INTRODUCTION: Polycystic kidney disease (PKD) is a genetic disorder characterized by renal enlargement due to cyst formation. Besides cystic complications, patients with autosomal dominant PKD (ADPKD) can develop vascular abnormalities. We investigated the impact of ADPKD on aortic morphometry and function in female and male PKD rats.
METHODS: Thoracic aortic rings were obtained from six-month-old female (n = 10; 307 ± 4 g) and male (n = 10; 470 ± 24 g) PKD/Mhm (Cy/+) rats, as well as age-matched control female (n = 9; 312 ± 6 g) and control male (+/+) (n = 13; 460 ± 14 g) rats. Vascular func

UNLABELLED: Pediatric advanced chronic kidney disease (CKD) profoundly affects not only the patient but the entire family system. This study explores how affected families-including patients, siblings, and primary caregivers-perceive and experience advance CKD as a chronic, life-limiting illness with life-prolonging but non-curative treatment options. A qualitative study was conducted involving seven families recruited from three pediatric nephrology centers. Semi-structured interviews were conducted with patients with CKD ≥ 4, at least one sibling and one parent per family. Both, patients and

Autosomal dominant polycystic kidney disease (ADPKD) affects approximately 12.5 million individuals globally and is one of the most common causes of end-stage renal disease. It is typically associated with a gradual increase in the volume of numerous cysts in both kidneys. Recent studies have highlighted the critical role of signal transducer and activator of transcription 3 (STAT3) in ADPKD pathogenesis, as it is highly expressed and persistently activated in ADPKD kidneys. Through screening of our in-house compound library, we identified compound WR-S-647 (4e) as a potent and specific inhibi

The mechanisms contributing to progressive kidney damage in autosomal dominant polycystic kidney disease (ADPKD) remain unclear. Renal microvascular (MV) rarefaction plays an important role in kidney disease, but its natural history, underlying mechanisms, and contributions to renal disease progression in ADPKD remain unknown. We hypothesized that intrarenal MV rarefaction is present early on and is preceded by vascular transcriptional and metabolic changes. Pkd1RC/RC and wild type (WT) mice (n = 16 each) were studied at 1, 6, and 12 mo. Total kidney volume (TKV) was measured in vivo (MRI), wh

PURPOSE: Autosomal dominant polycystic kidney disease (ADPKD) affects 1:1000, causing 5% to 10% of kidney failure. The primary disease gene, PKD1, has 6 pseudogenes with 97% to 99% homology, a >12-kb transcript, high GC content, and polypyrimidine tracts. Although long-range polymerase chain reaction with Sanger sequencing has been the "gold-standard," next-generation sequencing (NGS) is increasingly used.
METHODS: We performed exome sequencing (ES) on 203 ADPKD patients in 171 families from the Consortium for Radiologic Imaging Studies of Polycystic Kidney Disease cohort, with prior "gold-st

KEY POINTS: We developed assays to measure genetic variant effects on polycystin-1, the protein mutated in most autosomal dominant polycystic kidney disease. All tested pathogenic variants disrupted either polycystin-1 ciliary trafficking or channel function. Trafficking and channel function of some pathogenic variants was restored by low temperature culture to promote polycystin folding.
BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is the leading monogenic cause of kidney failure and affects millions of people worldwide. Despite the prevalence of ADPKD, limited mechanisti

Autosomal dominant polycystic kidney disease (ADPKD), caused by mutations in PKD1 or PKD2, is characterized by progressive and exponential enlargement of renal and hepatic cysts. However, the epithelial dynamics that generate this growth pattern remain incompletely understood. Using Brainbow/Confetti multicolor clonal lineage tracing in developmental and adult-onset ADPKD mouse models, we show that polycystin-deficient epithelial cells initiate clonal expansion at early stages of tubule dilation and continue to expand throughout cyst progression. Concurrently, cyst-lining cells undergo a progr

Autosomal-dominant polycystic kidney disease (ADPKD) is primarily of adult-onset and caused by pathogenic variants in PKD1 or PKD2. Yet, disease expression is highly variable and includes very early-onset PKD presentations in utero or infancy. In animal models, the RNA-binding molecule Bicc1 has been shown to play a crucial role in the pathogenesis of PKD. To study the interaction between BICC1, PKD1, and PKD2, we combined biochemical approaches, knockout studies in mice and Xenopus, genetic engineered human kidney cells carrying BICC1 variants, as well as genetic studies in a large ADPKD coho

