[Pregnancy complicated by patent urachus: A case report].

To improve clinicians' understanding, and to explore the diagnosis and treatment of pregnancy complicated with patent urachus. The clinical symptoms, ultrasound images, and delivery outcome of the pregnancy complicated with patent urachus were reported, and the literature was reviewed. The patient had umbilical leakage as a young child occasionally, the symptom of leakage was not obvious after she was 10 years old, and there was no asymptomatic before pregnancy and in the first and second trimesters. Umbilical leakage was present at gestational 33 weeks, and ultrasound showned a tubular struct

Patent Urachus and Associated Comorbidities in 101 Newborn Foals: A Retrospective Study.

BACKGROUND: Patent urachus is a common disease in newborn foals. However, despite its frequency, studies investigating symptoms, comorbidities, treatment and prognosis in a large number of affected foals have rarely been conducted. OBJECTIVE: This study aimed to describe the clinical symptoms, laboratory diagnostic findings and prognosis of foals with patent urachus. METHOD: Data from 101 foals with a patent urachus from 2006 to 2017 were analysed. RESULTS: Patent urachus occurred at a frequency of 7.8% of all foals up to the 14th day of life during the study period. More colts (74.3%) than

Imaging of the umbilicus.

The umbilicus is a scar of the umbilical cord containing various embryonic remnants that can lead to postnatal diseases. It is the collection point for blood and lymphatic flow and other structures. Hence, various diseases and conditions can occur. In this review, we aimed to categorize umbilical diseases based on the embryonic remnants, including urachal remnants, omphalomesenteric duct anomalies, and complications of the umbilical arteries and veins. The urachus, formed from the allantois, can result in anomalies such as patent urachus and urachal cysts, with infections being the most common

From prenatal detection to postnatal evaluation: a retrospective observational ultrasound study of patent urachus with allantoic cyst.

BACKGROUND: Patent urachus with allantoic cyst (PUAC) is a rare abnormal embryonic development characterized by a prenatal umbilical mass and postpartum bladder prolapse. The current literature is mainly case reports, and there is a lack of consecutively observational studies of this disease. OBJECTIVES: This study aims to investigate the impact of the disease on fetal development through a retrospective analysis of prenatal ultrasound progression, pregnancy outcomes, and postnatal follow-up, thereby establishing an evidence-based foundation for prenatal clinical recommendations. Solitary Plexiform Neurofibroma in the Urachus Associated With von Recklinghausen's Disease.

Neurofibromatosis type 1 (NF1), also known as von Recklinghausen's disease, is an autosomal dominant disorder characterized by multiple café-au-lait macules and cutaneous neurofibromas. Although neurofibromas are common in NF1, involvement of the urinary tract is rare, with the bladder being the most frequently affected site. Urachal neurofibromas are extremely rare, and their diagnosis and management remain challenging due to nonspecific imaging characteristics and their often asymptomatic presentation. We report the case of a 30-year-old man with clinical features of NF1 in whom a 5-cm mass

Urachus Remnant: Importance of Early Diagnosis in Preventing Complications.

The urachus remnant is a vestigial structure that results from incomplete involution of the cloaca and allantois, forming a duct between the bladder and the umbilical scar. Normally, it obliterates to form the middle umbilical ligament. Despite its rarity and often nonspecific symptoms, it can be incidentally discovered during imaging tests or due to complications, such as infections or neoplasms. The first diagnostic test usually ordered is an ultrasound, followed by computed tomography or magnetic resonance imaging for a more detailed evaluation. Early diagnosis and appropriate management, w

Suprapubic Open and Single-Port Laparoscopic Surgery for Urachal Remnant in Children: A Case Series.

