INTRODUCTION: Transitional care of patients with complex uro-genital malformations (CUGM), including spina bifida, bladder exstrophy-epispadias complex, cloaca, congenital adrenal hyperplasia, and vaginal atresia, remains a worldwide challenge. These conditions are associated with lower urinary tract dysfunction (LUTD), long-term urinary morbidity, and significant psychosocial consequences. The aim of this study was to investigate how pediatric urologists in Italy address sexuality and fertility in adolescents and young adults with CUGM.

OBJECTIVE: In classic bladder exstrophy (CBE), achieving optimal urethral coverage during epispadias repair (ER) remains surgically demanding. This study introduces a novel anatomical maneuver-external rotation (ExR) of the corpora cavernosa (CC)-and quantitatively compares it to internal rotation (InR) to determine which provides superior urethral coverage and penile length.
MATERIALS AND METHODS: We retrospectively analyzed 29 patients (aged 11-59 months) who underwent primary ER between 2020 and 2024 after prior bladder closure. In each case, the CC were fully mobilized, and neurovascular

INTRODUCTION: Isolated male epispadias is a rare congenital condition where the urethral opening is abnormally on the upper penis side. Its severity varies, affecting continence and often requiring surgery. Outcomes depend on severity, voiding problems, and the surgeon's expertise.
CASE PRESENTATION: Two boys, aged 7 and 9, each presented with abnormal urethral opening on the dorsum of the penis, with abnormal urinary stream (bent upwards), without a history of prior surgeries, which was consistent with isolated epispadias. Clinical evaluations revealed a subcoronal meatus on the dorsum of th

PURPOSE: Bladder exstrophy-epispadias complex (BEEC) is a rare urogenital malformation. Children risk long-term urinary and genital dysfunction. The study aimed to describe health-related quality of life (HRQoL) experiences among children with BEEC and ultimately guide the development of a BEEC-specific HRQoL questionnaire.
METHODS: Ten focus groups stratified by child age and sex were conducted in Sweden with 15 children aged 8-18, and 23 parents of children aged 2-17. Sessions were moderated, audio-recorded and transcribed verbatim. Reports of children's BEEC-related HRQoL were extracted an

BACKGROUND: Penopubic fistula formation remains one of the most frequent early complications following bladder exstrophy repair. Platelet-rich fibrin (PRF), an autologous, high-concentration platelet, white blood cells, and fibrin biomaterial that enhances tissue healing and regeneration, has shown potential for surgical applications. However, PRF has not been applied in primary bladder exstrophy repair. This study aimed to assess the effectiveness of autologous PRF membrane applications in primary bladder exstrophy repair in male patients.

INTRODUCTION: The Mitrofanoff principal entails creating a continent catheterizable channel, traditionally the appendix. In patients with nonfunctioning kidney, the ureter of this kidney can be an alternative This study aims to compare outcomes of ureter versus appendiceal Mitrofanoff channels (MCs).
PATIENTS AND METHODS: We retrospectively reviewed patients who underwent MC creation between January 2020 and June 2024 for patients with neurogenic bladder, exstrophy-epispadias complex, prune belly syndrome, or posterior urethral valves. Group A included patients who had a ureter as MC (n = 11)

Bladder exstrophy is a rare congenital malformation within the exstrophy-epispadias complex, usually diagnosed in the neonatal period. Untreated cases persisting into adulthood are exceptional and may be associated with major functional disorders, including pelvic organ prolapse. We report the case of a 25-year-old woman with an uncorrected complete bladder exstrophy, revealed by lifelong urinary incontinence and postpartum genital prolapse. Radiological assessment showed pubic symphysis diastasis without involvement of the upper urinary tract. Management requires a multidisciplinary approach

BACKGROUND: Isolated male epispadias (IME) is a rare congenital malformation. Surgical repair aims to improve urinary function, correct penile curvature, reconstruct the urethra and glans, and preserve future sexual function. Because available series are small, the influence of anatomical subtype and operative technique on outcome remains incompletely defined.
OBJECTIVE: To report single-center outcomes of Thiersch-Duplay and modified Cantwell-Ransley repairs for IME, with attention to anatomical subtype, complications, and age-appropriate continence outcomes.
METHODS: We retrospectively rev

