BACKGROUND Renal cysts and diabetes syndrome (RCAD), caused by heterozygous variants or whole-gene deletions in the HNF1B gene, is a rare, multisystem disorder often detected prenatally by ultrasound findings of bilateral cystic or hyperechogenic kidneys. CASE REPORT We present the case of a 21-year-old woman (G3P2) at 19 weeks of gestation referred for detailed fetal evaluation due to bilateral hyperechogenic, polycystic kidneys and severe oligohydramnios. After counselling, an amnioinfusion was performed to enable amniocentesis and cytogenetic testing. Chromosomal microarray analysis identif

Nephronophthisis type 4 (NPHP4) is a rare genetic kidney disorder progressing to end-stage renal disease (ESRD). In this study, we aimed to identify the genetic cause of NPHP4 in a pedigree with three affected individuals. Whole-exome sequencing was performed on the proband, and variants were filtered, analyzed, and evaluated using in silico tools. Co-segregation analysis was conducted using Sanger sequencing. Protein modeling for the wild-type and mutant forms was performed using AlphFold3. We identified a homozygous deletion (c.2999_3005delTGTGTGT/ p.Asn1000SerfsTer4) in exon 21 of NPHP4, wh

Wang et al. showcase a green mamba-derived compound, MQ232, which interferes with cyst growth in a mouse model of autosomal dominant polycystic kidney disease, similar to the only approved drug for autosomal dominant polycystic kidney disease treatment, tolvaptan. However, the authors reveal cyclic adenosine monophosphate-dependent and -independent components in MQ232's mode of action and gender differences in efficacy. Therefore, MQ232 may pave the way to novel treatment options for autosomal dominant polycystic kidney disease.

BACKGROUND: Beckwith-Wiedemann syndrome (BWS) is a congenital overgrowth disorder that may be associated with renal abnormalities, although benign renal manifestations presenting in adulthood are less well characterized.
CASE DESCRIPTION: We report the case of a 20-year-old woman with genetically confirmed BWS who was diagnosed with medullary sponge kidney (MSK) during adulthood after imaging evaluation for suspected renal abnormalities. Contrast-enhanced computed tomography and intravenous urography demonstrated characteristic medullary collecting duct dilatation with nephrocalcinosis, consi

BACKGROUND: Inherited kidney diseases (IKDs) and congenital anomalies of the kidney and urinary tract (CAKUT) are a clinically heterogeneous subset of chronic kidney disease and rank among the leading causes of kidney failure (KF), especially in younger patients. Although next-generation sequencing has expanded diagnostic opportunities, data linking genetic findings to clinical outcomes across both adults and children remain limited.
METHODS: We enrolled 256 patients with suspected IKD (175 adults, 81 children) at a single tertiary center between 2020 and 2023. Genetic testing was performed u

Pregnancy in women with autosomal dominant polycystic kidney disease (ADPKD) complicated by significant cardiovascular disease is high risk. We report a multiparous woman in her late 30s with ADPKD, chronic hypertension and prior anterior wall myocardial infarction who was found to have a left ventricular mural thrombus at 12 weeks' gestation. She was managed with anticoagulation, pregnancy-adjusted antihypertensive therapy and multidisciplinary care. Fetal growth restriction developed at 32 weeks, and emergency caesarean delivery was performed at 35 weeks for absent end-diastolic flow in the

Autosomal dominant polycystic kidney disease (ADPKD) is the most common genetic kidney disease, accounting for approximately 5% of kidney failure worldwide. While Alport syndrome (AS) is an inherited progressive disease that typically presents with microhematuria in childhood and subsequently progresses to hematuria, proteinuria, and progressive renal impairment. The coexistence of ADPKD and AS is rare. Here, we present a case of a child with ADPKD and AS who presented with bilateral renal cysts and hematuria. Furthermore, we described the clinical and genetic characteristics of the present ca

Autosomal Dominant Polycystic Kidney Disease (ADPKD) and X-Linked Alport Syndrome (AS) are the most common monogenic causes of chronic kidney disease (CKD), each capable of independently leading to end-stage kidney disease (ESKD). However, the concurrent presence of both disorders in a single individual is exceedingly rare and poses significant diagnostic complexity. We report a unique case involving dual diagnosis of ADPKD and AS, attributed to a paternal PKD1 variant and a maternally inherited COL4A5 variant. Initial genetic testing through next-generation sequencing (NGS) identified PKD1 an

Autosomal dominant polycystic kidney disease (ADPKD) is the most common hereditary kidney disorder. It is primarily caused by pathogenic variants in PKD1 and PKD2 genes. The natural history of ADPKD is characterized by progressive enlargement of the total kidney volume (TKV) due to continuous cyst development and expansion, ultimately resulting in a gradual decline of glomerular filtration rate (eGFR). ADPKD is a systemic disease that may involve hepatic cystic disease, intracranial aneurysms, and increased cardiovascular risk. The Mayo Imaging Classification, based on height-adjusted TKV and

Genetic kidney diseases are diverse. Autosomal dominant polycystic kidney disease and the Alport spectrum, together with many rarer genetic forms, cause chronic kidney disease (CKD) to a significant extent. Genetic diagnostics are increasingly being used, but genetically confirmed diagnoses are still often overlooked - even though a confirmed diagnosis often has a decisive influence on disease management.

