Identification of drug repurposing candidates for the treatment of polycystic kidney disease.

BACKGROUND AND PURPOSE: Autosomal dominant polycystic kidney disease (ADPKD) is a leading cause of end-stage renal disease with limited treatment options. Drug repurposing offers a promising strategy to find effective treatments. EXPERIMENTAL APPROACH: We identified birinapant, bardoxolone methyl and salicylic acid as repurposing candidates for ADPKD and applied thermal proteome profiling to identify disease-relevant targets. KEY RESULTS: Our results uncovered shared effects between the three drugs, including a thermal shift in ADP/ATP translocases (SLC25A4/5/6) caused by reduced ATP product

Nationwide epidemiological survey of autosomal dominant polycystic kidney disease in Japan in 2022.

BACKGROUND: Accurate epidemiological data on autosomal dominant polycystic kidney disease (ADPKD) are essential for healthcare planning and therapeutic strategies. However, nationwide prevalence estimates in Japan have not been updated for decades. METHODS: This was a nationwide survey conducted in Japan. Departments at hospitals were selected using stratified random sampling based on specialty and hospital size. The number of patients with ADPKD not receiving kidney replacement therapy (KRT), defined in this study as those who did not receive dialysis and did not newly undergo kidney transpl

Disruption of polycystin-1 cleavage impairs mitochondrial bioenergetics and calcium uptake in a substrate-dependent manner.

Metabolic and mitochondrial alterations are central in the pathogenesis of autosomal dominant polycystic kidney disease (ADPKD) since therapies targeting these alterations slow kidney disease progression in orthologous animal models. To investigate metabolic and mitochondrial defects in an animal model orthologous to ADPKD, we used male mice homozygous for a point variant in the GPS cleavage site of the polycystin-1 (Pkd1V/V) and evaluated oxygen consumption, Ca2+ uptake, and redox state in the mitochondrial fraction of Pkd1V/V kidneys and wild-type controls (WT). Our findings revealed mitocho

Exploring urinary allantoin and adenosine in pediatric autosomal dominant polycystic kidney disease: no clear association with disease severity.

BACKGROUND: Proteinuria and hypertension are commonly observed in children with autosomal dominant polycystic kidney disease (ADPKD), but reliable non-invasive biochemical markers for early disease activity or progression in pediatric ADPKD are lacking. Allantoin and the allantoin/uric acid ratio are recognized oxidative stress biomarkers in chronic kidney disease and may hold potential for early disease monitoring in ADPKD. METHODS: In this observational study, 34 patients with genotyped PKD1-ADPKD under 18 years with preserved kidney function were compared to 31 healthy controls. Urinary al

Thyroid Hormones Exhibit Promising Prognostic and Therapeutic Potential in Autosomal Dominant Polycystic Kidney Disease.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is a genetic disorder characterized by progressive cyst development and renal dysfunction. While thyroid hormones (THs) are known to regulate key pathways in various kidney diseases, their role in ADPKD pathobiology and therapeutic potential remains unexplored. Here, we aimed to elucidate the role of THs in ADPKD and evaluate whether their pharmacological modulation could serve as a therapeutic strategy. METHODS: Patient-derived renal epithelial cells were used to engineer 3D polycystic tubules and to test the anti-cystogenic ef

Autosomal dominant polycystic kidney disease: Current perspectives in 2026.

Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited kidney disease and is characterized by progressive kidney enlargement and cyst growth leading to progressive loss of kidney function. The two main genes implicated are PKD1 and PKD2; however, pathogenic variants in other less frequent genes can also cause the disease. Diagnosis may be established in various clinical settings, including family screening, the investigation of arterial hypertension, urological events such as hematuria or flank pain, or incidental imaging findings. Diagnostic confirmation relies on t

A three-year randomized, double-blind, placebo-controlled study of lanreotide in stage 2/3 autosomal dominant polycystic kidney disease.

