Auto-regulated bioprosthetic total artificial heart as bridge to transplantation: the EFICAS study.

BACKGROUND AND AIMS: Refractory advanced biventricular heart failure (RAHF) has a poor prognosis, and many patients are ineligible for left ventricular assist device (LVAD) support. The "historical" total artificial heart (TAH) is associated with high rates of stroke, bleeding, and haemolysis. The bioprosthetic TAH (bio-TAH), a pulsatile, autoregulated, and haemocompatible device, was assessed here as a bridge to transplantation. METHODS: EFICAS (NCT04475393) was a prospective, multicentre trial conducted at 10 French centres. Adults with RAHF who were ineligible for LVAD support and listed o

SBRT Plus Nivolumab in Advanced NSCLC: A Phase II Multicenter Study With Exploratory Bone Metastases Subgroup Analysis.

BACKGROUND: Immune checkpoint blockade provides durable benefit in a subset of patients with advanced non-small-cell lung cancer (NSCLC), but response rates remain limited after progression on standard systemic therapy. Stereotactic body radiotherapy (SBRT) may enhance tumor-antigen release and systemic immune activation, thereby improving the efficacy of PD-1 blockade. This multicenter, single-arm, phase II study evaluated the efficacy and safety of SBRT plus nivolumab in previously treated advanced NSCLC. METHODS: Eighty-three patients who had experienced disease progression after at least

Beyond Exons 19 and 21: mapping the clinical and molecular diversity of uncommon EGFR mutations in a Brazilian NSCLC cohort.

BACKGROUND: Uncommon and complex EGFR variants respond variably to tyrosine kinase inhibitors, and data on their frequency in Brazil are scarce. METHODS: We retrospectively analysed 141 patients with EGFR-mutated lung adenocarcinoma at a single Northeastern Brazilian centre. All profiling used treatment-naive diagnostic specimens: next-generation sequencing (NGS) in 111 patients (78.7%) and rapid PCR or single-gene assay in 30 (21.3%). Concurrent alterations and PD-L1 were assessed where panels covered those genes. Overall survival was estimated by Kaplan-Meier analysis and log-rank test. RE

p53 Co-mutations in EGFR-mutant NSCLC: biological insight and therapeutic implications.

Non-small cell lung cancer (NSCLC) harbouring mutations in the epidermal growth factor receptor (EGFR) represents a well-defined molecular subset that is amenable to targeted therapies. However, clinical heterogeneity in treatment response and resistance highlights the critical need for a deeper understanding of co-occurring genetic alterations. Among these, TP53 mutations are the most frequent co-mutations observed in EGFR-mutant NSCLC and are increasingly recognized for their prognostic and predictive relevance. With the emergence of next-generation sequencing in routine diagnostics and the

Generation of an induced pluripotent stem cell line, JHUi010-A, from an arrhythmogenic cardiomyopathy patient harboring pathogenic DSG2 variant c.2358delA.

We generated a human induced pluripotent stem cell (hiPSC) clonal line, JHUi010-A, from a 48-year-old female diagnosed with arrhythmogenic cardiomyopathy (ACM) carrying a heterozygous DSG2 c.2358delA variant using Sendai virus vectors expressing the Yamanaka factors. The line exhibits a normal karyotype, expresses stemness markers, and demonstrates trilineage differentiation capacity. Mycoplasma testing and short tandem repeat (STR) authentication confirmed line quality. Cardiomyocytes derived from JHUi010-A show reduced DSG2 localization, confirming the utility of this patient-specific model

Eosinophil biology in COPD: from scientific insight to clinical treatment action.

