Comprehensive Imaging Review of the Ureter.

The ureter is an important conduit between the kidney and the bladder and is frequently imaged to rule out cancer in the upper genitourinary tract. However, while malignancy can occur, a spectrum of other pathologic ureteral findings can exist, and these can be divided into congenital, intrinsic, and extrinsic abnormalities that range from postoperative and neoplastic conditions to inflammatory disease. This article provides a comprehensive overview of the ureter, including the ureteral anatomy, ureteral imaging strategies, and various ureteral conditions and their associated imaging features.

Case Report: Giant congenital hydronephrosis presenting as massive retroperitoneal cystic lesion with literature review.

Giant retroperitoneal cystic lesions encompass multiple differential diagnoses including renal cysts, lymphangiomas and congenital hydronephrosis. Congenital giant hydronephrosis is extremely rare in adult patients, with limited clinical data to guide preoperative differentiation and surgical management. We report a unique case of a 60-year-old male with a 10-year history of progressive giant left retroperitoneal cystic mass, accompanied by left abdominal distending pain. Preoperative contrast-enhanced ultrasound and CT demonstrated severe left hydronephrosis with markedly atrophic renal corte

Robot-assisted buccal graft uretero-pyeloplasty for pediatric recurrent UPJO: a multi-center case-series.

Redo pyeloplasty is technically more complex due to more significant scar tissue and risk for poor ureteral blood supply affecting outcomes. Use of buccal graft uretero-pyeloplasty was originally reported in the pediatric population back in 2017 as an alternative for complex reoperations. We hypothesize that in highly selected group of pediatric patients undergoing redo pyeloplasty, a buccal graft uretero-pyeloplasty would demonstrate efficacy. A total of 6 institutions identified patients undergoing a robot-assisted buccal uretero-pyeloplasty from 2014 to 2025. Demographic, pre-operative, int

[SINGLE-PORT LAPAROSCOPIC NEPHRECTOMY FOR CONGENITAL HYDRONEPHROSIS WITH A DISC KIDNEY AFTER THE EPISODE OF TRAUMATIC URINE LEAKAGE : A CASE REPORT].

A 7-year-old boy who had been under observation after being diagnosed with a disc kidney, right hydronephrosis and poor right renal function was urgently transported to our hospital for treatment of a right perirenal urinary leakage caused by abdominal trauma. We performed interventional radiology-computed tomography (IVR-CT) before nephrectomy of the nonfunctioning right hydronephrotic kidney. Preoperative IVR-CT revealed five and four arterial branches supplying the right and left kidney, respectively, derived from the abdominal aorta, bilateral common iliac arteries, and the sacral artery.

A clinical nomogram for predicting postoperative renal function improvement in children with UPJO.

BACKGROUND: Ureteropelvic junction obstruction (UPJO) is an important cause of congenital hydronephrosis and may result in impaired differential renal function (DRF) if not addressed in a timely fashion. Although pyeloplasty is generally effective, postoperative renal functional improvement varies among patients. Validated tools to estimate the probability of renal function improvement after surgery remain limited. Therefore, this study aimed to develop and externally validate a nomogram to predict postoperative renal function improvement in children with UPJO undergoing pyeloplasty. METHODS:

Association between grade of congenital hydronephrosis and time to natural resolution.

BACKGROUND: Standard of care for pregnancy includes a second trimester ultrasound which may lead to incidental findings of fetal uropathies. The most common fetal uropathy is congenital hydronephrosis (CH). There is limited information regarding the natural history of CH. The objective of this study was to describe factors associated with time to natural resolution of CH. METHODS: This is a retrospective cohort study of infants with CH using data from a single hospital's electronic medical record. Between January 2017 and December 2022, infants with CH were identified using ICD-10 codes and w

Differentiating Society for Fetal Urology Grades 2 and 3 Hydronephrosis: Diagnostic Accuracy of Resident Physicians.

