The ureter is an important conduit between the kidney and the bladder and is frequently imaged to rule out cancer in the upper genitourinary tract. However, while malignancy can occur, a spectrum of other pathologic ureteral findings can exist, and these can be divided into congenital, intrinsic, and extrinsic abnormalities that range from postoperative and neoplastic conditions to inflammatory disease. This article provides a comprehensive overview of the ureter, including the ureteral anatomy, ureteral imaging strategies, and various ureteral conditions and their associated imaging features.

The extrarenal calyx (ERC) is a rare congenital anomaly, associated with various other abnormalities of the urogenital system. We report a unique case of ERC in a solitary functioning kidney with a massively dilated pelvis that developed spontaneous rupture. A dismembered pyeloplasty was performed at the time of rupture. The patient did well post-operatively with a stable creatinine and stable SFU grade 2 hydronephrosis.

Ureteral triplication remains a very rare congenital malformation of the urinary tract with a wide spectrum of presentation. The sporadic nature of this condition and its association with other anomalies makes evidence-based management difficult. We report two cases of triplication in association with the VACTERL syndrome, one developing pelvi-ureteric junction obstruction and the other vesico-ureteric reflux.
