A spectrum of segmental multicystic renal dysplasia.

BACKGROUND: Multicystic renal dysplasia (MCDK) is a common anomaly well described in the literature, but less well described when involving only a portion of a kidney. OBJECTIVE: To present the imaging spectrum, natural history and associated anomalies of six kidneys with segmental MCDK. MATERIALS AND METHODS: Five children with segmental MCDK (one with bilateral segmental MCDK) referred to our hospital between 1989 and 1996 were reviewed. All had at least one ultrasound examination. Four had a voiding cystogram and three had renal scintigraphy. RESULTS: Four children had antenatal diagnosi

[Posterior approach to the bladder for implantation of the 800 AMS artificial sphincter in children and adolescents: techniques and results in eight patients].

Blind dissection of the bladder neck during implantation of an artificial urinary sphincter (AMS 800) may lead to injury of either the bladder, genital tract or even the rectum. Significant bleeding may occur. A posterior approach to the bladder neck allowing visual control of the anatomical structures is described. From November 1995 to February 1998, 8 adolescents (12-19 years old) underwent AMS 800 artificial urinary sphincter implantation for the treatment of severe incontinence associated with neurogen bladder; one patient had had a previous augmentation. Three had a simultaneous ileocyst

[Familial form of ureterocele with double ureter: 2 sisters and their father].

Double ureter is a frequent congenital malformation and the presence of a ureterocele on the ureter draining the superior renal pelvis is a not exceptional association. The abnormality is usually diagnosed in children by intravenous urography or ultrasound. It is extremely rare to find such a lesion in different members of the same family, as only a few cases have been published in the literature. Our case is original in that three members of the family presented the abnormality and it was the father who was affected, rather than the mother, as in the cases reported in the other two publicatio

[Single-system ectopic ureters. A review of 19 cases].

Single system ectopic ureters are usually associated with multiple congenital abnormalities and the corresponding renal units is frequently abnormal. 19 cases of single system ectopic ureters were diagnosed and treated in our hospital during the last 20 years. The patients were less than 11 years old. The abnormality was bilateral in two cases and the 63% of all had associated malformations, being the anorectal abnormalities the most frequent. The incidence of reflux into the ectopic ureter was 57%, and the association to unilateral or contralateral renal agenesis, 26.3%. Thirteen ectopic uret

Complete penoscrotal transposition and associated malformations.

Complete transposition of the penis and scrotum, or prepenile scrotum, is an uncommon congenital malformation. Concomitant genitourinary abnormalities, often life-threatening in nature, are frequently seen, and major malformations involving other organ systems may also be present. We report a newborn in whom complete penoscrotal transposition was accompanied by urethral atresia and radial dysgenesis.

[Bladder augmentation in reconstruction of the urinary tract (1985-1997)].

Bladder augmentation with intestinal or urinary segments has virtually replaced other treatments in the management of both neuropathic and no neuropathic bladder dysfunction that has not responded to pharmacotherapy nor other intervention. We present herein our experience in 55 patients who underwent augmentation cystoplasty. Their mean age were 12.2 years (range 2.5-22.8) and the mean follow-up time was 4 years (1-13.1). They were divided in three groups according to the diagnosis: vesical or cloacal exstrophy (14 patients), neuropathic bladder (36) and posterior urethral valves (5 patients).

Should renal ultrasonography be done routinely in children with first urinary tract infection?

To reassess the impact of renal ultrasonography on the care of children with first febrile urinary tract infection (UTI) we conducted a computer search and review of medical records of (1) all children who were admitted to our hospital with first febrile urinary tract infection and underwent renal ultrasonography during a 25-month period beginning February 1, 1995, (2) all children diagnosed by ultrasound to have hydronephrosis during the same time period. Of a total of 124 patients with UTI, renal ultrasound appeared normal or showed evidence of acute pyelonephritis in 105 (84.7%), and in ano

How they begin and how they end: classic and new theories for the development and deterioration of congenital anomalies of the kidney and urinary tract, CAKUT.

CAKUT are problems that often require surgical intervention or, in the worst case, lead to renal failure and the need for dialysis and/or renal transplantation. It is believed that these anomalies share a common genetic cause and to date there has been no good animal model with which to study these abnormalities. Although the abnormal interaction between the ureteral bud and metanephric blastema leads to renal hypodysplasia, vesicoureteral reflux, and ectopic ureters to name a few, the genetic and biochemical modulation of urinary tract development is not understood. Studies using the mouse st

Total anatomic urinary tract replacement and renal transplantation: a surgical strategy to correct severe genitourinary anomalies.

During renal transplantation, the donor ureter is normally anastomosed to the recipient bladder. However, preexisting anomalies of the lower urinary tract or previous surgical interventions may render the recipient bladder unusable. Although in such situations urine may be externalized via cutaneous ureterostomy or an ileal conduit, both techniques are frequently complicated by bacterial colonization or chronic infection. To overcome these problems, the authors have been treating such children via extensive, staged, bladder reconstruction (augmentation) before transplantation. On rare occasion

Construction of continent catheterizable urinary conduit from an isolated segment of colon.

BACKGROUND: Continent urinary diversion may be necessary in a range of urological abnormalities. In circumstances where the standard techniques are not possible, alternative innovative techniques may be used. METHODS: In a female patient with bladder exstrophy, a continent diversion was recommended. The appendix was not available, the ureters were not suitable and a continent stoma was fashioned from an isolated segment of colon. RESULTS: The stoma proved to be continent, although it was somewhat stenotic. However, clean intermittent catheterization maintained its patency. CONCLUSIONS: A co

[Hereditary renal agenesis . Report of a case].

