Despite the wide use of the international classification of megaureters, there is a lack of clear concepts especially for congenital malformations. We performed a follow-up study in 132 children with 171 megaureters operated on during the past 13 years. In 24 additional cases reconstructive surgery was not feasible due to advanced damaging of the parenchyma. In 97% of 171 megaureters, resection of the distal stenosis and ureterocystoneostomy was successful. In 20%, additional ureteric tailoring was necessary. Infundibulopelvic stenosis is a very rare kidney disease. This congenital disorder must be differentiated from multicystic dysplastic kidney, polycystic kidney disease, simple renal cysts and mega-(poly)-calicosis. Associated abnormalities of the ureter are rare. Ureteric obstruction was associated in our two patients, and in one case agenesis of the bladder and a duplicated genital tract were also present.

From 1981 to 1987, the authors observed 69 patients with ureteropelvic junction obstruction diagnosed by antenatal ultrasound, representing a total of 77 pathological renal units. Seventy renal units were treated by ureteropelvic resection, one renal unit returned to normal without treatment. There were 3 primary nephrectomies and 3 secondary nephrectomies for non-improvement after percutaneous nephrostomy. An early antenatal surgical management of ureteropelvic obstruction after antenatal ultrasound diagnosis does not seem to give better results than when diagnosis is made at a later stage. T

Fourteen cases of obstruction after ureteroneocystostomy in children have been cured by surgery between 1972 and 1986; nine patients were referred from outside our hospital. This failure as been observed after reimplantation of simple thin ureter (29 cases) megaureter (7) or duplication (4). We have seen 21 patients with unilateral congenital giant megaureter. Age ranged from 2 months to 8 years. This condition is characterized by giant focal segmental ureteral dilatation producing an elongated and distorted ureter; distally there may be congenital ureteral stenosis or atresia, or a ureter of approximately normal caliber. Bilateral hydroureteronephrosis following ileal conduit urinary diversion is not uncommon. It may be owing to ureteroileal stenosis, stomal stenosis or a poorly compliant ileal conduit. The standard evaluation of stoma size, conduit residual urine and a loopogram often fail to allow determination of the cause of ureteral dilatation. CT findings in three cases of ureteral metastases are reported. Metastases to the ureter is an uncommon cause of the upper urinary tract obstruction. The most common primary sites include breast, stomach and neoplasms from pelvic organs. Radiographic studies revealed an ureteral stenosis in all the presenting cases. A case of carcinoma in situ of the renal pelvis in a 70-year-old female is reported. The patient was admitted with the complaints of macrohematuria and left back pain. Urine cytology, which was carried out three times using urine samples collected directly from the urinary bladder proved to be negative. Results are reported of therapy of 18 neonates in whom obstetrical ultrasound imaging had demonstrated uni- or bilateral pyelo-caliceal dilatation due to stenosis of pyelo-ureteral junction (18 children--2 hydronephroses). Neonatal biologic, ultrasound and urographic examinations confirmed presence and assessed importance of the hydronephrosis. As a function of findings therapy consisted of immediate operation (10 neonates), delayed surgery (5 cases) or surveillance (3 cases). During 1980-1984 the ultrasound examination (US) performed in the third trimester revealed urinary tract dilatation in 18 foetuses. After delivery all the infants were reexamined within the first weeks with US, intravenous pyelography and micturition urethrocystography to confirm the malformation. Pelvic dilatation was seen in 12 foetuses, 5 bilaterally. The dilatation was caused by pelviureteric obstruction in 6 infants, 4 of which were operated on with dismembered pyeloplasty. A case of combined anomalies of the urinary tract, horseshoe kidney, retrocaval ureter and pelviureteric stenosis, is presented. Routine urological examination of a three-year-old girl who complained of recurrent cystitis, showed horseshoe kidney and right hydronephrosis. Suboperative observations of 73 patients have shown that lower polar renal vessels are often associated with other abnormalities (congenital fibrosis and segmentary hypoplasia of the parapelvic portion of the ureter resulting in the development of hydronephrosis, chronic urethritis, atrophy of the muscle layer and secondary stenosis of the ureter). The operation of choice is resection of the pyeloureteral segment and part of the pelvis followed by antevasal anastomosis.

A case of hypertension secondary to unilateral hydronephrosis due to congenital obstruction of ureteropelvic junction in a 6-year-old boy is reported. Preoperatively, the plasma renin activity (PRA) in the peripheral vein and in the affected kidney was elevated. Blood pressure decreased significantly following administration of converting enzyme inhibitor. Referring to a patient operated on the third day of life for pyeloureteral stenosis the 'prune belly' syndrome is described. The syndrome is delineated by urinary tract anomalies, undescended testicles and a prune like abdominal wall. Several other anomalies are commonly seen. In this patient the megaureters, the vesicoureteral reflux and megacystis are considered part of the syndrome. The pyeloureteral stenosis was only seen once before in a succumbed newborn.

The surgical treatment of circumcaval ureter (type I) should eliminate the ureteric obstruction which is of variable causation. At operation, the aetiology of the obstruction can be determined by inspection of the extent of ureteric dilatation, perfusion-pressure study and calibration of the lumen. Primary obstructive megaureter (P.O.M.) is a dilatation, different in size, of upper urinary tract without reflux and secondary to a functional obstructive malformation of terminal segment of ureter. Infection and renal function impairment complicate P.O.M. in a rather high percentage of cases. Based upon the experience with the operative treatment of 178 children with congenital stenoses of ureters and double urinary tracts the authors consider the upper heminephroureterectomy to be the main operation in the stenosis of the terminal part of the extra ureter. In stenosis of the prepelvic part of the ureter many modifications of pyeloureteral anastomoses can be used.