Hereditary tubulointerstitial kidney diseases typically manifest as slowly progressive chronic kidney disease. Rapidly progressive kidney failure with non-cystic nephromegaly is an exceptionally rare presentation posing significant diagnostic challenges. We describe a 64-year-old man presenting with rapidly progressive kidney failure and bilateral non-cystic kidney enlargement. Initial imaging, including computed tomography (CT) and magnetic resonance imaging (MRI), revealed no cysts. Kidney biopsy showed tubular ectasia and basement membrane abnormalities consistent with tubulointerstitial di

Autosomal dominant polycystic kidney disease is a systemic, hereditary disorder requiring life-long, multidisciplinary management. Approximately half of affected individuals progress to kidney failure by age 60 years. Recent advances in genetics, imaging, and disease pathophysiology have enabled earlier diagnosis and improved risk stratification and led to the development of disease-modifying therapies (such as tolvaptan) for high-risk patients. Despite the availability of comprehensive clinical practice guidelines, real-world implementation is hindered by disease heterogeneity, psychosocial i

Autosomal dominant tubulointerstitial kidney disease (ADTKD) is a genetically heterogeneous disorder characterized by progressive chronic kidney disease, primarily affecting the tubules and interstitium. It results from pathogenic variants in several known genes, including UMOD, mucin-1 (MUC1), REN, APOA4, and others. Despite its clinical significance, ADTKD is often under-recognized due to nonspecific histopathological and clinical features and limited referral for genetic testing, which remains the gold standard for diagnosis. In addition, due to technical difficulties, genetic testing for A

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is a leading hereditary cause of chronic kidney disease (CKD) and end-stage kidney disease (ESKD). While several clinical and genetic predictors of disease progression have been identified, the significance of urinary metabolic profile in ADPKD remains insufficiently explored. This study aimed to evaluate the association between 24-hour urinary parameters, renal function, and clinically relevant comorbidities in patients with ADPKD, and to explore whether these urinary alterations are specific to ADPKD or associated with declinin

Compared with cancers of other organs, precursor lesions of renal cell neoplasms are rarely discussed. However, there are specific scenarios where cysts or microscopic solid lesions are thought to precede tumor formation. In 2024, the International Society of Urological Pathology (ISUP) held a consensus meeting on precursor lesions of urologic neoplasms in Florence, Italy. This report details the findings of Working Group 3-Precursor Lesions of the Kidney. Papillary adenoma is likely the best-established precursor lesion in the kidney, thought to be an incipient form of papillary renal cell ca

Autosomal dominant polycystic kidney disease (ADPKD) is a systemic ciliary disorder, yet the impact on bone health remains insufficiently defined. Given the expression of polycystins in osteocytes, osteoclasts and extracellular matrix, a disease-specific bone phenotype has been hypothesized. This study aimed to comprehensively characterize the bone phenotype in ADPKD across stages of chronic kidney disease (CKD) and Mayo Imaging Classification (MIC). In this cross-sectional study, 81 patients with ADPKD, 15 with CKD of other etiologies, and 21 healthy individuals were included. Bone phenotype

Preimplantation genetic diagnosis for monogenic disorders makes it possible to identify and select embryos free of specific genetic mutations prior to uterine transfer, using embryos produced via medically assisted reproductive technologies. Since 2015, the Italian legal system has allowed parents who are carriers of, or affected by, monogenic hereditary conditions, including autosomal dominant polycystic kidney disease (ADPKD), to access preimplantation genetic diagnosis. However, the experience remains limited, and its application in particular to a late onset disease like ADPKD raises clini

BACKGROUND: Polycystic kidney disease (PKD) is a major hereditary disorder characterized by the formation of cysts in the kidneys of mammals, including humans and cats. It commonly leads to renal failure and is frequently associated with variants in the PKD1 gene. In cats, a specific PKD1 variant (chr E3:g.42858112 C > A) is a primary target for direct-to-consumer (DTC) genetic testing aimed at identifying PKD risk. However, its broad impact on the genetic structure of cat populations remains unclear. In this study, we investigated temporal and breed-specific dynamics of feline PKD and the PKD
