted surgery offers a minimally invasive alternative to open approaches and may reduce perioperative morbidity and length of stay. METHODS: Four adults with ADPKD/ESKD underwent fully robotic-assisted simultaneous bilateral nephrectomy/LDKT (RASBN/LDKT) at a single high-volume transplant center. All procedures used an intra-abdominal approach with a Pfannenstiel incision and

Minocycline infusion sclerotherapy for infected hepatic cysts in a patient with autosomal dominant polycystic kidney disease: a case report and literature review.

We report the case of a Japanese man in his 60 s undergoing hemodialysis for end-stage renal disease due to autosomal dominant polycystic kidney disease, who had experienced repeated hepatic cyst infections. Despite treatment with oral antibiotics, including minocycline and trimethoprim-sulfamethoxazole, he presented with fever and was admitted to our hospital. Diffusion-weighted magnetic resonance imaging demonstrated a high signal intensity in a large hepatic cyst, and percutaneous transhepatic drainage was performed. Enterococcus faecium was isolated from the cyst fluid. Three intracystic m

Abnormal purinergic signaling contributes to development of renal cysts in autosomal dominant polycystic kidney disease.

Polycystic kidney diseases (PKD) are a group of inherited nephropathies marked by the formation of fluid-filled cysts along the nephron, which cause renal failure. Bulk RNA sequencing of microdissected renal cysts from Pkd1RC/RC mice identifies 4,970 differentially expressed genes in comparison to intact collecting ducts. A poorly understood factor of cyst growth is the high ATP level in the cyst fluid, probably released by pannexin-1 hemichannels. Here, we evaluate a novel pannexin-1 inhibitor SIL14 as a potential therapeutic agent for cyst reduction and investigate proteins involved in cysto

Disruption of a six-nucleotide miRNA motif improves PKD1 dosage and ameliorates polycystic kidney disease.

Disrupting microRNA interactions to restore protein expression from haploinsufficient genes offers a promising precision-therapy strategy for monogenic disorders. PKD1 heterozygosity underlies autosomal dominant polycystic kidney disease (ADPKD), a disorder affecting nearly 12 million people worldwide, where reduced PKD1 dosage drives progressive cyst formation and kidney failure. We previously identified a 55-bp cis-repressive element in the PKD1 3'UTR. Here, we define a six-nucleotide miR-17 seed match within this element that is sufficient to reproduce PKD1 repression. In vivo base substitu

The Role of Immune Cells as Modulators of Progression in Polycystic Kidney Disease: A Systematic Review.

Autosomal dominant polycystic kidney disease (ADPKD) is a genetic disorder caused by mutations in genes encoding ciliary proteins. Increasing evidence suggests that immune cell infiltration, chronic inflammation, and dysregulated immune signaling pathways contribute to cyst growth, fibrosis, and progressive kidney function decline. Notably, features of chronic inflammation are already detectable in early stages of the disease, including infiltration by immune cells, elevated levels of proinflammatory cytokines, and immunologic crosstalk between immune and epithelial cells. These immune respons

Urinary interleukin-37 as a suggested protective biomarker of renal prognosis in autosomal dominant polycystic kidney disease.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is characterized by progressive cyst expansion and chronic inflammation. Interleukin-37 (IL-37) is an anti-inflammatory cytokine, but its role in ADPKD remains unclear. This study aimed to evaluate IL-37 expression in ADPKD patients and assess its clinical relevance in disease severity and renal prognosis. METHODS: IL-37 expression in kidney tissues and cell lines was examined by real-time PCR, Western blot, and immunofluorescence. IL-37 levels were measured by ELISA. This retrospective observational cohort study analyzed longit

2025 KDIGO Executive Summary: Diagnosis and Management of Pediatric Autosomal Dominant Polycystic Kidney Disease.

Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited kidney disorder, affecting 1 in 400 to 1,000 people worldwide. These significant care requirements reflect the importance of early diagnosis and prevention for affected patients and the need for evidence-based guidelines for treatment protocols and best practice measures. The 2025 KDIGO guidelines build on earlier frameworks such as the 2019 International consensus statement on the diagnosis and management of ADPKD in children and young people to improve screening, monitoring, and treatment methodology for pediat

The many faces of cholesterol in autosomal dominant polycystic kidney disease.

Autosomal dominant polycystic kidney disease has some effects on cholesterol metabolism and cardiovascular disease that are shared with other chronic kidney diseases and others that are distinctive and directly caused by disruption of polycystins. The pravastatin clinical trial by Gitomer et al. does not support treatment with statins for the only purpose of delaying disease progression. Emergence of novel lipid-lowering therapies presents new therapeutic opportunities but also risks. Recent studies suggest that cholesterol is important for traffic to and function of polycystins in primary cil

The Prevalence, Mechanisms, and Clinical Significance of Inferior Vena Cava Compression in Autosomal Dominant Polycystic Kidney Disease: A Multicenter Retrospective Cohort Study Based on TriNetX.

