Construction of a female urethra using the vaginal wall and a buttock flap: experience with 40 cases.

METHODS: From 1975 to 1996 a urethra was constructed using tubularized anterior vaginal wall covered with a buttock flap in 40 female patients in whom urinary incontinence was associated with a short or absent urethra. The underlying pathology included bilateral single ectopic ureters, cloacal malformation, urogenital sinus malformation, previous failed surgery of the urethra, severe trauma, myelodysplasia, female hypospadias, ectopic ureterocele with destruction of the urethra, congenital epispadias, bladder exstrophy, and previous hysterectomy and vaginectomy for clear cell carcinoma caused

Male epispadias: experience with 45 cases.

PURPOSE: We evaluate the long-term results of surgery for isolated (without exstrophy) male epispadias. MATERIALS AND METHODS: A total of 45 male patients with isolated epispadias were treated and followed at Debrousse Hospital between 1971 and 1993, 8 in whom repairs performed elsewhere had failed and 14 had total urinary incontinence. Incontinence was treated with a modified Young-Dees reconstruction of the bladder neck and the ureters were reimplanted in all cases. Long-term followup was available in 18 cases of a Cantwell-Young-Gross, 4 Duplay, 7 full thickness skin graft, 1 bladder graft

[Ureteral ectopia in pediatric age: treatment and evolution in 55 cases].

In the last twenty years 55 children with ureteral ectopia were diagnosed and treated in our Hospital. Thirty seven of them were female (67.3%) and they presented the urinary tract infection like the most frequent symptomatology. Seven children were diagnosed during the first days after being born because the detection of hydronephrosis in the antenatal ultrasound; 36 were associated with complete ureteric duplication (65.4%) and the other 19 with single system (34.6%). Congenital abnormalities associated were seen in 14 patients with more frequency in those which presented single-system ectop

Ischiopagus twins: an outcome analysis of urological aspects of repair in 3 sets of twins.

PURPOSE: We assessed the urological components of ischiopagus twins and outcomes of surgical correction. MATERIALS AND METHODS: We performed a longitudinal study of 3 pairs of twins who presented to a single institution for repair and analyzed the outcomes of certain aspects of repair. RESULTS: Three pairs of twins joined from diaphragm to pelvis and facing each other in an embracing posture were separated in the last 10 years at a single institution. Each pair of twins had a similar constellation of shared organs, including abdominal wall, bony pelvis, liver, bowel, crossed ureters, bladder

Results of the Mitrofanoff procedure in urinary tract reconstruction in children.

OBJECTIVES: To evaluate the success and long-term complications associated with the use of continent catheterizable conduits based on the Mitrofanoff principle in children. PATIENTS AND METHODS: The records of 43 patients (21 female and 22 male) who underwent the construction of a continent catheterizable stoma based on the Mitrofanoff principle between 1987 and 1996 were reviewed retrospectively. The mean age at surgery was 10 years (range 3-21) and the mean follow-up was 3 years (range 0.5-6.5). Twenty-eight of the 43 children underwent augmentation cystoplasty in conjunction with the Mitro

Giant megalo-ureter and duplex kidney in an asymptomatic adult.

An abdominal mass was palpated in an asymptomatic adult during a routine medical check-up. Ultrasonography and computed tomography scan diagnosed a simple renal cyst, a mesenteric cyst and a seminal vesicle cyst. At laparotomy a complete ureteral duplication and a giant ectopic megalo-ureter were diagnosed. Other complications were ruled out in the follow-up. Ureterectomy without heminephrectomy was performed and the patient remains asymptomatic 5 years after surgery.

[Congenital cysts of the seminal vesicles with ipsilateral kidney agenesis. Clinical aspects of 7 cases].

Seminal vesicle cysts with ureteral ectopy and ipsilateral renal agenesis or dysplasia are rare congenital malformations. This study reports 7 consecutive cases in a single center and, for the first time in the literature, reports long-term results on 5 cases. Patients were between the age of 15 and 57 years. The malformation was diagnosed in all cases with excretory urography, computed tomography, magnetic resonance imaging and cystoscopy. 5 patients were followed after diagnosis for 26 to 119 months (mean 52 months). Only 2 of 7 patients primarily presented with nonspecific lower urinary tra

[Surgical treatment of primary mega-ureter].

Primary megaureter is a congenital dilatation of the ureter secondary to obstruction of its terminal portion. It must be corrected surgically only when its obstructive nature can be demonstrated and/or in the presence of complications (especially when it is associated with vesicoureteric reflux). Surgical treatment must always include resection of the obstructive part and ureterovesical reimplantation with an anti-reflux device. Compliance with these 2 principles may require remodelling of the ureter depending on the size of the urethra and bladder. Many surgical procedures can be performed, b

Lower urinary tract abnormalities in children with multicystic dysplastic kidney.

PURPOSE: To determine the nature and frequency of associated structural abnormalities of the lower urinary tract in children with multicystic dysplastic kidney. MATERIALS AND METHODS: The authors reviewed imaging findings from studies in 59 children with unilateral multicystic dysplastic kidney. All patients had undergone renal ultrasonography (US), renal scintigraphy, and voiding cystourethrography. The diagnosis was based on characteristic imaging findings (echogenic, cystic kidney at US that did not function at scintigraphy). RESULTS: Nineteen (32%) patients had one or more abnormalities

[Solitary crossed renal ectopia and congenital vertebral anomalies].

