Smooth muscle cell apoptosis and defective neural development in congenital ureteropelvic junction obstruction.

PURPOSE: We assessed the smooth muscle cell apoptosis along with changes in cellular and extracellular components of the ureteropelvic junction in 23 patients with unilateral obstruction and compared them with 25 autopsies from ureteropelvic junction regions of age matched cadavers. MATERIALS AND METHODS: Tissue specimens obtained from pyeloplasty were divided into 3 sections-renal pelvis above the obstruction, obstructed ureteropelvic junction and ureter below the obstructed region. For the control group the normal ureteropelvic junctions of age matched infants were autopsied. In paraffin em

[Prenatal diagnostic of congenital unilateral hydronephrosis with megaureter--a case presentation].

Unilateral mild hydronephrosis with ureteral dilatation is a condition that can be detected by careful prenatal ultrasound examination. We present such a case, in a 26 year pregnant woman with hereditary antecedents (mother with Ombredan disease). The fetal ultrasound at 34 weeks indicated a left anechogenic sinuous image, identified as dilated ureter, and connected to a grade 2 hydronephrotic kidney. Normal data regarding the other kidney and the bladder made us recommend a close follow-up, considering this case as with a good prognosis. After birth, reassessment was favourable, and at 1 year

Tbx18 regulates the development of the ureteral mesenchyme.

Congenital malformations of the urinary tract are a major cause of renal failure in children and young adults. They are often caused by physical obstruction or by functional impairment of the peristaltic machinery of the ureter. The underlying molecular and cellular defects are, however, poorly understood. Here we present the phenotypic characterization of a new mouse model for congenital ureter malformation that revealed the molecular pathway important for the formation of the functional mesenchymal coating of the ureter. The gene encoding the T-box transcription factor Tbx18 was expressed in

Role of the hepatocyte nuclear factor-1beta (HNF-1beta) C-terminal domain in Pkhd1 (ARPKD) gene transcription and renal cystogenesis.

Hepatocyte nuclear factor-1beta (HNF-1beta) is a homeodomain-containing transcription factor that regulates tissue-specific gene expression in the kidney and other epithelial organs. Mutations of HNF-1beta produce congenital cystic abnormalities of the kidney, and previous studies showed that HNF-1beta regulates the expression of the autosomal recessive polycystic kidney disease (ARPKD) gene, Pkhd1. Here we show that the C-terminal region of HNF-1beta contains an activation domain that is functional when fused to a heterologous DNA-binding domain. An HNF-1beta deletion mutant lacking the C-ter

Concomitant anomalies in 100 children with unilateral multicystic kidney.

OBJECTIVES: To determine the incidence and type of associated urogenital anomalies in children with a unilateral multicystic kidney and to assess in children with nephrectomy the additional diagnostic value of cystoscopy and, in girls, of colposcopy. METHODS: This was a follow-up study of 100 fetuses with antenatally detected unilateral multicystic kidneys. After ultrasound confirmation of the diagnosis within a few days after birth voiding cystourethrography and isotope scan were performed in 83 of the surviving children to exclude vesicoureteral reflux and to establish renal function. Eight

Unfused renal ectopia: a rare form of congenital renal anomaly.

Unfused crossed renal ectopia observed 1 in 75,000 autopsies is a rare congenital anomaly. Typically one kidney is located in the proximity of the other kidney, and the ureter of the anatomically anomalous kidney crosses the midline to insert to the bladder in its normal anatomic position. Although renal function is usually not affected, the condition is generally accompanied by other congenital anomalies. In this case report, static and dynamic scintigraphic images of two patients with unfused crossed renal ectopia are presented. Besides properties of imaging modalities, clinical features are

[Arterial hypertension in associated with congenital unilateral stenosis of the ureteropelvic junction].

OBJECTIVES: To report one case of hypertension in association with congenital unilateral stenosis of the ureteropelvic junction (UPJ). METHODS: We describe the case of a 19- year-old male patient without urologic symptoms who was incidentally diagnosed of high blood pressure. Radiological tests showed right hydronephrosis suggestive UPJ stenosis. The insertion of a percutaneous nephrostomy allowed discontinuation of antihypertensive treatment. Dismembered pyeloplasty was indicated. RESULTS: Functional results were satisfactory. Blood pressure normalization without medical treatment was achie

Single vaginal ectopic ureter of fetal metanephric duct origin, ipsilateral kidney agenesis, and ipsilateral rudimentary uterine horn of the bicornuate uterus.

BACKGROUND: Congenital anomalies arising from the urogenital tracts are common. We experienced two cases of specific urogenital anomalies. CASE: Two Japanese women had a single ectopic ureter opening in the central wall of the vagina, ipsilateral kidney agenesis, and ipsilateral rudimentary uterine horn of the bicornuate uterus. One had squamous cell carcinoma of the uterine cervix, and the other had adenocarcinoma of the vagina arising from the ectopic ureter meatus, which was histologically confirmed as fetal metanephric duct remnant. One had vesico-vaginal fistula postoperatively, resultin

De novo Uroplakin IIIa heterozygous mutations cause human renal adysplasia leading to severe kidney failure.

Human renal adysplasia usually occurs sporadically, and bilateral disease is the most common cause of childhood end-stage renal failure, a condition that is lethal without intervention using dialysis or transplantation. De novo heterozygous mutations in Uroplakin IIIa (UPIIIa) are reported in four of 17 children with kidney failure caused by renal adysplasia in the absence of an overt urinary tract obstruction. One girl and one boy in unrelated kindreds had a missense mutation at a CpG dinucleotide in the cytoplasmic domain of UPIIIa (Pro273Leu), both of whom had severe vesicoureteric reflux,

Congenital mid ureteral strictures.

