Functional analysis of heterozygous variants in the SALL1 gene in 2 children with Townes-Brocks syndrome with FSGS.

BACKGROUND: To investigate the functional characteristics of heterozygous variants in the SALL1 gene in two children with Townes-Brocks syndrome (TBS), as well as the underlying mechanisms leading to chronic kidney disease. METHODS: We retrospectively analyzed the clinical manifestations, laboratory findings, and genetic backgrounds of two TBS patients admitted to our hospital presenting with chronic kidney disease stage 2 and focal segmental glomerulosclerosis (FSGS). The genetic function was investigated through cultured cells transfected with variants. The Sall1 protein expression level wa

Unilateral Multicystic Dysplastic Kidney in an Infant: A Case Report of Surgical Nephrectomy, Diagnostic Imaging, and Long-Term Outcomes.

Multicystic dysplastic kidney (MCDK) is a common congenital renal anomaly often considered benign when unilateral and managed conservatively. We report a female infant diagnosed antenatally with left-sided MCDK in the setting of maternal gestational diabetes mellitus (GDM). Postnatal imaging confirmed a nonfunctioning left kidney and a structurally normal right kidney with reduced glomerular filtration rate (GFR) of 49.7 mL/min/1.73 m². Laparoscopic nephrectomy at six months revealed complete parenchymal replacement by cysts with focal cartilaginous metaplasia. Over four years of follow-up, th

Prenatal Diagnosis of Vaginal Ectopic Ureter Insertion-Case Outcome and Literature Overview.

Background and clinical significance: Ectopic ureters are a rare urinary tract malformation, typically diagnosed in childhood and infrequently in adulthood. The prenatal detection by ultrasound and magnetic resonance imaging (MRI) of this clinical entity has scarcely been reported. Careful foetal scanning during the late second and third trimester might provide clues and lead to prenatal detection. However, even the postnatal diagnosis is challenging, and often delayed towards adulthood, since the condition may present with nonspecific symptoms, leading to underdiagnosis or misdiagnosis. In fe

Prenatal treatment of posterior urethral valves in a newborn with anorectal stenosis.

Posterior urethral valves (PUV) account for most cases of lower urinary tract obstruction (LUTO) in male fetuses, with a prevalence of 1 in 5,000 live births. Prenatal ultrasound findings include bladder wall thickening, keyhole sign, oligohydramnios, and ureteral dilatation. If untreated, PUV can cause pulmonary hypoplasia and renal failure, often requiring dialysis or transplantation. Treatment options include vesicoamniotic shunting and fetoscopic laser fulguration. PUV is rarely associated with anorectal malformations, posing a diagnostic and therapeutic challenge. This case report describ

Development and face validity testing of pyeloplasty surgical training models.

BACKGROUND: There are few realistic and accessible laparoscopic pyeloplasty training models. The aim of this study is to develop a functional training model through user-centered design that depicts the pathological features of pelvicoureteric junction obstruction (PUJO). METHODS: Realistic patient-matched anatomical models of the kidney, renal pelvis, and ureter were created based on segmentations of computed tomography (CT) scans of a paediatric patient with PUJO who underwent laparoscopic pyeloplasty. Three training model versions (polyjet, resin, and silicone) were produced, tested, and p

Ectopic ureter presenting as a scrotal fistula associated with unilateral atrophic kidney: a rare case report.

INTRODUCTION: An ectopic ureter is the one that reaches any place other than the trigone in the bladder, in the bladder neck or distal to it and is often associated with a single or duplicated collecting system, associated with renal dysplasia and obstruction. Ectopic ureter opening into the scrotal base as urocutaneous fistula is a rare finding. CASE PRESENTATION: A 14 years old male child presented with intermittent serous discharge from the base of scrotum for 2 months, and unilateral scrotal swelling for 1 week. On evaluation of the condition as scrotal sinus, the X-ray fistulogram and mi

Research Progress on Risk Factors or Prediction Models for Ureteropelvic Junction Obstruction in Children.

Ureteropelvic junction obstruction (UPJO), characterised by prenatal or postnatal renal pelvis dilation, represents the primary cause of congenital paediatric hydronephrosis. UPJO may lead to impaired renal function in paediatric patients. Its pathogenesis includes genetic predisposition and anatomical abnormalities. While spontaneous resolution may occur in some infants, progressive hydronephrosis can lead to renal impairment without intervention. The assessment of the degree of hydronephrosis, renal dysfunction and surgical indications in paediatric patients before surgery is beneficial for

Myelomeningocele in a newborn with VACTERL association.

