The multicystic dysplastic kidney in children.

Multicystic dysplastic kidney (MCDK) is the most common renal cystic disease in infancy and the second most common neonatal abdominal mass. It represents an extreme form of renal dysplasia. The key features in MCDK include variability in size, loss of kidney shape, absence of function and obstruction of ureter. Current management is still controversial. The prognosis is largely depended upon the problems of associated anomalies and the condition of the contralateral kidney. From September '82 to September '91, twelve patients with multicystic kidney have been treated at Taichung Veterans Gener

Unilateral renal agenesis may result from in utero regression of multicystic renal dysplasia.

Renal agenesis is generally thought to result from a lack of induction of the metanephric blastema by the ureteral bud, which may be secondary to ureteral bud maldevelopment and/or to a problem with the formation of the mesonephric duct. Multicystic dysplasia is thought to result from early ureteral obstruction as evidenced by the high frequency of associated proximal ureteral atresia. The recent obstetrical practice of large scale screening of large numbers of fetuses with sonography has resulted in a significant increase in the detection of this and other genitourinary anomalies. These findi

[Color Doppler ultrasound in differential diagnosis of unilateral congenital cystic kidney abnormalities].

Doppler-sonography was performed in 16 newborns with prenatally diagnosed unilateral cystic renal malformation. 11 babies with multicystic dysplastic kidneys showed doppler-sonographically reduced systolic flow velocities and elevated resistive index (RI) [RI = 90-100%] or lack of obtainable perfusion signals. 4 of them had to undergo nephrectomy, the other asymptomatic patients could be managed conservatively, spontaneous regression could be observed. 5 other infants suffering from cystic nephroma, severe ureteropelvic junction obstruction, hudge bleeding of the adrenal gland and hydronephros

Comprehensive planning of operative strategy for separation of ischiopagus tripus twins with particular reference to quality of life.

A 27-year-old mother was diagnosed by prenatal ultrasonography as having triplets at gestational age 32 weeks. Following cesarean section at 37 weeks, a pair of female babies were noted for the first time to be joined by a common pelvis with three lower limbs. They had separate upper gastrointestinal tracts, which joined in the distal ileum, leading to a common colon, rectum, and a single anus. Each twin had a functioning kidney, with a single ureter leading to a common bladder. A common urethra originating from the bladder neck ran into the urogenital sinus of one baby. Prior to the surgical

[Abdominal pain in childhood caused by stenosis of the pyelo-ureteral junction].

The authors describe three patients with episodes of abdominal pain where despite a negative finding in the urine sonography revealed dilatation of the hollow system. Urography proved stenosis of the pyeloureteral junction. The authors discuss the cause of abdominal pain. Consistent with data in the literature, the authors draw attention to pitfalls in the diagnosis of hydronephrosis. They recommend sonographic examination of children with abdominal pain, as a negative urinary finding does not rule out hydronephrosis. In this respect screening of hydronephrosis and other obstructive anomalies

Management of primary obstructive megaureter without reflux in neonates.

Systematic antenatal ultrasonography has significantly altered the conditions of diagnosis of megaureters. Pediatric urologists are now confronted with a large group of neonates with asymptomatic megaureter. Furthermore, reports of spontaneous resolution of primary megaureter without reflux have become common. We were confronted with 59 renal units in 48 neonates. We postulated that primary megaureter represented a significant obstructive uropathy when the kidney exhibited stasis and large pelvic and caliceal dilatations. So, 35 ureters were operated on initially. The other 24 cases were manag

[Clinical course of primary non-obstructive megaureter: analysis of our series].

Due to the widespread use of obstetrical ultrasonography with concomitant fetal screening, congenital uropathies are usually diagnosed in asymptomatic neonates. The main issue is to distinguish those cases of dilatation which are clearly obstructive and need surgical treatment from those which are within the normal anatomical range. Out of 47 patients with primary megaureter, 23 were nonobstructive, these are the aim of our study. There were 16 males and 7 females; 13 were located on the left side, 7 on the right side and 3 were bilateral, adding up to 26 reno-ureteral units. Prenatal diagnosi

[Pathologic development of the kidney].

In the absence of firmly established views on the development of nephropathy, we describe in this paper the embryogenetic and clinical aspects of kidney disease. Congenital reductive nephropathy always arises in the ureteral bud and is determined by two factors, endogenous dysplasia and endogenous obstruction. The nine well-known patterns of disease that may result are described herein. The most important starting points are as follows: (a) A dysplastically disorganized and hence refluxive trigone of the bladder induces, via pyramidal-medullary deficiencies, a defect of the metanephros and thu

Single system ectopic ureters and ureteroceles associated with dysplastic kidney.

Eight children forming an uncommon subgroup of renal obstructive dysplasia are presented. Each child had a nonfunctioning dysplastic kidney with a single collecting system with ectopic ureteral insertion and/or ureterocele. Five of the children had classic multicystic dysplastic kidneys, one had the hydronephrotic type of multicystic dysplastic kidney and two had hypoplastic kidneys. Other significant medical problems in 5 of the 8 children (63%) included VACTERL association, congenital heart disease and other genitourinary malformations. Unlike some children with unilateral multicystic dyspla

Single-system ectopic ureters: a review of 12 cases.