INTRODUCTION: A narrow operative field, difficult dissection of an inflamed urachal remnant (UR) and possible incomplete resection of UR at the bladder dome may be serious problems in laparoscopic surgery with umbilical and extraperitoneal approaches. To overcome these problems, we recently introduced suprapubic open and single-port laparoscopic surgery (SOSLS) for UR in children. MATERIALS AND SURGICAL TECHNIQUE: The root of UR was resected and the bladder wall was closed under direct vision through a 3 cm suprapubic transverse incision. A multichannel port with three trocars was applied to

Epidemiology and treatment patterns of urachal remnants in adult patients with omphalitis: a nationwide claims-based study in Japan.

PURPOSE: Urachal remnants are common in neonates and can persist into adulthood. However, their epidemiology in adults remains poorly characterized. We aimed to investigate the prevalence and clinical features of urachal remnants in adult Japanese patients with omphalitis using a nationwide claims database. METHODS: We analyzed data from the Japan Medical Data Center Claims Database (2005-2023) to identify patients  ≥ 15 years of age who were diagnosed with omphalitis. The prevalence of urachal remnants, associated diagnoses, surgical interventions, and time to surgery was examined, focusing

Diagnostic Challenges and Management of Urachal Malformations in an Infant.

BACKGROUND: The urachus is a vestigial remnant of the allantois which naturally degenerates after birth. Failure of closure of the urachus can result in urachal malformations, with the most common being a urachal cyst. Diagnosing a urachal cyst is challenging due to its diverse presentations, often leading to misdiagnosis. Although they are often asymptomatic, urachal cysts can be significant because they can lead to complications. CASE REPORT: We report a late preterm male infant born at 36 weeks + 5 days, in whom an antenatal anomaly scan revealed a unilocular intra-abdominal cyst adjacent

Glans ischemia in a 7-month-old boy who underwent correction of megaprepuce: case report and review of the literature.

Acute glans ischemia is a severe complication that can occur after penile surgery, such as circumcision, hypospadias repair, or disassembly of the corpora in epispadias. This rare condition is described in the current literature, especially after circumcision, although it is the less invasive procedure among those previously mentioned. We present a case of 7-month-old boy who developed glans ischemia after correction of megaprepuce. The condition was treated by intravenous unfractionated heparin. In our case, the complication of ischemia was complete dehiscence of the suture line at the corona

Modified anatomical repair of epispadias (MARE)-with preservation of glans-frenular unit during urethral disassembly.

BACKGROUND: In this video we report Modified Anatomical Repair of Epispadias (MARE)- where glans-frenular unit is preserved during urethral disassembly of Mitchell's technique. METHODS/TECHNIQUE: The epispadiac urethra and spongiosa were completely dissected off the corporal bodies; distally the urethra was dissected off, but the attachment at the frenulum and prepuce to hemi-glans was retained. The neourethra was tubularised using 7-0 Polydiaxone (PDS) over an 8Fr silastic catheter; neo-metaus was fashioned in a ventral position in the glans. The corporal bodies were approximated dorsally ve

Penopubic epispadias with intact prepuce presenting as phimosis in an infant: A case report.

Isolated epispadia is uncommon anamoly, occurring in approximately 1 out of 120,000 male births. To date, only 22 cases of epispadia have been reported with an intact prepuce and 8 cases with a phimotic intact prepuce. This paper presents the case of a 6-month-old male infant, who had been circumcised at three weeks of age, and was subsequently diagnosed with penopubic epispadias during a standard medical examination. His condition was successfully corrected using the Modified Cantwell-Ransley technique, leading to a favorable post-operative recovery. This case underscores the importance of th

Anterior urethral hamartoma in a female infant with anorectal malformation: A rare case report and literature review.

INTRODUCTION AND IMPORTANCE: Urethral hamartoma is highly unusual, as documented in the literature with only one case that ever reported. Here, we report a case of female infant with urethral hamartoma and anorectal malformation. CASE PRESENTATION: An 8 months old female infant was previously diagnosed with anorectal malformation (ARM) with rectovestibular fistula that had underwent colostomy. She presented with a vestibular mass protruding from her vestibule 4 months prior. Preoperative voiding cystourethrography (VCUG) and intraoperative cystoscopy supported that the mass did not originate

Hypospadias and Epispadias.