Renal agenesis is thought to result from a lack of induction of the metanephric blastema by the ureteral bud. Bilateral renal agenesis/dysgenesis (BRA/D) is rare, occurring in only one or two per 10,000 births. Despite its being moreover a sporadic event, there is a male to female ratio of 2.5 to 1 and approximately 20-36% of BRA/D present a familial recurrence, for genetic transmission most probably autosomal dominant with incomplete penetrance and variable expression, termed hereditary renal adysplasia (HRA). A case of prenatal diagnosis by ultrasound of renal agenesis/dysgenesis (left renal

[Blind ureter diagnosed through hematuria].

Blind ureter is a rare and incidental condition that, alike other congenital diseases, can be associated to other urological malformations. This papers presents one case report of a blind ureter diagnosed as a result of a apparent anaemic haematuria from the left renoureteral system due to unspecific diffused pyelitis.

Cryptorchidism as a caudal developmental field defect. A new description of cryptorchidism associated with malformations and dysplasias of the kidneys, the ureters and the spine from T10 to S5.

Cryptorchidism is a feature of abnormalities in the hypothalamo-pituitary-testicular axis, and almost all disorders of sexual differentiation in which a testis is present. We found cryptorchidism to be associated with malformations and dysplasias of the kidneys, the ureters and the spine from T10 to S5. The description of this association is new. The association was seen in 18% of cryptorchid boys younger than 3 years of age in a department of paediatric surgery, in 34% of cryptorchid foetuses who died in the third trimester, in 65% of cryptorchid patients with imperforate anus, and in all ind

The exstrophy-epispadias complex in the duplicated lower urinary tract.

PURPOSE: The exstrophy-epispadias complex in combination with a duplicated lower urinary tract is an extremely rare congenital malformation. We describe 2 cases of bladder exstrophy in association with a duplicated lower urinary tract. MATERIALS AND METHODS: We analyzed the case histories of 2 patients. The first case had anteroposterior duplication of the bladder communicating through an isthmus and a duplicate urethra. The anterior system was exstrophic and epispadiac. The second case had a duplicate bladder without epispadias. There was a communication between the anterior exstrophic and p

Renal transplantation into augmented bladders.

BACKGROUND: Urinary bladder augmentation is gaining popularity for the treatment of dysfunctional bladders in renal transplant patients. Although reported cases of adult and pediatric transplants into the augmented bladder have been favorable, the potential risk of urinary tract infection and graft failure under immunosuppression is still disputable. We report our experiences with 4 patients who underwent renal transplantation into an augmented bladder. METHODS: Between 1971 and 1996, 1275 renal transplants were performed at our institution. Of these transplants, 4 patients underwent renal tr

[The treatment of prune-belly syndrome at the University Hospital Center at Abidjan (Treichville)].

Over the past 10 years, we have seen sixteen children (15 males and 1 female) with the prune-belly syndrome. We have managed 12 of these children, but 4 of them were never reviewed. Abdominal wall reconstruction was performed in 11 children by dissection of spermatic vessels and fixing the testis in the scrotum. Because of our conservative approach to the management of urinary tract dilatation, we performed only one nephrostomy and 3 unilateral nephro-ureterectomies. Seven children were reviewed. The result of abdominal wall reconstruction was good in terms of cosmetic appearance and psycholog

The Mitrofanoff procedure in paediatric urinary tract reconstruction.

BACKGROUND: Urinary tract reconstruction is required in many congenital and some acquired urological conditions in childhood. The majority are managed by clean intermittent catheterization (CIC), for which purpose the appendix or other tubular structure may be used to provide a continent catheterizable abdominal stoma. METHODS: Between 1991 and 1996, 23 children underwent urinary tract reconstruction of varying complexity together with a continent diversion according to the Mitrofanoff principle. The appendix was used in 14 patients and the ureter in seven. Two patients had previously had an

[Vesicoureteral reflux and urinary incontinence in myelodysplastic patients: a comparison between those who had anti-reflux surgery alone and those who were treated by anti-reflux surgery combined with bladder augmentation with or without sling operation].

BACKGROUND: Vesicoureteral reflux and urinary incontinence are frequently observed among myelodysplastic patients. Since the conservative therapy is not always effective to improve these pathologies, surgical intervention is necessary for some of the patients. METHODS: Thirty-eight myelodysplastic patients were divided into 2 groups: Group A comprised 17 patients who had been operated by anti-reflux surgery (crossover method) alone and Group B 21 patients who had undergone anti-reflux surgery (Orikasa's method) and augmentation enterocystoplasty with or without sling operation. Vesico-ureteral reflux: a genetic condition?

Vesico-ureteral reflux (VUR) is a frequent condition, but in most instances, the precise cause is unknown. We here review the evidence of a genetic aetiology of VUR, inherited as an autosomal dominant trait, with variable expression. We discuss the possible pathogenetic relationship between VUR and other types of uropathies and possible strategies towards the identification of genes underlying VUR are presented. The isolation of the gene(s) responsible for uropathies will not only lead to a better insight into the embryology of the urological system, the pathogenesis of uropathies, but also to

[First-line vesico-psoas hitch ureteroneocystostomy for primary obstructive megaureter].

Between 1989 and 1997, 23 children with primary obstructive megaureter underwent surgical repair, routinely using a vesico-psoas hitch ureteroneocystostomy. Ten of them had a antenatal diagnosis. Mean postoperative follow-up was 3 years. No postoperative obstruction and no vesicoureteric reflux occurred. Assessment of renal function on the operated side showed improvement in 7 cases and stabilization in all other cases. No bladder dysfunction was observed. Vesicopsoas hitch is an advantageous procedure in this indication. It is almost always unnecessary to perform ureteral tailoring and its sp
