Nutritional adequacy of ketogenic diets as a novel dietary intervention for people living with autosomal dominant polycystic kidney disease.

BACKGROUND: Diet is a priority to the kidney community, with increasing interest in whether a ketogenic diet can slow autosomal dominant polycystic kidney disease (ADPKD) progression. People with ADPKD have unique nutritional requirements that need to be considered when designing a ketogenic diet for this population. The nutritional adequacy of an ADPKD ketogenic diet needs to be evaluated prior to testing in a clinical trial. OBJECTIVE: The current paper describes the (i) development of an ADPKD-Keto meal plan, in addition to modified PKD-Keto diets targeting the risk of nephrolithiasis and

miRNA profiling of a mutation-negative PKD cohort reveals PKD1/PKD2 ADPKD shared signatures and differences.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is mainly caused by mutations in PKD1 or PKD2 genes, but a subgroup of patients has no detectable mutation and remains understudied. We profiled microRNAs (miRNAs) in this mutation-negative group and compared them with PKD1, PKD2, and healthy controls. METHODS: Targeted miRNA profiling was used to measure miRNAs expression. We tested five prespecified contrasts using Welch's two-sided t-test (p < 0.05). For interpretation, experimentally supported miRNA-mRNA interactions were assembled and visualized into networks. RESULTS: miR

GV-001, An Oral Available Histone Deacetylase 6 Inhibitor for the Treatment of Autosomal Dominant Polycystic Kidney Disease.

Autosomal dominant polycystic kidney disease (ADPKD) is a rare genetic disorder characterized by progressive cyst formation. Emerging evidence suggests that histone deacetylase 6 (HDAC6) plays a pivotal role in the regulation of disease progression. In this study, a series of quinazoli-4-one derivatives were designed and synthesized, among which compound 6a (GV-001) exhibited superior potency and selectivity in both enzyme inhibition (IC50 = 1.18 nM) and immunoblotting assays. Subsequent analysis using the BioMAP fibrosis panel and an in vitro human kidney cyst model revealed that GV-001 mitig

Patient-derived kidney organoids recapitulate ADPKD and facilitate the identification of Rho pathway inhibitors as candidate therapeutics.

Autosomal-dominant polycystic kidney disease (ADPKD) remains refractory to curative therapy partly due to the lack of human models that recapitulate its adult-onset nature, genetic context, and multi-segment origin. Here, we established expandable, multi-lineage adult renal organoids (MAROs) directly from surgical specimens of ADPKD patients and healthy donors, creating a three-dimensional (3D) high-content screening platform. Patient-derived MAROs faithfully reproduced hallmark cystogenic features, including elongated primary cilia, polarity disruption, and elevated Rho GTPase/planar cell pol

Caring for Australians and New ZealandeRs with kidney Impairment guideline commentary on kidney disease: improving global outcomes 2025 guidelines for diagnosis and management of autosomal polycystic kidney disease.

Autosomal polycystic kidney disease (ADPKD) is the most common form of inherited kidney disease. The international guideline developer, Kidney Disease: Improving Global Outcomes (KDIGO), published a 2025 clinical practice guideline reflecting advances in genetic testing and the first disease-modifying agent, tolvaptan. However, KDIGO guidelines do not address healthcare access or equity issues relevant to the needs of our populations in Australia and New Zealand. The Caring for Australians and New ZealandeRs with kidney Impairment (CARI) Guidelines Working Group has provided a regional comment

CART for ascites accumulation and dialysis difficulty after hepatic cyst fenestration and bilateral nephrectomy in ADPKD: a case report.

Hepatomegaly with hepatic cysts constitutes a common extrarenal manifestation of autosomal dominant polycystic kidney disease (ADPKD), often resulting in mass effects and occasionally necessitating cyst fenestration to alleviate abdominal distension. Nonetheless, persistent cyst fluid leakage as a postoperative complication frequently leads to refractory ascites, for which treatment options are limited. In this report, we present a 62-year-old female patient with ADPKD undergoing maintenance hemodialysis. She received bilateral nephrectomy and hepatic cyst fenestration to mitigate abdominal di

Tolvaptan improves the diurnal blood pressure profile in patients with hypertension and autosomal-dominant polycystic kidney disease.