Eight boys and 4 girls with single-system ectopic ureters were seen during a 15-year period. The abnormality was bilateral in 4 patients and the majority had major associated urological and other abnormalities. Eight ectopic ureters were reimplanted successfully into the bladder and 4 nonfunctioning kidneys were removed, as were 2 "blind" ectopic ureters. One patient died during the neonatal period and the others have been followed from 1 to 15 years. Renal function was noted to be normal in 10 patients and 1 has mild renal failure. Bladder function was normal in 8 patients. Early surgery shou

[Endoscopic operation of congenital anterior urethral valve. Application of flexible renoureteroscope for antegrade urethroscopy].

4 children with anterior urethral valve were treated by TUR-Valve. Before TUR, valve anatomy could be easily seen by antegrade urethroscopy through cystostomy using a 10.8Fr caliber flexible renoureteroscope and destruction of the valve could be recognized after TUR. Antegrade urethroscopy with a flexible fiberscope was so helpful to TUR-Valve and the findings of voiding cystourethrography and micturition have remarkably improved. This technique is not so difficult to apply and thought to be one of the best methods to recognize valve anatomy and degree of resection. This technique is also appl

[Early diagnosis of congenital ureterocele].

Ureterocele is a congenital urinary abnormality characterized by the presence of an intrabladder hernia or cystic ballooning of the lower end of an ureter lying between the mucosa and muscle of the bladder. The abnormality leads to urinary retention and recurrent urinary tract infection which can cause irreversible damage to the kidney. It can become very large and is the most common cause of bladder outlet obstructions in infant girls. It may be associated with serious urologic abnormalities in either sex. This abnormality can be suspected in the fetus by ante-natal echography and confirmed b

[Congenital valves of the ureter. Comments on 7 cases studied in childhood].

Ureteral valves are a rare cause of congenital obstructive uropathy in pediatric age. Symptoms onset is variable and depends directly on the degree of valvular obstruction. Seven cases of ureteral valves were studied in the Radiology Department of "Bambino Gesù" Pediatric Hospital in Rome; every type of technique available in the hospital was employed. For each case, the authors carefully describe the clinico-radiological approach which led to the diagnosis of obstruction due to the presence of one or more valvular structures (hydronephrosis). Intravenous urography proved to be the most sensit

Contralateral renal abnormalities in multicystic-dysplastic kidney disease.

Multicystic-dysplastic kidney (MCDK) disease has been associated with contralateral genitourinary tract abnormalities; however, neither the nature nor the prevalence of the contralateral renal lesion has been delineated. We retrospectively studied patients with MCDK disease to determine the prevalence of contralateral renal abnormalities and of other urologic anomalies. Forty-nine patients, ranging in age from neonates to 6 years, were examined by renal ultrasonography, excretory urography or nuclear medicine scanning, and voiding cystourethrography; 51% of the patients had an associated urolo

[Fetal hydronephrosis].

Antental intervention in cases where the foetus was suspected of having hydronephrosis has been the subject of intense discussion in recent years where the diagnosis was technically possible. The object of this investigation was to assess the end result in foetuses with hydronephrosis diagnosed prenatally and who were treated expectantly without intervention in utero. Fourteen consecutive cases of suspected foetal hydronephrosis are described. In four of the cases the hydronephrosis was transient without consequences for subsequent renal function. Three infants died, however, immediately after

Diabetes insipidus, diabetes mellitus, optic atrophy and deafness (DIDMOAD syndrome). A clinical study in two Sudanese families.

Four Sudanese children with DIDMOAD syndrome (diabetes insipidus, diabetes mellitus, optic atrophy and deafness) are reported. They were two boys (aged 15 and 16 years) in one family and a boy and a girl (aged 16 and 6 years, respectively) in another family. Diabetes mellitus was first to appear (at 3-8 years) followed by deafness and visual failure; and the disease ended fatally in one patient (aged 20 years). In the other three, diabetes insipidus was confirmed using water deprivation test for 8 hours. The maximum urine osmolality ranged between 131-523 mOsm/kg, whereas the corresponding pla

[Blind-ending bifid ureter associated with adult congenital hydronephrosis].

We present a case of cephalad blind-ending bifid ureter coexisting with adult congenital hydronephrosis that had not clinically manifested previously and had been incidentally detected during surgery. The possibility that both anomalies of development may be related etiopathogenically is discussed. This and other malformations should be sought during the preoperative evaluation of a patient with congenital hydronephrosis.

[A case of persistent left superior vena cava with a horseshoe kidney].

A case of persistent left superior vena cava with a horseshoe kidney was found in a Japanese male cadaver (72 years old) during a dissection for students in 1989. The main findings were as follows. The right superior vena cava (21.2 mm average diameter) was normal and opened into the right atrium. The left superior vena cava (8.4 mm average diameter) was smaller than the right one and ran in the coronary sinus to reach the right atrium. There was a small transverse anastomosis (8.9 mm average diameter), which corresponded to the normal left brachiocephalic vein, between the right and left supe
