Outcomes of the Surgical Stone Management in Pelvic Ectopic Kidneys: A Retrospective Comparison of Three Different Approaches.

Background/Objectives: This study evaluates and compares the surgical outcomes of open pyelolithotomy, laparoscopic pyelolithotomy, and retrograde intrarenal surgery (RIRS) in the management of pelvic ectopic kidney stones. Methods: A retrospective analysis was conducted on 47 adult patients with pelvic ectopic kidney stones who underwent surgery between January 2009 and January 2024. Patients were categorized as open pyelolithotomy (n = 15), laparoscopic pyelolithotomy (n = 14), or RIRS (n = 18). Stone-free (SF) rates were assessed in the early postoperative period (1st or 2nd day), in the 1s

Ectopic Right Kidney Imaged With Dynamic 82Rb PET/CT.

Ectopic kidney in the thoracic region are infrequent in clinical practice. A 64-year-old man underwent a Rubidium (82Rb) positron emission tomography/computed tomography (PET-CT) scan for the evaluation of myocardial ischemia due to a diagnosed 2-vessel coronary artery disease. While no myocardial perfusion abnormalities were detected, the scan showed the presence of a diaphragmatic high stand on the right side, with evidence of an ectopic kidney. We here present static and flow dynamic images of this rare finding1-3.

Laparoscopic nephrectomy in crossed-fused inferior ectopic kidneys - Defusing complexities: A case series.

BACKGROUND: Crossed fused renal ectopia manifests as an operative challenge to the minimally invasive urologist. Provided that certain preset steps are meticulously followed and the anatomy is understood, it is not a difficult task to operate on these cases. PATIENTS AND METHODS: This is a retrospective study of 15 cases at our Dr. RMLIMS, Lucknow over a period of 5 years. RESULTS: The mean operative time was 117.3 minutes and the mean blood loss was 99.3 ml. Two arteries were encountered in 53.3 % of cases, but they can vary in number from a single artery to even six, as found in our series

Pathological ectopic kidney: Rare cases report and review of literature.

INTRODUCTION: Ectopic kidneys (EKs) are a rare congenital anomaly, occurring in approximately 1 in 900 individuals, where the kidney fails to ascend to its typical lumbar position. Although often asymptomatic, EKs are more susceptible to pathological conditions such as obstruction, infection, and stone formation due to their abnormal positioning. CASE PRESENTATION: We present three instructive cases of pathological ectopic kidneys, each demonstrating unique clinical challenges and management strategies: DISCUSSION: These cases underscore the value of advanced imaging for diagnosis and functio

Incidental Pelvic Ectopic Kidney Discovery During Pre-TAVR Imaging: Implications for Surgical Planning.

BACKGROUND Renal ectopia (RE) is a congenital malformation in which one or both kidneys fail to reach their normal anatomical position. Most cases are asymptomatic, but they can occasionally lead to complications. This case report presents the incidental finding of a pelvic ectopic kidney during a pre-transcatheter aortic valve replacement (TAVR) evaluation, showing the importance of recognizing anatomical variations in surgical planning for vascular access. CASE REPORT A 68-year-old man undergoing evaluation for severe symptomatic aortic stenosis was incidentally found to have a left pelvic k

Pelvic Ectopic Kidney in an Adult: Robotic-Assisted Surgical Treatment of Unrecognized Ureteropelvic Junction Obstruction.

Ureteropelvic junction obstruction (UPJO) is observed in approximately 30% of patients with ectopic kidneys. Due to the narrow pelvic space and risk of injuring aberrant structures, an ectopic pelvic kidney with UPJO presents a unique treatment challenge. Most experiences in treating UPJO in pelvic ectopic kidneys using robotic surgical systems are based on the pediatric population. Only a few cases of successful robotic-assisted surgery in adult patients with this condition have been described. This case reports illustrates that the indications for robotic-assisted surgery for UPJO may safely

An Asymptomatic Intrathoracic Kidney Secondary to a Bochdalek Hernia in a Geriatric Patient.

A Bochdalek hernia is a congenital defect that permits abdominal organs to herniate into the thoracic cavity. While commonly diagnosed in neonates due to symptomatology, it is often found incidentally in adults. The case presents an 84-year-old male from a skilled nursing facility complaining of left-sided chest pain radiating to the left shoulder, accompanied by shortness of breath and nausea. Initially worked up for an acute coronary cause or pulmonary embolism, imaging revealed an incidental right-sided Bochdalek hernia with intrathoracic displacement of the right kidney. Given the absence

Intrathoracic Kidney Detected on DTPA Scan.

Intrathoracic kidney is very rare and has the lowest prevalence among all renal ectopias. We report the case of a 70-year-old male who was referred for diethylenetriaminepentaacetic acid (DTPA) scan as a part of presurgical work-up for a right renal mass. PriorUltrasonography abdomen incorrectly reported left kidney as normal in location. 99mTc-DTPA renal scan combined with single-photon emission computed tomography - computed tomography revealed impaired global glomerular filtration rate with left kidney in ectopic (intrathoracic) location. 18-fluorodeoxyglucose positron emission tomography-c

Renal Preservation During Open Abdominal Aortic Aneurysm (AAA) Repair in a Patient With a Pelvic Kidney: A Case Report and Surgical Insight.