Hepatitis C virus-associated HCC (HCV-HCC) is a prevalent malignancy worldwide and the molecular mechanisms are still elusive. Here, we screened 240 differentially expressed genes (DEGs) of HCV-HCC from Gene expression omnibus (GEO) and the Cancer Genome Atlas (TCGA), followed by weighted gene coexpression network analysis (WGCNA) to identify the most significant module correlated with the overall survival. 10 hub genes (CCNB1, AURKA, TOP2A, NEK2, CENPF, NUF2, CDKN3, PRC1, ASPM, RACGAP1) were identified by four approaches (Protein-protein interaction networks of the DEGs and of the significant

Gender bias in machine learning: insights from official labour statistics and textual analysis.

Streptococcus pneumoniae infection may result in asymptomatic carriage, mucosal or invasive disease. We hypothesize that self-limiting or fatal disease outcome follows infection with S. pneumoniae differential activation of the host immune response. BALB/c and C57BL/6 mice were inoculated intranasally with S. pneumoniae serotype 3 strain WU2 and serotype 14 strain DW14 and mortality, bacterial load, pathological changes in the lungs and cytokines mRNA levels in the spleen were analysed. No differences between the C57BL/6 and the BALB/c inbred mice were observed except for the severity of their

Concurrent Congenital Umbilicobiliary Fistula and Vesicourachal Diverticula in a Dog.

Congenital umbilicobiliary fistula is a rare disease reported in humans and dogs. A 2-month-old, intact, male French Bulldog presented with a greenish-yellow discharge dripping from the umbilicus since birth. Complete blood count and serum biochemistry were within normal limits, but serum alkaline phosphatase activity was mildly elevated. A positive contrast cystogram was performed to rule out a patent urachus and confirmed the presence of a vesicourachal diverticula, a type of urachal anomalies. An abdominal ultrasound and computed tomography fistulogram demonstrated a communication between t

Medial Pterygoid Abscess Masquerading as a Temporomandibular Joint Disorder: A Case Report.

OBJECTIVE: To review the long-term results of bladder neck reconstruction (BNR) in patients with classical bladder exstrophy or epispadias, and to review the concept of continence surgery in these two groups, stressing the difficulty in finding an adequate balance between urine storage (which implies high outlet resistance and low storage pressure) and complete bladder emptying (which implies low outlet resistance and a transient increase in bladder pressure); surgery cannot achieve 'continence' (which implies active mechanisms) but only 'dryness' (which implies passive mechanisms). Perioperative management of primary classic bladder exstrophy: A single institutional pathway to success.

PURPOSE: Appropriate perioperative management is crucial in patients undergoing classic bladder exstrophy closure (CBE). Therefore, the authors sought to review their intra and postoperative management of patients with CBE undergoing primary closure and examine the impact of this pathway on patient outcomes. METHOD: A prospectively maintained institutional approved exstrophy-epispadias complex database was reviewed for patients with CBE who had undergone primary closure between 2016 and 2022 and whose closure was performed within one year of age. Innervation pattern of the unclosed detrusor muscle in classic bladder exstrophy: a study of patients with urothelial overexpression of nerve growth factor.

PURPOSE: An overexpression of nerve growth factor (NGF) in the urothelium is discussed to lead to neuronal hyperinnervation of the bladder detrusor. The aim was to assess the sensory and sympathetic innervation of the detrusor in unclosed exstrophic bladders patients with known overexpression of NGF in the urothelium. METHODS: Full-thickness bladder biopsies were prospectively obtained from 34 infants at delayed primary bladder closure between 01/2015 and 04/2020. The bladder biopsies were immunohistochemically stained with antibodies against S100, calcitonin gene-related peptide (anti-CGRP),