BACKGROUND: The Society for Fetal Urology (SFU) grading system is widely used to assess the severity of congenital hydronephrosis (CHN). Distinguishing between Grades 2 and 3 is clinically critical yet diagnostically challenging because of vague definitions. Grades ≤ 2 are usually observed, and Grades ≥ 3 require closer follow-up; therefore, the 2-3 boundary represents a critical threshold. However, the diagnostic accuracy of inexperienced physicians remains unassessed. METHODS: Six kidneys (three per grade) with consensus diagnoses by four pediatric nephrology experts were selected as repres

Comparison of nuclear renographic renal cortical transit time with diuretic drainage time parameters in congenital hydronephrosis.

INTRODUCTION: Ureteropelvic junction obstruction (UPJO) is a frequent cause of congenital hydronephrosis (CHN). There is no gold standard diagnostic test for UPJO. Serial diuretic renogram is often used as a surrogate for diagnosing UPJO. Cortical transit time (CTT) is a renogram parameter reported helpful for management. Our primary objective was to study the association of CTT with other diuretic renogram parameters, and secondarily with clinical management. METHODS: We retrospectively reviewed 295 charts for CHN and pyeloplasty. We included 64 consecutive pyeloplasties (treatment group), a

The role of voiding cystourethrogram in the assessment of bilateral antenatally-detected isolated hydronephrosis.

INTRODUCTION: Isolated hydronephrosis (IH) is considered the most common cause of congenital hydronephrosis. Although voiding cystourethrogram (VCUG) is one of the main diagnostic tools in the workup of IH, there is debate about performing it routinely for all cases. In this study, we evaluated the role of VCUG in assessing bilateral IH in terms of its utility and potential complications. Moreover, we aimed to evaluate the outcome of the management of bilateral HN. PATIENTS AND METHODS: A retrospective review of patients' charts who presented with IH with non-visualized retrovesical ureters f

Ureteric Injury From an Indwelling Catheter in the Context of an Undiagnosed Duplex Ureter in a Labouring Woman: A Case Report.

Indwelling urinary catheter (IDC) insertion is routine in obstetric practice and is considered low risk. Inadvertent ureteric catheterisation is rare and has been reported only in a few patients with complex backgrounds, mainly neurogenic bladder. This report describes a case of iatrogenic ureteric injury secondary to IDC insertion in an otherwise well woman, who was later found to have an undiagnosed duplex renal collecting system. Interestingly, an antenatal ultrasound in this case detected a fetal duplex renal collecting system. The case describes a woman in her thirties in spontaneous labo

Hepatocyte growth factor blocks epithelial-mesenchymal transition and fibrosis of ureteral epithelial cells in UPJO.

BACKGROUND: Ureteropelvic junction obstruction (UPJO) is a leading cause of congenital hydronephrosis. Its etiology is associated with dysregulated extracellular matrix (ECM) remodeling and epithelial-mesenchymal transition (EMT). While Hepatocyte Growth Factor (HGF) is a known antagonist of fibrosis in other organs, its role in human UPJO pathogenesis remains unexplored. METHODS: Stenotic ureteropelvic junction (UPJ) tissues from pediatric UPJO patients (n = 40) and normal ureteral tissues from duplex kidney controls (Ctrl, n = 10) were obtained in our hospital. Analyses included demographic

A Case of Hydronephrosis Associated With Horseshoe Kidney Successfully Treated by Retroperitoneoscopic Isthmusectomy Without Pyeloplasty.

UNLABELLED: Pyeloplasty is generally considered the standard treatment for congenital hydronephrosis associated with horseshoe kidney. We report a case successfully managed mainly by retroperitoneoscopic isthmusectomy without pyeloplasty. CASE PRESENTATION: A 23-year-old man presented with left flank-to-back pain. CT revealed horseshoe kidney and left hydronephrosis. The left ureteropelvic junction was considered compressed by renal malrotation, causing obstruction. Retroperitoneoscopic isthmusectomy was performed using an articulating endoscopic linear stapler, resulting in hydronephrosis im

Genetics of CAKUT.