BACKGROUND: Autosomal-dominant polycystic kidney disease (ADPKD) is a genetic disorder frequently associated with non-dipper hypertension, with tolvaptan being the only currently approved treatment. However, the effects of tolvaptan on blood pressure (BP) in patients with ADPKD and hypertension remain unclear. METHODS: This retrospective study examined the effect of tolvaptan on 24-h ambulatory BP monitoring parameters. Changes in ambulatory BP-related parameters, including annual rate of change in total kidney volume (TKV), were assessed in 23 patients with ADPKD and hypertension before and

Attribute-based cross-classification reveals sex- and age-specific prognostic impact of anemia in ADPKD.

BACKGROUND: Anemia is less prevalent in autosomal dominant polycystic kidney disease (ADPKD) owing to preserved erythropoietin production. However, its impact on kidney prognosis remains unclear. Given sex-related differences in hemoglobin (Hb) levels, we hypothesized that the prognostic relevance of anemia may vary by sex and age. Therefore, we aimed to identify subgroup-specific risk patterns using an attribute-based cross-classification approach to support individualized anemia management in ADPKD. METHODS: We analyzed 552 Japanese patients with ADPKD from a single-center cohort. The prima

Carbon dioxide contrast angiography-and computed tomography-guided percutaneous embolization of the renal allograft artery in a patient with graft intolerance syndrome and iodinated contrast allergy: a case report.

Graft intolerance syndrome (GIS) is a well-recognized complication after kidney allograft failure, characterized by fever, graft pain, and systemic inflammation. Management options include continued immunosuppression, graft nephrectomy, and renal artery embolization, each associated with specific risks. We report a case of GIS successfully treated with percutaneous renal allograft artery embolization using carbon dioxide angiography in a patient with iodinated contrast allergy. A 66-year-old woman with autosomal dominant polycystic kidney disease underwent living-donor kidney transplantation a

Miniaturization of CRISPRa plasmids for efficient delivery into renal epithelial cells and Pkd1 transactivation.

BACKGROUND: Autosomal Dominant Polycystic Kidney Disease is caused by loss-of-function mutations in PKD1 or PKD2 genes, leading to reduced polycystin protein levels. Increasing PKD1 expression via CRISPR activation (CRISPRa) represents a promising therapeutic strategy; however, delivery of large CRISPRa plasmids into renal epithelial cells, and particularly primary cells, remains inefficient due to size-related barriers. We aimed to enable Pkd1 transactivation by miniaturizing CRISPRa plasmids into ~ 6 kb vectors using a one-pot method to enhance cellular uptake in mouse kidney epithelial cell

The Kinetics and Mechanism of Niche Filling in Healthy and Cystic Mice.

KEY POINTS: The rate and mechanism of kidney-resident macrophage niche filling differs between healthy and cystic mice. An incompletely filled kidney-resident macrophage niche is associated with worsened polycystic kidney disease outcomes. BACKGROUND: Previous data indicate that bone marrow-derived Ly6c hi monocytes can become tissue-resident macrophages when the niche is available. However, the balance between monocyte recruitment and tissue-resident macrophage self-proliferation following partial depletion is unclear. METHODS: We investigated the kinetics and mechanism of kidney-resident m

Designing genome editing experiments with EditABLE.

While many computational tools exist for designing CRISPR-Cas experiments, there is a need for a centralized resource that combines individual tools to predict the most efficient genome editing strategy for a given application. To fill this gap, we develop EditABLE (EditABLE-app.stanford.edu), an online resource that provides optimal CRISPR editors and guide RNAs based on user provided sequence data with functionalities for base editing, prime editing, and integrase-mediated editing. We demonstrate the utility of EditABLE by applying it to one of the most common monogenic disorders, autosomal

Modelling kidney cystogenesis using human kidney tubuloid cultures.