INTRODUCTION: Somatostatin analogues reduce liver and kidney cyst volume in autosomal dominant polycystic kidney disease (ADPKD), but randomized trials have not demonstrated a consistent benefit on kidney function decline. We tested whether the analogue lanreotide slows glomerular filtration rate (GFR) decline over three years in adults with ADPKD stages 2/3. METHODS: In this randomized, double-blind, placebo-controlled trial, we enrolled adults with ADPKD and measured GFR (mGFR) 30-89 ml/min/1.73 m2. Patients were randomized to receive monthly injections of lanreotide 120 mg or 0.9% sodium c

Autosomal Dominant Polycystic Kidney Disease: Genetic Insights, Phenotypic Spectrum, and Advances in Prognosis and Treatment.

Autosomal dominant polycystic kidney disease is a leading cause of inherited kidney failure, primarily due to pathogenic variants in PKD1 (80%) and PKD2 (15%), with rare contributions from genes like GANAB and DNAJB11. This review summarizes the understanding of autosomal dominant polycystic kidney disease's genetic and phenotypic diversity, emphasizing genotype-phenotype correlations that influence disease severity and progression. Truncating PKD1 variants are linked to earlier kidney failure (median onset in the fifth to sixth decade) and a faster estimated glomerular filtration rate decline

Characterization of Kidney and Liver Cystic Phenotype Associated with GANAB Using Advanced Imaging Biomarkers.

BACKGROUND: Monoallelic pathogenic variants in GANAB cause autosomal dominant cystic kidney and liver disease, but quantitative imaging phenotypes remain incompletely defined. METHODS: We performed a retrospective study of 16 individuals with GANAB variants and available abdominal imaging. Deep learning-based cyst segmentation quantified kidney and liver volumes and cyst metrics, including height-adjusted total kidney volume (htTKV), height-adjusted total liver volume, total cyst number (TCN), and height-adjusted total cyst volume. RESULTS: Hepatic involvement was common, with polycystic liv

Empagliflozin in ADPKD with suspected IgA nephropathy: antiproteinuric response without adverse events.

SGLT2 inhibitors have demonstrated robust cardiorenal benefits across diverse chronic kidney disease populations but remain underutilized in autosomal dominant polycystic kidney disease (ADPKD) due to lack of disease-specific data and theoretical safety concerns. We report a 58-year-old man with genetically confirmed PKD2-associated ADPKD who developed progressive proteinuria (0.8 g/24 h increasing to 2.0 g/24 h over three years) despite optimal ACE inhibition. Clinical findings suggested concomitant IgA nephropathy, but biopsy was contraindicated. Empagliflozin 10 mg/day was initiated off-lab

GLP-1 receptor agonists in ADPKD: from metabolic rationale to phenotype-enriched translational testing.

Autosomal dominant polycystic kidney disease (ADPKD) remains therapeutically anchored to vasopressin V2-receptor antagonism, yet progression heterogeneity and persistent unmet need increasingly suggest residual disease biology beyond cAMP-centered control. Converging experimental and observational human phenotype data suggest that metabolic reprogramming, mitochondrial dysfunction, impaired fatty-acid oxidation, obesity, and visceral adiposity may modify cyst growth, kidney-volume expansion, eGFR decline, or treatment-response heterogeneity, although causal and therapeutic evidence remains inc

Genetic and Clinical Evaluation of Pediatric Cystic Kidney Diseases: Autosomal Dominant and Autosomal Recessive Polycystic Kidney Disease.

Cystic kidney diseases are an important cause of pediatric morbidity and mortality. Most genetic cystic kidney diseases are "ciliopathies", a group of disorders that share a common feature of abnormal structure or function of the primary cilia. Ciliopathies represent a broad range of clinically and genetically heterogenous disorders, which affect multiple organ systems. Autosomal dominant polycystic kidney disease (ADPKD) and autosomal recessive polycystic kidney disease (ARPKD) are the 2 most common etiologies, affecting 1:1000-2500 and 1:20,000-40,000 individuals, respectively. Although hist

[Chronic abdominal pain in elderly patient].

A 75-year-old female patient with autosomal-dominant polycystic kidney disease and a history of kidney transplantation presented to the emergency department with dysuria, chills and abdominal pain. A detailed review of her medical history revealed that her acute pyelonephritis was masking a chronic abdominal pain issue accompanied by significant bilirubinemia. MRCP showed a complete cystic transformation of the left liver lobe and liver fibrosis. Consequently, the diagnosis of Caroli syndrome, a rare cystic liver disease with fibrosis, was established despite her advanced age.