KEY POINTS: The causal relationship between serum osmolality and kidney outcome remained unclear in patients with autosomal dominant polycystic kidney disease. Serum osmolality may be a possible surrogate marker for the clinical monitoring of autosomal dominant polycystic kidney disease patients, especially when access to copeptin level is limited. High serum osmolality conveys possible detrimental effect on the kidney outcomes.
BACKGROUND: Current treatment of autosomal dominant polycystic kidney disease (ADPKD) is mainly focused on inhibiting cystogenesis through arginine vasopressin suppre

Machine learning (ML) is a valuable tool in healthcare, enabling the prediction of disease progression through data-driven regression and nonlinear modeling. Unlike traditional statistical methods, ML can identify complex interactions among explanatory variables. Autosomal dominant polycystic kidney disease (ADPKD) is a common cause of chronic kidney disease (CKD), often progressing to end-stage renal failure. Accurately predicting CKD progression in ADPKD patients is essential for personalized treatment strategies. This study analyzed data from 2,737 patients with ADPKD enrolled in the Japane

ATP-citrate lyase (ACLY) is a crucial cytosolic and nuclear enzyme that catalyzes the synthesis of acetyl-CoA from citrate, serving as a central metabolic node linking carbohydrate metabolism to de novo lipogenesis and histone acetylation. Accumulating evidence positions ACLY not merely as a metabolic enzyme but as a key pathogenic driver in various kidney diseases. This review systematically examines the multifaceted roles of ACLY in renal physiology and pathology. During development, ACLY is essential for nephron progenitor cell maintenance and nephrogenesis. In diabetic kidney disease, hype

Obturator hernia, a rare condition that accounts for only 0.07-1% of all hernias, typically occurs in older adult, thin, multiparous women owing to weakening of the obturator membrane and increased intra-abdominal pressure. Conditions such as chronic constipation, chronic obstructive pulmonary disease, ascites, peritoneal dialysis (PD), and autosomal dominant polycystic kidney disease (ADPKD) are considered risk factors for an obturator hernia. Diagnosis is often delayed because of nonspecific symptoms, and computed tomography imaging is essential for early detection. We report the case of a 6

This study presents a novel endoscopic laser internal drainage technique for pediatric endophytic simple renal cysts. Tailored to anatomy using flexible ureteroscopy or needle-perc approaches, it creates a cyst-collecting system fistula via holmium laser for durable decompression. With a high success rate and minimal invasiveness, it offers a safe and effective alternative to traditional methods for problematic pediatric cysts.

Autosomal dominant polycystic kidney disease (ADPKD) is a common monogenic genetic disease characterized by bilateral renal cyst formation and enlargement. The purpose of this study was to explore the therapeutic effects of emodin, which is a key active constituent of traditional Chinese herbal medicine Rhubarb, on ADPKD and its underlying mechanisms. Herein, we found emodin significantly inhibited renal cyst progression in ADPKD. RNA-sequencing analysis and network pharmacological analysis indicated that emodin may exert its therapeutic effects by modulating cell proliferation and oxidative s

KEY POINTS: Long-term exposure to fine particulate matter was associated with faster decline in kidney function among patients with polycystic kidney disease. Associations were stronger in younger patients and those with preserved kidney function at baseline. The association was weaker among patients prescribed tolvaptan.
BACKGROUND: Polycystic kidney disease (PKD) is the fourth leading cause of ESKD. Although the condition is primarily genetic, environmental exposures such as air pollution may influence disease progression.
METHODS: We investigated whether long-term exposure to fine particu

We enrolled 553 patients with autosomal dominant polycystic kidney disease (ADPKD) to investigate the association between hemoglobin (Hb) levels and kidney outcomes. Renal outcome was defined as a 50% reduction in estimated glomerular filtration rate or the initiation of renal replacement therapy. Survival analyses and logistic regression were used to generate time-series McFadden’s pseudo-R-squared (pseudo-R²) values as a goodness-of-fit index to compare the relative contribution of Hb cut-offs over follow-up time, and the mean pseudo-R² values from the first to the 17th year (1–17Y mean) wer

Cystic lesions of the kidney represent a heterogeneous group of conditions ranging from non-neoplastic processes to benign and malignant neoplasms. With the widespread use of cross-sectional imaging, these lesions are detected with increasing frequency, often incidentally during radiologic evaluation. The Bosniak classification system remains the primary radiologic framework for estimating malignancy risk and guiding management decisions for cystic renal masses. However, imaging findings alone are frequently insufficient for definitive diagnosis, and accurate classification often requires inte
