TTC21B: From Modifier to Causative Gene in Joubert Syndrome.

Biallelic pathogenic variants in TTC21B, encoding the retrograde intraflagellar transport protein IFT139, are a known cause of renal and skeletal ciliopathies. Single heterozygous variants in this gene were found to be enriched in a cohort of patients with various ciliopathy phenotypes, including three individuals with Joubert syndrome (JS), a neurodevelopmental ciliopathy characterized by a peculiar cerebellar and brainstem malformation. It was then suggested that TTC21B could act as a modifier gene for ciliopathies. Here we report a patient homozygous for the hypomorphic founder variant p.Pr

Renal dysplasia in horses: 25 cases (1991-2021).

BACKGROUND: Renal dysplasia (RD) is a congenital malformation with variable clinical expression and limited characterization in horses. HYPOTHESIS/OBJECTIVES: We hypothesized that RD occurs in horses across a broad age range and may be overrepresented in specific breeds. Our objective was to describe clinical, clinicopathologic, and histopathologic findings in horses with RD. ANIMALS: Twenty-five horses with RD identified from a veterinary teaching hospital between 1991 and 2021. Horses included neonates, young horses, and adults, with ages ranging from 0 days to 24 years and representing mu

Placenta-derived CXCL1 is associated with fetal renal dysplasia upon gestational cadmium treatment.

Congenital anomalies of the kidney and urinary tract are the leading cause of chronic kidney disease in children. Cadmium (Cd is a developmental toxicant that acts primarily by impairing placental function. However, the underlying placenta-origin injury mechanisms of gestational Cd-induced fetal renal dysplasia have not been fully elucidated. Here, we established a mouse model by administering pregnant CD-1 mice a single intraperitoneal injection of cadmium chloride solution (0, 2.25, and 4.50 mg/kg) on gestational day 8. We demonstrated that gestational Cd treatment induced placental senescen

Severe bilateral ocular coloboma and microphthalmia in a neonate with WDR37-associated neurooculocardiogenitourinary syndrome.

We describe a neonate with severe bilateral ocular abnormalities associated with WDR37-related neurooculocardiogenitourinary syndrome. Ocular findings identified at birth included bilateral microphthalmia, microcornea, corneal edema, marked anterior segment dysgenesis, and bilateral colobomatous posterior pole abnormalities. The patient also had multiple systemic anomalies, including complex congenital heart disease, renal dysplasia, skeletal abnormalities, and cerebellar malformation. Clinical exome sequencing identified a de novo pathogenic variant in the WDR37 gene, confirming the diagnosis

Correlation of pre-operative sonographic and nuclear scan parameters with intraoperative renal histopathology in children with congenital pelviureteric junction obstruction.

BACKGROUND: Pelviureteric junction obstruction (PUJO) is the most common cause of pediatric hydronephrosis; however, distinguishing obstructive and non-obstructive dilatation remains challenging. Existing diagnostic modalities are limited by the lack of validation against objective measures of renal injury. Preoperative imaging is critical for guiding surgical decision-making; however, its correlation with histopathologic renal damage has not been fully elucidated. OBJECTIVES: To evaluate the association between preoperative sonographic and nuclear scintigraphy parameters and intraoperative r

Bilateral Glomerulocystic Kidney Disease With Extensive Embryonal Hyperplasia in a Setting of HNF1B Mutation.

Glomerulocystic renal disease has numerous etiologies, including HNF1B mutations. In addition to cysts, morphologic renal findings in a setting of HNF1B mutations include cystic renal dysplasia, solitary functioning kidney, horseshoe kidney, and oligomeganephronia. Embryonal hyperplasia resembling nephrogenic rests has been reported in rare cases of glomerulocystic disease, but none have been genetically characterized. We report a case of bilateral glomerulocystic kidney disease (GCKD) showing extensive embryonal hyperplasia in a setting of germline HNF1B mutation with progressive renal failur

Unilateral Multicystic Dysplastic Kidney in a Fetus Associated With Parental Genetic and Environmental Risk Factors: A Case Report.