COPD is a common lung disease that causes significant global morbidity. Strikingly, elevated blood and/or airway eosinophil levels are observed in up to 40% of COPD patients. These innate immune cells are clinically associated with increased exacerbation frequency and risk. Eosinophils are key in COPD symptom management and a drive to uncover their related immunopathology is critical to our understanding of disease progression. Recent biologic treatments targeting eosinophils have had success in reducing symptoms in eosinophilic COPD patients. However, the underlying pathways that lead to thes

Spermidine attenuates monocrotaline-induced pulmonary arterial hypertension with HADHA-associated improvements in fatty acid oxidation and mitochondrial redox homeostasis.

Metabolic reprogramming, impaired mitochondrial quality control, and mitochondrial redox imbalance are increasingly recognized as central drivers of pulmonary arterial hypertension (PAH). Spermidine (SPD) has been reported to exert cardiovascular protective effects; however, its effects on endothelial metabolism and redox homeostasis in PAH remain incompletely characterized. HADHA, the α-subunit of mitochondrial trifunctional protein, participates in long-chain fatty acid β-oxidation (FAO) and provides a potential link between fatty acid utilization, mitochondrial respiration, mitophagy-relate

CT-defined pulmonary emphysema severity independently predicts long-term outcomes after curative gastrectomy for gastric cancer.

BACKGROUND: Respiratory dysfunction has been reported as a prognostic factor in gastric cancer. In contrast, pulmonary emphysema evaluated by CT may reflect systemic vulnerability beyond airflow limitation and has been implicated in cancer-related outcomes; however, its prognostic relevance in gastric cancer remains unclear. METHODS: This study included patients with pathological stage I-III gastric cancer who underwent curative gastrectomy between July 2014 and August 2025. Pulmonary emphysema was assessed on preoperative chest CT using the Goddard scoring system. Prognostic factors for dise

Markers of Autonomic Function and Cancer Mortality Risk in Healthy Adults.

BACKGROUND: Dysfunction of the autonomic nervous system increases cardiovascular risk, but its contribution to cancer outcomes is less well-characterized. OBJECTIVES: The aim of this study was to evaluate associations between physiological measures reflective of autonomic function and all-site and site-specific cancer mortality in healthy adults. METHODS: The study included 75,308 participants (26.8% women) in the Cooper Center Longitudinal Study who completed a preventive medicine examination with a treadmill test between 1971 and 2018. Gompertz mortality models estimated associations betwe

BACKGROUND: The comprehensive relation between air pollutants and venous thrombosis is currently not available. METHODS AND RESULTS: The Web of Science, PubMed, Scopus, and Embase databases were searched up to July 1, 2025, for population-based studies of the associations of air pollutants with venous thrombotic events, including deep vein thrombosis (DVT), pulmonary embolism (PE), or both (venous thromboembolism [VTE]). The relative risks (RRs) and corresponding 95% confidence intervals (95% CIs) were derived using a random-effects meta-analysis. A total of 23 studies were identified from an

Diagnosis and management of subsegmental pulmonary embolism.

Subsegmental pulmonary embolism (SSPE) accounts for 5-10% of PE diagnoses, but the optimal diagnostic and management strategies, as well as the long-term outcomes remain debatable. Recent studies have provided more insights into outcomes in this population, and a randomized controlled trial is ongoing to evaluate the role of anticoagulation in patients with isolated SSPE. In this article, we use cases to illustrate the challenges in diagnosis and to discuss management approaches. We aim to provide evidence-based recommendations whenever possible and point out potential areas for future researc

Days Alive and Out of Hospital Around Lung Transplantation: Predictors and Clinical Implications.

BACKGROUND: Days alive and out of hospital (DAOH) is a validated patient-centered outcome that remains unexplored in lung transplantation (LT). It assesses the proportion of days a patient spends alive and outside of hospital. Quantifying DAOH trajectories around LT could provide patients with realistic expectations and better prepare them for the demanding post-transplant period. MATERIALS AND METHODS: DAOH was calculated for the year before (DAOH-BF) and two years after first LT (DAOH-AF1, DAOH-AF2) for all patients transplanted in France between 2020 and 2023. Clinical variables were colle