PURPOSE: Congenital mid ureteral stricture is rare. We report 7 cases, and discuss the differences in preoperative evaluation and surgical management compared to other obstructive entities. MATERIALS AND METHODS: Medical records and imaging studies of 7 children identified with mid ureteral strictures between 1998 and 2002 were reviewed retrospectively. Five newborns presented with prenatal hydronephrosis, and 2 children presented at age 15 years, one in the course of evaluation of blunt trauma, and one due to pain and abdominal mass. Imaging studies included renal ultrasound, voiding cystour

[Ureteral triplication--a case report].

INTRODUCTION: Of all children born with congenital anomalies, 30-40% have urogenital abnormalities. Most of them are asymptomatic, but if any symptoms occur, they are usually symptoms of urinary infections. CASE REPORT: A four-year old girl was admitted because of recurrent urinary infections and in order to perform a thorough clinical examination. Clinical examination showed an asymmetrical gluteus and presence of a fibroma in the same region. Detailed radiologic examinations (ultrasonography, plain X-ray, excretory urography, voiding urethrocystography) revealed a triplicate ureter on the r

Fryns syndrome: a review of the phenotype and diagnostic guidelines.

Fryns syndrome (FS) is the commonest autosomal recessive syndrome associated with congenital diaphragmatic hernia (CDH) and comprises CDH, pulmonary hypoplasia, craniofacial anomalies, distal limb hypoplasia, and internal malformations. Although there have been more than 50 case reports on probands with FS, the diagnostic guidelines were formulated from a review of eight patients and modifications to the guidelines have only once been suggested. Recently, several case reports have described new anomalies in FS and other papers have highlighted the variation in expressivity found in FS. This pa

Assessment of CT urography in the diagnosis of urinary tract abnormalities.

BACKGROUND: To evaluate the accuracy of helical computerized tomography urography (CTU) in diagnosing urinary tract disease. METHODS: We collected 102 patients who underwent CTU from March 2001 to September 2002. The clinical symptoms of these patients included: flank pain or hematuria, which were clinically suggestive of urinary system disorders. All patients received CT scan with or without intravenous contrast medium administration, except patients who had allergy history or poor renal function. Reformatted CT urography was performed and the accuracy of imaging diagnosis was evaluated. RE

Bilateral megaureters in the Adriamycin rat model.

Congenital obstructive uropathy is associated with significant morbidity and mortality in the human neonate. The pathophysiology of congenital obstructive uropathy is poorly understood. There are very few experimental models of prenatal obstruction of the urinary tract, except in the fetal lamb or inbred rats. Prenatal exposure to Adriamycin in a rat model leads to a spectrum of malformations including urinary tract anomalies. We hypothesized that Adriamycin administration during a particular time frame could yield a high incidence of urinary tract anomalies and therefore designed this study t

Horseshoe kidney with a circumcaval ureter.

Horseshoe kidney accompanied by a totally circumcaval ureter has rarely been reported in the same patient. However, both entities are well recognized as being uncommon anomalies of the genitourinary system. Variability in the normal course of the ureter makes it difficult to differentiate a normal variant from a congenital anomaly or a deviation due to other factors such as inflammation or cancer. We present a case of simultaneous horseshoe kidney and a right ureter that ran in a circular fashion (360 degrees) around the inferior vena cava. Contrast-enhanced CT showed that the ureter passed po

Neoplasia in horseshoe kidney with pyelic fusion and crossed single ureter.

Horseshoe kidney with pyelic fusion and crossed single ureter is a rare anomaly, with only 3 cases described in the literature. Such anomaly can be accompanied by other abnormalities, such as congenital scoliosis and situs inversus totalis. We present one case of this malformation associated with malignant neoplasia, treated with partial nephrectomy.

[Outcome of pregnancy in patient with cloacal dysgenesis].

The cloacal dysgenesis or persistent cloaca is an anomaly included in urogenital tract malformations (UGM) and, in particular, in anomalies due to a missed development of the urogenital septum. The UGM occur during 6-11th weeks of gestation owing to a stop and/or alteration of the normal development and subdivision process of the cloaca, the embryonic structure from which gives origin to the ano-rectal segment of midgut, bladder, ureters, vagina and uterus. The case of a patient submitted to many surgical treatments for urogenital tract congenital malformations is described; when pregnant, she

Calcineurin is required in urinary tract mesenchyme for the development of the pyeloureteral peristaltic machinery.

Congenital obstructive nephropathy is the principal cause of renal failure in infants and children. The underlying molecular and cellular mechanisms of this disease, however, remain largely undetermined. We generated a mouse model of congenital obstructive nephropathy that resembles ureteropelvic junction obstruction in humans. In these mice, calcineurin function is removed by the selective deletion of Cnb1 in the mesenchyme of the developing urinary tract using the Cre/lox system. This deletion results in reduced proliferation in the smooth muscle cells and other mesenchymal cells in the deve

[Ureteropelvic stenosis, agenesia of the contralateral kidney and sinistral inferior vena cava].

INTRODUCTION: Ureteropelvic stenosis occurs as a result of intraluminal or extraluminal alterations at the ureteropelvic junction. It is the most common cause of upper urinary tract obstruction. CASE REPORT: We report about a rare case of a man with ureteropelvic junction obstruction and a renal pelvic stone accompanied by other congenital anomalies, including a sinistral inferior vena cava and agenesis of the opposite kidney. Multiple vessels were present in front of the renal pelvis, while the renal vein itself was situated dorsal to the left renal pelvis, resulting in compression and obstr
