Glans ischemia in a 7-month-old boy who underwent correction of megaprepuce: case report and review of the literature.

Acute glans ischemia is a severe complication that can occur after penile surgery, such as circumcision, hypospadias repair, or disassembly of the corpora in epispadias. This rare condition is described in the current literature, especially after circumcision, although it is the less invasive procedure among those previously mentioned. We present a case of 7-month-old boy who developed glans ischemia after correction of megaprepuce. The condition was treated by intravenous unfractionated heparin. In our case, the complication of ischemia was complete dehiscence of the suture line at the corona

Modified anatomical repair of epispadias (MARE)-with preservation of glans-frenular unit during urethral disassembly.

BACKGROUND: In this video we report Modified Anatomical Repair of Epispadias (MARE)- where glans-frenular unit is preserved during urethral disassembly of Mitchell's technique. METHODS/TECHNIQUE: The epispadiac urethra and spongiosa were completely dissected off the corporal bodies; distally the urethra was dissected off, but the attachment at the frenulum and prepuce to hemi-glans was retained. The neourethra was tubularised using 7-0 Polydiaxone (PDS) over an 8Fr silastic catheter; neo-metaus was fashioned in a ventral position in the glans. The corporal bodies were approximated dorsally ve

Penopubic epispadias with intact prepuce presenting as phimosis in an infant: A case report.

Isolated epispadia is uncommon anamoly, occurring in approximately 1 out of 120,000 male births. To date, only 22 cases of epispadia have been reported with an intact prepuce and 8 cases with a phimotic intact prepuce. This paper presents the case of a 6-month-old male infant, who had been circumcised at three weeks of age, and was subsequently diagnosed with penopubic epispadias during a standard medical examination. His condition was successfully corrected using the Modified Cantwell-Ransley technique, leading to a favorable post-operative recovery. This case underscores the importance of th

Hypospadias and Epispadias.

Hepatitis C virus-associated HCC (HCV-HCC) is a prevalent malignancy worldwide and the molecular mechanisms are still elusive. Here, we screened 240 differentially expressed genes (DEGs) of HCV-HCC from Gene expression omnibus (GEO) and the Cancer Genome Atlas (TCGA), followed by weighted gene coexpression network analysis (WGCNA) to identify the most significant module correlated with the overall survival. 10 hub genes (CCNB1, AURKA, TOP2A, NEK2, CENPF, NUF2, CDKN3, PRC1, ASPM, RACGAP1) were identified by four approaches (Protein-protein interaction networks of the DEGs and of the significant

Medial Pterygoid Abscess Masquerading as a Temporomandibular Joint Disorder: A Case Report.

OBJECTIVE: To review the long-term results of bladder neck reconstruction (BNR) in patients with classical bladder exstrophy or epispadias, and to review the concept of continence surgery in these two groups, stressing the difficulty in finding an adequate balance between urine storage (which implies high outlet resistance and low storage pressure) and complete bladder emptying (which implies low outlet resistance and a transient increase in bladder pressure); surgery cannot achieve 'continence' (which implies active mechanisms) but only 'dryness' (which implies passive mechanisms). PATIENTS

Perioperative management of primary classic bladder exstrophy: A single institutional pathway to success.

PURPOSE: Appropriate perioperative management is crucial in patients undergoing classic bladder exstrophy closure (CBE). Therefore, the authors sought to review their intra and postoperative management of patients with CBE undergoing primary closure and examine the impact of this pathway on patient outcomes. METHOD: A prospectively maintained institutional approved exstrophy-epispadias complex database was reviewed for patients with CBE who had undergone primary closure between 2016 and 2022 and whose closure was performed within one year of age. Electronic medical records for eligible patien

Innervation pattern of the unclosed detrusor muscle in classic bladder exstrophy: a study of patients with urothelial overexpression of nerve growth factor.

PURPOSE: An overexpression of nerve growth factor (NGF) in the urothelium is discussed to lead to neuronal hyperinnervation of the bladder detrusor. The aim was to assess the sensory and sympathetic innervation of the detrusor in unclosed exstrophic bladders patients with known overexpression of NGF in the urothelium. METHODS: Full-thickness bladder biopsies were prospectively obtained from 34 infants at delayed primary bladder closure between 01/2015 and 04/2020. The bladder biopsies were immunohistochemically stained with antibodies against S100, calcitonin gene-related peptide (anti-CGRP),

Palliative Care for Pediatric Urology.

Palliative care in the field of urology has largely been limited to adult oncologic conditions. Although there is a plethora of established literature suggesting the advantageous impact of palliative care, there is limited integration of palliative care in adult urology. This underutilization is further exacerbated in pediatric urology, and palliative care in pediatric urology remains an underexplored area despite the prevalence of several life-limiting conditions in this patient population. This paper highlights the potential need for palliative care intervention in a variety of urologic cond

Unusual morphology of isolated male epispadia: A rare case report.