Repair of an abdominal aortic aneurysm (AAA) becomes significantly more complex when complicated by the presence of atypical renal vasculature and an ectopic kidney, as these anatomic variations pose distinct surgical challenges. In these cases, the surgeon must not only address the aneurysmal part of the aorta but also manage the aberrant renal anatomy to ensure the preservation of kidney function. This is critical in open AAA repair, where prolonged aortic clamping and manipulation near aberrant renal vessels can compromise renal perfusion. Preservation of renal function under these circumst

Subdiaphragmatic Ectopic Kidney: A 12-Year Follow-Up Case Report.

This 12-year follow-up of a prenatally misdiagnosed subdiaphragmatic ectopic kidney highlights the challenge of distinguishing renal agenesis from anatomical variants. Conservative management proved effective without surgical intervention. Awareness of such anomalies can prevent misdiagnosis, particularly in thoracic imaging, and guide appropriate long-term surveillance through childhood and adolescence.

Thoracoscopic Plication for Neonatal Diaphragmatic Eventration With Intrathoracic Ectopic Kidney: A Rare Case Report.

Coexistence of congenital diaphragmatic eventration (CDE) and an intrathoracic ectopic kidney (ITEK) is extremely rare and may lead to substantial respiratory distress in neonates. This study reports the case of a small-for-gestational-age male neonate presenting with respiratory distress and requiring nasal continuous positive airway pressure. Computed tomography revealed right-sided CDE accompanied by an ITEK. At 44 days old, the infant underwent thoracoscopic diaphragmatic plication. CO2 insufflation during surgery led to spontaneous reduction of the ectopic kidney into the abdominal cavity

Urinary tract anomalies and their role in pediatric stone formation.

PURPOSE: This review aims to evaluate the role of congenital urinary tract anomalies in the pathogenesis of pediatric nephrolithiasis. By examining specific structural abnormalities such as ureteropelvic junction obstruction (UPJO), horseshoe kidney, ectopic kidney, exstrophy-epispadias complex, and ureterocele, we aim to elucidate how these anomalies contribute to urinary stasis, infection, metabolic disturbances, and ultimately, stone formation in children. METHODS: A narrative literature review was conducted focusing on congenital anomalies of the urinary tract and their association with p

99m Tc EC Renal Scintigraphy in a Rare Case of Retrovesical Kidney: Insights Into Protocol Considerations and Diagnosis.

A 28-year-old man presenting with swollen feet underwent a 99m Technetium Ethylene-Dicysteine (EC) renal diuretic scintigraphy to evaluate the renal function, which demonstrated a zone of increased tracer uptake in the retrovesical space, indicative of an ectopic kidney with tracer accumulation in the dilated pyelocalyceal system and good excretion following Lasix administration. SPECT/CT imaging further confirmed the presence of the ectopic kidney, which was positioned horizontally just posterior to the bladder, exhibiting normal cortical thickness with a dilated pyelocalyceal system in its u

Crossed fused renal ectopia complicated by a renal stone: endoscopic management with retrograde intrarenal surgery.

Crossed fused renal ectopia (CFRE) is an uncommon and congenital anomaly in which both kidneys are fused and located on one side of the body, often going undetected due to its asymptomatic nature. When symptomatic, it can present unique challenges in the management of nephrolithiasis due to altered anatomy and impaired drainage. We report a rare case of a young adult male with a large renal calculus in a left-to-right crossed-fused ectopic kidney-an anatomical variant seldom encountered in clinical practice. The patient was successfully treated with retrograde intrarenal surgery (RIRS) using a

Unilateral absence of the pulmonary artery, patent ductus arteriosus, and ectopic Kidney: A multimedia case report.

This report describes a rare case of unilateral absence of the right pulmonary artery (UAPA) with several artery malformations, patent ductus arteriosus (PDA), and an ectopic kidney. A 65-year-old male with arrhythmia underwent chest computed tomographic angiography (CTA), which revealed an absence of the right pulmonary artery with increased tortuous vascular shadows in the right lung hilum. The patient underwent PDA at the age of 13. An ectopic left kidney was detected via urinary ultrasound. This anomaly in adults requires treatment according to symptoms, and there is often no commonly acce

Acute obstructive pyelonephritis revealing a crossed non-fused renal ectopia in the early postpartum period: A case report.

Non-fused crossed renal ectopia (CRE) is a rare congenital anomaly where one kidney migrates contralaterally without fusion. We report a 31-year-old woman presenting nine days postpartum with febrile abdominal pain revealing a left non-fused crossed ectopic kidney complicated by bilateral ureterohydronephrosis and acute pyelonephritis. Management involved bilateral ureteral stenting and antibiotic therapy with favorable recovery. This case highlights the potential for pregnancy-related physiological changes to unmask previously silent renal anomalies and underscores the diagnostic importance o

Ectopic Kidney Masquerading as a Pelvic Mass: A Case Report.

Renal ectopia is a congenital anomaly caused by abnormal migration of the kidneys during the process of embryogenesis. Under typical development, the kidneys ascend to the lumbar region during the early gestational period. A failure in this process can result in ectopic kidneys, most commonly found in the pelvis. Although often asymptomatic, pelvic kidneys can be complicated by pelviureteric junction obstruction (PUJO), infection, reflux, or malrotation in a few cases. We present a rare case of a pelvic kidney with PUJO presenting as a lower abdominal mass in an elderly female. A 60-year-old f
