Metabolic and mitochondrial alterations are central in the pathogenesis of autosomal dominant polycystic kidney disease (ADPKD) since therapies targeting these alterations slow kidney disease progression in orthologous animal models. To investigate metabolic and mitochondrial defects in an animal model orthologous to ADPKD, we used male mice homozygous for a point variant in the GPS cleavage site of the polycystin-1 (Pkd1V/V) and evaluated oxygen consumption, Ca2+ uptake, and redox state in the mitochondrial fraction of Pkd1V/V kidneys and wild-type controls (WT). Our findings revealed mitocho

BACKGROUND: Proteinuria and hypertension are commonly observed in children with autosomal dominant polycystic kidney disease (ADPKD), but reliable non-invasive biochemical markers for early disease activity or progression in pediatric ADPKD are lacking. Allantoin and the allantoin/uric acid ratio are recognized oxidative stress biomarkers in chronic kidney disease and may hold potential for early disease monitoring in ADPKD.
METHODS: In this observational study, 34 patients with genotyped PKD1-ADPKD under 18 years with preserved kidney function were compared to 31 healthy controls. Urinary al

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD) is a genetic disorder characterized by progressive cyst development and renal dysfunction. While thyroid hormones (THs) are known to regulate key pathways in various kidney diseases, their role in ADPKD pathobiology and therapeutic potential remains unexplored. Here, we aimed to elucidate the role of THs in ADPKD and evaluate whether their pharmacological modulation could serve as a therapeutic strategy.
METHODS: Patient-derived renal epithelial cells were used to engineer 3D polycystic tubules and to test the anti-cystogenic ef

Autosomal dominant polycystic kidney disease (ADPKD) is the most common inherited kidney disease and is characterized by progressive kidney enlargement and cyst growth leading to progressive loss of kidney function. The two main genes implicated are PKD1 and PKD2; however, pathogenic variants in other less frequent genes can also cause the disease. Diagnosis may be established in various clinical settings, including family screening, the investigation of arterial hypertension, urological events such as hematuria or flank pain, or incidental imaging findings. Diagnostic confirmation relies on t

INTRODUCTION: Multicystic dysplastic kidney (MCDK) is a congenital condition where noncommunicating cysts replace normal renal parenchyma, affecting about 1 in 4,300 live births. Unilateral MCDK is often asymptomatic, it may cause flank pain, hematuria, or hypertension. CASE DESCRIPTION: A 40-year-old male with history of hypertension since age 20 was referred to our clinic for evaluation of cystic kidney. He had no family history of kidney disease. His blood pressure was 145/85 mmHg. Serum creatinine was 0.8 mg/dL. Urinalysis did not show hematuria or proteinuria. Renal ultrasound showed an e

INTRODUCTION: Somatostatin analogues reduce liver and kidney cyst volume in autosomal dominant polycystic kidney disease (ADPKD), but randomized trials have not demonstrated a consistent benefit on kidney function decline. We tested whether the analogue lanreotide slows glomerular filtration rate (GFR) decline over three years in adults with ADPKD stages 2/3.
METHODS: In this randomized, double-blind, placebo-controlled trial, we enrolled adults with ADPKD and measured GFR (mGFR) 30-89 ml/min/1.73 m2. Patients were randomized to receive monthly injections of lanreotide 120 mg or 0.9% sodium c

Autosomal dominant polycystic kidney disease is a leading cause of inherited kidney failure, primarily due to pathogenic variants in PKD1 (80%) and PKD2 (15%), with rare contributions from genes like GANAB and DNAJB11. This review summarizes the understanding of autosomal dominant polycystic kidney disease's genetic and phenotypic diversity, emphasizing genotype-phenotype correlations that influence disease severity and progression. Truncating PKD1 variants are linked to earlier kidney failure (median onset in the fifth to sixth decade) and a faster estimated glomerular filtration rate decline

BACKGROUND: Monoallelic pathogenic variants in GANAB cause autosomal dominant cystic kidney and liver disease, but quantitative imaging phenotypes remain incompletely defined.
METHODS: We performed a retrospective study of 16 individuals with GANAB variants and available abdominal imaging. Deep learning-based cyst segmentation quantified kidney and liver volumes and cyst metrics, including height-adjusted total kidney volume (htTKV), height-adjusted total liver volume, total cyst number (TCN), and height-adjusted total cyst volume.
RESULTS: Hepatic involvement was common, with polycystic liv

SGLT2 inhibitors have demonstrated robust cardiorenal benefits across diverse chronic kidney disease populations but remain underutilized in autosomal dominant polycystic kidney disease (ADPKD) due to lack of disease-specific data and theoretical safety concerns. We report a 58-year-old man with genetically confirmed PKD2-associated ADPKD who developed progressive proteinuria (0.8 g/24 h increasing to 2.0 g/24 h over three years) despite optimal ACE inhibition. Clinical findings suggested concomitant IgA nephropathy, but biopsy was contraindicated. Empagliflozin 10 mg/day was initiated off-lab

Autosomal dominant polycystic kidney disease (ADPKD) remains therapeutically anchored to vasopressin V2-receptor antagonism, yet progression heterogeneity and persistent unmet need increasingly suggest residual disease biology beyond cAMP-centered control. Converging experimental and observational human phenotype data suggest that metabolic reprogramming, mitochondrial dysfunction, impaired fatty-acid oxidation, obesity, and visceral adiposity may modify cyst growth, kidney-volume expansion, eGFR decline, or treatment-response heterogeneity, although causal and therapeutic evidence remains inc