OBJECTIVE: To describe a case of solitary crossed renal ectopia that had been incidentally discovered in a healthy male with a previous history of spinal surgery. METHODS/RESULTS: A 43-year-old male who had undergone two surgical procedures for severe scoliosis of the thoracic spine when he was 7 years of age, consulted for symptoms of unspecific urethritis that had just remitted. Abdominal ultrasound and excretory urography revealed a solitary crossed ectopia on the right side and the spinal x-ray showed congenital fusion of the cervical vertebrae (Klippel-Feil syndrome). CONCLUSIONS: Cross

[Endoscopic therapy of ectopic ureterocele].

For a long time the standard treatment of an ectopic ureterocele, combined with a double collection system, was an upper-pole resection with or without ureter resection. Between 1980 and 1992, 27 children were treated initially with an endoscopic incision of the ureterocele. Twenty-four of these children were followed up. In 11 of 24 cases the endoscopic incision was sufficient and in 5 of 24 other children a reflux operation had to be performed afterwards, so that in 66% of the children the upper-pole resection was not necessary. We presume that the endoscopic incision of an ectopic ureteroce

A suspected case of bilateral crossed renal ectopia or bilateral jet effect.

Crossed renal ectopia is a relatively unusual congenital anomaly. Bilateral crossed renal ectopia is considered the rarest form of this anomaly. A 19-year-old girl with intermittent flank pain was admitted to our department. Various clinical and laboratory examinations were carried out. Excretory urography showed a normal upper urinary tract anatomy, whereas the distal ends of the ureters crossed each other in the pelvis, which suggested bilateral crossed renal ectopia.

Expression of keratinocyte growth factor in embryonic liver of transgenic mice causes changes in epithelial growth and differentiation resulting in polycystic kidneys and other organ malformations.

Expression of human keratinocyte growth factor (KGF/FGF-7) was directed to hepatocytes during the later period of mouse gestation using a human apolipoprotein E (ApoE) gene promoter and its associated liver-specific enhancer. Human KGF was detectable in liver extracts and serum prepared from e17.5-e19.5 embryos, concomitant with the appearance of morphological abnormalities in several organs which express KGF receptor. The most striking phenotypic aberration in the ApoE-hKGF transgenic embryos was marked hyperplasia and cystic dilation of the cortical and medullary kidney collecting duct syste

[Bilateral persistence of the common mesonephric duct in children. Apropos of a case. Review of the literature].

We report a case of bilateral persistence of the common mesonephric duct in a child. This anomaly is an error of implantation of the vas deferens in the distal part of the Wolffian duct. This abnormality is extremely rare (6 cases have been reported in the world literature). The diagnosis is usually incidental during laparotomy or on retrograde cystography (vesico-ureteric and vesico-deferential reflux). It is simple to treat: bilateral deferential ligation in view of the context of multiple malformations, in order to prevent ascending genital infections (prostatitis, epididymitis, testicular

Type II ureteral triplication associated with ectopic ureter.

We report a case of type II ureteral triplication associated with ectopic ureter. The ureter, draining the upper renal segment, entered the vagina, and the other two ureters, draining the middle and lower renal segments, had joined and entered the bladder. The anatomic findings are compatible with those predicted by the Weigert-Meyer law. A thorough evaluation of such an anomaly is essential for effective management.

Kidney transplantation to patients with congenital malformations of the distal urinary tract.

Fourteen of 1000 consecutive kidney transplant patients had congenital malformations affecting the bladder or urethra: six had congenital valvulus of the urethra, two congenital sclerosis of the bladder outlet, and six a neurogenic bladder. Pretransplant surgery had been performed in all patients: reimplantation of ureter (n = 11), resection of congenital valvulus (n = 7), and nephrectomy (n = 6). Four patients had an intestinal bladder. Age was 0-17 (median 1) years at diagnosis. Follow-up time was 3-10 (median 5) years. Special transplant surgery techniques were required in five patients. Congenital megalourethra.

The authors report on two patients who had fusiform megalourethra with unusual features. The first patient had left hydronephrosis with duplication of the ureter, oblique sagittal septum in the urinary bladder, and Y duplication of urethra with megalourethra of both channels. The second had features of the abdominal muscle deficiency syndrome, with anterior ectopic anus, rectourethral fistula, and fusiform megalourethra. Both patients were treated by preliminary urinary diversion. The first patient subsequently had staged reconstruction. The other patient died of urinary sepsis. The authors be

Isolation of cosmids corresponding to the chromosome breakpoints of a de novo autosomal translocation, t(6;19)(p21;q13.1), in a patient with multicystic renal dysplasia.

Hydronephrosis caused by pelvi-ureteric junction obstruction (PUJO) is a frequent urological malformation assumed to result from a deficient development of the ureteric bud. The exact etiology of pelvi-ureteric junction stenosis is unknown, but there is convincing evidence for a genetic cause, with linkage analysis predicting a hereditary hydronephrosis locus on chromosome 6p. We encountered a patient with a de novo autosomal t(6;19)(p21;q13.1) and attendant bilateral multicystic renal dysplasia (MRD), bilateral PUJO resulting in massive hydronephrosis, and an associated von Mayer-Rokitansky-K

[Radiologic aspects of the coexistence of vesicoureteral reflux and other abnormalities of the urinary tact in children].

In this report cases of the coincidence of vesicoureteral reflux with different congenital abnormalities have been described. Radiological and ultrasonographic examinations of 240 children with 324 vesicoureteral refluxes showed the presence in 8.7% additional congenital anomalies such as: duplex pelvicaliceal system and duplex ureter, ureterocele, horseshoe kidney, primary megaureter, ureteropelvic junction obstruction, posterior urethral valves, neurogenic bladder. The coincidence of these abnormalities with vesicoureteral reflux caused some diagnostic and therapeutic difficulties. In conclu