Background and Objectives: Autosomal dominant polycystic kidney disease (ADPKD) is a leading cause of end-stage renal disease (ESRD). Progressive renal cyst growth in ADPKD can exert mass effects, including compression of the inferior vena cava (IVC), a rare but clinically significant complication with implications for hemodynamic stability and renal outcomes. This study evaluated the prevalence of IVC compression in ADPKD and its impact on progression to ESRD, mortality, and overall survival. We aimed to provide quantitative measures to elucidate its prognostic significance. Materials and Met

Discovery of a potent STAT3 inhibitor WR-S-647 for the treatment of ADPKD.

Autosomal dominant polycystic kidney disease (ADPKD) affects approximately 12.5 million individuals globally and is one of the most common causes of end-stage renal disease. It is typically associated with a gradual increase in the volume of numerous cysts in both kidneys. Recent studies have highlighted the critical role of signal transducer and activator of transcription 3 (STAT3) in ADPKD pathogenesis, as it is highly expressed and persistently activated in ADPKD kidneys. Through screening of our in-house compound library, we identified compound WR-S-647 (4e) as a potent and specific inhibi

Vascular transcriptional and metabolic changes precede progressive intrarenal microvascular rarefaction in autosomal dominant polycystic kidney disease.

The mechanisms contributing to progressive kidney damage in autosomal dominant polycystic kidney disease (ADPKD) remain unclear. Renal microvascular (MV) rarefaction plays an important role in kidney disease, but its natural history, underlying mechanisms, and contributions to renal disease progression in ADPKD remain unknown. We hypothesized that intrarenal MV rarefaction is present early on and is preceded by vascular transcriptional and metabolic changes. Pkd1RC/RC and wild type (WT) mice (n = 16 each) were studied at 1, 6, and 12 mo. Total kidney volume (TKV) was measured in vivo (MRI), wh

Optimizing next-generation sequencing for genetic diagnosis in autosomal dominant polycystic kidney disease.

PURPOSE: Autosomal dominant polycystic kidney disease (ADPKD) affects 1:1000, causing 5% to 10% of kidney failure. The primary disease gene, PKD1, has 6 pseudogenes with 97% to 99% homology, a >12-kb transcript, high GC content, and polypyrimidine tracts. Although long-range polymerase chain reaction with Sanger sequencing has been the "gold-standard," next-generation sequencing (NGS) is increasingly used. METHODS: We performed exome sequencing (ES) on 203 ADPKD patients in 171 families from the Consortium for Radiologic Imaging Studies of Polycystic Kidney Disease cohort, with prior "gold-st

Disruption of Polycystin Ciliary Localization and Channel Function by Autosomal Dominant Polycystic Kidney Disease-Causing Polycystin-1 Variants.

KEY POINTS: We developed assays to measure genetic variant effects on polycystin-1, the protein mutated in most autosomal dominant polycystic kidney disease. All tested pathogenic variants disrupted either polycystin-1 ciliary trafficking or channel function. Trafficking and channel function of some pathogenic variants was restored by low temperature culture to promote polycystin folding. BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is the leading monogenic cause of kidney failure and affects millions of people worldwide. Despite the prevalence of ADPKD, limited mechanisti

Epithelial Dynamics of Cystogenesis in Genetic Models of Autosomal Dominant Polycystic Kidney Disease.

Autosomal dominant polycystic kidney disease (ADPKD), caused by mutations in PKD1 or PKD2, is characterized by progressive and exponential enlargement of renal and hepatic cysts. However, the epithelial dynamics that generate this growth pattern remain incompletely understood. Using Brainbow/Confetti multicolor clonal lineage tracing in developmental and adult-onset ADPKD mouse models, we show that polycystin-deficient epithelial cells initiate clonal expansion at early stages of tubule dilation and continue to expand throughout cyst progression. Concurrently, cyst-lining cells undergo a progr

BICC1 interacts with PKD1 and PKD2 to drive cystogenesis in ADPKD.

Autosomal-dominant polycystic kidney disease (ADPKD) is primarily of adult-onset and caused by pathogenic variants in PKD1 or PKD2. Yet, disease expression is highly variable and includes very early-onset PKD presentations in utero or infancy. In animal models, the RNA-binding molecule Bicc1 has been shown to play a crucial role in the pathogenesis of PKD. To study the interaction between BICC1, PKD1, and PKD2, we combined biochemical approaches, knockout studies in mice and Xenopus, genetic engineered human kidney cells carrying BICC1 variants, as well as genetic studies in a large ADPKD coho
