This case report describes a newborn diagnosed with very-early-onset autosomal dominant polycystic kidney disease (VEO-ADPKD) and congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency. The patient presented with prenatally detected kidney cysts and exhibited clinical and laboratory features consistent with salt-wasting, including markedly elevated 17-hydroxyprogesterone levels, requiring treatment with hydrocortisone, fludrocortisone, and sodium chloride. Genetic analysis revealed a de novo truncating variant in PKD1 and two variants in CYP21A2. Hypertension developed at 3 year

Autosomal dominant polycystic kidney disease (ADPKD) is the most common hereditary renal disorder and primarily arises from mutations in either the polycystic kidney disease 1 (PKD1) or 2 (PKD2) gene. Although traditionally categorized as an adult-onset disease, cystogenesis often begins in utero, and structural changes may become evident during childhood. Although most pediatric patients with ADPKD remain asymptomatic, hypertension occurs in 20%-40% of cases, and albuminuria may also be present. Both findings represent early, treatable manifestations that can significantly influence long-term

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is increasingly recognized as a systemic disorder with early vascular involvement, even before significant kidney function decline. Arterial stiffness and ocular microvascular alterations may reflect subclinical vascular changes in this population. However, data regarding the relationship between systemic arterial stiffness and choroidal microvascular structure in early ADPKD are limited.
MATERIALS AND METHODS: A total of 41 patients with ADPKD and 40 controls were included. In this single-center, cross-sectional study, patients

Ferritin dysregulation is implicated in numerous pathological conditions; however, its role in autosomal dominant polycystic kidney disease (PKD) remains poorly understood. Ferritin expression is increased in cyst-lining epithelial cells and macrophages in both PKD mouse and human kidneys. To investigate the functional significance of ferritin/iron homeostasis in disease progression, we generated conditional knockout of ferritin heavy chain (FTH) in collecting duct or myeloid lineage cells of PKD mice. FTH deletion in either cell type did not impact renal cyst growth. Notably, loss of FTH expr

Autosomal dominant tubulointerstitial kidney disease -UMOD is characterized by progressive renal interstitial inflammation and fibrosis. However, its underlying mechanisms remain unclear. Here, we identify a large ADTKD pedigree harboring a novel UMOD p.H36Y mutation. Using CRISPR/Cas9 technology, we generated a UmodH36Y/+ mouse model that recapitulates the key phenotypes observed in affected individuals, including renal dysfunction, cyst formation, and interstitial inflammation. Multi-omics analyses in kidneys from male UmodH36Y/+ mice revealed marked macrophage pyroptosis. Mechanistically, t

Autosomal dominant polycystic kidney disease (ADPKD), the most prevalent hereditary kidney disorder, is characterized by the progressive formation and expansion of kidney cysts that ultimately destroy normal renal parenchyma. Runt-related transcription factor 1 (Runx1), a highly conserved regulator of gene expression, orchestrates diverse cellular signaling pathways. Here, we identify Runx1 as a critical driver of cyst growth in ADPKD. Runx1 expression is markedly upregulated in Pkd1 mutant renal epithelial cells and kidney tissues through a cAMP-dependent mechanism. Pharmacological inhibition

Congenital abnormalities of the biliary tree are increasingly identified in adulthood as incidental findings on cross-sectional imaging. They range from clinically silent anatomical variants to complex malformations with relevant clinical or surgical implications. This review provides a practical framework for radiologists who encounter these entities in adult patients, organizing them into four categories: congenital cystic biliary dilation, fibropolycystic liver disorders arising from ductal plate malformation, anatomical variants of the biliary tree, and rare developmental anomalies such as

KEY POINTS: Peritoneal dialysis can be used successfully in patients with autosomal dominant polycystic kidney disease. Compared with days receiving hemodialysis, peritoneal dialysis days were associated with better patient survival, which has increased over time. Compared with hemodialysis days, peritoneal dialysis days were associated with a higher rate of kidney transplant, which has increased over time.
BACKGROUND: Peritoneal dialysis (PD) is a form of home dialysis that is underutilized in the United States. Patients with autosomal dominant polycystic kidney disease (ADPKD) are frequentl

ObjectiveAutosomal dominant polycystic kidney disease (ADPKD) is a progressive hereditary kidney disorder with limited treatment options. Although semaglutide has demonstrated kidney-protective effects in other populations, its renal effects in ADPKD remain unknown.MethodsWe conducted a retrospective propensity score-matched cohort study using the TriNetX Research Network. Adults with ADPKD between 2000 and 2024 were identified. Patients receiving sodium-glucose cotransporter-2 inhibitors, tolvaptan, or other glucagon-like peptide-1 receptor agonists were excluded. Semaglutide users were match

