[Ureteropelvic stenosis, agenesia of the contralateral kidney and sinistral inferior vena cava].

INTRODUCTION: Ureteropelvic stenosis occurs as a result of intraluminal or extraluminal alterations at the ureteropelvic junction. It is the most common cause of upper urinary tract obstruction. CASE REPORT: We report about a rare case of a man with ureteropelvic junction obstruction and a renal pelvic stone accompanied by other congenital anomalies, including a sinistral inferior vena cava and agenesis of the opposite kidney. Multiple vessels were present in front of the renal pelvis, while the renal vein itself was situated dorsal to the left renal pelvis, resulting in compression and obstr

Congenital unilateral ureteropelvic junction obstruction of the rat: a useful animal model for human ureteropelvic junction obstruction?

OBJECTIVES: To investigate the expression of endothelin-1 (ET-1) and adrenomedullin (ADM) in the renal pelvis, stenotic ureteropelvic junction, and ureter of 20 male Wistar rats with congenital unilateral ureteropelvic junction obstruction; the normal contralateral kidneys served as controls. The molecular pathophysiology of congenital ureteropelvic junction obstruction is still unclear. The implication of altered peptidergic innervation is under discussion. Our study group has recently been able to demonstrate a significant increase in ET-1 and a significant decrease in ADM in prestenotic and

[Appendix-ureteroplasty in a child: report of a case].

The use of vermiform appendix as a tube to replace right ureteral segment has been reported, rarely in child. Herein is reported a case of right ischemic ureteral stenosis following a reimplantation of the ureter for high grade reflux secondary to posterior urethral valves with only one functioning kidney. A long ureteral defect was bridged successfully by appendix interposition and then reimplanted in the bladder at four years of age. The interest of ureteroappendiculoplasty provides temporary solution to repair long ureteral defect, in spite of uncertain future, especially in childhood.

Congenital stenosis of ureteropelvic junction: assessment with multislice CT.

PURPOSE: To assess the feasibility of Multislice spiral CT (MSCT) with volume rendering (VR) and maximum intensity projection (MIP) reconstructions in recognizing ureteropelvic junction obstruction (UPJO). MATERIALS AND METHODS: Twenty-seven patients with UPJO underwent MSCT. To visualise the excretory system, 50 ml of uroangiografic contrast medium was administered by drip infusion via the cubital vein 180 minutes before the CT scan. MSCT was performed after the injection of 120 ml of contrast medium at a flow rate of 3.5 ml/sec. Dual-phase scanning was performed 25s and 55s after contrast m

Robotically assisted laparoscopic dismembered pyeloplasty: a chronic porcine study.

OBJECTIVES: To evaluate the feasibility and efficacy of robotically assisted laparoscopic dismembered pyeloplasty in an animal model. We also investigated whether the use of a device that lacks tactile feedback results in tissue damage and subsequent stenosis or leaking of the anastomosis. METHODS: Robotically assisted dismembered pyeloplasty was performed on 10 farm pigs. After 1 month, all pigs were anesthetized again for intravenous urography and to remove the kidney and ureter on the operated side. Histopathologic examination of all specimens was performed. RESULTS: All animals survived

[A case of unilateral giant hydronephrosis with contralateral dwarf kidney].

A 54-year-old woman was admitted to our hospital for oliguria and left lower abdominal pain. She had renal dysfunction with a serum creatinine of 9.1 mg/dl and blood urea nitrogen of 96.5 mg/dl. Plain computed tomography and magnetic resonance imaging revealed right dwarf kidney and left giant hydronephrosis with extravasation of urine. MR-urography revealed left dilated ureter caused by ureterovesical junction (UVJ) stenosis. Therefore, percutaneous nephrostomy was immediately performed to treat postrenal failure, with resulting collection of approximately 1,650 ml of urine. Subsequently, lef

[Lithiasis in complete ureteral duplication and prostate cancer: combined surgery treatment].

The diagnosis of congenital anomalies of the urogenital tract in the adult is uncommon. Complete ureteral duplication represents a congenital anomaly of the ureters, usually draining a duplex kidney, with a second orifice entering the bladder, urethra or other structures. In the majority of cases, this pathology is completely asymptomatic; sometimes it's diagnosed because of complications such as infections, hydronephrosis, lithiasis, or in the evaluation of the urinary tract. Early diagnosis of localized prostatic adenocarcinoma is increased due to worldwide use of PSA screening and the optim

Surgical management of a ureteral defect with ureterorrhaphy and of ureteritis with ureteroneocystostomy in a foal.

A 2-day-old male foal developed uroperitoneum. Initial exploration of the abdomen via ventral midline celiotomy failed to localize the source of uroperitoneum. Bilateral nephropyelocentesis and antegrade urography were performed, and a defect in the right ureter and stenosis of the left ureter were identified. With the foal in dorsal recumbency in the Trendelenburg position, ureterorrhaphy was performed on the right ureter. After ureterorrhaphy, a ureteral catheter was maintained as a stent. The stenotic left ureter was transected, and ureteroneocystostomy was performed using a drop-in mucosal

Urologic injuries associated with repair of anorectal malformations in male patients.