VACTERL association refers to the concomitant occurrence of congenital malformations such as vertebral defects, anorectal malformations, cardiac anomalies, tracheoesophageal fistula, renal anomalies, and limb defects. This case describes a newborn girl who presented with anomalies compatible with this association, such as scoliosis, sacral dysgenesis, right lower phocomelia, and multicystic renal dysplasia, also associated with myelomeningocele, which was surprising as it occurred concomitantly with the other congenital malformations. Although neural tube defects are not part of the classic cr

Long-Term Renal Transplant Success Is Possible in Hypoparathyroidism, Sensorineural Deafness, and Renal Dysplasia Syndrome: A Case Report.

INTRODUCTION: Hypoparathyroidism, sensorineural deafness, and renal dysplasia (HDR) syndrome is caused by haploinsufficient GATA3 variants. Renal disease is present in 72% of patients with HDR syndrome, with a widely variable age of onset and rate of progression. Overall prognosis is primarily dependent upon the severity of renal disease. Four patients have been previously reported to have kidney transplants due to HDR syndrome, but there are minimal data describing transplant outcomes. We describe a case of a 74-year-old male with HDR syndrome receiving a living related renal transplant lasti

GATA3 Deletion Associated With Juvenile Idiopathic Arthritis: Expanding the Phenotypic Spectrum of Hypoparathyroidism, Sensorineural Deafness, and Renal Dysplasia (HDR) Syndrome.

Hypoparathyroidism, sensorineural deafness, and renal dysplasia (HDR) syndrome is caused by pathogenic variants in the GATA3 gene located on chromosome 10p14. Here we present a 10-year-old girl with HDR syndrome who also has oligoarticular juvenile idiopathic arthritis (JIA). The patient presented for genetics evaluation with bilateral sensorineural hearing loss, oligoarticular JIA, and a renal cyst. Trio-based exome sequencing and chromosome microarray analysis revealed a pathogenic heterozygous ~1.45 Mb deletion that included the entire GATA3 gene, consistent with a diagnosis of HDR syndrome

Heterozygous variants of uncertain significance in NPHS1 and CRB2 in a newborn with congenital nephrotic syndrome of the Finnish type and multiple fetal anomalies: a case report.

BACKGROUND: Congenital nephrotic syndrome of the Finnish type (CNF) is an autosomal recessive disorder resulting from mutations in the NPHS1 gene, which encodes nephrin, an essential protein in the podocyte slit diaphragm. Pathogenic mutations of NPHS1 cause substantial proteinuria detectable at birth. Variants of uncertain significance (VUS) are recognized mutations, but their effects on health are not yet understood. This uncertainty makes it challenging to identify specific types of VUS that may contribute to disease development. The association of CNF with NPHS1 VUS remains unclear. CASE

Recurrent pelvi-ureteric junction obstruction due to eosinophilic pelvi-ureteritis.

Eosinophilic pelvi-ureteritis is an uncommon inflammatory condition characterised by eosinophil infiltration in the urinary tract, which can lead to varied clinical presentations ranging from urinary obstruction to reflux. Here, we report the case of a boy in early childhood initially diagnosed with right-sided pelvi-ureteric junction (PUJ) obstruction (PUJO) presenting as an abdominal lump with gross hydronephrosis. The patient underwent laparoscopic pyeloplasty over a double-J stent. Postoperatively, the patient exhibited features of recurrent PUJO after stent removal. The patient was taken

Combined Urogenital and Musculoskeletal Malformations in a Dog: A Case Report.

A dog was examined due to urinary incontinence. On physical exam a bipartite scrotum, hypospadias, and hypodontia were observed. Retrograde cystourethrography and abdominal ultrasonography revealed bilateral ectopic ureters and renal dysplasia, respectively. The hypospadias and ectopic ureters were surgically corrected, and the patient's renal disease and recurrent urinary tract infections were medically managed over the next several months. This paper describes possible embryological etiologies for this unique case.

Case Report: Diagnostic overlap of OHVIRA syndrome and Gartner duct cyst: challenges in imaging and management.

This case series explores the diagnostic overlap between OHVIRA syndrome (obstructed hemivagina and ipsilateral renal anomaly) and Gartner duct cyst (GDC) with ipsilateral renal dysplasia in two prepubertal girls. Both cases exhibited similar imaging features on CT and MRI-including unilateral renal agenesis, ectopic ureter with abnormal insertion, cystic lesions posterolateral to the bladder- which led to significant diagnostic challenges. Despite differing in pathology, the overlap in embryologic origins and imaging findings made differentiation difficult. This series underscores the importa