BACKGROUND: Pelviureteric junction obstruction (PUJO) is the most common cause of pediatric hydronephrosis; however, distinguishing obstructive and non-obstructive dilatation remains challenging. Existing diagnostic modalities are limited by the lack of validation against objective measures of renal injury. Preoperative imaging is critical for guiding surgical decision-making; however, its correlation with histopathologic renal damage has not been fully elucidated.
OBJECTIVES: To evaluate the association between preoperative sonographic and nuclear scintigraphy parameters and intraoperative r

BACKGROUND: Autosomal recessive polycystic kidney disease (ARPKD) is a rare, inherited disorder primarily affecting the kidneys and liver. Disease-causing variants in PKHD1 lead to a disruption of the encoded protein fibrocystin/polyductin. This study aims to identify disease causing variants in PKHD1 in families from the Dhofar region of Oman.
METHODS: We conducted a case series of six families with antenatal diagnoses of ARPKD and postnatal deaths. Genetic testing was performed on neonates using Sanger sequencing and next-generation sequencing (NGS) to detect variants in PKHD1. In silico an

Autosomal recessive polycystic kidney disease (ARPKD) is a severe inherited disorder caused primarily by mutations in PKHD1 and in a minority of cases, CYS1. These genes encode fibrocystin and cystin, respectively. ARPKD typically manifests in infancy with enlarged kidneys, progressive cyst formation, and an estimated peri-natal high mortality rate of 20%. Given the lack of efficient therapies and the genetic complexity of many rare diseases such as ARPKD, strategies that restore functional proteins defective in the disease may offer a disease-modifying approach. Glomerulocystic renal disease has numerous etiologies, including HNF1B mutations. In addition to cysts, morphologic renal findings in a setting of HNF1B mutations include cystic renal dysplasia, solitary functioning kidney, horseshoe kidney, and oligomeganephronia. Embryonal hyperplasia resembling nephrogenic rests has been reported in rare cases of glomerulocystic disease, but none have been genetically characterized. We report a case of bilateral glomerulocystic kidney disease (GCKD) showing extensive embryonal hyperplasia in a setting of germline HNF1B mutation with progressive renal failur

INTRODUCTION: Autosomal recessive polycystic kidney disease (ARPKD) is caused primarily by pathogenic variants in PKHD1, encoding fibrocystin/polyductin. In Cys1cpk/cpk (cpk) mice, the kidney and liver lesions closely phenocopy ARPKD. Cys1 encodes cystin, a myristoylated protein that traffics to the primary cilium and nucleus. We recently reported the first patient with ARPKD due to a homozygous CYS1 splicing variant.
METHODS: To study this, we utilized high-resolution ultrasound, array Comparative Genomic Hybridization and exon sequencing to identify an ARPKD-like kidney lesion and CYS1 defe

Autosomal recessive polycystic kidney disease (ARPKD) leads to severe renal cysts and progressive kidney dysfunction, with no approved treatments. The absence of such cystic phenotypes in Pkhd1-/- mice underscores the need for novel models that better recapitulate the human disease. We developed kidney organoid-on-chip models that mimic patients' distal-nephron cysts, identifying RAC1/c-FOS as potential therapeutic targets. However, critical questions remain regarding RAC1 activation during cyst formation, cyst origins, and underlying molecular mechanisms. Using a multifaceted approach, organo

Background/Objectives: Autosomal recessive polycystic kidney disease (ARPKD) is a severe ciliopathy caused by biallelic pathogenic variants in PKHD1, characterized by variable renal and hepatobiliary involvement. The widespread use of next-generation sequencing (NGS) has revealed a large number of rare PKHD1 variants, creating major challenges in distinguishing molecularly confirmed ARPKD from a broader spectrum of PKHD1-associated disease. Methods: We performed an integrated clinical and molecular analysis of 68 individuals referred for suspected ARPKD. Using phase-aware and family-informed A

Cystic kidney diseases are an important cause of pediatric morbidity and mortality. Most genetic cystic kidney diseases are "ciliopathies", a group of disorders that share a common feature of abnormal structure or function of the primary cilia. Ciliopathies represent a broad range of clinically and genetically heterogenous disorders, which affect multiple organ systems. Autosomal dominant polycystic kidney disease (ADPKD) and autosomal recessive polycystic kidney disease (ARPKD) are the 2 most common etiologies, affecting 1:1000-2500 and 1:20,000-40,000 individuals, respectively. Although hist

A 75-year-old female patient with autosomal-dominant polycystic kidney disease and a history of kidney transplantation presented to the emergency department with dysuria, chills and abdominal pain. A detailed review of her medical history revealed that her acute pyelonephritis was masking a chronic abdominal pain issue accompanied by significant bilirubinemia. MRCP showed a complete cystic transformation of the left liver lobe and liver fibrosis. Consequently, the diagnosis of Caroli syndrome, a rare cystic liver disease with fibrosis, was established despite her advanced age.

Nephronophthisis (NPH), a rare but serious ciliopathy, is the predominant genetic cause of end-stage kidney disease (ESKD) in children and adolescents. Renal tubular dilatation and cyst formation are key pathological features of NPH. The mechanisms underlying renal cystogenesis remain poorly understood. This study employed sex-balanced Nphp1 deletion gene knockout (Nphp1KO) mice at 8, 12, and 24 weeks of age to investigate the spatiotemporal dynamics of renal tubules and cyst development, aiming to identify the origins of renal cysts. Quantitative evaluation of cyst progression revealed a sign