Multicystic dysplastic kidney (MCDK) is a congenital renal anomaly identified on prenatal ultrasound. They often arise sporadically and unilaterally. Our case involves an isolated unilateral MCDK in a fetus born to a mother with generalized anxiety disorder (GAD), obsessive-compulsive disorder (OCD), and gastroesophageal reflux disease (GERD), with the father having chronic occupational lead exposure and a congenital disorder of glycosylation type 1A (PMM2-CDG). Our case highlights the multifactorial etiology of renal dysplasia and its potential role of glycosylation defects and environmental

Prolonged Survival With Homozygous Deletion of Exon 9 in Perlman Syndrome: A Case Report.

Perlman syndrome is a rare autosomal recessive overgrowth disorder characterized by macrosomia, nephromegaly, renal dysplasia, and characteristic facial features. It has both similarities and differences to other more common overgrowth syndromes. Pathogenic homozygosity is extremely rare in nonconsanguineous relationships. Survival is predicted by differences among various germline mutations in the DIS3L2 gene on chromosome 2q37.1, with prolonged survival documented in heterozygous mutations allowing partial exoribonuclease function. Although homozygous deletions of exon 9 are rare and have be

Adult Diagnosis of Solitary Kidney and Renal Dysplasia in a Male Born Prematurely as a Twin: A Case Report.

BACKGROUND: Congenital anomalies of the kidney and urinary tract (CAKUT) are a common cause of chronic renal disease and usually manifest either antenatally or in the early years of life. Solitary Kidney with Renal Dysplasia presents early in life due to regular antenatal sonographic scans. The rarity of diagnosis being deferred until adulthood has important implications when there is prematurity and other associated congenital anomalies, such as Congenital Heart Disease. CASE PRESENTATION: We present a 23-year-old male experiencing sequential generalized edematous changes, decreased urine ou

Meckel-Gruber syndrome: a rare and fatal congenital disorder (case report).

Meckel-Gruber syndrome is a rare congenital disorder characterized by multiple malformations. It transmits via a recessive autosomal mode. It is characterized by an occipital encephalocele, polydactyly, and polycystic renal dysplasia. The diagnosis could be established via ultrasound. In fact, it is actually the key method for the early screening of this lethal malformation with at least two of its main characteristics. However, the diagnosis is confirmed by karyotype analysis. Herein, we present a case of Meckel-Gruber syndrome diagnosed in a fetus from a consanguineous marriage in a 40-year-

Premature Rupture of Membranes after Serial Amnioinfusions in Fetuses with Early Anhydramnios due to Severe Renal Disease/Lower Urinary Obstruction.

INTRODUCTION: This study estimates the frequency of preterm premature rupture of membranes (PPROM) in fetuses with early anhydramnios due to severe renal or urinary disease treated with serial amnioinfusions (AIs). METHODS: Fetuses diagnosed with early anhydramnios (<25 weeks gestation) secondary to severe renal disease or urinary tract obstruction underwent serial AIs at two academic centers, to reduce uterine compression and prevent pulmonary hypoplasia. Fetuses with structural or chromosomal anomalies were excluded. AIs were performed to maintain an amniotic fluid index (AFI) between 15 an

Transient neonatal diabetes mellitus as an early diagnostic clue to HNF1B-related disease - two case reports and a literature review.

BACKGROUND: Pathogenic variants in the HNF1B gene cause a multi system disorder encompassing organ abnormalities-primarily affecting the kidneys and pancreas-as well as metabolic disturbances, collectively referred to as HNF1B-related disease. While maturity-onset diabetes of the young type 5 is a well-recognized manifestation, neonatal diabetes mellitus (NDM) associated with HNF1B is exceedingly rare, and has only been reported in patients harboring single nucleotide variants. CASE PRESENTATION: We describe two unrelated female children presenting with transient NDM caused by a complete HNF1

Double Threat in a Newborn: High Anorectal Malformation with Pouch Colon and Bilateral Multicystic Dysplastic Kidneys - A Rare Case Report.