Congenital anomalies of the kidney and urinary tract (CAKUT) represent a heterogeneous group of developmental disorders and are the leading cause of pediatric chronic kidney disease worldwide. The phenotypic spectrum is broad, encompassing kidney agenesis, hypodysplasia, multicystic dysplastic kidneys, vesicoureteral reflux, obstructive uropathies, and other malformations affecting the kidneys, ureters, and urethra. Advances in genetics have begun to unravel the molecular pathways underlying these diverse phenotypes, yet the complexity of CAKUT reflects contributions from both monogenic varian

Furosemide Administration Enhances Diagnostic Confidence in the Evaluation of Ureteral Ectopia in Dogs Using Ultrasound.

Ultrasonography is frequently utilized in patients with urinary incontinence to screen for structural abnormalities such as ectopic ureters (EUs). Furosemide can be administered during an ultrasound for a better assessment of the ureters and their insertions. Furosemide decreases urine specific gravity (USG), and differences in specific gravity allow a better visibility of ureteral jets on ultrasound; however, it is unknown whether furosemide has utility in patients with hyposthenuria or isosthenuria. The aims of this study were to determine if furosemide administration improves the diagnosis

Mid-ureteric injury resulting from malposition of an indwelling urethral catheter.

We describe a case of a gentleman in his late 70s with left ureteric injury secondary to insertion of a urethral catheter into the left ureter. The resulting left ureteric injury required emergency percutaneous nephrostomy (PCN) insertion. This patient presented acutely to the urology service with haematuria and presumed urinary tract infection owing to a pre-existing bladder cancer diagnosis. A urethral catheter was inserted on admission for monitoring urine output and when his urethral catheter was noted to be blocked with clot overnight it was changed to a larger catheter. An 18Ch catheter wa

Fetal Hydronephrosis: A Narrative Review of Diagnostic Pathways, Prognostic Markers, and Management.

Fetal hydronephrosis is the most frequently detected prenatal urinary tract abnormality and encompasses a broad clinical spectrum, ranging from transient physiological dilation to clinically significant obstructive uropathy with potential long-term renal consequences. The broader term urinary tract dilation integrates renal pelvic and calyceal dilation with abnormalities of the renal parenchyma, ureters, bladder, and amniotic fluid volume, thereby supporting more comprehensive risk stratification. This narrative review synthesizes evidence on fetal urinary tract development, prenatal diagnosis

Complete ureteral duplication with occult calculi: A case report.

Duplication of the kidney and ureter is a relatively rare congenital malformation compared with other systemic malformations. It is even rarer for the duplicate to have stones inside; therefore, the present study reports the case of a patient with complete repetitive ureteral malformations combined with occult stones who showed marked improvement in symptoms and good recovery after surgical treatment. The patient received a timely diagnosis and achieved a good treatment outcome. The patient was satisfied with the treatment and had a good follow-up. Early and accurate diagnosis and timely treat

KidMesh: Computational Mesh Reconstruction for Pediatric Congenital Hydronephrosis Using Deep Neural Networks.

Pediatric congenital hydronephrosis (CH) is a common urinary tract disorder, primarily caused by obstruction at the renal pelvis-ureter junction. Magnetic resonance urography (MRU) can visualize hydronephrosis, including renal pelvis and calyces, by utilizing the natural contrast provided by water. Existing voxel-based segmentation approaches can extract CH regions from MRU, facilitating disease diagnosis and prognosis. However, these segmentation methods predominantly focus on morphological features, such as size, shape, and structure. To enable functional assessments, such as urodynamic simu

TGF-β1 induces epithelial-mesenchymal transition and fibrosis of ureteral epithelial cells via targeting RACK1 in ureteropelvic junction obstruction.

BACKGROUND: Ureteropelvic junction obstruction (UPJO) is a frequent cause of congenital hydronephrosis. It involves both structural and functional changes at the ureteropelvic junction (UPJ), where the ureter connects with the renal pelvis. The mechanisms behind tissue remodeling and fibrosis in this condition are not fully understood. This study explores the role of epithelial-mesenchymal transition (EMT) and examines how receptor for activated C kinase 1 (RACK1) may regulate this process in UPJO. METHODS: Stenotic UPJ tissues were collected from patients with congenital UPJO and analyzed fo
