Family-based screening for polycystic kidney disease: the role of health literacy.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is the most common hereditary kidney disorder globally, and early identification of affected family members is essential. The goal of this study was to investigate family screening uptake among relatives of ADPKD patients and to identify patient-level factors affecting screening. METHODS: This cross-sectional study was conducted at Etlik City Hospital, Ankara, with the approval of the local ethics committee. Sociodemographic data, clinical parameters, and family screening outcomes were collected, and health literacy was assessed

Cilia-to-basement membrane signaling is a biomechanical driver in models of autosomal dominant polycystic kidney disease.

Autosomal dominant polycystic kidney disease (ADPKD), the leading genetic cause of kidney failure, results from loss-of-function mutations in PKD1, encoding polycystin-1 (PC1). PC1 localizes to the primary cilium. In the absence of PC1, adverse signaling from the primary cilium orchestrates cyst formation, but the biomechanical underpinnings of this cilia-dependent cyst activation (CDCA) remain unclear. Combining tubule-specific orthologous mouse models with a tubule-on-chip platform, we show that PC1 and cilia govern the composition, mechanical properties, and shape of the tubular basement me

Metabolomics in renal cystic disease: links to mechanisms and identification of pathways for potential prediagnosis.

Polycystic kidney disease (PKD) is a common hereditary kidney disorder with an insidious onset and complex mechanisms, making early diagnosis challenging. Current diagnostic methods, including imaging and genetic testing, have limitations. Imaging detects cysts only once formed, and genetic testing is costly, labor-intensive, and unable to guarantee complete diagnostic accuracy or predict onset and progression. Metabolomics may offer an approach to uncover PKD mechanisms by detecting metabolic alterations that precede clinical manifestations, enabling identification of disease-specific biomark

Kidney outcomes and safety of sodium-glucose cotransporter-2 inhibitor therapy in autosomal dominant polycystic kidney disease: systematic review and meta-analysis.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is the most common hereditary kidney disease and among the leading causes of kidney failure globally. Sodium-glucose cotransporter (SGLT)2 inhibitor therapy is a novel therapeutic alternative for chronic kidney disease but patients with ADPKD were excluded from pivotal trials. Our aim is to evaluate the efficacy and safety of SGLT2 inhibitor therapy in ADPKD patients in terms of kidney and cardiovascular outcomes. METHODS: We conducted a systematic review and meta-analysis adhering to PRISMA guidelines and registered with PROSPE

Autosomal dominant tubulointerstitial kidney disease in Chinese patients.

BACKGROUND: Autosomal dominant tubulointerstitial kidney disease (ADTKD) is recognized as a significant cause of genetic kidney disease. However, data from Asian populations remain limited. METHODS: We evaluated 70 patients meeting KDIGO clinical criteria for ADTKD at Peking University First Hospital between 2010 and 2024. Genetic testing was performed using whole-exome sequencing, with PacBio long-read sequencing employed for cases meeting clinical criteria but lacking identifiable causal variants. Plasma and urinary uromodulin levels were measured and compared between ADTKD, IgA nephropathy

Clinicopathologic Features of Metaplastic Breast Carcinoma: Experience from A Cancer Research Centre in India.

Rapidly growing global demand for nutrient-dense foods is currently testing planetary boundaries, while global food systems are failing to alleviate a persistent burden of malnutrition in human populations. Naked clams (Teredinidae) have recently emerged as a promising candidate to address these deficiencies, offering high-nutrient, low-environmental impact "blue food" production. While previous research demonstrated the viability of maintaining adult naked clams in modular systems, full life-cycle culture and quantified growth performance remained unexplored. This study evaluates the capacity

The Hidden Iceberg of ADPKD: Early Organomegaly-Driven Malnutrition and Sarcopenia Beyond Preserved eGFR.

Autosomal dominant polycystic kidney disease (ADPKD) is the most frequent monogenic kidney disease (≈4 cases per 10.000 inhabitants) and a major cause of end-stage kidney disease (ESKD). Beyond progressive cystic enlargement of the kidneys and frequent extrarenal involvement, adults with ADPKD often exhibit a distinctive "body phenotype" with central adiposity and marked abdominal distension due to renal and hepatic organomegaly. In this setting, conventional anthropometric indices such as body mass index (BMI) and crude body weight are of limited value, as they cannot distinguish nutritional

Long-Term Tolvaptan Administration in Chinese Patients with Autosomal Dominant Polycystic Kidney Disease: A Retrospective Study in Real Clinical Practice.

