Chronic Chagas' disease: possible mechanism of sinus bradycardia.

OBJECTIVE: To determine the cause of sinus bradycardia in patients with chronic Chagas' disease by studying their sinus node function with pharmacological agents. DESIGN: Cohort study. SETTING: Referral tertiary care hospital in Cordoba, Argentina. PATIENTS: Thirty-seven chagasic patients (19 females and 18 males, mean age 41 +/- 12 years) were divided into three groups: group 1, 14 patients with positive serology for Chagas' disease but no evidence of cardiac involvement; group 2, 14 patients with Chagas' disease and electrocardiographic abnormalities but without signs or symptoms of conge

Experimental Chagas' disease: electrophysiology and cell composition of the neuromyopathic inflammatory lesions in mice infected with a myotropic and a pantropic strain of Trypanosoma cruzi.

C3H/HeN mice infected with the pantropic/reticulotropic Trypanosoma cruzi RA strain disclosed electromyographic signs (EMG) of neuropathic damage, while those infected with the myotropic CA-I strain showed EMG suggestive of primary muscle involvement. Although both strains induced inflammatory infiltrates in hamstring muscles (HM), damage was more severe in mice infected with CA-I. In sciatic nerves (SN) of mice infected with the RA strain, increased inflammatory changes, amastigote nests, and myelin digestion chambers were consistently found during the course of infection. On the other hand,

Functional changes of the sciatic nerve in mice chronically infected with Trypanosoma cruzi.

Early histological studies carried out in the sciatic nerve of mice chronically infected with Trypanosoma cruzi showed demyelination and scanty axonal degeneration. The experiments reported in this paper were designed to assess the functional state of the sciatic nerve and of some of the muscles it supplies. For these purposes 14 mice were infected with trypomastigotes (clon K-98, CA-I strain) 12 months before the investigation. Results were compared with 13 normal mice matched by age and weight. Hamstring muscles were studied electromyographically by means of a fine coaxial needle electrode a

Central nervous system involvement in Chagas' disease. An updating.

A review was made of the available literature on central nervous system (CNS) involvement in Chagas' disease. Thirty-one works concerning the acute nervous form and 17 others dealing with the chronic nervous form, all presenting neuropathologic studies, were critically analysed. Based on this analysis, an attempt was made to establish the possible natural history of CNS involvement in Chagas' disease. Among others, the following facts stand out: 1) the initial, acute phase of Trypanosoma cruzi infection is usually asymptomatic and subclinical; 2) only a small percentage of cases develop enceph

FL-160 proteins of Trypanosoma cruzi are expressed from a multigene family and contain two distinct epitopes that mimic nervous tissues.

The partial sequence of a gene encoding the COOH terminus of a protein of apparent molecular weight of 160 kD associated with the flagellum of trypomastigotes of Trypanosoma cruzi (FL-160 now renamed to FL-160-1) has been previously reported. The COOH terminus of FL-160-1 has an epitope, defined by 12 amino acids, which molecularly miMics a nervous tissue antigen of 48 kD found in myenteric plexus, sciatic nerve, and a subset of cells in the central nervous system. We now report that FL-160 is a family of highly related genes. The sequence has been determined for the entire open reading frame

[Chronic Chagas' cardiopathy causing congestive heart failure in childhood: a clinical and histopathological study of a case with emphasis on the lesions of the intracardiac conduction and autonomic nervous systems].

A case of decompensated chagasic cardiopathy in a nine-year-old boy from the south of the State of Goiás, is described. He developed congestive heart failure four months before death. The serological reaction for Chagas' disease and the xenodiagnosis were positive. Electrocardiograms showed sinusal tachycardia, ventricular and supraventricular extrasystoles, left anterior hemiblock, complete right bundle branch block and signs of chambers overload. The echocardiogram demonstrated chamber dilatation with diffuse hypocontractility. He presented a downhill course complicated with several pneumoni

Cardiac arrhythmias in Chagas' heart disease.