The coexistence of bilateral multicystic dysplastic kidneys (MCDKs) and high anorectal malformation with pouch colon is exceptionally rare. The combination results in a nonsurvivable condition due to complete renal failure, posing complex diagnostic and management challenges. A full-term male neonate presented with absent anal opening, limb and auricular deformities, and progressive anuria since birth. Antenatal history was unavailable. Clinical examination revealed bilateral clubfoot and normal male genitalia. Laboratory evaluation showed steadily worsening renal function. Ultrasound demonstr

An unusual segmental retinal venous abnormality detected during retinopathy of prematurity screening in an infant with a PAX2 variant.

PURPOSE: Congenital abnormalities of retinal venous development are rare and may be difficult to distinguish from prematurity-related vascular changes when detected during retinopathy of prematurity screening. In premature infants, atypical posterior pole vascular findings should not be automatically attributed to retinopathy of prematurity alone. The purpose of this report is to describe an unusual segmental retinal venous abnormality detected during retinopathy of prematurity screening in a premature infant with a PAX2 variant and renal dysplasia. METHODS: We report a premature male infant

The pedfix technique: a new minimally invasive method of peritoneal dialysis catheter insertion with secure fixation to the abdominal wall-a preliminary experience.

PURPOSE: Peritoneal Dialysis (PD) is the most common modality of renal replacement therapy in children with acute kidney injury (AKI). Various open and laparoscopic techniques for the placement of peritoneal dialysis (PD) catheter, with or without intra-abdominal fixation, have been described. The incidence of catheter migration after PD is 0-19%. We present a new, modified technique which minimizes the risk of catheter migration and blockage. MATERIALS AND METHODS: For 18 years a laparoscopic assisted PD catheter insertion has been used by the first author with a single 5 mm Port at the umbi

Subcutaneous Alemtuzumab as Salvage Therapy for Acute Kidney Transplant Rejection Following Thymoglobulin Anaphylaxis: A Pediatric Case Report.

BACKGROUND: Acute kidney allograft rejection is a significant cause of graft dysfunction in pediatric transplant recipients. Standard of care for moderate to severe acute T-cell-mediated rejection among pediatric transplant recipients includes lymphocyte-depleting agents such as rabbit anti-thymocyte globulin (rATG). However, intolerance to or contraindications to rATG may limit its use and necessitate alternative immunosuppressive strategies. Alemtuzumab, a humanized monoclonal antibody targeting CD52, induces profound depletion of T- and B- lymphocytes and has been used in the transplantatio

[Genetic analysis of a fetus with renal dysplasia/hypoplasia due to a variant of GREB1L gene and literature review].

OBJECTIVE: To carry out clinical and genetic analysis for a fetus with absent kidneys and bladder. METHODS: Clinical data of the fetus were collected. Whole exome sequencing (WES) and Sanger sequencing were carried out on fetal tissue and peripheral blood samples from its family members. A RDDC online tool was used to predict the impact of the variant on the GREB1L gene splicing, which was further validated by in vitro minigene assays. Long-range PCR was employed to verify the exonic deletional variant. An I-TASSER server was used to predict the three-dimensional structure of the mutant prote

Prenatal ultrasound diagnosis of sirenomelia: A case report.

Sirenomelia is an extremely rare and almost invariably lethal congenital malformation characterized by variable fusion of the lower limbs and severe associated visceral anomalies. We report a case prenatally diagnosed at 20 weeks of gestation in a 25-year-old nondiabetic, nonconsanguineous woman referred for suspected polymalformative fetal syndrome. Prenatal ultrasound demonstrated severe oligohydramnios, bilateral small hyperechoic kidneys suggestive of renal dysplasia, fused lower limbs, absent external genitalia, and a single umbilical artery, findings strongly supporting the diagnosis of