INTRODUCTION: Tolvaptan is the first drug approved by the FDA for rapidly progressive polycystic kidney disease (PKD). However, there are no studies reporting real-world clinical practice using tolvaptan to treat Chinese patients with autosomal dominant polycystic kidney disease (ADPKD). METHODS: This retrospective multicenter cohort study analyzed ADPKD patients diagnosed by genetic or imaging tests, comparing those treated with tolvaptan (15 mg, twice daily) to unmedicated patients. Clinical course, estimated glomerular filtration rate (eGFR), total kidney volume (TKV), Short-Form 36 (SF-36

Serum and urine prognostic biomarkers for autosomal dominant polycystic kidney disease: a systematic review and meta-analysis.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD), the leading monogenic cause of kidney failure, exhibits heterogeneous clinical progression. This systematic review and meta-analysis synthesize and evaluate current evidence on blood and urine prognostic biomarkers in ADPKD, addressing gaps in understanding their role in predicting progression and guiding clinical trial selection and management. METHODS: We searched PubMed, Embase, and Cochrane up to April 2025 and screened articles in duplicate. We included longitudinal studies evaluating the blood and urine prognostic biomark

Abdominal arterial calcification profile in kidney transplant candidates: a comparison between patients with autosomal dominant polycystic kidney disease and those with other causes of kidney failure.

BACKGROUND: Abdominal arterial calcification is a well-established predictor of cardiovascular events and mortality in kidney failure. Autosomal dominant polycystic kidney disease (ADPKD), a common genetic cause of kidney failure, presents with a distinct vascular profile. However, the burden of abdominal arterial calcification in ADPKD remains inadequately characterized. Clarifying these differences may improve cardiovascular risk stratification and transplant timing. METHODS: We conducted a retrospective cohort study of 4088 adult patients listed for kidney transplant at three Mayo Clinic s

ACR Appropriateness Criteria® Autosomal Dominant Polycystic Kidney Disease.

Ultrasound is the imaging study of choice for the initial diagnosis of autosomal dominant polycystic kidney disease (ADPKD) due to its high diagnostic accuracy and ability to detect kidney cysts as small as 2 to 3 mm. MRI of the kidneys is also highly sensitive at detecting small cysts and is an alternative to US. MRI is the preferred modality for determining total kidney volume (TKV). TKV can be used as an imaging biomarker to predict kidney function decline, track disease progression, and evaluate the effectiveness of treatment. CT abdomen and pelvis with contrast is the test of choice for d

Association Between Tolvaptan Exposure and Venous Thromboembolism Risk in ADPKD: A Real-World Propensity-Matched EHR Study.

AIM: Venous thromboembolism (VTE) has increasingly been reported in autosomal dominant polycystic kidney disease (ADPKD), although the mechanisms underlying this association remain incompletely understood. Whether tolvaptan exposure influences VTE risk in ADPKD has not been well studied. METHODS: We performed a retrospective cohort study using the TriNetX Global Collaborative Network, including adults with ADPKD (ICD-10-CM Q61.2) from 2000 to 2025. Patients with inherited thrombophilia or prothrombin G20210A mutation were excluded. Propensity score matching (1:1) was conducted across 24 covar

First Reported Asian Cases of Robot-Assisted Simultaneous Bilateral Nephrectomy With Kidney Transplantation in Autosomal Dominant Polycystic Kidney Disease: A Case Series.

BACKGROUND Autosomal dominant polycystic kidney disease (ADPKD) is frequently associated with massive kidney enlargement, which complicates kidney transplantation by limiting intra-abdominal space. Simultaneous bilateral nephrectomy with kidney transplantation can reduce surgical burden and avoid staged procedures, but it is technically demanding and rarely performed using robotic surgery. No such cases have been reported previously in Asian patients. CASE REPORT Two adult patients with end-stage kidney disease due to ADPKD presented with severe abdominal distension, pain, early satiety, and i

A HaloTag Knock-In Resource for In Vivo and In Vitro Analysis of Endogenous Polycystin-2 Localization, Turnover, and Transport.