Pregnancy in Autosomal Dominant Polycystic Kidney Disease: Maternal and Fetal Considerations: A Narrative Review.

BACKGROUND AND AIMS: Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited kidney disorder and an important cause of kidney failure among individuals of reproductive age. Although its management in nonpregnant adults is well established, the implications of ADPKD during pregnancy remain less clearly defined. The aim of this review is to summarize maternal and fetal risks and provide practical management considerations for pregnancy in individuals with ADPKD. METHODS: A narrative review of the literature was conducted using PubMed/MEDLINE and relevant clinical pract

Kidney transplantation after chronic active Epstein-Barr virus infection remission.

Chronic active Epstein-Barr virus is a rare but severe condition characterized by persistent Epstein-Barr virus activation, leading to chronic lymphoproliferation, significant morbidity, and mortality. We report the case of a 34-year-old male with autosomal dominant polycystic kidney disease who achieved chronic active Epstein-Barr virus remission through hematopoietic stem cell transplantation in childhood. Despite long-term remission, the patient developed end-stage renal disease and underwent ABO-incompatible living-donor kidney transplantation. Post-transplant monitoring over 7 months reve

Diagnosing Mendelian Kidney Disease: Hidden Niches in the (Kidney) Genome.

A certain proportion of patients with Mendelian diseases are overlooked, although substantial technical advances in molecular genetics have been achieved. Massively parallel sequencing (MPS) increasingly identifies genetic variants of unknown significance, which may remain clinically unhelpful. Furthermore, difficult niches in the genome exist, which cannot be solved by standard MPS. In the reported family, autosomal dominant kidney disease leading to renal failure in middle adulthood runs through the maternal and paternal family. Comprehensive genetic analyses and customized functional evalua

Macroscopic hematuria in children with autosomal dominant polycystic kidney disease - report from four European tertiary centers.

UNLABELLED: In adults with autosomal dominant polycystic kidney disease (ADPKD), episodes of macroscopic hematuria are relatively common and are predictors of hypertension. We hypothesized that a history of macroscopic hematuria will be associated with hypertension also in children with ADPKD. We retrospectively analyzed 289 children (median age 10.2 years) with ADPKD who were followed in four European tertiary nephrology centers. The presence of ≥ 1 episode of macroscopic hematuria (MaHu) defines a group of MaHu+ patients; the absence defines a group of MaHu- patients. Hypertension was define

Native nephrectomy in relation to kidney transplantation in autosomal dominant polycystic kidney disease patients; a narrative review of the literature.

Autosomal dominant polycystic kidney disease (ADPKD) is one of the leading causes of kidney failure in adults. Many of these patients subsequently require kidney transplantation. There is currently vast debate on whether these patients should undergo native nephrectomy, and if so, the timing of the nephrectomy, pre-transplant, post-transplant, or simultaneously alongside transplantation. There is also uncertainty in the literature on whether unilateral or bilateral nephrectomy should be undertaken. Therefore, this narrative review aimed to summarize the most up-to-date literature regarding the

Modulation of cyst growth in autosomal dominant polycystic kidney disease: mechanistic insights and therapeutic opportunities.

Autosomal dominant polycystic kidney disease (ADPKD) is one of the most common inherited kidney disorders and is characterized by the progressive formation and expansion of fluid-filled cysts, ultimately leading to kidney failure. Although caused by reduced dosage of the polycystin proteins, the disease phenotype arises from a broad disruption of epithelial physiology rather than a single linear pathway. Loss of polycystin function destabilizes epithelial homeostasis and sensitizes cyst-lining cells to proliferative and secretory cues. A central consequence is the emergence of a self-reinforci

β-Hydroxybutyrate: a renoprotective hormone in polycystic kidney disease.

Autosomal dominant polycystic kidney disease (ADPKD) is a common genetic form of chronic kidney disease characterized by highly variable rates of progression. This variability suggests that nutritional, lifestyle, and environmental factors may significantly influence disease course. Despite its prevalence, pharmacological treatment options for ADPKD remain very limited, underscoring the urgent need for safe and effective therapeutic strategies. Recent research has identified ADPKD as a metabolic disorder at the cellular level, marked by impaired mitochondrial function and a pathological relian