INTRODUCTION AND IMPORTANCE: Patent urachus is a rare congenital anomaly resulting from failed obliteration of the embryonic urachus. The educational value of this report lies in showing a pragmatic diagnostic and surgical pathway for a symptomatic infant in a resource-limited setting where ultrasonography, fistulography, and MRI were unavailable.
CASE PRESENTATION: A 2-month-old male infant presented with a 3-week history of persistent umbilical discharge and intermittent fever. Examination showed a small patent umbilical opening with continuous seropurulent discharge and mild peri-umbilical

Abnormalities of the urachus detected in infancy and early childhood are often interpreted as persistent congenital abnormalities requiring surgical correction. However, growing clinical experience suggests that a proportion of these findings may reflect ontogenetic immaturity of the urachus rather than fixed pathological conditions. As a transient embryological structure, the urachus undergoes postnatal remodeling and involution, which may explain the spontaneous regression of urachal anomalies observed during follow-up. This paper proposes ontogenetic immaturity of the urachus as a biologica

Colourachal abnormalities are conditions that occur due to defective obliteration of urachus into the median umbilical ligament during the developmental period. The case of a middle-aged male who presented multiple times with recurrent abdominal pain, vomiting, and intermittent bloody loose stools is presented. On final admission, he developed signs of acute abdomen. Despite multiple visits, his workup remained inconclusive. Explorative laparotomy was performed which showed Colo-urachal fistula with infected urachus from which air was released upon incision and pus drained. The patient underwe

BACKGROUND: We report Bladder Turn-in & Epispadias Repair (TIER) as a first step of two-staged Bladder Exstrophy repair.
TECHNIQUE: Circumferential incision was made around the bladder and urethral plate. Penis was degloved and corporal bodies were mobilised. The epispadiac urethra and spongiosa were dissected off the corporal bodies leaving the frenulum and prepuce attached to hemi-glans. Perineal muscle was identified by muscle stimulator and wrapped around the bladder neck. Bladder was closed leaving reimplantation/bladder neck repair to second stage. The neourethra was tubularised and mea

Epispadias is a rare congenital disorder characterized by an abnormal dorsal opening of the urethra due to the failure of the urethral plate to tubularize. It is usually part of broader conditions like bladder or cloacal exstrophy and rarely occurs in isolation. We present the case of a 14-year-old girl with isolated epispadias who had lifelong urinary incontinence without urgency. Genital examination revealed a bifid clitoris, underdeveloped labia minora, and a widely open urethral meatus with continuous leakage of urine. Urethrocystoscopy showed a short, patulous urethra, a poorly coapting e

Isolated female epispadias without bladder exstrophy is a rare anomaly. We report a six-year-old girl who presented with urinary incontinence since birth and recurrent urinary tract infections. Examination revealed a bifid clitoris and a wide urethral opening. She underwent single-stage surgical repair via a perineal approach. Postoperatively, she achieved urinary continence with a good stream and could retain urine for 90-120 minutes. Female epispadias is often misdiagnosed as refractory enuresis or recurrent urinary tract infections. Early recognition and timely surgical correction significa

INTRODUCTION: Female epispadias is a rare congenital anomaly resulting from disrupted mesodermal migration during embryogenesis, leading to anterior urethral displacement, a wide bladder neck, and a bifid clitoris without bladder exstrophy. Diagnosis is clinical, and defects are classified according to the degree of meatal displacement.
CASE PRESENTATION: The first case involved a 5-month-old infant with a congenital abnormal urethral opening, dorsal urethral cleft, cephalad-displaced meatus, and a bifid clitoris, with normal imaging. She underwent urethroplasty without bladder neck reconstru

INTRODUCTION: Female epispadias represents an abnormal or incomplete fusion of the dorsal urethra. There can be varying degrees of urethral malformation with or without coexisting abnormalities of the bladder neck. Patients often have a bifid clitoris, depressed mons pubis, and ill-defined labia minora. Occurring in 1:400,000 live female births and is rarer than other Bladder Exstrophy-Epispadias Complex (BEEC) diagnoses, including isolated male epispadias. Like all pathologies within the BEEC, this condition is associated with varying degrees of widened pubic diastasis. This study sought to e

A 3.5-year-old, 5.2-kg mixed-breed dog was presented with persistent stranguria after being neutered due to dystocia 6 months ago. Plain abdominal radiographs revealed an intrapelvic soft-tissue attenuating mass that displaced the urinary bladder cranially and constricted the descending colon. Abdominal ultrasonography yielded inconclusive results. Computed tomography revealed the mass as a tubular, hypodense structure compressing the urethra and the descending colon. This finding was defined as hydrocolpos induced by an imperforate hymen, segmental vaginal aplasia, or a vestibulovaginal disor