Chagas' disease is a chronic parasitosis affecting most Latin American countries. Its most important clinical manifestation is a late developing chronic myocarditis and, much less frequently, an early acute myocarditis. Chagasic myocardial damage is microfocal and disseminated throughout the heart. In most cases, the coexistence of areas of myocytic degeneration, inflammatory infiltration, and fibrosis suggests a permanent evolving process. Commonly, chronic chagasic myocarditis resembles a dilated cardiomyopathy, with characteristic ECG abnormalities (atrial and ventricular extrasystoles, int

[Chronic nervous form of Chagas disease: clinical course and anatomopathology of a case followed-up for 20 years].

The chronic nervous forms of Chagas' disease involve both the central and peripheral nervous systems although they are not detected at the frequency indicated by Chagas in his initial observations. The present report concerns a patient with chronic Chagas' disease since childhood who progressively developed involvement of voluntary motility, muscle tone, coordination, and cranial nerves. The patient also had Chagas' heart and colon disease. The patient died after surgery for hemicolectomy and was autopsied. Histologic study of the central nervous system revealed demyelination of the spinocereb

Abnormal progression of a liquid meal through the stomach and small intestine in patients with Chagas' disease.

This study describes the abnormal pattern of gastrointestinal progression of a liquid meal in patients with the digestive form of chronic Chagas' disease. This condition is known as a natural model of intramural denervation of the gut. Sixteen patients with clinical and radiographic evidence of esophageal and/or colonic involvement and 18 healthy volunteers were studied. Orocecal transit time after the ingestion of a 10% lactulose solution (180 ml) tagged with 99mtechnetium was measured by a conventional H2 breath technique. Gastric emptying and the arrival of the front of the meal to regions

Scintigraphic study of the gastrointestinal transit of a liquid meal in patients with chronic Chagas' disease.

Gastric emptying and small bowel transit of a liquid meal (isotonic dextrose) were assessed by a scintigraphic technique in 16 patients with Chagas' disease involving the esophagus and/or the colon, including one case with mega-jejunum, and in 10 control subjects. Initial gastric emptying was faster in the Chagas' disease group than in controls (gastric retention at 15 min, median and (range): 52% (15-86) vs 71% (43-97), P < 0.01) although there was no significant difference in total gastric emptying (T1/2: 20 min (4-132) vs 31 min (13-280), P > 0.05). Chagas' disease.

Chagas' disease, caused by Trypanosoma cruzi, is an important cause of morbidity in many countries in Latin America. The important modes of transmission are by the bite of the reduviid bug and blood transfusion. The organism exists in three morphological forms: trypomastigotes, amastigotes, and epimastigotes. The mechanism of transformation and differentiation is currently being explored, and signal transduction pathways of the parasites may be involved in this process. Parasite adherence to and invasion of host cells is a complex process involving complement, phospholipase, penetrin, neuramin

Central nervous system involvement in experimental Trypanosomiasis cruzi.

A review of the available literature on central nervous system involvement in experimental trypanosomiasis cruzi is undertaken. From a critical analysis of 26 works on experimental infections with Trypanosoma cruzi (23 on the acute phase, 2 on the chronic phase, and one describing sequentially both phases), all supported by neuropathologic studies, it can be concluded that: 1) central nervous system involvement during the acute phase, in the form of encephalitis in multiple foci, with variable intensity of the parasitism and inflammatory changes, is frequent and well documented; 2) in animals

Histopathological and immunohistochemical study of the brain in the acute and chronic phases of experimental trypanosomiasis cruzi in dogs.