BACKGROUND/PURPOSE: Serious injuries to the urinary tract may occur during the repair of an anorectal malformation, especially in boys. This review of a large series of patients characterizes factors that may either lead to, or prevent, those injuries. METHODS: A retrospective review of 1,003 boys with anorectal malformations was performed. RESULTS: A total of 129 injuries in 1,003 patients were identified. Five hundred seventy-two of the 1,003 patients (group A) underwent definitive repair at the authors' institution. In this group, there were 19 urologic injuries; 1 bladder perforation, 1

Megaureter.

Obstructive megaureter is shown by ureteral dilatation most frequently due to an obstacle at the level of ureterovesical junction. The obstacle is caused by a stenotic tract and/or the absence of peristalsis. The diagnosis of primary obstructive megaureter is usually prenatal. The pediatric surgeon together with the pediatric radiologist and nephrologist should pose the indication for surgery. The main criteria on which this approach should be indicated are the grade of megaureter and the renal function. Megaureters of low-mid grade should be followed by a "wait and see" approach because they

Ureteral valves.

Ureteral valves together with stenosis and diverticula represent congenital malformations of the ureteral tract between the pyeloureteral junction and the ureterovesical junction. They are rare malformations and even more rarely they cause an obstacle to urine flow. The proximal subjejunal ureteral tract is more frequently involved. The most reliable pathogenetic hypotheses suggest a delay in the canalization of fetal ureter or an ischemic damage. The true ureteral valves should be distinguished from Ostling embryonic folds that can be documented in 5% of newborns and disappear with growth. Tr

Extracorporeal shock wave lithotripsy in ureteral and kidney malformations.

INTRODUCTION: Extracorporeal shock wave lithotripsy (SWL) has long been accepted worldwide in the treatment of kidney stone disease. Upper ureter calculi in ureteral and kidney malformations are rather frequent (10-25%). The aims of this retrospective study were to determine whether malformations might impair fragment expulsion. MATERIAL AND METHODS: From 1986 to 1995, 203 patients with ureteral and kidney malformations were treated and followed up for a minimum of 1 year to a maximum of 9 years. Malformations included: ureteropelvic junction stenosis, horseshoe kidney, renal malrotation, ren

Endoscopic classification and endourologic therapy in proximal incomplete ureteral duplication pathology.

OBJECTIVE: To assess whether patients with proximal incomplete ureteral duplication (PIUD) had functional or anatomical Y junction zone pathology. In such cases, stasis and infection, with or without associated renal scarring, are often found. The results after minimally invasive treatment (retrograde endoincision of ureteral stenosis, localized on one limb) can be evaluated. PATIENTS AND METHODS: In a retrospective analysis of 807 ureteroscopies, we found 29 cases with PIUD pathology dating from July 1996 to March 1999. The investigative protocol involved: intravenous pyelography (IVP) with

Quadricuspid aortic valve, parossistyc supraventricular tachycardia and double right kidney: an uncommon association.

Fourteen years after surgery for replacement of the aortic valve, an interesting case previously unreported was brought to our attention. The female patient came to our OP Dept for a routine follow-up: she had been found at surgery to have a quadricuspid aortic valve. Operation dated October 1985. At a careful appraisal of the world-wide literature, we noticed that such an association of pathologies had never been reported before: quadricuspid aortic valve, paroxystical supraventricular tachycardia and right double kidney with double renal pelvis and double proximal ureter. Other anomalies ass

Congenital giant megaureter: first neonatal case.

The authors report the first neonatal presentation of a congenital giant megaureter (CGM), a very rare unilateral urinary anomaly, which is defined as "a ureter whose lumen is congenitally, focally and segmentally dilated to more than 10 times the normal diameter, in presence of normal bladder volume and function." The definition of CGM is purely descriptive; it can be secondary to several different diseases, and in the series of 21 CGM (12 in duplex system) reported by Chen-Ju Huang, the morphology of the distal ureter varied from a near normal caliber to a ureteral stenosis, a ureterocele or

Pyeloureteral junction stenosis and ureteral valve causing hydronephrosis.

A young female patient underwent ureteral resection and Hynes-Anderson pyeloureteroplasty because of pyeloureteral junction stenosis and a ureteral valve (UV), which caused hydronephrosis. Despite using ultrasonography and intravenous urography the ureteral valve was observed only on exploration. UV should be considered as a possible cause of upper urinary tract obstruction; retrograde urography can be essential.

Congenital bilateral ureteral stenosis and hydronephrosis in a neonatal puppy.

Three days after an uneventful parturition, a Brittany spaniel/beagle puppy (Canis familiaris) was nursing but not gaining weight as rapidly as were its littermates. Although its diet was supplemented, the puppy died 10 days after birth. The renal pelves were enlarged and filled with urine. Both ureters were thin throughout their length, and urine could not be expressed from either kidney into its respective ureter. The bladder contained no urine and was firmly embedded in the umbilicus. Histologically, both kidneys were hydronephrotic and contained hypoplastic collecting tubules. The diameter
