Bladder exstrophy (BE) is a rare congenital malformation of the genitourinary tract, as well as surrounding structures, within the spectrum of the exstrophy-epispadias complex. This review aims to summarize the current literature on repair procedures, specifically the modern staged repair and complete primary reconstruction, along with their associated complications and complication rates. A particular focus is placed on long-term management strategies to address these complications as patients transition into adulthood. Additionally, this review highlights the management of sexual health for

INTRODUCTION & IMPORTANCE: Bladder exstrophy epispadias complex (BEEC) is a rare congenital midline malformation defect which requires complex, staged reconstruction of the bony pelvis and the genitourinary tract.
PRESENTATION OF CASE: A 26-year-old G3SA2 with previously reconstructed bladder exstrophy presented at 16 weeks +5 days gestation with severe bilateral hydronephrosis and inability to self-catheterize her Mitrofanoff diversion. She had bilateral nephrostomy tubes placed for the duration of pregnancy. Her pregnancy course was further complicated by recurrent pyelonephritis and progre

We report a highly unusual case of penile duplication (diphallia) accompanied by epispadias, neurogenic bladder with markedly reduced compliance, and lumbar vertebral fusion. Comprehensive clinical, imaging, and urodynamic examinations delineated complex genitourinary and spinal anomalies necessitating multidisciplinary management. The discussion reviews embryological implications, compares existing literature, and addresses long-term follow-up considerations. This case underscores the clinical and academic importance of individualized evaluation and tailored intervention for exceedingly rare

Epispadias is a rare congenital anomaly with an abnormal dorsal urethral opening. Isolated subcoronal cases are uncommon and present specific surgical challenges. We report a 15-year-old male with isolated subcoronal epispadias presenting with an upward urinary stream since birth. He underwent dorsal Thiersch-Duplay urethroplasty without intraoperative complications. Postoperatively, vital signs remained stable, wound healing was satisfactory, and urinary stream improved. No leakage, infection, or incontinence was noted during follow-up. This case highlights dorsal Thiersch-Duplay urethroplast

PURPOSE OF REVIEW: This review synthesizes the recent literature on lower urinary tract symptoms in pediatric patients with congenital conditions. We will discuss emerging insights from multiple studies on pathophysiology, prevalence, clinical impact, management strategies, and long-term outcomes.
RECENT FINDINGS: Surgical advancements have increased early continence rates to 80% in exstrophy-epispadias complex (EEC), but attrition of continence remains a challenge. Studies have shown 40% of males with PUV and 80% of patients with EEC report moderate or severe LUTS which significantly impact

Wormian bones are accessory ossicles that typically occur within cranial sutures. Their presence within fontanelles, particularly concurrent with craniosynostosis, represents an uncommon finding that poses unique diagnostic and management considerations. We present a one-month-old male infant born at 35 weeks of gestation, who presented with an abnormal skull configuration. Physical examination revealed scaphocephaly with a palpable sagittal ridge, bilateral temporal hollowing, and concurrent distal shaft epispadias. Computed tomography (CT) with three-dimensional (3D) reconstruction demonstra

PURPOSE: This review aims to evaluate the role of congenital urinary tract anomalies in the pathogenesis of pediatric nephrolithiasis. By examining specific structural abnormalities such as ureteropelvic junction obstruction (UPJO), horseshoe kidney, ectopic kidney, exstrophy-epispadias complex, and ureterocele, we aim to elucidate how these anomalies contribute to urinary stasis, infection, metabolic disturbances, and ultimately, stone formation in children.
METHODS: A narrative literature review was conducted focusing on congenital anomalies of the urinary tract and their association with p

INTRODUCTION: Cloacal Exstrophy (CE), also known as OEIS complex (Omphalocele, Exstrophy of the cloaca, Imperforate anus, and Spinal defects), is the rarest and most severe congenital malformation of the bladder exstrophy-epispadias spectrum, characterized by an open bladder and exposed portions of the large intestine, along with bony anomalies. The objective of this study is to describe the experience of a high-volume exstrophy center's perioperative pathway, with a focus on analgesic use as well as urologic, orthopedic, and dietary interventions, for bladder repair of cloacal exstrophy.

INTRODUCTION: Bladder exstrophy is an embryologic malformation characterised by a deficiency of the anterior midline, resulting in urogenital and skeletal complications. The aim of this article is to identify the factors influencing outcomes in patients with bladder exstrophy, a condition for which published literature remains limited.
MATERIAL AND METHODS: A retrospective cohort study analysed medical records from 2011 to 2017. Patients with bladder exstrophy who underwent anterior and posterior innominate osteotomy were evaluated for clinical outcomes in an orthopaedic outpatient clinic. An

BACKGROUND: Reconstruction of the genitalia in patients with classic bladder exstrophy (CBE) is essential to support satisfactory sexual function and fertility. However, due to the complexity of both urinary and genital reconstructive procedures, long-term sexual function may be significantly impaired in adulthood.
OBJECTIVES: To assess long-term sexual outcomes and the prevalence of sexual and erectile dysfunctions in patients with classic bladder exstrophy (CBE). Secondary objectives were to identify predictive factors of sexual dysfunction and describe fertility outcomes.

A 17-year-old male and a 30-year-old female with bladder exstrophy-epispadias complex and failed primary repair during infancy presented with continuous urinary leakage and an open bladder plate. Considering their age and desire to avoid a stoma and catheterization, a Mainz II continent urinary diversion was performed with robotic assistance. The ureters were implanted into an S-shaped, low-pressure, high-capacity sigmoid pouch using an extramural serosal tunnel technique to create an anti-refluxing anastomosis. At 3-month follow-up, both patients were completely continent, able to hold urine

INTRODUCTION: Bladder exstrophy-epispadias complex (BEEC) is a devastating congenital anomaly of the urinary tract and is associated with an increased risk of bladder cancer. The etiology of the BEEC is unknown, but a clear genetic component has been highlighted. Currently, all genetic studies suffer from small populations, limited to a European background.
OBJECTIVES: To identify copy number variations (CNVs) in a South Asian and North American bladder exstrophy cohorts. To identify novel non-European CNVs to expand the literature beyond its European ethnic background.
STUDY DESIGN: Patient

PURPOSE: To describe the presentation and outcome of surgical management strategies applied to the adolescent and adult patient population (age >14 years) with bladder exstrophy/epispadias complex (BEEC) who presented with persistent incontinence even after previous numerous surgeries at our institution.
METHODS: A retrospective review of the electronic medical records of patients with BEEC managed from January 1998 to December 2022 was undertaken. Patients aged >14 years at presentation, with detailed medical records were selected. Of these, we identified patients who underwent surgical inte

OBJECTIVE: To determine the age at primary bladder exstrophy (BE) closure from 2012 to 2023. We hypothesized that the median age at primary BE closure increased from within the first week of life to more delayed closure.
METHODS: A retrospective, cross-sectional analysis was performed using the Epic Cosmos database. BE patients were identified using a combination of diagnostic and procedure codes. The use and timing of pelvic osteotomies, epispadias repairs, postoperative complications, and length of stay were also noted. The primary outcome was the age at the primary BE closure. Delayed clos

INTRODUCTION: Urinary continence is a goal of bladder exstrophy epispadias complex (BEEC) surgical management. However, it is often not achieved until school age or later. Little is known about the mental health implications of this incontinence on caregivers and whether this should affect timing of surgical interventions to achieve continence. We hypothesized that caregivers of children with continuous incontinence or dry intervals <1 h per day/night exhibit adverse mental health outcomes.
OBJECTIVE: To evaluate the relationship between urinary incontinence of children with BEEC and caregive
