Various techniques of ureteral reimplantation have been described for different indications. After a retrospective study concerning 109 cases and 178 ureteral reimplantations, the authors propose the selective indications for each technique noting the most important results. OBJECTIVES: The possible solutions to a case of ureteral duplication complicated by obstruction of one of the collecting systems are reviewed.
METHODS/RESULTS: We report an uncommon case of ureteropelvic junction obstruction in the lower system of an incomplete duplication of the collecting systems that was resolved by pyeloureterostomy.
CONCLUSIONS: Anastomosis of the lower renal pelvis to the upper ureter may provide a simple solution for the functional recovery of an important renal segment.

We have wanted to appraise the diagnostic reliability of fetal ultrasonography (FU) and its correlation with postnatal ultrasonography (PU). For two years we have studied 20.506 children born in 12 Pediatric Division of Veneto, all provided with FU. In 56 megaureters treated by the folding technique, good results were obtained in 52 (93%). Complications in four ureters included stenosis (2) and vesico-ureteral reflux (2). The width of the megaureters and minor renal calyces, the degree of hydronephrosis, and the serum urea and creatinine levels decreased significantly after the operation. The renal parenchyma thickness and the creatinine clearance level increased significantly.

Secondary, ureteroneocystostomy is the procedure most commonly used for repair of ureteral stenosis or necrosis in renal transplant patient. The clinical characteristics and natural history of 55 cases with antenatally diagnosed fetal uropathy were investigated. The authors report their experience on a case of crossed ectopic kidney without fusion associated with stenosis of the pyeloureteral junction and secondary pyelocaliceal staghorn calculosis. After a brief summary of the various types of kidney ectopia and related principles of operation they describe their case report underlying the rarity of their case and the surgical approach. They consider the anterior transperitoneal approach as the most indicated for this type of malformation as it permits a better surgical view of the kidney and of the vascular structures.

Four patients with severe ureteral strictures recovered adequate renal and ureteral function after application of an ileal patch graft to the area of stenosis. No metabolic abnormalities resulted from use of the ileal patch graft. Candidates for this type of repair have severe ureteral strictures after prior attempts at repair. This work is a report on our preliminary experience with the treatment of iterative stenosis of the pyeloureteral junction (JPU) through a retrograde endoscopic approach. From June, 1991, to June, 1992, we used this method to operate 6 cases of failed surgical pyeloplasty due to abnormalities of the JPU. Anomalies of the vesicoureteric junction are important, particularly obstruction and reflux, as they may predispose to urinary tract infection. Over a 5-year period, 52 babies were referred with dilatation of the urinary tract detected antenatally or/and postnatally by ultrasound. Sixteen had an anomaly of the vesicoureteric junction: 9 had vesicoureteric reflux, 3 had ureteroceles, 1 had urethral stenosis with secondary reflux and 3 had stenosis of the vesicoureteric junction. Transurethral endoureteropyelotomy was performed in a total of 37 patients with primary ureteropelvic junction obstruction or various ureteral strictures.
