Challenges in umbilical pathology: far beyond the obvious-a case report.

BACKGROUND: Umbilical disorders in neonates are frequently benign, with umbilical granulomas, hernias, and transient inflammatory changes representing the majority of presentations. However, persistent or atypical umbilical findings may signal rare congenital anomalies originating from incomplete involution of the urachus or the omphalomesenteric duct. Early recognition of these conditions is crucial to avoid misdiagnosis, unnecessary antibiotic exposure, or delayed referral. CASE DESCRIPTION: We describe a 27-day-old term newborn, previously healthy, who initially presented with a single ves

Bladder Exstrophy in a Female Fetus: Ultrasound and Autopsy Findings.

Introduction: Bladder exstrophy (BE) is a congenital anomaly within the exstrophy-epispadias complex (EEC), characterized by a defect in the closure of the lower abdominal wall and bladder. Case report: A female fetus at 20 weeks + 2 days showed at ultrasound (US) a homogeneous echogenic mass with a lobulated surface, protruding from the anterior lower abdominal wall. The bladder was not visualized. Doppler US displayed umbilical arteries laterally to the mass. The kidneys appeared normal. Amniotic fluid was regular. Legal termination of pregnancy occurred at 21 weeks + 4 days. Autopsy grossly

The gene mutation pattern of urachal squamous cell carcinoma resembles that of gastrointestinal tumors: a rare case with favorable prognosis following surgery and adjuvant chemotherapy.

BACKGROUND: Urachal carcinoma (UrC) is a rare epithelial malignant tumor of the urinary system, with an age-standardized incidence of 0.32 per million population, accounting for 0.35–0.70% of all bladder tumors. Adenocarcinoma is the most common histological subtype of UrC, while squamous cell carcinoma is extremely rare, representing only 2% of all UrC cases, and is associated with a high mortality rate. Due to its occult onset and high malignancy, urachal squamous cell carcinoma (USC) is often diagnosed at an advanced stage, and current understanding of this disease remains limited. Herein,

Neighborhood Economic Deprivation, Hispanic Ethnic Enclaves, and Congenital Anomalies in Texas.

BACKGROUND: Congenital anomalies are common, costly, and potentially life-threatening. The impacts of structural determinants of health on risk for these conditions have scarcely been studied. We evaluated associations between residence in economically disadvantaged neighborhoods or Hispanic/Latino enclaves and congenital anomalies. METHODS: We performed a registry linkage study of 7.25 million births and 332,935 cases with anomalies in Texas, 1999-2018. We defined exposure groups using maternal residential address at delivery or termination and tract-level Yost socioeconomic index and Hispan

The space of Retzius: anatomy, imaging features, and patterns of disease spread.

The space of Retzius is a clinically important but often overlooked extraperitoneal compartment that serves as a common site of disease involvement and a key pathway for the spread of pelvic and retroperitoneal pathology. Its complex anatomical relationship with other extraperitoneal/retroperitoneal spaces renders it prone to involvement in different pathologic processes. Most cases represent secondary involvement rather than primary pathology. CT and MRI play a pivotal role in depicting the anatomy of the space of Retzius, characterizing lesions, and identifying routes of disease extension. T

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Duplicate bladder exstrophy in a female infant: A case report.

Duplicate bladder exstrophy, a rare variant of the bladder exstrophy-epispadias complex, was diagnosed in a 5-month-old female infant presenting with a 4 × 4 cm lower abdominal mass. Physical examination revealed absent umbilical scar, wide symphysis pubis, and anteriorly displaced genitalia and anus. Imaging ruled out classic bladder exstrophy and vesicoureteral reflux. Surgical exploration confirmed a suprapubic exstrophic mucosal plaque without communication to a functional bladder. Management involved excision of the exstrophic plaque and primary abdominal wall closure. Post-operative cour

Will my Child Walk Funny? The Rotational Profile of Infants and Children With Classic Bladder Exstrophy.

BACKGROUND: Bladder exstrophy is a rare, complex disorder that involves the urinary, reproductive, gastrointestinal, and musculoskeletal systems and is marked by an incomplete lower abdominal wall, leaving the bladder exposed to the outside world. Skeletal findings in this condition include externally rotated innominate bones. While the resulting acetabular retroversion might be expected to cause out-toeing, there is no published data on the gait parameters or rotational profile of children with classic bladder exstrophy (CBE) in isolation. This study, therefore, characterizes the rotational p

Umbilical morphometry and centile curves in boys aged 1-14 years: Establishing reference values for umbilical reconstruction.

UNLABELLED: Umbilicoplasty is essential for the reconstruction of congenital malformations such as bladder-exstrophy-epispadias complex and gastroschisis. Reference values for umbilical morphometry are critical for anatomically acceptable outcomes. OBJECTIVE: To measure vertical height and width of the umbilicus in male participants aged 1-14 years and generate age-specific centile curves for umbilical reconstruction. METHODS: This retrospective study analyzed computed tomography data from 887 male participants aged 1-14 years; those with previous abdominal surgeries or pathologies were excl

Bladder exstrophy: navigating long-term outcomes.

Bladder exstrophy (BE) is a rare congenital malformation of the genitourinary tract, as well as surrounding structures, within the spectrum of the exstrophy-epispadias complex. This review aims to summarize the current literature on repair procedures, specifically the modern staged repair and complete primary reconstruction, along with their associated complications and complication rates. A particular focus is placed on long-term management strategies to address these complications as patients transition into adulthood. Additionally, this review highlights the management of sexual health for

Isolated subcoronal epispadias managed with dorsal Thiersch-Duplay urethroplasty: A case-based insight into technique selection and outcomes.

Epispadias is a rare congenital anomaly with an abnormal dorsal urethral opening. Isolated subcoronal cases are uncommon and present specific surgical challenges. We report a 15-year-old male with isolated subcoronal epispadias presenting with an upward urinary stream since birth. He underwent dorsal Thiersch-Duplay urethroplasty without intraoperative complications. Postoperatively, vital signs remained stable, wound healing was satisfactory, and urinary stream improved. No leakage, infection, or incontinence was noted during follow-up. This case highlights dorsal Thiersch-Duplay urethroplast

Concurrent Sagittal Craniosynostosis and Anterior Fontanelle Wormian Bone: Radiological Findings and Surgical Management in an Infant With Epispadias.

Wormian bones are accessory ossicles that typically occur within cranial sutures. Their presence within fontanelles, particularly concurrent with craniosynostosis, represents an uncommon finding that poses unique diagnostic and management considerations. We present a one-month-old male infant born at 35 weeks of gestation, who presented with an abnormal skull configuration. Physical examination revealed scaphocephaly with a palpable sagittal ridge, bilateral temporal hollowing, and concurrent distal shaft epispadias. Computed tomography (CT) with three-dimensional (3D) reconstruction demonstra