In less than 10% of cases, males may have isolated epispadias, which is caused by failure in the urethral tubularization process, leading to dorsal urethral defect. This case report presents a unique instance where epispadias was associated with ambiguous genitalia. A 5-year-old boy diagnosed with epispadias. The penis resembled external female genitalia, with scrotal skin covering it. He underwent a two-stage operation without complications. The aim of the surgical techniques is to correct these anomalies and restore urinary continence and sexual function. Long-term outcomes of the surgery ca

Multi-Institutional Bladder Exstrophy Consortium After 8 Years: The Short- and Intermediate-Term Outcomes.

PURPOSE: Bladder exstrophy (BE) poses challenges both during the surgical repair and throughout follow-up. In 2013, a multi-institutional BE consortium was initiated, which included utilization of unified surgical principles for the complete primary repair of exstrophy (CPRE), real-time coaching, ongoing video capture and review of video footage, prospective data collection, and routine patient data analysis, with the goal of optimizing the surgical procedure to minimize devastating complications such as glans ischemia and bladder dehiscence while maximizing the rate of volitional voiding with

Updates on the Care of Cloacal Exstrophy.

Cloacal exstrophy is the most severe congenital anomaly of the exstrophy-epispadias complex and is characterized by gastrointestinal, genitourinary, neurospinal, and musculoskeletal malformations. Individualized surgical reconstruction by a multidisciplinary team is required for these complex patients. Not infrequently, patients need staged surgical procedures throughout childhood and adolescence. Following significant improvements in medical care and surgical reconstructive techniques, nearly all patients with cloacal exstrophy now survive, leading to an increased emphasis on quality of life.

Urethral duplication associated with epispadias: Case report.

INTRODUCTION AND IMPORTANCE: Urethral duplication is a rare congenital urinary tract anomaly. Its association with male epispadias is extremely rare. Due to various anatomical types, patients with urethral duplication can have a wide range of clinical presentations. Managing epispadias associated with urethral duplication involves different techniques and one should identify the functional urethra before proceeding with the surgery. It is associated with better functional outcomes than bladder epispadias exstrophy. CASE PRESENTATION: We report a case of a 4-year-old male child presented with

EZH2 specifically regulates ISL1 during embryonic urinary tract formation.

Isl1 has been described as an embryonic master control gene expressed in the pericloacal mesenchyme. Deletion of Isl1 from the genital mesenchyme in mice leads to an ectopic urethral opening and epispadias-like phenotype. Using genome wide association methods, we identified ISL1 as the key susceptibility gene for classic bladder exstrophy (CBE), comprising epispadias and exstrophy of the urinary bladder. The most significant marker (rs6874700) identified in our recent GWAS meta-analysis achieved a p value of 1.48 × 10- 24 within the ISL1 region. In silico analysis of rs6874700 and all other ge

Optimizing cesarean delivery in a patient with exstrophy-epispadias via prenatal MRI and surgical planning: A case report.

Due to advancements in medical care and reconstructive surgery, more patients with bladder exstrophy are reaching reproductive age and considering pregnancy. Complications are more common during pregnancy in this patient population, given medical comorbidities and prior surgical intervention. In the case of cesarean delivery, careful planning is necessary to prevent inadvertent injury to the bladder or bowel segments. This report concerns a patient with a history of infant closure of bladder exstrophy, followed by two bladder augmentations and a catheterizable channel who presented for care at

Pathophysiology of Congenital Anomalies of the Kidney and Urinary Tract: A Comprehensive Review.

Congenital anomalies of the kidney and urinary tract (CAKUT) represent a broad range of diseases with differing mechanisms, clinical presentations, and prognoses. With an estimated prevalence of between 4 and 60 per 10,000 births, CAKUT represents a sizable number of patients for pediatric and adult nephrologists as therapies have progressed, allowing longer life spans. Many CAKUT disorders are associated with genetic mutations, and with advances in genomic sequencing, these genes are being identified at an increasing rate. Understanding these mutations provides insight into these conditions'

Bladder exstrophy-epispadias complex: The effect of urotherapy on incontinence.

INTRODUCTION: Achieving urinary continence is a key goal in children born with the bladder exstrophy-epispadias complex (BEEC). Unfortunately, this goal is only moderately achieved despite sometimes extensive surgical treatment. Undergoing repeated hospitalization and operations may consequently have a negative impact on quality of life. We therefore believe that other, conservative treatment options should be explored in an earlier stage of incontinence treatment in BEEC patients. As part of this, an intensive urotherapy program based on was offered to patients with persistent incontinence af