A histopathological and immunohistochemical study was undertaken of the brains of 12 young purebred Pinscher dogs, eight of which were inoculated intraperitoneally with the Colombiana strain of Trypanosoma cruzi. The four non-infected dogs formed the control group. An immunoperoxidase method was employed for demonstrating amastigotes of T. cruzi in tissue sections. Three infected dogs died in the acute phase; the other five were sacrificed at different stages of the chronic phase, one of them having survived for almost three years after the inoculation. Histopathological and immunohistochemica

Cerebral evoked potentials in human chronic Chagas' disease.

Seventy five patients with the diagnosis of chronic Chagas' disease were studied by employing EPs techniques. Two of them had delayed arrival of the signal to the Erb's point and one to the spinal cord when looking at SEPs. Two patients had increment of the time interval between waves Ist and IIIrd, when studying PEATs. These findings were interpreted as due to peripheral nerve fibers damage, a feature described in previous papers. The most striking finding was the prolonged time interval between waves N13 and N20 (SEPs) found in two patients and between waves IIIrd and Vth (PEAT) seen in 7 af

A sequential study of the peripheral nervous system involvement in experimental Chagas' disease.

To search for the sequential compromise of the spinal cord, nerves, and skeletal muscle in mice chronically infected with Trypanosoma cruzi, animals were subjected to electromyographic investigation, end-plate recordings, and histological studies at 7, 15, 37, 60, 90, 120, 180, 270, and 360 days postinfection. Electromyographic studies showed signs of motor unit remodeling as early as 15 days postinfection, when diminished duration and amplitude of motor unit potentials pointing to a primary muscle involvement were found. Thereafter, certain features of denervation, reinnervation, and primary

[American trypanosomiasis (Chagas disease) and the nervous system].

Ten to twelve million people irregularly distributed mainly through extensive rural areas of Latin America are afflicted by American trypanosomiasis (Chagas disease). Trypanosoma cruzi is the etiological agent, and it is naturally transmitted to humans by hematophagous hemiptera of Triatominae sub-family. These hemiptera feed by biting and usually defecate in the area near the puncture wound. Mucous membranes of breaks in the continuity of skin serve as passage ways for the parasite present in the excrement of the bug. Acute and chronic forms of American trypanosomiasis occur. Valsalva maneuver: a test of the functional state of cardiac innervation in Chagasic myocarditis.

The ratio between maximal and minimal R-R intervals measured during the Valsalva maneuver is an adequate means of studying the functional state of the cardiac parasympathetic nervous system. We studied the changes in heart rate evoked during the Valsalva maneuver in 49 asymptomatic Chagasic patients with different degrees of myocardial involvement. The Chagasic patients were divided in three groups. Twelve patients had no evidence of heart disease except for an abnormal myocardial biopsy. Sixteen patients had a normal electrocardiogram but revealed evidence of localized myocardial damage as sh

Peripheral nervous system damage in experimental chronic Chagas' disease.

Electromyographic and histopathologic studies were performed in Rockland mice chronically infected with CA-I Trypanosoma cruzi strain. At 4 months post-infection the emg failed to show spontaneous activity, but a diminished interference pattern was detected in half of the infected group, while mean motor unit potential amplitude and duration were increased, compared with controls. An active denervation was observed at 6 months which persisted up to 9 months, when motor unit potential showed a significantly lower mean activity and duration. At 12 months most of the infected mice developed a red

Epitopes common to trypanosomes (T. cruzi, T. dionisii and T. vespertilionis (Schizotrypanum)): astrocytes and neurons.

An autoimmune mechanism is commonly invoked to explain the occurrence of neuronal destruction in the chronic phase of Chagas' disease. Monoclonal antibodies raised against T. dionisii (DION) and T. vespertilionis (VESP), and cross-reactive with T. cruzi recognize antigens in cultured cerebellar cells from embryonic and postnatal mice, as revealed by indirect immunofluorescence. Astrocytes (labelled with rabbit anti-GFAP antibody) showed positive reactions with DION 12.7, VESP 8.2 and VESP 9.3 while neurons (labelled with either tetanus toxin or anti-neuron-specific enolase antibody) reacted wi