KEY POINTS: A HaloTag knock-in resource allows direct visualization of endogenous polycystin-2 (PC2), enabling quantitative analysis of its localization, turnover, and transport dynamics. PC2-HaloTag labeling establishes PC1-dependent and Tulp3-dependent control of PC2 ciliary targeting in vivo and in cells. BACKGROUND: Polycystin-2, encoded by PKD2 , is a cation channel essential for kidney physiology. Dysfunction of Pkd2 causes autosomal dominant polycystic kidney disease. Currently, our understanding of cystogenesis in the kidney is limited by the difficulty of visualizing the localization

Somatostatin analogues for ADPKD: ending the confusing story of mGFR and eGFR.

This commentary discusses the Lanreotide In Polycystic kidney disease Study (LIPS) that is published in this issue of Kidney International. It represents the last randomized evaluation of somatostatin analogues in patients with autosomal dominant polycystic kidney disease. Lanreotide did not demonstrate a beneficial effect on the primary outcome, measured glomerular filtration rate decline, but did suggest an effect on creatinine-estimated glomerular filtration rate slope. The discrepancy between both outcomes highlights important challenges in the interpretation of kidney function end points

A Single Incision for Simultaneous Liver and Kidney Transplant With Right-Side Nephrectomy for Polycystic Liver and Autosomal Dominant Polycystic Kidney Disease.

We describe a new surgical approach for simultaneous liver and kidney transplant in which we utilized the same reverse L incision used for liver transplant to perform ipsilateral native nephrectomy and kidney transplant. The 48-year-old male patient had auto-somal dominant polycystic kidney and significant polycystic liver disease. He underwent multidisciplinary assessment and was listed for simultaneous liver and kidney transplant. The liver and kidney graft were donated from a donor after neurological determination of death. After hepatectomy and orthotopic liver transplant, a right native n

Living donor kidney transplantation after living donor liver transplantation from the same donor in a patient with autosomal dominant polycystic kidney disease: a case report.

Sequential living donor liver transplantation (LDLT) and living donor kidney transplantation (LDKT) from the same donor are extremely rare, with fewer than 10 such cases reported worldwide. These procedures present unique surgical, ethical, and immunological considerations. A 63-year-old man with autosomal dominant polycystic kidney disease (ADPKD) underwent LDLT from his wife for massive polycystic liver disease complicated by portal hypertension. Three years later, he received LDKT from the same donor following comprehensive evaluation for end-stage renal disease due to ADPKD. Pre-implantati

Peritoneal catheter rupture in an obese patient with the peritoneal wall anchor technique.

Drainage failure during peritoneal dialysis is most commonly due to mechanical complications, such as catheter migration and obstruction. However, intraperitoneal catheter rupture is extremely rare. We report the case of a 48-year-old man with autosomal dominant polycystic kidney disease (ADPKD) who developed intraperitoneal catheter rupture. Despite a thorough analysis of the removed catheter, the definitive cause of rupture could not be determined. Chronic mechanical stress, exacerbated by obesity (body mass index 40.74 kg/m2), was thought to induce catheter rupture. Additionally, the perito

Quantitative analysis of d-amino acids in renal samples using derivatization.

d-Amino acids are the enantiomers of l-amino acids that occur in minimal concentrations within organisms and exert significant effects on physiological regulation and pathological progression. Abnormal d-amino acid levels are closely associated with neurological disorders and metabolic disturbances. The present research is dedicated to establishing a highly sensitive and reliable quantitative detection approach for evaluating metabolic changes in d-amino acids linked to kidney disease, aiming to assist clinical diagnosis. Samples were processed by precipitating proteins, drying them with nitro

Myofibroblast- specific autophagy drives cyst growth in autosomal dominant polycystic kidney disease.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is characterized by progressive cyst expansion, fibrosis and inflammation, leading to kidney failure. Myofibroblasts (MFs) often accumulate around cysts and promote fibrosis and cyst growth, but the cellular mechanisms enabling their pro-cystogenic activity remain unclear. Here we examined the role of autophagy within MFs, on their paracrine stimulation of cyst expansion in ADPKD. METHODS: Autophagy was assessed in human ADPKD nephrectomy tissue, primary human ADPKD renal myofibroblasts (ADPKD-MFs) and male RC/RC mouse model of