Polycystic kidney disease (PKD) is a group of inherited disorders characterized by cystic lesions in the kidneys and multiple organs, primarily including autosomal dominant polycystic kidney disease (ADPKD) and autosomal recessive polycystic kidney disease (ARPKD). ADPKD is mainly caused by variations in the PKD1 and PKD2 genes. Its clinical manifestations include progressive renal cyst growth, hypertension, and multi-system complications. ARPKD, on the other hand, is primarily caused by mutations in the PKHD1 gene. It commonly occurs in infants and young children, with hepatorenal cystic fibr

Pancreatic cystic lesions (PCLs) may occur in several hereditary syndromes, although their frequency, phenotype, and clinical implications differ considerably across entities. This review summarizes the spectrum of hereditary-associated PCLs, focusing on von Hippel-Lindau syndrome, autosomal dominant polycystic kidney disease, cystic fibrosis, multiple endocrine neoplasia type 1, and other hereditary pancreatic cancer predisposition syndromes. The lesions most commonly encountered are simple cysts, serous cystic neoplasms, and intraductal papillary mucinous neoplasms, whereas cystic pancreatic

BACKGROUND: With regard to complications of portal vein (PV) and inferior vena cava (IVC) thrombosis in autosomal dominant polycystic kidney disease (ADPKD), several cases have been reported based on imaging findings. However, only one autopsy case has been described and no systematic analysis has been conducted to date. This retrospective study aimed to review autopsy cases from our department over the past 37 years to clarify the frequency and background factors of thrombosis formation in ADPKD.
METHODS: Among 4001 autopsies performed at our institution from 1987 to 2023, 10 ADPKD cases wer

BACKGROUND Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited renal disorder, typically presenting with hypertension, hematuria, or progressive renal dysfunction. However, it can have a broad range of initial presentations and, in uncommon cases, may mimic other abdominal disorders. This report presents a case of ADPKD manifesting as an epigastric mass in a patient without a family history, highlighting its importance in the differential diagnosis of upper abdominal masses. Background and Objectives: The genetic architecture of late-onset autosomal dominant polycystic kidney disease (ADPKD) remains incompletely defined. Pathogenic mutations in two genes that encode polycystin proteins, PKD1 and PKD2, prevail among these patients. Detection of PKD1 alterations by a sequencing method is challenging as this is a large gene with high GC content and multiple pseudogenes. Although next-generation sequencing (NGS) has become the cornerstone of molecular diagnosis, its limited availability in several settings and its limited sensitivity for structural variants may create

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD), primarily driven by PKD1 or PKD2 mutations, is a prevalent hereditary nephropathy for which large-animal models capturing human renal anatomy and disease tempo remain urgently needed. This study aimed to establish a site-specific human PKD2 (hPKD2) transgenic porcine model as a versatile platform capable of supporting two temporally distinct applications: longitudinal evaluation of whether sustained PKD2 overexpression is sufficient to induce renal pathology, and-should overexpression prove phenotypically silent-future crossbree

BACKGROUND: The prevalence of intracranial aneurysms (IAs) in individuals with autosomal dominant polycystic kidney disease (ADPKD) is about 6%-25%. This rate is higher than that of the general population and is associated with increased morbidity and mortality. Studies in Africa are sparse. Therefore, we aimed to describe the clinical manifestations, imaging features, and long-term prognoses of patients with cerebrovascular complications to inform screening and improve patients' awareness.
METHODS: We reviewed the records of patients with ADPKD admitted to Inkosi Albert Luthuli Central Hospi

Most cases of autosomal dominant polycystic kidney disease (ADPKD) are caused by mutations in PKD1, which reduce polycystin-1 (PC1) levels below a critical functional threshold. Normalizing PC1 dosage mitigates disease progression; therefore, we sought to develop a CRISPR activation (CRISPRa) strategy to transcriptionally upregulate endogenous PKD1. We systematically screened multiple single-guide RNAs using an EGFP-reporter platform and identified potent candidates targeting the proximal PKD1 promoter in mouse and human cell models. Our results demonstrate that CRISPRa effectively increased e

Transcription factors from the family of Signal Transducers and Activators of Transcription (STAT) are activated by numerous cytokines. Two members of this family, STAT5A and STAT5B (collectively called STAT5), have gained prominence in that they are activated by a wide variety of cytokines such as interleukins, erythropoietin, growth hormone, and prolactin. Furthermore, constitutive STAT5 activation is observed in the majority of leukemias and many solid tumors. Inactivation studies in mice as well as human mutations have provided insight into many of STAT5's functions. Disruption of cytokine
