The authors describe three patients with episodes of abdominal pain where despite a negative finding in the urine sonography revealed dilatation of the hollow system. Urography proved stenosis of the pyeloureteral junction. The authors discuss the cause of abdominal pain. Consistent with data in the literature, the authors draw attention to pitfalls in the diagnosis of hydronephrosis. They recommend sonographic examination of children with abdominal pain, as a negative urinary finding does not rule out hydronephrosis. Between 1974 and 1990 in 16 children (5 boys and 11 girls) with a duplex kidney and bifid ureter surgery was indicated. The mean age of the 15 already operated patients was 7 years (0.3-12 years). 14 had uretero-ureteric reflux, 1 had a stenosis of the ureteric bifurcation, and 1 had a lower pole pelvi-ureteric junction obstruction. An interpyelic anastomosis was performed in 14 and a ureteroneocystostomy "en bloc" in 1. The follow-up was 3-10 years with a mean of 5 years. We report a case of ureteric valve associated with pyeloureteral obstruction, exhibited as fistula after pyeloplasty. We performed ureteroplasty for megaureter by ureteral plication with ease in two cases with unilateral megaureter due to uretero-vesical stenosis and obtained good results in each case. Because ureteral plication theoretically preserves lower ureteral blood supply and eliminates suture line leakage, this technique is thought to be superior to excisional tapering techniques in ureteroplasty for a megaureter.

We reported two cases of nephrotic syndrome associated with hydronephrosis. A forty four year old male patient who suddenly complained of nephrotic syndrome, showed left hydronephrosis caused by lower ureteral stenosis. A family with autosomal dominant inheritance of sacral agenesis is described. Ten members were affected; four had associated presacral teratomas and anterior sacral meningoceles, giving rise to serious complications in three, including bacterial meningitis, local recurrence of teratoma and perianal sepsis. The authors report a case of Cockett's syndrome discovered by ilio--cava angiography at a patient who was operated on a bilateral retro-iliac ureter which occasioned an important renal insufficiency. 35 children and adolescents were submitted to ureterocalicostomy for PUJ obstruction. The surgical procedure is explained and specially the importance of performing a regular, muco-mucosal anastomosis to avoid its secondary stenosis. 9 patients have an excellent result with absence of clinical symptoms, sterile urinalysis and normal IVP. 20 patients have a good result with absence of clinical symptoms, sterile urinalysis and improvement of the dilatation of the cavities. 2 secondary nephrectomies have had to be performed because of a dysplastic kidney (1 patient) or of a parenchymal infection

Ureteral ectopias, which mostly occur in ureteral duplications, become noticeable in women quite early by characteristic clinical signs (dribble, or urinary incontinence). In males, it is usually only a stenosis-conditioned dilatation of the ureter that results in an uncharacter-istic pattern of complaints. The article presents the clinical pattern of signs and the x-ray findings in a man of 42 year of age, who had not reported any complaints until then, with an excessive dilatation of an extopically discharging single ureter.

This paper is a review of 63 cases of congenital Pyeloureteral Union Syndrome. A total of 47 patients were treated using the Anderson-Hynes reconstructive technique with 3-4 years follow-up. Incidence was analyzed by sex, age and side affected, without a clear predominance of one over the others being shown. Mean age at the time of diagnosis was 39-43 years. Urinary tract anomalies are common. Prenatal diagnosis is important and enables either special obstetric management or termination of pregnancy and probably in the future, intrauterine intervention. Fifteen adult patients diagnosed as having congenital stricture of the ureteropelvic junction were treated by percutaneous endoureteropyelotomy. We evaluated the results and complications of 85 ureteroneocystostomies performed from 1978 to 1987. These varied according to the etiology of the disorder; the success rate was 100% for malformations, 87.5% for gynecologic ureters, and 80% for stenosis of the end of the ureter. Several surgical techniques were employed. The Politano-Leadbetter procedure was the most widely-used (40% of cases), and the Cohen technique achieved the best results (100% success rate). All the failures (7.1% of the cases) were due to stenosis of the reimplanted ureter.

PURPOSE: To critically appraise four common uses of intravenous pyelography in adults.
DATA IDENTIFICATION: We reviewed the literature on the use of intravenous pyelography for four clinical indications.
STUDY SELECTION: We analyzed the literature to determine the proportion of intravenous pyelograms producing clinically useful information (yield) in four clinical situations. The best estimate of its accuracy (sensitivity and specificity) for associated pathologic findings was determined. The implications of using the test on the outcomes of patients and costs were evaluated.

Endourologic treatment of the ureteropelvic junction syndrome was performed 48 times in 45 patients (1 was bilateral, and 2 required repeat treatment) via a percutaneous incision of the stenosis, and placement of a modelling double-J catheter, which was left indwelling for 4 to 6 weeks. Evaluation of the results between 6 months and 3 years revealed a success rate of 81.5% for secondary stenosis and 73.7% for congenital stenosis. If the foregoing technique fails, pyeloplasty can be performed without difficulty.

Between September 1984 and April 1988, 56 children underwent surgery for an obstructive uropathy discovered on antenatal ultrasound examination. These included 31 cases of stenosis of the pyelo-ureteral junction (PUJ), 9 cases of vesicorenal reflux, 6 mega-ureters (MU), 6 ureteroceles, 3 posterior urethral valves and 1 ectopic ureteral orifice. We present our experience with 32 cases of ureterocele in children treated between 1972-1984. Five children with ureterocele in a single system underwent ureterocelectomy and ureteral reimplantation. Ten cases were managed by ureteropyeloanastomosis with ureterocelectomy and reimplantation with good results. Nine heminephrectomies and eight nephrectomies were done. The most common complication was ureterovesical stenosis in a "double barrel" reimplant. We were able to preserve renal parenchyma drained by a ureterocele in 15 cases.

Between 1974 and 1986 96 children underwent the Lich-Grégoir antireflux procedure, in 63 patients on both sides in one session. 17 patients had double ureter. In totally 153 controlled operations we found 4 persistent reflux (2.6%) and 1 stenosis (0.7%). The success ratio is, therefore, 96.7%.