BACKGROUND: Renal cyst formation, as observed in autosomal dominant polycystic kidney disease (ADPKD), is a life-threatening condition with no effective cure yet. The molecular mechanisms underlying primary cilia dysfunction, which causes cyst formation and disease development, are not well understood. Human kidney tubuloids offer a promising model system to investigate the disease mechanisms of PKD in physiologically relevant 3D structures. However, their inherent cystic morphology poses a challenge in effectively modelling kidney cystogenesis. Therefore, our study aims to refine the culture

HIF-Regulated Pannexin-1 Channel Drives Luminal ATP Accumulation in Kidney Cysts.

Autosomal dominant polycystic kidney disease (ADPKD) is marked by the progressive development of bilateral kidney cysts, leading to compression of intact surrounding tissue and a subsequent decline in kidney function. Accumulation of ATP within the cyst fluid significantly drives cyst expansion by activating purinergic receptors, thereby promoting calcium-dependent chloride secretion. Moreover, the induction of hypoxia-inducible factor 1α (HIF-1α) in cyst-lining epithelial cells further enhances chloride conductance. However, the mechanisms underlying ATP release into the cyst fluid and the ro

Overweight and Obesity Are Associated with Lower Renal Blood Flow in Autosomal Dominant Polycystic Kidney Disease.

INTRODUCTION: We have previously reported that overweight and obesity are independently associated with more rapid disease progression in adults with autosomal dominant polycystic kidney disease (ADPKD). In this study, we perform a cross-sectional analysis to evaluate whether overweight and obesity are also associated with renal blood flow (RBF) in this patient population. METHODS: A total of 134 non-diabetic adults with ADPKD and estimated glomerular filtration rate (eGFR) ≥60 mL/min/1.73 m2 who participated in a randomized controlled trial of pravastatin therapy were categorized as normal w

Endocannabinoid System-Related Inflammation and Progression of Autosomal Dominant Polycystic Kidney Disease.

The endocannabinoid system is a complex regulatory network whose functioning is associated with maintaining homeostasis and regulating immune response. The aim of this study was to evaluate the relationship between endocannabinoid system activity, inflammation, and the progression of chronic kidney disease (CKD) in patients with autosomal dominant polycystic kidney disease (ADPKD). The study included 105 participants: 60 individuals with ADPKD and 45 healthy volunteers. From a single venous blood draw, concentrations of anandamide (AEA), 2-arachidonoylglycerol (2-AG), tumor necrosis factor α (

Regulation of the PKD2 channel function and associated disease phenotypes by RASSF4.

Autosomal dominant polycystic kidney disease (ADPKD) is one of the most common monogenic genetic disorders, caused by mutations in receptor PKD1 or ion channel PKD2, and is characterized by progressive renal cyst development with additional hepatic and extrarenal manifestations. As effective treatments for ADPKD remain limited, further investigation into the function and regulation of PKD proteins is needed. Using biotin-based proximity labeling combined with mass spectrometry in human embryonic kidney (HEK) cells, here we identify Ras association domain family member 4 (RASSF4) as a potential

Effectiveness and mechanistic effects of green mamba related peptide MQ232 in an orthologous mouse model of autosomal dominant polycystic kidney disease.

INTRODUCTION: Tolvaptan, a vasopressin V2 receptor (V2R) inverse agonist, is the only treatment specifically approved for autosomal dominant polycystic kidney disease (ADPKD). In addition to inhibiting cAMP synthesis, it has other effects on V2R signaling. Our goal was to mechanistically characterize the effects of MQ232, an optimized peptide of the green mamba snake and a new V2R inverse agonist, in Pkd1RC/RC mice. METHODS: Pkd1RC/RC mice were treated with MQ232 (0.5, 2, 4, or 8 nmols/kg/hour via minipump), tolvaptan (0.2% in chow), or vehicle from four to 16 weeks of age. MRI measurements o
